Like every organ, the kidneys are of great importance for human health. The kidney, which is located on both sides of the abdominal cavity and resembles a bean in shape, works like a filter in the human body and ensures that harmful and waste materials are removed from the body and the blood is cleaned. Thanks to this article, you can have information about the open nephrectomy operation used in the treatment of kidney diseases.
What is nephrectomy?
A healthy functioning of one of the kidneys in the human body is sufficient for the continuation of vital functions. However, in some cases, malfunctions in the kidney, which is so vital for human health, can cause an  increase  in urea and creatinine  in the body, resulting in kidney failure , damage to kidney tissue, and the formation of cancer in the kidney. In this case, it may be necessary to remove the sick kidney from the body by operation. The operation to remove the kidney from the body is called nephrectomy.
What is Open Nephrectomy?
Nephrectomy, which is the process of removing the kidney from the body, can be performed by open surgery or laparoscopic surgery. If the operation is performed with the open surgery method, this procedure is called open nephrectomy. In some cases, specialists may decide that it is not appropriate for the patient to remain under anesthesia for a long time. In this case, the laparoscopic (closed) nephrectomy method may be preferred as an alternative to the open nephrectomy method. Today, although the post-operative period is more comfortable, patients and specialists prefer the laparoscopic nephrectomy method, open nephrectomy is still one of the most effective methods used in the treatment of kidney diseases in cases where the laparoscopic nephrectomy method is not suitable.

In cases where the donor and the recipient must be together in the case of chronic kidney failure, which is seen as a result of long-term tissue damage in the kidney tissue,
In disorders of the pancreas.
There is the possibility of treatment with open nephrectomy method in many diseases from kidney cancer to kidney injuries, polycystic kidney diseases and pancreatic problems.
Nephrectomy can be grouped into two classes as partial nephrectomy and radical nephrectomy.
In some cases, removing the tumor tissue is sufficient to treat the disease. This procedure is known as partial nephrectomy. In some cases, since it is not possible to leave intact kidney tissue, the entire kidney must be removed during the operation. This procedure is called radical nephrectomy. If both kidneys are damaged, both kidneys may need to be removed from the body, but in this case, a kidney transplant is needed because it is not possible for the patient to live without their kidneys. Open nephrectomy operations are operations where the recipient and donor are together. Before this operation, some analyzes are performed on the receiver and the transmitter. If, as a result of these analyzes, the results of the recipient and the donor are compatible, the operation is started. The kidney from the donor is placed in the recipient. During this procedure, the healthy kidney is placed on the right or left side of the recipient. The process of taking a healthy kidney from the donor and attaching it to the recipient is known as donor nephrectomy.
In cases where kidney transplantation is required, the patient’s own kidney is not removed if the diseased kidney is not at risk of cancer or if there is no infection.
Open Partial Nephrectomy
In kidney cancer patients, nephrectomy operation, in which tumor tissue is removed and intact kidney tissue is left, is known as partial (partial) nephrectomy. During the partial nephrectomy procedure, the surgeon reaches the tumor through the opened incisions. For this reason, open nephrectomy method is preferred in partial nephrectomy operations. Open partial nephrectomy may be preferred in the following cases:
kidney cancers,
Infections in the kidney that cannot be treated with drug therapy,
Kidney failure,
The possibility of deterioration in the function of the other kidney,
Injuries to the kidney,
Conditions in which the blood supply of the kidney decreases as a result of occlusion in the vessels leading to the kidney,
Hypertension as a result of problems in the blood supply of the kidneys  ,
Congenital kidney anomalies.
During open partial nephrectomy, after an incision is made in the abdominal wall, the vessels leading to and exiting the kidney are clamped to prevent bleeding in the kidney. After the diseased tissue is removed, the clamp is removed and blood flow is restored to the kidney.
The duration of the open partial nephrectomy operation varies according to the size of the cancerous or impaired tissue. The procedure can be performed within 1-2 hours on average. However, as with any open surgery, there are some complications that may occur during open nephrectomy. Some of these complications are as follows:
During open partial nephrectomy, bleeding may occur that requires urgent blood supply.
The healing process can be long and accordingly  urinary incontinence  problems may occur.
Since the operation is performed in the form of open surgery, pain may be felt in the operation area for a few weeks after open partial nephrectomy.
Since open nephrectomy is performed under general anesthesia, allergic reactions to anesthesia may occur in some cases, but this is very rare because the necessary analyzes are performed by a specialist team before anesthesia.
After open partial nephrectomy surgery, patients are discharged within an average of 1 week. In the post-hospital period, patients are asked to pay attention to some points. In this process, the patient;
Should consume plenty of fluids,
Avoid doing heavy exercises until the surgical wounds are fully healed,
He should not neglect to go to his check-ups.
Open Radical Nephrectomy
Radical nephrectomy is a frequently used treatment method in kidney cancer treatments. In some cases, it is not possible to remove a part of the kidney, especially in the later stages of kidney cancer. In this case, the kidney and the fat tissue under the kidney must be completely removed. The process of removing the kidney completely is called radical nephrectomy. If the other kidney is healthy, one kidney will also perform the task of the kidney that was taken. However, in cases where there is a tumor in both kidneys and both kidneys need to be removed, kidney transplantation is necessary. In this case, open nephrectomy is performed with an operation where the donor and the recipient are together. If the kidney with tumor is not intervened, the tumor continues to grow and begins to damage other organs. This leads to many different health problems.
Radical nephrectomy can be performed as laparoscopic or open nephrectomy. In some cases, the operation is started with the closed surgery method and open nephrectomy can be started during the procedure.
General anesthesia is used during the open radical nephrectomy procedure. An incision is made under the ribs and the kidney is reached.
As with other surgeries, open radical nephrectomy has some risks. These risks can be listed as follows:
In patients who underwent radical nephrectomy with open surgery, pain after the operation is a common side effect.
In some cases, although rare, bleeding may occur during or after the operation.
Infection of the surgical wound is one of the rare side effects. In order to prevent this situation from happening, it is important to have dressing as the doctor says.
One of the rare conditions is intestinal obstruction.
After the partial nephrectomy procedure, as well as after the open radical nephrectomy, the patient will be sick for a while;
Should consume plenty of fluids,
Avoid lifting heavy loads
Intense exercises should be avoided.

GUESS WHAT!!! a tutorial for rayman origins texture mods like how i made uglette

this is for origins only for now, ill make a separate post for legends later (but itd follow most of the same steps)
first of all. use rayman control panel, if you dont have it somehow then what are you waiting for?? make sure its updated to the latest update too (14.0.0.9 as of writing this) for improved modding support

step 1: getting the textures
in control panel, click origins and then open the archive explorer

youll get a whole directory of a bunch of folders, what you need for the textures is just the itf_cooked folder. if you want all the textures just right click the folder, click export & convert, and convert them to png or something for convenience

if you converted all the images and dont need any things that arent images, just use windows search on the extracted folder to prune all the remaining .ckd files to save on space, but this isnt as necessary
you can also extract just the sprites you want to work with, or heck get them from spriters resource if you want, but still keep control panel in mind as you need the original directory of the files
if you want to replace the playable characters, their textures are are in the following folders
rayman sprites: itf_cooked/pc/actor/playablecharacter/rayman/animation globox sprites (you will also need mosquito sprites for him): itf_cooked/pc/actor/playablecharacter/globox/animation itf_cooked/pc/world/common/shooter/playablecharacter/shootermoskitoglob/animation teensy sprites: itf_cooked/pc/world/1_jungleworld/friendly/teensy/animation ui icons for everyone: itf_cooked/pc/world/common/ui/ui_bar

step 2: the art itself
once you have the original textures all you need to do is to edit them, or redraw them. this may be a tedious process though! what i did is painstakingly edit each original spritesheet to be 2x size, then aliased to put it into a format that i can easily work with and edit, i mostly did so with a whole bunch of magic wand usage and tweaks
i did finish all the templates for all playable characters, i will see if i can release them somehow eventually? but yeah for now figure out your own way to edit them for now

either way you can figure out some way to draw new art for them. save it in a folder separate from all the other assets obviously. here for example i made a more rayman 3-like grand mimimus. i also frankensteined an icon together

step 3: dds things
now that youve done the textures for the mod, duplicate them and save them as .dds files
this isnt necessary, as larger resolutions will also work (probably as long as the dimensions are still a power of 2), but you can resize the textures to the original resolution as well. i will be doing that personally
if you dont have a program that supports dds files, i just use paint.net. i provide a save configuration for it specifically, you can get it here if you need
this is updated!!! after much experimenting this one looks the best to me. the most important settings are outlined in orange, but try to match the rest too

make sure you have generate mipmaps on specifically or else the sprite will end up pixelly

however some additional, more jpeg-y crust will be applied onto the sprite from the mipmaps themselves. so heres the technical part thats probably complicated... if you dont wanna bother with this try to copy the rest of my save configuration, i think it works the best here
i experimented a bit with what looked best and picked fant with no gamma correction, but you can try saving multiple dds files with different settings, viewing them with whatever lets you view the mipmaps themselves (windows image viewer does) and just picking which looks better at a smaller scale

(to see the differences look closely at the eyes specifically, its probably hard without overlaying these one over another and directly comparing though)
one thing i can say is bicubic and fant are definitely the best for matching the original sprites' mipmaps, as theyre not too blurry (like bicubic smooth or bilinear) while remaining fairly good looking
these are the original dds files' settings as viewed in dxt viewer. most of this doesnt seem to be applicable for the save settings but the compressed format is specifically dxt3 (which is also present in its header if you view it in hxd), so for the first save setting you probably have to pick BC2 (Linear, DXT3) as i have

TLDR: i know this is complicated, try to just copy the dds save configuration from the start of this step i guess???

step 4: renaming things
once you have done that, the weird thing you need to do is to rename the .dds into files that the game can actually read (even though theyre still dds files, and can be renamed back to .dds for them to function). for convenience you can just duplicate the files again
pay attention to the original file formats which you can see in control panel! as you can see, you should rename the majority of the extensions from .dds to .tga.ckd

windows will stop recognizing them, likely the same for the programs you use, but that shouldnt be a big deal. if you want, you can rename them back to .dds to make them usable

the important part here, dont miss it!!! is that all the ui icons are not tga.ckd, but png.ckd!!! even though theyre actually still dds files under the surface. just rename them correctly and pay attention to the right extension, or else the icon wont work

step 5: actually making the mod
now that you have the relevant files, click create mod in control panel

select the files module, and then make and select a folder to use as the mod folder, which should give you this result

you should create a GameData folder in added_files, after which the entire regular filepath follows, starting from bundle_PC.ipk, then itf_cooked, then so on. for example, these are (nearly) the full directories of my files
\mod\files\added_files\GameData\bundle_PC.ipk\itf_cooked\pc\world\1_jungleworld\friendly\teensy\animation\teensy_a1.tga.ckd \mod\files\added_files\GameData\bundle_PC.ipk\itf_cooked\pc\world\1_jungleworld\friendly\teensy\animation\teensy_e1.tga.ckd \mod\files\added_files\GameData\bundle_PC.ipk\itf_cooked\pc\world\common\ui\ui_bar\ui_bar_teensy_a3.png.ckd
obviously all the textures should have the same filename and the same file path as the textures theyre replacing
then go back to the base mod folder and edit the metadata.jsonc file in something like notepad++. the file itself has commented-in extensive instructions so just follow them

as the mod creator thingy says you can also optionally create a thumbnail
anyway once youve done all of that, select all the files in your mod folder and zip them. you cant just select the folder WITH the mod things and then zip that, as then control panel wont read it because the base of the zip directory doesnt have the metadata file

step 6: finally implementing the mod!!!
now that you have the zip ready, click the mod loader and select this in the lower left corner

navigate to your zip and select it. it should appear here under installed mods

make sure you press the checkmark to the left of it and then press the apply button in the lower right

after a short wait for the mods to apply you can go turn on the game and test it

if you want to make any further edits to the textures, youd have to resave them as .dds files again, and rename them to tga.ckd/png.ckd and such, then just replace them in the mod's directory (or add new files to it) and just zip it again. its fairly easy to work with i hope
this is overall kind of a complicated tutorial but its not too bad once you get the hang of it

omfg y'all am I insane or is this healthcare professional a fucking nutjob
(Spoiler: I know she's a fucking nutjob but would deeply appreciate others chiming in because I still feel crazy 😭)
Okay, so I am still reeling from this provider interaction and feel like I'm going crazy. To sum up, she downplayed covid in general, suggested that antivirals were more dangerous than covid itself, and I believe she tried to scare me and my partner into not asking for Paxlovid at all.
Background info:
My partner and I are both mid-30s. We both have health conditions that place us in the high risk category for serious covid complications (asthma, CKD, overweight/obese, etc.).
Because we have several high risk factors, we have been super careful for the past 3+ years, and fortunately haven't caught covid at all before now (to our knowledge).
We are primarily concerned about long term damage and possible long covid. We are aware that even though omicron has lower risk of long covid, it's still around 10-14% risk. Especially considering our high risk health conditions, we want to do everything we can to prevent long covid.
We are aware that Paxlovid is an oral antiviral that received full approval a couple of months ago for high risk groups.
What happened:
My partner and I both tested positive for covid today using an at-home test, so we decided to go to the local clinic. They offered to see us together and we agreed, so we're in the same exam room.
The tech who was taking our info asked, "You tested positive with an at-home test? When?" "Yes. We took the tests about an hour ago." She seemed confused. "So why are you here?" "To…. confirm with a PCR I guess? Or to get antivirals?" She seemed annoyed and quickly left.
[Edited to add - the LVN and tech discussed between themselves, before they left the room, that the provider said a PCR wasn't necessary since we had a positive home test and symptoms.]
When the provider (CNP) came in, she started off with the same question. "Okay, so I understand that you tested positive with at-home tests. So... why are you here?" We gave her the same answer: "To get Paxlovid? ...I guess?"
She scoffed and said, "I'll prescribe you Paxlovid if that's what you want, but I don't recommend it." Well, that wasn't what we expected to hear. She explained that she doesn't recommend it because it's only authorized for emergency use and "doesn't really do as much for you as you'd like to think." She went on to explain that it interacts with "a ton of really common medications" so it can be very risky to use anyway, possibly even more risky than covid itself. My partner spoke up to say, "Yes, we saw that the only medication of mine that appears to have an interaction is amlodipine but that it isn't a contraindication." and she quickly said, "Right, but Paxlovid is just very risky if you have anything cardiovascular going on."
O…kay….
She then explained that Paxlovid isn't really for mild cases anyway, it's really just for people who are at high, high risk for severe covid symptoms and would, like, die in 24 hours without treatment. We found this odd (and in fact blatantly wrong) based on our own knowledge of Paxlovid and covid, so we didn't quite know what to say. I felt very put off and like I couldn't trust what she was telling us. It's at this point that I suspected this woman was trying to dissuade us from taking Paxlovid for… I have no idea what reason. But we live in West Texas and it's extremely red here, so… draw your own conclusion. (I did.)
She then started talking down to us (IMO) by explaining that we would still have to deal with the coughs and sniffles even if we took the antivirals. As though we were only there for a quick fix, because our little stuffy noses were so annoying. I finally spoke up and explained that we're less concerned about acute symptoms and more concerned about long-term effects and possible long covid.
This really made my blood boil. She proceeded to tell me that 1) Paxlovid won't help with preventing long covid, 2) omicron doesn't really cause long covid, that's just the original covid strain that does that, so no one is really seeing new cases of long covid, and 3) no one's getting severely sick from covid anymore anyway, and we appear to have a mild case, so I shouldn't really be concerned. You know, folks, based on my own knowledge, those are some pretty sweeping unsupported statements. So the more she talks, the more concerned I get.
She then starts telling us about how there's all sorts of adverse reactions that can happen with Paxlovid. Horrible reactions that could totally ruin your life! Worse than long covid! But no specific reactions mentioned at this point - just general "Oh, it's so bad" pearl-clutching statements. We sat silently through this -- I think wondering if this was actually happening.
Our continued silence finally caught her attention. "Oh, I know, most people don't expect to hear all this!" "Uh huh." At this point, I'm trying to not to pinch the bridge of my nose and sigh. "So what are the risk factors for these reactions?" "Well, they're far-reaching effects that can ruin your health and your life! It can happen to anyone. Things like hemorrhagic stroke--" I am extremely annoyed and cut her off. "Okay, fine, but that doesn't answer my question. What are the RISK FACTORS for these reactions?" "There are none." "None? No risk factors. No correlations. For any of these reactions." "No. It affects everyone equally. Age, sex, race, doesn't matter. You aren't safe because you're young." "No specific health conditions that would put one at higher risk of these bad reactions?" "No, it affects EVERYONE the same."
^^ If y'all don't see why that kind of claim is absolute bullshit I don't know what to tell you. I'm not 100% sure, but I don't think I've ever heard of any condition/reaction that that has zero identifiable risk factors or correlations, that just somehow affects everyone equally. She pulled that out of her ass, full stop.
Anyway, we just stared at her again because what else do you say to that kind of fearmongering bullshit? She followed up with, "And this drug is only approved for emergency use, so you know, you're taking a big risk if you choose to take Paxlovid. Like, I have to confirm in our system when I send the prescription that you understand the risks. And it is a risk."
Here's the thing. I remembered seeing a couple of months ago that Paxlovid was now fully approved for high risk groups. You know, like me and my partner. She's trying to tell us that Paxlovid is somehow more dangerous than covid/the risk of developing long covid. As best I can tell based on current data, risk of developing long covid from omicron is around 10-14%. Is this woman seriously trying to tell me that Paxlovid has greater than a 10-14% chance of causing severe issues with no known correlations/risk factors, and was somehow still fully approved for high risk patients a couple of months ago? Something is seriously off here.
If I had to listen to any more of this bullshit I think I was going to lose my absolute mind and verbally rip into her. She already said she'd prescribe it if we wanted it, and I didn't want to risk that changing by pissing her off, so finally I was like, "Okay, I guess can we have a moment to discuss this together, then?" I figured we'd be able to exchange a quick, "This woman is crazy. Yes, let's ask for the Paxlovid", but she goes, "Oh, no, I'll just go ahead and submit the prescription. None of the pharmacies are open until tomorrow morning anyway, so that'll give you plenty of time to consider it. And if you decide you don't want it, just don't fill it. But if you decide you do, the prescription is already submitted."
O....kay... then...
So she leaves and soon the LVN comes in with the appointment summary printoffs. The LVN asks us if the provider discussed with us the cardiovascular risks associated with Paxlovid.
"No...? I mean, she said it can interact with blood pressure medication but that's it."
"Well, it can have some pretty nasty cardiovascular side effects. I used to work in cardio at the peak of covid and we got calls all the time about whether or not this cardio patient could take Paxlovid."
No mention about what the answer actually was for all those phone calls -- just the vague implication that it's terrifying to take Paxlovid because it might hurt your heart. Again, this just didn't match what we knew about Paxlovid, but we also aren't confident enough to call them on it ourselves.
We got out of there as quickly as we could and agreed in the car that we would look up more info online when we got home; however, our inclination was already to fill the prescription and take the Paxlovid. We tried looking up these adverse effects she warned us about. All we can find is info about medication interactions, which we already knew. We found studies stating that cardiac patients benefited more than most groups from taking Paxlovid. We found ample information suggesting, as we already knew, that with our health conditions we are absolutely recommended to take Paxlovid and that we personally have no contraindications.
It occurred to us after we got home that the provider never told my partner what to do in terms of adjusting the amlodipine while taking Paxlovid. They'll have to ask the pharmacist tomorrow and may skip the amlodepine tomorrow until they can ask their regular doctor about it on Monday. Sucks that this is happening over a weekend and the only medical professionals available will be the same folks we just saw, because they sound absolutely INSANE.
In case anyone is wondering, uh, yeah, as best I can tell, current data suggests Paxlovid helps reduce the acute illness period AND may reduce the risk of long covid. And I couldn't find a single goddamn thing about severe reactions like hemorrhagic stroke. So I don't know what the actual fuck these people were doing trying to blow smoke up our asses, but it PISSES ME THE FUCK OFF. They have done this to other people. And even if we complain, I'm sure they will be allowed to continue doing this to other people because covid deniers and downplayers are the norm here, not the outliers.

Understanding why eGFR laboratory reports include African American and non-African American results
Glomerular filtration rate (GFR) is the best way to measure how well your kidneys are working, but this test is complicated and cannot be easily done in a doctor’s office. To get around this, laboratories use mathematical equations to estimate the glomerular filtration rate instead of measuring it. This is why laboratories report estimated GFR or eGFR.
Two commonly used estimating equations for eGFR are the CKD MDRD (Modification of Diet in Renal Disease) and the CKD EPI (Chronic Kidney Disease Epidemiology Collaboration) equations. Using these math equations, eGFR is calculated from the amount of creatinine in the blood.
Creatinine is a waste product that comes from the normal wear and tear on your body’s muscles and also from the foods you eat. Everyone has creatinine in their bloodstream. However, creatinine levels can differ between people. This reason for this difference may not only be related to kidney disease – it may be affected by several other factors, such as age, sex, and body weight.
Race was originally included in eGFR calculations because clinical trials demonstrated that people who self-identify as Black/African American can have, on average, higher levels of creatinine in their blood. It was thought the reason why was due to differences in muscle mass, diet, and the way the kidneys eliminate creatinine. Since a patient’s race is not always used when laboratory tests are ordered, laboratories used different eGFR calculations for African American and non-African American and included both numbers in their lab results.
The use of race in calculating eGFR has been a subject of debate. Race is not a biological concept, but a social construct. Using race as a factor for calculating eGFR does not account for the diversity within communities of color. Also, people who self-identify as multiracial might not want to be put in a single racial bucket.
What is the NKF doing to address concerns about using race in measuring eGFR?
In 2020, the National Kidney Foundation (NKF) and the American Society of Nephrology (ASN) formed a joint task force to review the use of race in eGFR calculations. On September 23, 2021, the Task Force announced a new race-free calculation for estimating eGFR. The National Kidney Foundation is working with the nation’s laboratories to get this new calculation into use as quickly as possible.  The NKF is also creating materials to show how this new equation may impact patient care.
How is eGFR used?
