Công dụng của thuốc Pentacillin Injection 1.5g trong việc điều trị bệnh

Thuốc Pentacillin Injection 1.5g được dùng để điều trị:

Nhiễm khuẩn đường hô hấp trên và đường hô hấp dưới gồm: viêm xoang, viêm tai giữa và viêm nắp thanh quản, viêm phổi do vi khuẩn.

Nhiễm khuẩn đường tiết niệu và viêm thận- bể thận.

Nhiễm khuẩn trong ổ bụng.

Viêm màng não.

Nhiễm khuẩn da, cơ, xương, khớp.

Nhiễm lạicầu không biến chứng.

Chỉ định ở những bệnh nhân phẫu thuật ổ bụng hoặc vùng chậu có thể bị nhiễm bẩn phúc mạc để giảm tỷ lệ nhiễm khuẩn vết thương hậu phẫu.

Hướng dẫn sử dụng thuốc

Cach sử dụng

Thuốc Pentacillin Injection 1.5g được sử dụng qua đường tiêm.

Đối tượng sử dụng

Bệnh nhân sử dụng thuốc theo chỉ định của bác sỹ.

Liều dùng

– Ngư��i lớn:

Nhiễm khuẩn nhẹ 1,5-3g/ngày.

Nhiễm khuẩn vừa tăng tới 6g/ngày.

Nhiễm khuẩn nặng tăng tới 12g/ngày, chia 3-4 lần.

Liều tối đa là 4g sulbactam/ngày.

– Trẻ em, trẻ nhỏ, sơ sinh:

150 mg/kg/ngày, chia 3-4 lần, riêng sơ sinh  < 1 tuần, chia 2 lần/ngày.

– Lậu liều đơn 1,5g + uống 1 Probenecid.

– Dự phòng quanh phẫu thuật 1,5g-3g tiêm lúc khởi mê, nhắc lại mỗi 6-8 giờ/24 giờ sau mổ.

Chống chỉ định

Qúa mẫn cảm với các thành phần của thuốc.

Tác dụng phụ

Đôi khi: Ban, tiêu chảy, đau nơi tiêm, phản ứng dị ứng.

Hiếm khi: đau ngực, mệt, khó chịu, nhức đầu, ớn lạnh, bệnh não động kinh, ngứa bồn nôn, viêm ruột, viêm ruột giả mjac, tiêu khó, viêm âm đạo, giảm các huyết  cầu.

Tương tác thuốc

Cả Ampicillin và Sulbactam đều tương kỵ về mặt lý – hóa với aminoglycosid và có thể làm mất hoạt tính của aminoglycosid in vitro.

 Probenecid ức chế cạnh tranh sự thải trừ của Ampicillin và Sulbactam qua ống thận, do đó làm tăng và kéo dài nồng độ của cả hai thuốc này trong huyết thanh.

Có sự gia tăng tần suất phát ban trên người bệnh có acid uric máu cao đang được điều trị đồng thời bằng Allopurinol và Ampicillin .

Ampicillin được thông báo là có ảnh hưởng đến xét nghiệm tìm glucose trong nước tiểu bằng phương pháp đồng sulfat, nhưng không ảnh hưởng đến xét nghiệm bằng phương pháp glucose oxidase .

Thận trọng khi dùng thuốc

Chỉnh liều ở bệnh nhân suy thận, có thai và cho con bú.

Dược lực

Ampicilline là kháng sinh bán tổng hợp thuộc họ Beta-lactamines, nhóm Penicilline type A, độc tính thấp, phổ kháng khuẩn rộng.

Dược động học

– Hấp thu: Hấp thu tốt qua đường tiêu hoá nhanh và gần như hoàn toàn. Sau khi tiêm bắp 1 liều 500mg, sau 1 giờ đạt nồng độ dỉnh trong huyết tương, uống liều 500mg sau 2h đạt nồng độ đỉnh trong huyết tương. – Phân bố: Khoảng 20% ampicillin liên kết với protein huyết tương. Ampicillin có thể tích phân bố lớn, khuếch tán qua nhau thai và tuần hoàn của thai nhi và vào nước ối. Ampicillin không qua được hàng rào máu não. Trong viêm màng não mủ, do hàng rào máu não bị viêm và thay đổi độ thấm, nên thông thường ampicillin thâm nhập vào hệ thần kinh trung ương đủ để có tác dụng. – Thải trừ: thuốc thải trừ nhanh qua ống thận(80%) và ống mật.
