You may do a lot of different art and can offer lots of types of commissions. But try focusing on just one or two offerings based on your enjoyment and market demand.
Be clear on what you do or don’t need from your client in regards to creative input.

A Commission Order Form??
Pros:
More intuitive and structured for the client than multiple emails
Would feel more formal and real for the client
Cons:
I’ll have to transfer the data from the forms manually...? Or at least I have no idea how I plan to store the data. In the recipe? Or maybe I can provide a link to the filled out form in the recipe?
I’ll have to reach out to them by email first and my email might go to junk. --> Maybe provide the form link in the first email from client? So CTA publically is to email me, once client has email me then I will provide the form link. 
What Would I Include?
Client name
Best email address
Mailing address
Deadline (write in)
Menu options with quick blurb of size, medium, deliverables: Horse commission, logo commission...
A box to describe the commission. The more detail, the better
A way to attach photos. Include photos that convey the concept, mood, color scheme, reference for characters, a quick doodle you’ve done.
A box to describe your company if relevant: colors, font, mood, mission, product/services provided, 
Will you need commercial rights? (Do you plan to make money directly or indirectly using the art/design? Examples include t-shirt design, poster, flyer, logo, art prints, promotional purposes, etc...)
Is it ok if I publically stream the commission on Twitch and post WIPs publically?
What would be the best weekday and time for you to come to an art stream so you can provide real time feedback about your commission? 
A link to the TOS and a checkbox saying you’ve read the TOS or a quick quiz about the TOS? Type the answer in. Not yes/no questions.

Stripe (Payment Platform)
Cost??? Per transaction fees. No maintenance or subscription fees.
How do I get the money??? Stripe will pay you every 2 days (industry and country depending). The amount will be deposited into a linked bank account. the very first payout will take about a week.
Integrates with Ko-Fi to take credit card payments (as opposed to just Paypal payments.). This would then make Ko-Fi very user friendly for any client to use. No account needed for the client to pay you.
