Anthony’s Stupid Daily Blog (1136): Thu 8th May 2025
At long last I finally had an appointment at The Spire to discuss the results of my MRI and possible treatment just five months after I originally had the scan done and four months after I initially requested the appointment. I’ve always wanted to create a comic book character and while I’m aware the idea of a superhero who becomes more powerful the angrier he gets has already been done I would like to create one who becomes more powerful the more fuckwits he encounters. Any time he wanted to transform he could just go on X and type in the phrase “Alex Belfield Hurty Words” and the comments made by his fans would supply him with enough power to get him through any battle. Anywho I was sat in the waiting room for forty minutes and had to endure the noise of SunFm. I can’t understand why every business seems insistent on playing music, presumably reasoning that they don’t want to sit in silence, read and/or listen to their own music on headphones. This hospital must be getting sponsored by SunFm and have to force their patients to listen to it in the waiting room since they need the money. Finally I got called in to see the doctor, fully expecting there to have been some further fuckup like instead of The Spire the company I booked the MRI through sent the results to a Sprite factory or something. Luck for me the scans had finally come through and the doctor gave them a once over and told me that the discs were in good shape. I should have asked “SO WHY THE FUCK DOES IT HURT THEN?” because every time I move my neck from side to side of feels like there’s something poking out from my shoulder blades and sometimes it feels like I’ve been stabbed in the middle of the spine. I asked him if there was a chance that the bulging disc could herniate and he said yes but there’s no way of knowing when this would happen. So basically if the fucker does herniate at some stage there’s going to be fuck all I can do because if I go to the doctor and say it hurts even more she’s just going to look at her notes, see that the surgeon at The Spire says there’s nothing wrong with me and just send me to MSK for more fucking useless stretches. Luckily he’s agreed to give me some steroid injections which will hopefully relieve the pressure on my spine and around the disc. Obviously I’m hoping it works but I have a feeling it will have about the same effect as putting one of Luna’s Paw Patrol plasters on it. If it has no effect whatsoever hopefully I’ll be able to go back to the Spire, tell the doctor that there’s been no improvement and then he’ll refer me for surgery.

One week post diagnosis, this is what I know so far:
1. The nodules in my chest that were revealed in my routine chest CT scan are positive for cancer
2. The pathology shows they have the same profile as the breast cancer I was initially diagnosed with four years ago: hormone positive, HER2 negative. This is good news because it is highly treatable and slow growing compared to other breast cancers.
3. The PET scan showed that the cancer has also spread to my liver and bones (ribs, spine) and the lymph nodes in my chest. But the lesions are tiny because they were detected early and treatment should be able to prevent further growth or spread.
4. The best news I got this week was that my brain MRI came back completely clear. Metastasis to the brain is a whole different world of treatment.
5. No chemo needed! I never want to go through it again so I am eternally grateful that my treatment this time around doesn't include it.
This is the plan for next week (I am planning to stay in Rochester MN for the next week to get this done at Mayo:
6. A cervical spine MRI to see the extent of growth in the spine and determine if I need localized radiation
7. An oophorectomy (getting my ovaries removed) because the cancer is strongly hormone positive and this is an effective first line of defense.
8. Starting endocrine therapy: oral drugs to remove any hormones in my system
9. Starting targeted therapy (Ribociclib) to prevent cancer growth. This drug wasn't even around four years ago which shows how fast medicine is progressing.
Mayo is an amazing place to get treated: the organization and efficiency of patient care is unlike anything I have experienced before at MSK, Mt Sinai or Hartford Healthcare. Appointments are scheduled back to back with minimal waiting, tests are run and results obtained within a day at most. Every tiny detail down to the design of the patient gowns is thought through.

I'm not dead...
Which should be obvious from the last few weeks...that being said as there are people on here I actually know in real life for whom I have dropped off the face of the planet.
Hi. I'm back.
Put a cut 'cause those who don't actually know me probably aren't going to give a shit as to what the hell happend.
Three years ago my Mom got diagnosed with lung cancer and my life went off into a total spiral. I basically lived out of my office for the first year and a half so that I had time to spend the rest of it at MSK at Mahattan or Nassau depending on what treatment was going on at any given day.
The last year has been a careful balance of good months (weeks...days) and bad ones - though we seem to be in an upswing currently. February 2022 - 1st surgery. May 2023 - "declared remission" after 3 scans over 18 months. December 2023 - requested for re-scans 'cause of fevers and night sweats starting again. April 2024 - finally agreed to re-scan. same cancer back; but in lymph nodes instead this time. May 2024 - October 2024 - monthly chemo that worked for the first 3 months (after a scan in August) and then stopped working for the next 3 months (after a scan in November). December 2023 - February 2024 - different medication (a daily pill at home - which made life infinitly easier) that miraculously did not make her sick and also amazingly in 3 months (based on last months scan) shrank all of her cancer 50% and lowered the "heat index" (ie: how bright it is on the pet scan) from 16 to 2 overall.
So fingers crossed this trend continues.
Professionally (brace yourselves) I've been working in finance adjacent. Sort of. The firm I work for gets hired by companies to run their shareholder meetings and my team run the communication center for direct contact with shareholders among other things.
My elderly inner punk is currently not on speaking terms with me and regularly accuses me of becoming a corporate shill. Long Island is expensive. And, hey, technically get to say that I work for an international corporate communications firm since we have Canadian offices. (Which is going to be interesting in the coming weeks. Fuck.) Also, somewhere in Oneonta a few of my professors - who weren't too impressed with me on my more hungover days - are falling over in shock.
So yea, that's what's up.
I have skimmed through here (and instagram) every once in a while just to see if I could somewhat keep up with people - as Twitter (...I've had an account since that website opened I am not calling it 'X') and has become immeasurably toxic so don't expect me back on that anytime soon.

good evening Mimi! may I ask a medicine question (definition is probably more like it)? I’m also struggling through One Piece (honestly, “inspired” by you, but I’m a solid 100 episodes behind you 😬)
in episode 144, a derelict ship falls from the sky. Robin then restores a skull that has two holes in the back. she says it’s “craterization” and Chopper confirms it. however, even with his brief explanation, I don’t really understand it. when I translate that term to myself, it brings me to “cauterization”. the only thing I can find for “craterization” itself in English is “to cut out a piece of bone”.
Unfortunately, I can’t remember if you wrote anything about this term in your watch through (I know you mentioned scurvy, though). did you have the term in school perhaps? can you say a bit about it? thank you very much!
tbh usually whenever a chunk of bone is removed from the skull it's either termed craniotomy or craniectomy or just like vaguely resection, i've never heard craterization used like colloquially in medicine but a google search does say craterization is removing a piece of bone like you said.
(there are a million different terms just for removing bone which is why despite being out of residency and reading thousands of neuro and msk scans i still find out about new procedures and terms all the time)
the way craterization is described is more like a 'debridement' because it seems like it's for osteomyelitis but i think this is more of an ortho term and ortho tends to not deal with things above the neck
interestingly enough, it's something i could expect an archaeologist to use as terminology as opposed to a physician (not needing to be THAT discerning) and it's very in line with chopper agreeing, knowing what she means in technicality (doctoring involves a lot of knowing what the term is specifically by specialty in and out of medicine so you tend to like have a million different terms for the same thing)
that being said i do remember there were a couple of things chopper said that i was like hm... not medicine but also it's anime and he is also a talking reindeer so i think it's passable
this feels like a super long reply to your question but yeah
*** another note! i am american and sometimes terminology will be used differently depending on where you trained in medicine so maybe craterization is a more common term in the uk or canada! i just don't know personally and do not use it in my daily lingo!

Anthony's Stupid Daily Blog (1056): Tue 18th Feb 2025
Off to the town for a massage this morning but before I went in I dropped off some DVD’s at the charity shop. I’ll be honest I’m surprised at the sheer amount of movies that I’ve deemed unworthy of staying in my collection. At one point I must have actually deemed Night at the Museum and Leap year entertaining enough to take up space in my house but upon rewatching them I’ve discovered them to actually be absolute dogshit. I’m happy that these DVDs will now go towards raising money for the British Heart Foundation but I still can’t back all the time I’ve wasted watching these train wreck movies. I popped in to the massage place and because I’ve been here a few times before I didn’t need to sign all their tedious paperwork because they already had my details on record. I told the massage lady that my MRI had shown no problems in the upper spine and that I suspected the problem might be in my shoulders and asked her to mainly focus on them. The whole thing was quite painful but in an enjoyable way as she was digging her thumb right under my shoulder blades and at certain points they were making such a cracking noise I was scared they were going to break apart like a Christmas cracker. The only thing I hate about massages is the whole where you put your face as it’s really uncomfortable and the towel they line it with doesn’t help. Why can’t they make a ring out of the stuff they make neck pillows out of and stick that in the hole instead? Or at least give you some heroine so you won’t notice the discomfort in your forehead? I’ll be honest it seems as though the massage made fuck all difference as I moved my neck to the side when she was done and it was just as uncomfortable as before but I enjoyed it while it was happening.
I decided to get a taxi to the Spire for my appointment to discuss my MRI results because there was only an hour and a half to go and in the past I’ve made the mistake of relying on the buses of Sunderland to be relatively on time and it’s ended up fucking up my plans quite a bit. I walked up to the desk at reception and told her I had an appointment at the o’clock but she told me they didn’t have my appointment on record. She asked me to spell out my name so she could check again and she said that I did originally have an appointment but that it was cancelled and my GP should have told me. I told her that I rang up my GP literally not half an hour ago to confirm if they’d sent my MRI results to them and hey confirmed they had and at no point did they say “oh no your appointment has been cancelled”. This bitch behind the counter just shrugged and said I would need to talk to my GP. I scrunched up the appointment letter and threw it on the floor as I walked out realising I’d spent twenty quid on a taxi for no fucking reason thanks to NHS incompetence yet again. When I got home after an hour in the cunting bus I rang up my GP and asked her what the fuck was going on. She asked if I’d received a text asking if my referral had been rejected which I had but I then received a letter a week later telling me that I had an appointment so I figured what must’ve happened is some fuckwit rejected the referral but then someone with common sense looked it over and decided they shouldn’t have rejected it and gave me an appointment. According to the receptionist the opposite happened i.e someone with common sense agreed to the appointment and then some fuckwit decided they shouldn’t have and cancelled it. Unfortunately they didn’t tell me that it was the appointment that had been cancelled rather than the referral. Anywho to wrap things up I need to go through MSK again even though the whole reason for getting a private MRI scan is because MSK kept fobbing me off with stretches. So this receptionist said she’d set up an appointment with them which should be happening some time next month at least I hope it will I’ll probably get there and find out MSK shut down and that building has been a McDonalds for the last six months.
