I come from a country where healthcare is free at the point of service and after my recent experience in Florida, I’ve never been more grateful.

I’ve been holidaying (vacationing) in Florida for a number of years now, in fact my husband and I have visited around 12 times over the past 13 years (sometimes more than once a year). I’m not unfamiliar with the climate and always have an array of potions, lotions and tablets to treat most ailments. However, I was totally unprepared for what would happen on this trip.

Having flown into Miami from the UK especially for the David Cook gig, we made our way over to Ft. Lauderdale to meet up with some friends who had travelled down from Orlando to join us for their first DC experience. It was dark and very rainy when we arrived so after a spot of dinner we all retired for the night. The following morning (gig day) was gorgeous, the sun was out, it was a beautiful clear sky so we all slathered on the factor 30 and made for the beach.

That night as I stepped out of the shower my legs, and in particular my feet, were neon. I’ve never seen sunburn like it. I couldn’t understand it. I’d reapplied several times during our beach session, with my arms and chest, that had been subjected to the same meticulous routine, being a little pink but of no real concern.

Fast forward 48 hours we are sat in the waiting room of a walk in clinic in Orlando waiting to see a doctor. We’d travelled back up north to spend a few days with our friends, but on the way my right foot had started to swell and blister, then my left and soon I could barely walk.

Concert and Orlando leg of our trip totally ruined I had no choice but to seek medical help so a friend drove me to a nearby walk in centre (I won’t say exactly which one, but I will say it’s near a popular tourist attraction!).

I have to admit I didn’t feel like I was sitting in the waiting room for too long and the front desk staff were polite enough, though perhaps lacking in empathy. It was when I stepped through the doors to the consultation area my issues started. I was ushered into a corner cubicle with no further instructions other than to lie on the bed, not unsurprising given my ailment, and the curtain was drawn.

In the cubicle to my right was a little boy and his mum both suffering with an ear infection and to my left another woman from the UK suffering from Hives, I could hear every word of their consultations.

I waited about 30 minutes for the doctor to come in, take one look at my feet from the other side of the cubicle, tell me what I already knew (I had second degree burns) and lecture me about “my kind” not understanding the power of the sun. (Don’t worry I set him straight on that one!)

It must have been another 20 minutes or so before the nurse arrived with my prescription, in the interim period a number of other nurses, popped their heads around the curtain to pass judgment on me, some offering sympathy most shaking their heads and saying “should’ve worn sunscreen”. This of course resulted in me telling my final visitor, a second doctor, who’d been sent by the first not for another opinion but just to look, that I was “not a freak show!”

About 90 minutes after arriving, and with no sign of any forthcoming information, I hobbled off the gurney to the nurses station with my prescription to ask if I could go, aware my poor friend had to work in the morning, only to be greeted with a look of disdain for daring to ask. Thankfully I only had to sign for my drugs and pay. Oh yes pay. It cost $336 for a doctor to stand across a room from me, prescribe me anti-inflammatory drugs and tell me to pop the blisters after three days!

I left the centre feeling humiliated and while I’m not against paying for healthcare I do think you should get what you pay for. Back in the UK if I’d presented with the same symptoms a doctor would have put some gloves on, had a look and, if he thought the blisters needed popping would more than likely have done it there and then and dressed the wound appropriately. What’s more it would be free! I love the US but somebody needs to look at the healthcare and reversing Obamacare? Big mistake!

As for me, well 36 hours after my visit to the clinic my feet are still blistered and swollen, I’m still struggling to walk and feeling very sorry for myself. But I’m desperately trying to salvage something from the trip as we head down to Miami tomorrow to close out our vacation.

It’s been an experience but one I hope I never have to repeat. Factor 50+ sunblock all the way for my tootsies from now on.

YJ: What does conscious and inclusive wellness mean to you?

Mia Caine: Wellness shouldn’t look one way. What I do in my day-to-day that makes me feel healthy and well is my choice. For each of us, the things we do to put our best foot forward are different. So much of wellness is based on what you need to buy: a $200 product or a $20 green juice. Wellness is centered on those with privilege and a certain amount of expendable income who can afford to spend the money to be well. But wellness can come from the small habits in your everyday life that can influence the rest of your life; the little things like meditating. I write in a journal every morning and night, and that's really helped my goal setting and keeping up with my intentions. There are just so many ways to be well beyond buying product after product. When I started blogging, I felt this pressure to go to all these boutique fitness places; that I needed to be wearing a certain type of athleisure wear. But when you do the math and add up the dollars it just doesn't make sense. Financial health is an important part of living a healthy life, and it’s unnecessary to spend all this money just for the appearance of being healthy and well. You can’t put a price tag on real wellness.

On Being Lit: A Personal Essay on Mental Illness, Homelessness, and Compassion

Theo Karantsalis of High Times Reports:

A former librarian shares his experiences with schizophrenia, bipolar disorder, and living with a fragmented mind.

This essay has been re-published with the permission of the author.

The fine line between sanity and madness blurs as a jittery librarian in downtown Miami with body sores, unkempt hair, and tattered clothes gives fiery sidewalk lectures.

Onlookers who recognize the senior librarian shake their heads as they toss a dollar or a get-well note into a tip jar with the hashtag #litwithfire scrawled on it. As the 57-year-old hippie throwback from San Francisco rambles on about spiritual warfare, chem trails, and legalizing weed, a struggle between rational and irrational thought plays out on the public stage.

I am Theo Karantsalis, a longtime college library administrator who has suffered from serious mental illness since childhood, or for about 50 years. This includes multiple suicide attempts, drug addiction, and bizarre behavior resulting in police standoffs, countless trips to jail, dozens of lawsuits, restraining orders, and lots of psychiatric intervention.

A crumpled doctor’s note in my pocket is a reminder to take medicine which includes immune system injections, large doses of anti-psychotics, anti-depressants, and mood stabilizers. It reads: “chronic mental illness, poor coping skills, evidence of paranoid and persecutory delusions, most recent episode manic, severe, with psychotic features.”

Over the years, a team of doctors have helped keep me afloat with schizoaffective disorder bipolar I type, or a combination of schizophrenia and bipolar disorder.

Regardless of how consistent one is with treatment, there are risks of unexpectedly going off the rails, as I did earlier this year. This left coworkers in our tight-knit College community scratching their heads and whispering.

When I requested emergency leave last April, the official reason was something valid and visible: psoriatic arthritis, which has left me covered head to toe with skin lesions that make it hard to sit, stand, or walk. If need be, I was ready to stack the request with multiple sclerosis, a condition that for twenty years has given me a neurological golf umbrella to hide any mental misfires.

One thing that I could never admit was that I had been sucked into a psychotic black hole. I recklessly spent thousands on Rolex watches, passed out $100 bills to strangers, filed a bizarre lawsuit against the College in federal court, chose to sleep on the streets, and recently fired two psychiatrists for being spies for the CIA.

While in a psychotic state, one often has poor insight or an ability to perceive that he or she is ill.

Years ago, when I started my librarian career with Miami-Dade County, I looked the other way as homeless bathed and shaved in our restroom sinks or violated other minor rules like sleeping or eating. Deep down, I knew that I was just one psych ward visit away from joining them. And I did.

Ending up on the streets outside the main library – disheveled, delusional and with Diogenic indifference – I saw first-hand what it was like to panhandle near the metro rail station, watch the sun rise from a decrepit alley, wait for handouts of day-old muffins and coffee, smoke discarded cigarette butts, and feel the disdain some locals have for the downtrodden.

At the College, the mentally ill make up a slice of the folks we serve, and many colleagues have confided in me that they too suffer, albeit quietly. And for good reason, as words like “schizophrenia” or “bipolar” conjure up fear and the related stigma might affect a promotion or a career.

There were signs in the months leading up to my break that things might be amiss like handing out psychedelic business cards and custom bookmarks that detailed my extensive medical and drug use history, including LSD, cocaine and meth. I also wore the same wrinkled and stained clothes for weeks on end, quickly lost about 75 pounds, stopped shaving or combing my hair, and often shed clothes and spoke to wildlife by the lake.

While in a psychotic state, one often has poor insight or an ability to perceive that he or she is ill.

And even if I did have a sliver of insight, who might I have reached out to at the College about being under attack by interdimensional demons? Or that meetings were a waste of time, as we should just send mind messages back and forth via ESP? Or that out-of-tune foreign radio stations in my mind scrambled my thoughts and words jumped from my computer screen onto the desk and scattered into the walls?

Perhaps it is time for the College to address mental illness from within the ranks so we can better understand and help each other, as well as those with similar issues seeking our services.

As I wind down my wonderful 15-year journey at the College, I leave you with a simple, best-life practice that has helped me deal with police, jail, court, and living on the streets.

The reason those of us suffering from schizophrenia and bipolar may appear distant, aloof, or otherwise detached is because our minds tend to run on different operating systems. Though I now have flickers of clarity and reason, my thoughts and speech remain fragmented and disorganized, drowned out by noises and visions from another dimension.

We live in another world, at times a wondrous and magnificent world, but one that is often disconnected from your version of the world. We remain somewhat tethered to Earth in various degrees, some with a fat rope, and others, like me, with a tiny thread. And the library is the magnet that instinctively pulls us as we seek direction, meaning and purpose.

As I glance down from another galaxy, like Major Tom floating in a tin can, the signal bars waft in and out of service and I wonder what life will be like when, and if, I eventually land.

But I think my spaceship knows which way to go.

Miami’s antiquated permitting system gets a reboot, but developers remain wary

Being a developer can take a good amount of patience, but in Miami that virtue has long been put to the test.

Initial approval for a commercial or residential project can take between six months to a year. Physical plans are carried from one city department to the next, and any changes in an application can lead to more submissions and more delay. Once the initial step is secure, a developer usually can wait up to a year for a building permit.

Add on to that the roughly two years — on average — that it takes a project to be built, and it can take more than four years to complete a development.

“With the speed at which the market changes, that’s a really long timeframe,” said attorney Javier Aviñó, a partner at Bilzin Sumberg, and part of its land development and government relations practice.

“If you discourage developers from wanting to do projects in Miami, you’re really hurting your neighbors, the tax base, the new schools down the block,” added developer Avra Jain.

But Miami’s antiquated permit system just got a high-tech update, which city officials say will streamline the process and cut the wait time. Developers and other industry players remain wary, saying other problems persist.

Rolled out in December, the new permit system, Electronic Plan Review, recorded about 800 applications for new projects with the building department in the first month. Applicants can get feedback from city staff within two hours of submitting plans, compared to two weeks, the city said.

Multiple city departments can also review the same project simultaneously through Electronic Plan Review. Previously, a single set of plans would have to be walked to the different departments. Corrected plans can also be re-submitted electronically, another time-saver, the city says.

The system — by Arizona-based Avolve Software Corp. — has already been installed in smaller municipalities like Tamarac and Wellington. Avolve claims “100 cities, counties and states” use the system, according to its website.

Miami’s goal is not to speed up construction, but to reduce the turnaround time from when a developer or builder submits their application to when the developer receives a permit, according to the city’s building department director, Jose Camero.

Using the new technology, developer Melo Group got through its first round of reviews within six weeks — something Camero said would have taken “a good four to six months” before the city went digital.

Kemarr Brown — the city enterprise program manager who is in charge of implementing ePlan — said the focus now is training developers, contractors and owners on how to use ePlan and working out any kinks.

Overall, the city has spent roughly $7 million overhauling the building department, about $1 million of which went into a physical upgrade of its offices at the administrative building along the Miami River. The majority was spent on software and hardware, with funds also going toward IT consultants, professional services, licensing and permitting, and construction, Brown said.

The overhaul comes amid what a dozen developers, contractors, brokers and architects interviewed for this story agree remains a painstakingly slow permitting and approval process.

The data largely backs their claim.

Between 2014 and 2018, wait time for permits rose dramatically in Miami in all categories, according to a Real Deal analysis of city data.

For alteration and remodeling permits, condo projects experienced the biggest increase in waiting times, from 8 days in 2014 to 40 days in 2018. For single-family homes, it went from 43 days to 119 days. For an office remodeling job, the wait went from 35 days to 83 days over the same period. Retail alterations permit wait times — not including restaurants — also rose, from 80 days in 2014 to 103 days in 2018.

As for new construction permits? That category also experienced a slowdown. It took developers a combined 2,164 days to get their permits for new construction in 2018. That’s double the number of days it took in 2014, according to TRD’s analysis.

The amount of time it took developers to secure the necessary permits for new condominium projects jumped to 645 days last year, from 268 days in 2014, according to TRD’s analysis. The data does not include larger, more complex projects that often go through a separate phased permitting process.

Most of the industry players who spoke for this story did so on condition their names be withheld for fear of antagonizing city officials. One such developer said permit applications often end up “languishing on people’s desks.”

And one attorney who works with developers added: “A small project — as small as a single-family home — can take just as long as a high-rise development and that is crazy.”

But improvements can also be seen.

In mid-December, Melo Group filed a building permit for a 1,042-unit apartment tower at 501 North Miami Avenue in downtown Miami. By the end of January, all of the necessary city departments had reviewed the 55-story project and provided comments. The Melo Group plans to break ground by 2020, but will likely get its building permit sooner, ahead of expectations.

Still, developers say the city has not addressed a glaring issue: staffing.

“At the end of the day, there have to be enough people to review the plans. There isn’t sufficient human capacity to accomodate the volume of work that is being put through,” Aviñó said. “The city needs to hire more people that have the skillset to be able to process these things effectively and efficiently.”

After the 2008 financial crisis, some municipalities in South Florida were forced to lay off employees, in some cases outsourcing their building department responsibilities. Over the past decade, those communities of Deerfield Beach, Weston, Pembroke Pines and Lauderdale-by-the-Sea hired private companies to run their inspection and permitting processes.

In the city of Miami, the building department had 61 employees in 2007. That shrunk to 55 by 2012 then rose to 91 as of this January, according to data provided by the city.

When the market peaks, “we try to bring in consultants to bridge the gap” and to avoid having to lay off employees, Camero said. “We had a substantial number of consultants in 2007.”

Around the same time, the city was also facing its share of financial issues. The city and its former budget director, Michael Boudreaux, were found guilty by the Securities and Exchange Commission of fraud. Boudreaux essentially “cooked the books,” orchestrating transfers of millions of dollars from the city’s capital improvement fund to its general fund to cover up budget deficits in the general fund, according to the SEC.

In 2010 and 2011, Miami was forced to lay off staff, just when construction cranes began popping back up. Development has boomed ever since, including massive projects like the $1 billion Brickell City Centre, along with condo towers like Aria on the Bay, Biscayne Beach, Echo Brickell, and 1010 Brickell, and many others.

Camero said the building department now has a number of open positions, including for building, plumbing and electrical inspectors, as well as supporting staff. For now, it is again using consultants, to help bridge that gap between ePlan and projects still using Miami’s old system.

For rescue and relief workers, the Ultimate Service TITAN can serve as a place to find shelter, food and a much-needed place to rest between shifts, thanks to the temperature-controlled box that features a resting area, stove, microwave, sink and refrigerator.

In conjunction with Red Cross Blood Drive efforts, the Ultimate Service TITAN could also serve as a place to give or collect blood donations from the field.

After a disaster strikes, roads can be difficult to pass from washouts, mud and debris blocking roadways. The ICON lift kit on the Ultimate Service TITAN includes adjustable- height coil-over shock absorbers tuned specifically for TITAN XD, custom rod end bearings and patent-pending Delta Joints – helping provide increased ground clearance and enhanced ride quality both on- and off-road.

Body armor to help protect the vehicle from the elements comes via an Addictive Desert Designs Stealth Bumper. A WARN ZEON Platinum 12S winch was added for debris removal, rescue and recovery efforts and to help ensure the Ultimate Service TITAN can maneuver through many difficult situations. ICON Compression wheels and Nitto Ridge Grappler tires help assure this one-of-a-kind service vehicle will get through the challenging road conditions often seen during natural disasters.

To avoid a fuel shortage – a common concern in disaster-stricken areas – the Ultimate Service TITAN is equipped with a custom TITAN Fuel Tank that holds up to 75 gallons to serve for long periods of time without interruption.

From there, focus shifted to the bed – a crucial piece of the service vehicle puzzle. To provide shelter and privacy for patients receiving care and rescue workers, a B&S Customizing Lightweight Bed Box, complete with a Rockit West custom wrap, serves as the backbone of this mobile command center. Inside the bed box is a Goose Gear Custom Interior, carefully crafted with the Red Cross and its volunteers in mind.

Inside the bed box is where the Ultimate Service TITAN really earns its name. Unique storage for first aid supplies, a desk, a couch, a mobile kitchen, a generator, solar panels, Wi-Fi and a wrap-around awning are just a few of the features that allow the vehicle to transform to meet first responder needs.

Nissan Ultimate Service Titan Deploys In Miami, Ready To Rescue

It’s a purpose-built mobile command center and first aid unit.

Nissan’s history of making one-off Titan pickup trucks continues with the Nissan Ultimate Service Titan. The Japanese automaker designed the truck with the American Red Cross in mind, making the pickup a mobile command center and first aid unit for when disaster strikes. And it looks striking.

“We hope this Ultimate Service Titan, created in lockstep with the specific needs of the American Red Cross, will help them continue to be a great example of how a Titan of the Community can truly make a difference,” said Jeremy Tucker, vice president, Marketing Communications and Media, Nissan North America.

Nissan based the custom truck on its Titan XD Diesel Midnight Edition Crew Cab, using its heavy-duty commercial-grade ladder frame to support the temperature-controlled B&S Customizing Lightweight Bed Box. The box, which has a Rockit West custom wrap, houses a mobile command center and a host of creature comforts needed in an emergency. Inside, there’s a Goose Gear Custom Interior that features a desk, couch, mobile kitchen – stove, sink, and microwave, a generator, solar panels, a bed, and a Wi-Fi hotspot. There’s also storage for first aid supplies and a SnoMaster refrigerator for blood storage.

Outside, the Ultimate Service Titan gets several features to add to its rugged appearance and off-road capability. There’s an Addictive Desert Designs Stealth bumper, Warn Zeon Platinum 12S winch, Icon Compression wheels wrapped in Nitto Ridge Grappler tires, Baja Designs auxiliary lights, three-inch Icon suspension lift kit, and a 75-gallon custom fuel tank. This isn’t the first partnership between Nissan and the American Red Cross. In the last five years, Nissan has raised $2.5 million in donations for the charity. The Nissan Ultimate Service Titan makes its debut at the Miami Auto Show, Oct. 5-13, at the Miami Beach Convention Center. Nissan will donate the truck to the American Red Cross in South Florida after the show ends.
