chapter 20: YUZI: unknown trigger or removed scene
I don't know what the trigger for this scene is. It looks like it's supposed to play after the broadcast, but before you have the chance to visit Seagull and save Shinji and Mika.
It's very unusual for an entire scene like this to be removed from the game, but I can't think of anything that would make this scene possible, since Natsuki scans the city and shows you where everyone is immediately after the broadcast. It seems to match with the other scene at Production Art where Arthur talks to you after the broadcast, so that would leave only two locations open during this phase of the day, which seems very weird for two non-essential conversations.
Unlike the scenes at the Civic Hall, there is no alternate version with Yuuji still infected. In fact, it looks like he's been given his shop back. That's not possible in the finished game, so I think it must be an actual removed scene from an earlier version of the plot.
Since Yuuji mentions Mika coming to his store about a news story once, I think that must be talking about the mysterious fire subplot that appears in the code. So there were in fact scenes developed around some of those removed items that made it into the game! This and the matching scene with Arthur appear to be the only ones that were missed when the game was being revised.

Chapter 20: Production Art: probably removed scene
I believe this scene matches with some dialogue set at YUZI in the same chapter. That scene at YUZI is impossible in the final version of the game, so I believe these two scenes are remnants of an earlier version of the game. They must have been missed when the devs were taking out obsolete content, probably because they're so close to the end of the game.
It looks like there are different versions of this conversation, so maybe this earlier version of the game let Arthur get reinfected by another Silent, or emotionally damaged by some earlier dialogue choice?

One of the weirder mistakes in the game: ever since the statue subplot came up on this playthrough, I've been asking myself where it supposedly is at the train station.

There's absolutely no statue here. I've zoomed in as far as I can go and there's nothing.
Then I visited in chapter 20...

Lo and behold, there is a version of the background art with a statue! Just not the one that's used for most of the game. This must be something left behind from a previous stage of development, a version of the story where the train station either doesn't have a statue at all, or there's a plot point about it being removed. Very strange that they didn't notice the primary background art was missing a huge item the characters keep saying is present.

[NOTE: As with Yayoi and Akira, I've never gone for the Edward-only ending, but I'm guessing this plays if you mess up BOTH Sakai and Honoka, which causes the game to end before you deal with Graham.]

千年よりも昔から、 悲しいオフェーリアは白い幻となり、 はてしない黒い大河を流れてゆく。 千年よりも昔から、 オフェーリアの切ない恋の想いは 暮れゆくそよ風に、 あの恋の詩をつぶやく…。
Long ago pale ghostly Ophelia across the night sky misses someone as she reads the love poem with the breeze...
For a thousand years ago and more, the sorrowful Ophelia has been a white phantom drifting in the endless black river. For a thousand years and more Ophelia's feelings of painful love whisper her love verses to a darkening zephyr...
[NOTE: I just tried translating this and then looked up one phrase only to discover this is a REAL POEM written by Arthur Rimbaud... So I don't know how they messed it up this badly. Arthur is reciting the first two verses when you first meet him in chapter 00. Here's what he's actually supposed to be saying:
On the calm black water where the stars are sleeping White Ophelia floats like a great lily; Floats very slowly, lying in her long veils... - In the far-off woods you can hear them sound the mort.
For more than a thousand years sad Ophelia Has passed, a white phantom, down the long black river. For more than a thousand years her sweet madness Has murmured its ballad to the evening breeze.]

[NOTE: As with the Akira-Triple Step scene, I didn't get this scene in my current playthrough and don't know what triggers it. I'm guessing it plays if you mess up with Sakai AND Honoka, which causes the game to end early without ever tackling Graham.
None of this really needs to be retranslated, but here it is.]

その時、レイは追手も 出さずに以後の調査を ストップさせた。
Farancks. The people of Photo have infiltrated the organization  in the past, but we’ve stopped receiving communication with them.
Ray did not send pursuers and the investigation came to a complete hault.
[ATSUKI:] PHALANX. Members of FORT once infiltrated that organization, but FORT lost contact and their whereabouts remain unknown.
Ray didn't send anyone after them, and all further investigation was halted.

『私たちがちっぽけ  な騎士だとすると、  相手はドラゴンにも  相当する』と語った。
The reason was, ”The enemy was   more of a   threat than we  imagined.”
They said that,  ”We were  like mere Knights,  and our opponents   were proportionate  to dragons.”
[ATSUKI:] The reason he gave then was, "The enemy is much larger than we imagined."
He said, "If you think of us as tiny knights, then our opponent is the equivalent of a dragon."

いかなる国家組織にも 恐れず立ち向かう彼が、 何もせずに成り行きを 見ているということで、 その不気味さがわかる。
今回の事件は? ファランクスの主力的 なものなのか? それとも枝葉的な事件 なのか?
どちらにしてもすぐに それはわかるだろう。
Regardless the national organi- zation, watched quietly without taking action. 
What incident? Is Farancks the driving force? Or were they coincidences?
We’ll know for sure soon.
[ATSUKI:] He's the one who faces down even national organizations without fear. It's very eerie for him to sit back and watch events unfold without doing anything.
What's happening here? Is this part of PHALANX's ultimate goal? Or is it just a minor incident?

ナツキが向こうから 手を振りながら走ってきた。

Atsuki mistakes her for a doctor from her white clothes. It’s Honoka Hino.
Honoka sees Atsuki nd walks over to him.
/Atsuki took her for a doctor because of her white gown, but in fact the owner of the voice was Hino Honoka.

chapter 19: Good hospital scene
おっはよ~~!!
今日はすがすがしい朝だね。
Good morning!!
What a crisp morning.
[HONOKA:] Gooood morning!!
Sure is brisk, isn't it?

こんなに気分��良い朝は 久しぶりっていった感じだよ。
それもこれも、西条のおかげ みたいなものだけどね。

[HONOKA:] I feel like it's been a long time since I had such a nice morning.
And I get the feeling I owe it all to you, Saijou.

目が気味悪いくらいに光っていて、 吸い込まれそうな感じがしてね。

底知れない深さって言うのか……。
その目を見ていると、 金縛りみたいになるんだよ。

When I looked into his eyes, I totally froze up.

[NOTE: Even though Edward was the one in Kiryuu's backstory, this one line identifies the stranger as Graham. In your final battle, his Silent quotes the same line from Saint Augustine of Hippo: "Do what thou wilt." So he used his mind control abilities, which, as we learned from Hoshino and Yamato, can be used on someone without infecting them. But the Silent infection we removed yesterday was part of Edward's Silent, Maggot. You have to go by this one line and a statement in the good route that she and Hibiki both had their senses of self eroded like Tanaka.
It seems like Aleister Crowley also used the phrase "Do what thou wilt" when defining his philosophy of Thelema: "Do what thou wilt shall be the whole of the Law." It's possible the devs got the quotation from him instead, since we know they were looking through books about the occult while writing the Silent dialogue.]

『なに言ってやがるんだい!』
って蹴飛ばしてやろうと思ったら、 もうどこにも居ない。
”What the hell is that?!”
I was gonna beat him down, but he was gone.
[HONOKA:] I was about to give him a good kick and yell,
"What the hell are you talking about?!"... but he was already gone.

気味の悪い話だろ? まるで真昼の幽霊だよ
そこから何かさ、気分がすっかり 落ち込んでしまってね。
Isn’t that creepy? Like a ghost in the afternoon.
Since then, I’d been feeling so depressed.
[HONOKA:] Isn't that creepy? Like seeing a ghost in the middle of the day.
After that, I started feeling completely depressed.

だけど、もう大丈夫さ!
I only thought about bad and unhappy things...

[NOTE: As with the other scene in chapter 20, there are actually four different versions of this scene, depending on whether you left them infected or not. I fixed both of them, so I can't see versions 2-4 in my game, but luckily you can tell exactly who's talking just from their speaking styles .]

Mystery: what is that thing on the left lapel of Graham Miller's suit? It can't be blood, because no one ever comments on it.

If you zoom way, way in on the ripped version of his sprites, you can see that it's purple.

Personally, I've always associated his design with the March Hare, and if he has a jam stain on his lapel, that makes it seem like the similarity was intentional?

Incidentally, something I've only just noticed is that Graham's name in katakana is グレアム, Gureamu, which would be pronounced Gray-um in the British style--although looking at the Wikipedia list of Grahams, it seems like the Japanese pronunciation is just a toss up that has nothing to do with American vs. British. (Your other option is Gurahamu, which doesn't really reflect either pronunciation.)
