My Upcoming Research Study: The Effects of Yogic Breathing on Chronic Sinus Symptoms
This week I have been putting together a presentation that I will be giving about my new research project. It's a study of a yogic breathing technique called Bhramari Pranayama as an adjunctive treatment for people with chronic nose and sinus issues.
Essentially the premise is this:
Our sinuses produce nitric oxide, which then in turn effects our nasal airway and our lungs. The nitric oxide can improve nasal airflow, up-regulate mucociliary clearance, and enhance anti-viral immune activity. Additionally, humming actually temporarily increases the amount of nasal nitric oxide released by about 15 fold. Therefore my study is intended to examine the effects of this pranayama technique that utilizes humming on patients with chronic nose and sinus symptoms.
Typically for patients with these symptoms, we start them out on a steroid nasal spray (flonase) as well as sinus irrigations (something like a neti pot).
I always hear from people outside the medical field about how no one studies these things--non-pharmacologic interventions, the beneficial effects of non-proprietary supplements, or other alternative medical options. People often think that if big pharma (or someone expecting to profit) isn't paying for a study, it cannot happen.
This really isn't true likely 90-99% of the time. The problem isn't funding. I'm conducting a prospective randomized control study with human subjects in order to evaluate the benefits of yogic breathing for patients--something that if found to be helpful will bring in no additional monetary profit for anyone. How much will my study cost? $0.
But do you know what it did require?
Two things: interest and opportunity.
Firstly I, a resident physician, had an idea. I learned about nasal nitric oxide and thought it was cool. I read about how humming has a bolusing effect by transiently increasing nitric oxide output by 15x. And then I recalled that there are pranayama techniques that utilize humming.
With my interest piqued, I spoke with one of my bosses, an attending physician at an academic medical center. He's the head of our Rhinology and Skull Base Neurosurgery division and he is cool as hell. He's all about healthy lifestyle and benefiting patients as much as possible. He loved the idea immediately.
And lastly we roped in a medical student. Med students are very helpful with doing the grunt work of collecting the data into spreadsheets, running the statistical analyses and such. Sometimes they bring some excellent ideas of their own as well. In return for their work, med students are often given a significant portion credit upon publication of the study and this allows them the opportunity to add some scholarly publications to their CV. I don't really need more publications under my name, but they do.
My point with sharing all of this is that people often claim there are health benefits to doing or imbibing certain things but that they'll never be studied because there's no money to be made. And it may be true that private companies such as those in the pharmaceutical industry may not have such interest; their existence in a capitalistic economy relies on profitability. But this is part of why academic institutions are so important--because learning and discovery is part of the essential mission there. Profit doesn't dictate their avenues of research.
When it comes to the study and validation of alternative/complimentary medicine, the focus really needs to be on raising awareness and interest. Talk to your doctors, nurses, physician assistants, etc. The good ones listen. The younger they are, the more likely they are to be open-minded about it too (the older ones are hit or miss--some are so cool and some are very old school).

Alright, imagine this.
I'll use whole cloth new material as an example, so this won't be an allegory to any one specific religious group, but will involve religious culture parallels.
Imagine some religion exists that is absolutely teatottle. Ideologically, they argue, no chemicals that enter the body and could be construed to be used as a drug are acceptable. Vitamins and minerals and proteins are fine, but imbibing substances to numb pain or cover up mental disorders are unacceptable. Only institutionalization, surgery and talk therapy are acceptable. They have decided, no use of drugs is acceptable, unless it's something like an antibiotic or antibacterial. Getting high bad.
And it doesn't matter how wrong they are. You can point to the clinical, objective, empirical proof that medicine is good and worth the risks, we just need better systems in place to manage those with crippling addiction and help prevent overdoses or dependences.
But ideologically that does not abide by their ideal world, where no one has access to drugs and no one can abuse them because no one has access to them. Their ideal world, no one is able to overdose, because no one can access drugs.
People disagree. But those that hate drugs want them gone, and it becomes a constant vigilance to push them back so they DON'T get their way. They are not even okay with Over the Counter drugs in supermarkets; To use so much as a headache medicine, you should need approval by a doctor and the entire pharmacological industry, or else no pain relief or cure for constipation via medicine for you. To them, any drug use that isn't prescribed is a crime, and should be considered a devious violation of the system worth federal jailtime.
So they start a heavy handed, emotionally charged, ideologically driven conversation about the need for, "common sense drug reform," and "miniscule reimaginings of drug access," that all but tries to whittle away what you're allowed to access, how much, how often.
Ideally, the anti-drug users would have it become something you're federally jailable for if you use anything stronger than a Halls, Vicks or Ricola cough lozenge, a certain number over both a period of hours, days and weeks.
In order to legally be permissed to access medicine, you'd need A.) to schedule a doctor's visit or go to tthe ER. B.) To be confirmed as NEEDING that medicine by the doctor, dependent entirely on if the doctor felt you needed or deserved that antihistamine or migraine relief medicine. C.) Receive your prescription, then head to the pharmacy. It gets entered into a big federal record. You get it marked that you received X many pills of Y type with Z active ingredients on the date. This is permanent and is used to correlate any mysterious patterns that may involve you or your product in them.
You can't so much as take just a little bit more cough medicine than the minimal dose reccommended by the people that don't want you to have that medicine at all, politically, in the name of public health and safety.
And every single time someone is caught abusing drugs, even drugs unrelated to you and your ailment, there's cries for change, security, mourning our, "toxically masculine" society and its "drug abuse culture," and every fucking asshole raising a ruckus on cocaine, meth or heroin becomes another nail in the coffin to legitimate access to drugs.
You now need a license for pain relievers, every single pill you acquire is catalogued on what it is and when, you're put through rigamaroll that's almost as painful as just sitting there suffering and letting the pain pass than actually pursuing help for your condition. Your freedom to just go to the god damned counter and pick up a box of cough medicine, pain reliever for tooth problems, nasal spray, even fucking silverdine ointment for burns, gets compromised and the screws put to it every time some privileged asshole in San Francisco is found dead from an overdose with needles in his arm.
"Why do you even NEED a box of 24 pain tablets? If you have a problem bigger than that, you should just see your doctor."
"Why do you even NEED antihistamines? If your pets are such a detriment to your health and immune system, maybe what's needed is getting rid of the pets."
"Why do you even NEED alcohol? Clearly you'd be better served by a therapist."
And each and every argument you make is countered with pointing to the government to provide something to change your behavior or deny you access to something they don't want you to have.
You shouldn't want to have a box of 50-80 antacids, you should solve your problems with a more thorough examination of your biology and reasses your diet for foods you're sensitive or allergic to, and not eat those.
You shouldn't NEED cotton swabs for your ears; those are bad for you, so they should be controlled and regulated if used for medical purposes.
And again, every time someone illegally goes around this system you virtually have to opt into in order to regulate your behavior, only really making consequences for those that agree to live under it, the wheel turns and the straps tighten until you're limited to only possessing what medicine the pharmacist will give you for notarized consumption in front of the prescribing pharmacist. The number of pills you can have on hand legally goes down. The number of pills you're allowed to acquire in a window of time goes down. If there's ever an abuse that required a supply of consumer level pills, they go looking to see if you've been hoarding and not using your prescripted supplies, because to even use these drugs, they decide you waive your rights to unreasonable search and seizure; these are controlled substances monopolized by the government and society, and society reserves the right to treat your medicine like its property. Even if it's inside your body.
Eventually the anti-drug advocates get their way. No one that is law abiding can legally use medicine except to ake the edge off of excruciating pain, not solve it. They whoop up how they've saved so many lives and prevented so many overdoses by hard drugs, every time they make it harder to buy pepto bismal or antifungal cream. While the criminals that use them to poison people remain poisoning people, their statistics unaffected and the laws and regulations not preventing a single deliberate homicide.
This is how it feels to be a gun rights advocate in the face of people that despise guns. Arguing for the right to medicine and privacy in the face of someone that thinks you'd be better served not having the option for medicine or the discretion to use it yourself, so why not just say 1 pill over the federal limit s tantamount to conspiracy to kill, and give them a negative mark on their permanent record, a massive fine and legal ban on medicine until the legal mea culpa.

According to Banerjee, there are two environmental sustainability aspects to packaging – carbon footprint and waste management and/or recycling. “The industry is balancing both aspects and moving more towards a circular economy by using a mix of materials, formats, manufacturing, and end-of-life solutions.”
Apart from reducing plastic use, how can the industry reduce its overall carbon footprint, such as the sensible use of water, power, inks, and chemicals? Banerjee says sustainability is not limited to material, format, or processing but is holistic. “However, we need to understand and find the right balance. The supply chain also plays an important role, including sourcing and manufacturing. Most companies are leveraging sustainable energy sources, water harvesting, and recycling to the extent possible and limiting the use of inks and chemicals. An example would be digital printing, the CMYK color palette, and so forth.” 
As biodegradable and compostable packaging, renewable materials, and biopolymers have become buzzwords with many companies claiming such solutions, we ask about their feasibility in the long term as well as their cost-effectiveness. Banerjee explains that multiple aspects of feasibility, availability, manufacturing, costs, and sustainability are involved. For instance, most novel solutions currently have limited availability, which restricts their application. When availability increases, the industry will adopt these solutions.
The discussion shifts to packaging recycling and the circular economy, especially in the pharma and food industries. Banerjee sees an urgent need to overcome the thought process of downcycling. “Moreover, many packaging materials and formats used today are recycled laterally and used for the same application. Recycled PET is a good example where it is used for beverage bottles. There are similar examples of using recycled paper for secondary and tertiary packaging,” he said.

Speaking about the pharma industry confronting the sustainability challenge, he says while pharma companies are evaluating several alternatives, in Japan an interesting approach is biopolyethelene blisters. A consortium of four companies – Takeda, Astellas Pharma, Daiichi Sankyo and Eisai is collaboratively working on this development.
Takeda's recent WorldStar award was for a nasal spray device – the innovation of an electromechanical device and packaging for a drug product in a specific therapeutic area. Banerjee, who was present in Bangkok to receive the award, says it addresses the safety requirements for patients using a drug in that therapy. He says packaging design in the pharma industry is directed towards providing patient-centricity, dosage adherence, safety, and regulatory considerations. The new-age drugs of cell, gene and similar therapies have specific needs that are being addressed with a mix of design and technology, leveraging science.
We ask how he envisages the future of the packaging industry in a country like India where there is a surging demand for hygienically packed products after the global Covid pandemic but where the per capita consumption of packaging seems low. According to Banerjee, for the part of the population with a similar range of per capita income as the West, the packaging consumption is similar. “However, emerging economies have a substantial population that has a low per capita income and that skews the consumption numbers. As more sections of the population increase their per capita, the lifestyle evolves and so does the use of packaged products.”
