Sam's statement seems like a key to figuring out how these things work. The little plastic skeleton is not already an external or a cursed object. It seems to be just a normal plastic skeleton. It's actually just Sam's fear of it that makes it dangerous. But it's still just one person and still just a plastic skeleton, so all it can do is papercuts.
I had already been mulling over this, but think about how that translated to Heinrich Unheimlich. He started out as a regular toy that was kind of scary. The local children made it into an urban legend, and then he came to life. As the legend progressed, so did he, getting more powerful. Then as the legend has waned he has become weaker once more.
For a less prominent or obvious example we can also look at episode 18. In this case, the dead woman speaks about being a child and being told not to go into the abandoned house. She is told all the ways she could be hurt if she plays inside and she becomes afraid of it, but as she learns to fear the house, even after it is destroyed it lives on in her mind. Her fear has transformed the house in an effect similar to the spiral in TMA, trapping her inside twisted halls and rooms that are entirely false, unable to escape even in death.
For another case that is fresh on my mind we have the Growler in episode 27. Mr. Magnus himself specifies that it is called such because of the noise the wheels make on the pavement. It is a name that calls to mind frightening imagery as opposed to merely loud. He says many people prefer the quieter carriage, even though it may not be as comfortable. You could imagine from his description the carriage as a wild animal, or a demonic beast ready to devour you should you willingly crawl into its belly. And that's before he describes the actual nightmare in disguise.
With the emphasis on alchemy and transformation through fear, one could come to the conclusion that there are no categorical entities such as the deep, or the lonely, or the stranger, but that each thing is created individually through the collective fear of those that behold it.
In other words, you could take identical creepy dolls and tell two different people to spend time with them and observe them separately, and you could potentially end up with two different entities based on what each of them feared most about it. Then you could take those same people and give them one doll and their fear would give the one thing both characteristics. And you could keep multiplying that endlessly.
