Reduced lip volume, dry skin and pucker lines around the lips can age the face considerably, and that’s before we’ve even considered the nose to mouth folds and marionette lines that can add to the problem.
Thankfully however, there’s now plenty that can be done to both hold back and improve the signs of ageing in this part of your face, so that you’ll be able to enjoy younger, plumper lips for longer.
Hydration of the lips is very important to reduce the signs of ageing. Dry, flaky lips are not only uncomfortable, they look thinner than their well hydrated counterparts, and it’s this thinness that makes lips appear older. The skin produces less oil as we age, and this, coupled with the fact that the lips have no protective epidermal layer, means that they’re a prime area for moisture loss. Apart from staying well hydrated by drinking plenty of water (something we can’t emphasise highly enough) balms are a very important way of maintaining moisture in the lips. A rich moisturising balm that contains a high SPF will help the lips appear smooth, even and full.
Fine lines around the lips, or the ‘smokers pout’ as it’s come to be known are mainly (although not always) caused by smoking. The lines are due to the repetitive pursing of the lips.but are one side effect of smoking.

Got pigmentation? Want to shift it?
Q: I have some brown spots and patches on my Face that seem to get worse in the Sun. How are they caused and how can I get rid of them to make them look less visible?
A:
Dr Pradnya says these brown spots or patches are caused by a variety of reasons that include sun exposure,hormone changes and hyperpigmentation from certain Aesthetic Treatments. These include Laser or IPL for Hair removal and can particularly affect darker skins. Certain chemical peels can also cause pigmentation post treatment if the skin is not properly prepared prior to having the peel.
The main cause of brown spots or pigmentation however is sun exposure.
Age spots are caused by an excessive production of melanin. You are likely to develop brown spots or pigmentation in skin that is exposed to UV radiation. They also develop as the skin ages. Areas of your skin exposed to the sun will include the face,back of hands, shoulders and forearms.
A brownish patch that develops on the face after sun exposure is called melasma and it is due to overproduction of melanin by the pigment cells called melanocytes which are then deposited in the dermis. There is also a genetic disposition to this type of skin pigmentation..Known triggers for melasma include sun exposure and pregnancy (though the melasma can fade after delivery). Hormone treatments eg contraceptive pill,HRT and implants are also a factor in about 25% of women. Some medications can cause a phototoxic reaction that triggers melasma. Hypothryoidism can also be a causative factor.
These patches can also commonly arise in healthy,non pregnant adults and can persist for decades. UV exposure deepens the pigmentation because it activates the melanocytes to produce more melanin.It is more common in women than men and its appearance can be quite distressing to the individual
Common areas affected by this condition are forehead,cheeks and the nose.
Age spots or melasma formation cannot be prevented but the chances can be reduced by simple preventation.
Avoiding sun exposure especially between the hours 11am-3pm when the sun’s rays are the most intense.
Wearing sunscreen daily. This should be a minimum of SPF 30 and contain UVA and UVB protection. Wearing a physical sunscreen is vital to protect the skin from the photo aging effects.
Wearing protective clothing eg long sleeved clothes and wearing a hat.
Treatments are now available for treating pigmentation in the skin. These will include skin lightening agents though some contain Hydroquinone. This has been clinically proven to reduce pigmentation in the right concentrations but recent clinical research has shown pigmentation can get worse once the treatment has stopped so it has to be carefully controlled by a qualified medical aesthetic practitioner .
There are other skin lightening topical treatments available that do not contain hydroquinone. Kojic Acid as part of a skincare treatment is well known to reduce pigmentation as well and we offer such skincare in our Clinic. This is from Image Skincare.
Any medication to treat pigmentation should be taken under advice in a Medical Aesthetic Clinic as it is a very difficult condition to treat.
Not all cases of pigmentation can be treated with topical agents so adjunctive procedures such as chemical peels,microdermabrasion or microneedling ( Dermaroller) may be used for more stubborn cases,though these have to be exercised with caution due to an increased risk of hyperpigmentation.Results are normally seen after a few treatments.
It can also be treated with Laser but this should be approached with caution in darker skins too as the pigmentation can be made worse post treatment. Pre treatment with Skin lightening agents will need to be used prior to laser treatments to avoid hyperpigmentation.

Bar code lips!
Aesthetics as we all know is a constantly evolving discipline that is essentially all consumer driven. It goes through flux and change both with new techniques, products and even more on areas of the face and body.
Most focus I have noticed this last year has been on the lips and more importantly, the area around the lips. These lines are known to aesthetic practitioners as the peri-oral lines but they were previously coined’ smokers lips’. The latest name for these vertical lines that appear around the mouth, mainly upper lip are now called bar-codes.
So why do they develop? Peri- oral lines are influenced by a number of factors and these include age,genetics,sun exposure,repetitive facial movements,lip threading, waxing,smoking and through drinking through a straw.
Lips have am much thinner stratum corneum compare to the facial skin which makes it venerable to UV damage as well which causes sun damage and the vertical wrinkles.
The muscles that surround the mouth are constantly being used and the repeated contraction of the upper lip muscles when we talk,eat,drink and kiss, the skin becomes thinner due to collagen loss and the slowdown of natural fibroblast cell activity.
Threading and waxing the upper lip also creates lines too because they damage the skin and make it thicker.
Peri-oral lines are more visible in women than men because women have thinner skin in this area.
There are various ways to treat these lines. Firstly, dermal fillers are used that can be used to augment the lips to help with this issue. Personally I use very light dermal filler like Perfectha Fine Lines or Emervel Touch. I use a serial puncture technique whereby small amounts are injected in dots around the upper lip area. I also cross hatch depending on the severity of the lines but I have to be mindful of not over filling so the upper lip area is not overfilled. (£200-260)
Fractional laser resurfacing is also another way treatment that can resurface this area and promote collagen formation. There is usually a downtime associated with these techniques and patients would normally need a course of 3-5 treatments.
Wrinkle relaxing injections such as Botox can also be used but they have to be very very carefully placed to relax the upper lip muscle. Care must be taken not to alter the patients natural smile. ( from £200)
One thing that I would advise is to see an aesthetic practitioner that who performs lip treatments regularly.
The good news is that there are treatments out there to reduce or get rid of the bar codes but be warned, over the years they may well come back

Skin care in the 50′s
In this day and age, we all seem to be more health conscious and skin aware than before and as a result of this, being fifty or more is no longer considered ‘old’.
There are plenty of celebrities out there that look amazing, for example Courtney Cox and Demi Moore and so there is no excuse for not looking after yourself and letting it all go.
Maintaining a youthful glow is more of a challenge now with your oestrogen levels declining as menopause is imminent. This is turn means your skin is drier and thinner.
As appearance is important for some people, there is plenty to be done to make sure your skin does not suffer. From Vitamin supplements to specialist skincare products, diet and exercise to non surgical cosmetic procedures, the options available are as effective as you want them to be.
Here are some tips for your skincare in this fifth decade and make sure you age the way you want.
1. Diet and exercise
Food and physical activity is vitally important to women in their 50’s and you will find it contributes to a wonderful overall sense of wellbeing and appearance. Winter Skin Tips
Winter weather is not fun for our skin. Cold temperatures and low humidity results in dry air which takes valuable moisture away from the skin every second. Without care, the skin will dry leading to cracking and bleeding and harsh winter weather especially if combined with cold winds can make the problem worse. Central heating robs the air of moisture as do hot showers,baths and harsh cleansers. Additional moisture can help but you will need to do more to keep your body and facial skin looking youthful and smooth. So to reduce chapping,itching,redness and to keep the skin more healthy and comfortable this Winter, try my Skin tips!
1. Wash in lukewarm water. Though a hot shower or bath seems glorious at the time, the heat of the water actually will strip your skin of valuable oils making it more prone to redness and itching.
2. Moisturise immediately after you have washed. Applying body lotion/moisturiser to damp skin helps seal that dampess into the skin. Keep a bottle of handcream /body lotion nearby and use liberally after every time you wash your hands/body.
3. Some moisturisers have petroleum based ingredients that actually further dry your skin during the winter months. Choose a smart formula that has natural nourishing ingredients and go for an oil based rather than water based solution, as it more likely to help your skin retain moisture in winter.
4. Protect your skin by wearing gloves & scarves. Do not forget to wear sunscreen as the winter sun can be just as damaging as summer sun. Make sure the product contains titanium dioxide and zinc oxide to get the best protection.
5. Drink Water! We tend to drink less in the winter because we turn to hot drinks but do not forget your skin needs hydration from the inside. A little warm water with lemon is refreshing and hydrating at the same time.
6. Moisturise at night. Drier areas like hands,feet,knees and elbows have thin skin and tend to lose moisture faster than other areas. Use a deep/richer moisturiser at night and do not forget to moisturise your face with a product designed for overnight repair.
7. Exfoliate. It is important to help the skin slough off the dead cells in winter because if the dead cells are not sloughed off, moisture can not get in. I recommend an exfoliating mask with a combined ingredient of glycolic acid that will accelerate skin turnover. I also recommend a body lotion that contains glycolic acid to increase cellular turnover to keep your body smooth. Only use the exfoliating facemask once or twice per week.
8. Winter skin is more fragile so avoid allergans that will cause your skin to flare up.
9. Hydrate from the inside out. Eating food high in water content can help your skin hydrate from the inside eg watermelon,apples,kiwi and watery veggies like tomatoes,celery,cucumbers and carrots. Also make sure you are getting enough Vitamin C and zinc to support healthy production of collagen and elastin, components needed for a glowing complexion. Consume more fatty fish and flaxseed to give your skin the building blocks it needs to keep your skin supple.
10. Change your cleanser. Use a more hydrating cleanser and do not leave your skin too naked for too long as dehydrating it can lead to more dryness.

Rosacea
Rosacea is a common condition that is poorly understood that mainly affects the face. Symptoms often begin with flushing but other symptoms can develop as the condition progresses such as burning and stinging sensations,permanent redness ,spots ( papules and pustules)and small blood vessels that become more visible. There can also be raised red patches ( plaques) on the skin.
Persistent facial redness, known as erythema is like a blush that does not go away and if often mistaken for someone who drinks heavily. The redness usually affects the cheeks,nose and chin but can also spread to other areas such as the forehead,neck and chest.
In addition to skin problems,many people can also develop symptoms that affect the eyes. This is known as ocular rosacea. Symptoms of this can include dry eyes,irritated/bloodshot eyes as well as feeling like there is something in your eyes. Eyelids can also inflame, a condition called blepharitis though this is rare.
Rosacea is a cyclic condition so there are periods when the symptoms subside. It can also be linked to the menstrual cycle. Many of the symptoms of rosacea can be controlled to a degree with treatment. Unfortunately changes to physical appearance can have a significant psychological and social impact making on the sufferers.
The exact of rosacea is unknown although a number of factors have been suggested,including abnormalities in the blood vessels of the face.
Several triggers have been identified that make rosacea worse. These include: sunlight exposure,stress, strenuous exercise,not/cold weather,hot drinks,alcohol and caffeine. Spicy foods can also trigger an attack.
Roscaea appears to be quite common and more frequent in women than men. First diagnoses tend be in the 30-50 age bracket.
Up to 1 in 10 people may have it. It commonly affects white Caucasians but can also occur in Asians and African origin skins.
There is no currently no cure available for this condition but treatment can help symptoms.
Long term treatment is necessary although there will be periods when your symptoms will improve. For most people, treatment will involve a combination of self help measures and medication such as avoiding known triggers eg alcohol,spicy foods and caffeine.
Creams and gels that can help when directly applied to the skin. Products containing Vitamin C are known for healing blood capillaries and benefical to rosacea sufferers.
If Rosacea is associated with spots, then a course of oral antibiotics can help clear them up. In some cases, procedures such as laser and Intense pulse light ( IPL) treatment may be helpful. These treatments are aimed at the visible blood vessels causing them to shrink and make them less visible.
I have found in my Clinical Practice that using a correct skincare range that is dedicated for roscaea skins helps enormously as well as having Skin Peels that reduce the redness, soothe the Skin and that are Vitamin C infused. The downside of Rosacea is that there can be the added problem of pigmentation associated with this condition so it is important to have skincare that helps with this and a hydroquinone free skincare is best.

Cosmetic attitudes!
Have attitudes changed towards cosmetic procedures in the last decade?
Thousands of middle aged men are sneaking out for secret lunchtime Botox/Botulinum Toxin injections without telling their partners, I have recently found out through recent research.
Up to one in five men have treatment and not told their partner/spouse. Men aged 45-55 are least likely to let on to a partner/spouse that they have nipped out for a quick injection to stave off wrinkles.
They could be trying to keep their looks to keep up with a partner who is also having cosmetic help to stay younger, experts reckon.
Men are also more likely than women to seek the treatment in order not to appear too old at work where they are worried about age discrimination.

I too have found this to be true from my own experience.My own recent Google analytics from my website www.revitalise-rejuvenate.co.uk have shown that on average 54% of men are now looking at my website at any one time. I have treated more men for botulinum toxin this year than ever and this is in line with current cosmetic trends.
The most common male customers I noticed checking out the website were 35 to 45 year olds and this seems to be the National trend. Newly divorced men are also seeking such treatments now that they are back on the dating scene.

Other cosmetic procedures that have seen a rise are liposuctions with a 28% increase in 2012. Other treatments also include rhinoplasty.
This increasing trend of males wanting to have some kind of cosmetic procedure I feel marries to the ever growing pressures on men and women to look and feel better. Glossy magazines once for women to look good is now portrayed in Mens glossy magazines such as Mens Health and Esquire. Men are also seeking treatment out for waxing and manicures.

Excessive sweating?
Excessive sweating or medically known as Hyperhidrosis is a common condition in which a person sweats excessively.
The sweating may affect the whole of your body or only affect certain areas.Commonly affected areas are armpits,palms of your hands, sole of feet and your face.
Both sides of the body are usually affected and can be embarrassing and distressing.
Causes for this condition are thought to be a result of a problem with a part of the nervous system that controls sweating. This is known as primary hyperhidrosis. Hyperhidrosis that does have an identifiable cause is known as secondary hyperhidrosis. This can have many different triggers including pregnancy,anxiety,certain medications and low blood sugar and an over active thyroid gland.
It can be treated in a number of different ways. Powerful antiperspirants can help.
Antiperspirants containing aluminium chloride is often used to treat hyperhidrosis. This works by plugging the sweat glands. It is normally applied at night and washes off in the morning.
Lifestyle changes include wearing loose and light clothes,avoiding triggers like spicy foods and alcohol that could make sweating worse. Wearing white or black clothing can also help minimise the signs of sweating.
There are other treatments that are available to treat this condition. These include Anticholinergics. These work by blocking the affects of a chemical called acetylcholine,which the nervous system uses to activate the sweat glands.
Iontophoresis may help if you have excessive sweating that affects your hands and feet. It can also be used to treat the armpits although this is usually less effective. It involves treating affected areas of skin with a weak electric current passed through water or a wet pad. This is thought to help block the sweat glands.
There are other treatments out there that also include Botulinum Toxin injections into the armpits which have been proven to be successful though the effects are short lived and require re treatment.
Small injections are injected in a grid like pattern in the armpits. This blocks the transmission of nerves that supply the sweat glands. Perspiration normally diminishes with 48 hours and effects last for approximately 6 months. The treatment is then repeated at this stage.
There are a number of treatments that are also available that include severing/clipping the nerves that control sweating. This is normally performed under General Anaesthetic though it carries a higher risk of complications.
Hyperhidrosis is common. It is estimated to affect between 1-3 people every 100 in the UK.
It can develop at any age although primary hyperhidrosis typically starts during childhood or soon after puberty. Though embarrassing, there are a number of treatments available to help sufferers of this condition.
