Botox® vs. Dermal Fillers: How Do They Differ?

Botox® and dermal fillers are two convenient beautification techniques. Despite the popular misconception, the only things these procedures have in common are that both are non-surgical and are administered via injections. These are thought of as complementary procedures and can be combined for enhanced results, but one should not be considered as the equivalent of or a substitute for the other. In fact, the types of aging or irregularities they target, their ingredients, and the way they function are very different. Dr. Nowell Solish, who provides cosmetic dermatology in the Toronto area, explains some of the key details to know about the two.
There are two reasons why most people seek fillers or Botox®: to reverse the consequences of aging, or to enhance their existing facial features. As our chronological age increases, most people notice that their skin is no longer as supple, smooth, and firm as it once was. This process, known as intrinsic aging, causes the skin to decrease its production of connective proteins like collagen and elastin, new skin cells, and moisture. And the skin is also repeatedly battered by external forces like environmental factors, pollution, and exposure to UV radiation from sunlight.
How Do They Work?
The main factor that sets Botox® and products like Juvederm® apart is that the former is what is known as an injectable neuromodulator while the latter is a dermal filler, otherwise known as a soft tissue filler. Once injected into the skin, Botox® works by changing the way nerves interact with muscles in isolated spots on the face. It is a protein from a natural substance that weakens nerves for a specific length of time nerves for a specific length of time. The overlying skin appears smoother.
Dermal fillers add volume to the soft tissue beneath the skin with non-toxic, gel-like substances designed to fill in the hollow spaces and thin areas that have been affected by the loss of collagen and other ingredients. Most of the popular, temporary dermal fillers like Juvederm® contain hyaluronic acid – a carbohydrate found naturally in the body’s tissues that is capable of pulling in and holding onto large quantities of water to lubricate tissues.
What Types of Wrinkles Do They Address?
Not all wrinkles are the same. Some of the main types include dynamic expression wrinkles, permanent elastic creases, compression wrinkles, and gravitational folds. Botox® is the gold standard for making dynamic wrinkles disappear. These are ones that result from a history of repeated muscle movements in the face and appear when you make certain facial expressions. Older skin is no longer as springy as it once was and cannot return to its original position as easily, so the overlying skin bunches together. Most of the time, this medicine is used on the lines that occur on the upper section of the face, like horizontal forehead lines, elevens between the eyebrows, and crow’s feet.
On the other hand, dermal fillers are meant for any wrinkles that are not caused by movement, i.e. static wrinkles and gravitational folds. Many filler brands like Juvederm® consist of a range of different products that are designed with unique purposes. Fillers usually go on the parts of the face that Botox® isn’t generally meant for – like the middle and lower thirds including the cheeks, mouth area, and lips.
Juvederm® decreases the depth of moderate to severe wrinkles in these areas like nasolabial folds, and also finetunes the more delicate skin on lip lines. One version of it, Voluma®, augments the cheeks to make them rounder, more youthful, or more defined, depending on what the patient wants.
