And then John, still laughing, realizes what he just heard and does a sort of puzzled look back up at Sherlock:

When he was in a case, Sherlock is badly injured with a blow to his head and loses consciousness. He wakes up in a hospital, (he deduces because of the smell of ether and the feel of the rough sheets and hospital bed on which he lies), and what intrigues and terrifies him immediately is that everything is dark. He feels a air shake at his side, and the scent of the John’s cologne smell. His brain, half foggy, is still faster than most humans of medium intelligence, and he realizes, with a cold terror that runs through his entire body, that the injury he suffered, just an idiot injury in a case which was not even a “five”, it cost him his job. His heart quickens, and soon the air does not seem to fit properly into his lungs. He overhears John's voice asking him to be calm, he feels John's fingers on his face and the warmth of John’s palm flowing through his skin directly on his cheeks, he feels they are wet and that he is crying. He listens John's voice, desperate, and only realizes after a while that he is babbling non-stop "I cannot see you John, I cannot see you..."
How will he deduce? How will he work if he can not see?!
He feels John's lips kissing his eyes and the request for calm again. A part of him automatically deduces that John already knew, maybe he had woken up before, maybe they have already gone through it, and his traumatized mind has locked the painful memory in some chest chained in the bottom of a dark basement in his mental palace.
"I'll be your eyes... I'll be on your side Sherlock, always and forever... I'll be your eyes ..." John speaks, his voice thick with tears. He's crying too.
Sherlock falls on the comfort of John’s arms, and he does not know how, nor how long, but he will have to learn to see the world through John's eyes.
He does not want to, and now his words are painful denials, they are abusive language, insults, pain, and offenses that John does not deserve, but he just squeezes him stronger in his arms and repeats without stopping how much he loves him...
"I do not want to live like this," he says. John replies, "I love you," "I do not want your pity," he says. John replies "I love you" I do not want to need you, "he says. John replies" I love you " "But I'm not "amazing” without my eyes," he says.
John replies: “And I'm still going to love you, for who you are, not for what you can show ro the world. I'll still love you for who you make me be.... And I would be your eyes, your legs, your ears, whatever it takes, because, Sherlock Holmes, I love you.
Sherlock relents in John's arms, and he does not want to think about what he has lost now, but in what he have, his John, regardless of anything and any situation, he have John’s unshakeable love, John’s loyalty, and he knows that it will be so long as they live. Despair is bittersweet, he realizes, and closes his eyes to the inner darkness of unconsciousness, because he knows that when he wakes up again, even if he does not see, John will be there, by his side, always, always and forever by his side.
