(Ermm!! Dumb headcanon incoming!!!) since piccolos nails are so inconsistent throughout the series, I attempted to make a (sort of) explanation for it ;p

There's a two handed Makankosappo? I never noticed that before. Do you have a picture of it?

It's when Gohan's too afraid to do his part in Piccolo's "Ambush Nappa" plan, ruining their opportunity to take him out.

This relatively short and unceremonious firing marks the second and final appearance of the Makankosappo in the Dragon Ball manga. It's not nearly as memorable as its use against Raditz, where the plot actively revolved around it and it scored the killshot.

So if you were writing Piccolo from the perspective of wanting to take this technique of his and evolve it in new and interesting ways, then this is the version that you'd build on. This is the "true" Makankosappo, and it's where you would want to start developing new ways of using the technique.

But if you were only interested in Referencing the Cool Moment and turning "Remember that time Piccolo did the thing?" into a staple signature move of his attack list, then you'd just keep using the memorable and iconic one-hand version even when he has a perfectly good second hand.
