EZ neemt deel aan de Trinidad and Tobago Manufacturers' Association “Doing business in Suriname” seminar

Het ministerie van Economische Zaken, Ondernemerschap en Technologische Innovatie (EZ) heeft mee gedaan aan het “Doing business in Suriname” seminar dat gehouden werd door Trinidad and Tobago Manufacturers' Association (TTMA). De TTMA is van 28 mei tot en met 1 juni 2024 voor een handelsmissie in Suriname en heeft als doel om business-to-business bijeenkomsten te hebben, zodat zij een productieve samenwerking kunnen hebben en zaken kunnen doen met Suriname. Tijdens dit seminar waren verschillende relevante actoren aanwezig waaronder Directeur Ondernemerschap (DO) van het ministerie EZ, Karishma Mathoera, die ook een van de sprekers was tijdens dit seminar. Het seminar werd gehouden op donderdag 30 mei in de Banquet Hall van hotel Torarica. Tijdens het seminar hebben alle relevante instanties zoals het Surinaams Standaarden Bureau (SSB), Suriname Business Forum (SBF), Belastingdienst Suriname, Maritieme Autoriteit Suriname (MAS) en Ramps Logistics een korte presentatie gegeven over hun instantie. Het doel van de presentatie is om de delegatie van TTMA een bredere horizon te geven over de instanties, zodat er een productieve en goede samenwerking kan ontstaan tussen Trinidad & Tobago (T&T) met Suriname. Onder de sprekers bevond zich de directeur van TTMA, Rudy Rampersad, die sprak over het belang van het CARICOM raamwerk en de relatie met Suriname. De directeur heeft aangegeven dat zij hier zijn met 20 bedrijven van T&T, omdat zij een ongelooflijke kans en potentie zien om zaken te doen met Suriname. Rampersad benadrukt om de geest van samenwerking en verkenning te omarmen en heeft eenieder aangemoedigd om door te gaan met het hebben van zinvolle discussies en het opbouwen van relaties terwijl de vele voordelen van Suriname ontdekt worden. Er wordt onder andere ook op regionaal niveau gewerkt, waarbij de CARICOM Interactieve Marktplaats en Opschortingsprocedure (CIMSuPro) de markttoegangsmogelijkheden binnen de regio zal verbeteren via de Interactieve Marktplaats en ook de opschortingsprocedure (opschorting van het gemeenschappelijk buitentarief) zal vergemakkelijken. De DO- directeur hoopt op de steun van Trinidad te kunnen rekenen bij het volledig implementeren en benutten van dit platform. Verder benadrukt Mathoera dat Suriname bezig is met het implementeren van het Suriname Electronic Single Window (SESW), om de ondernemers beter te faciliteren in de administratieve procedure voor import, export en doorvoer. De DO- directeur heeft vervolgens aangegeven dat deze initiatieven verwelkomd worden. Necrotizing Sialometaplasia of Palate by Radka Cholakova in Open Access Journal of Biogeneric Science and Research

Abstract

Necrotizing sialometaplasia (NS) is a rare, benign, inflammatory condition, occasionally with ulcers, which is self-limiting and affects mainly salivary tissue. Purpose: To present a case of NS associated with a systemic connective tissue disease in a female patient. Material/Methods: A 56-year-old female patient with ulcers of the palate, surrounded by a red halo, for 2 weeks, which did not resolve when treated with topical antiseptics. Clinical and radiological methods, together with pathohistological analysis, were used to make the diagnosis. Immunohistochemical analyses to diagnose patient's systemic disease were performed at a rheumatology clinic. Results: The definitive diagnosis was made based on a pathological examination and the tests performed in the rheumatology clinic, which found that this was a case of necrotizing sialometaplasia of the minor salivary glands and a concomitant systemic connective tissue disease. Conclusions: NS is a rare disease, with an excellent prognosis, without any possible preventive strategies.

Introduction

Necrotizing Sialometaplasia (NS) is a rare, benign, inflammatory condition, sometimes with ulcers, which is self-limiting and affects mainly salivary tissue. This disease is classified as a “tumour-like lesion” in the WHO classification of salivary gland tumours. It was first described in 1973 by Abrams, Melrose and Howell [1], and in the following year, Dunlap and Barker reported five diagnosed cases [2]. This lesion can be confused with a malignant disease, thus resulting in unnecessary radical surgery.

Materials and Methods

A female patient of visible age of 56, corresponding to her calendar age, with complaints of a non-healing “aphthous ulcer”, which appeared 2 weeks earlier. Treatment with antiseptic mouthwashes and topical application of propolis was administered. The aphthous ulcer did not resolve, but the pain subsided over time. The initial examination showed two ulcers with dimensions of about 2 mm, located on either side of the median palatine suture, with a red halo around them. After treatment with SOLCOSERYL (MEDA Pharma GmbH & Co. KG, Germany), the ulcers resolved, but the red spots remained (Figure 1). The patient was referred for a CBCT in order to detect erosions in the palatine bone under the ulcers (Figure 2). The size of the formation did not decrease and, therefore, an excisional biopsy was performed in full thickness of one of the lesions, and then was provided for histopathological examination. The wound was covered with a PRF membrane and healed without complications. Pathological examination showed that there were cords of the lining multilayered squamous non-keratinizing epithelium, deep below it, without atypicality, and with rapid proliferation of granulation tissue, which covered the minor salivary gland ducts, with squamous cell metaplasia. The patient was a moderate smoker (up to 10 cigarettes/day). She had cholelithiasis without clinical manifestations. She had had surgery for a benign neoplasm of the breast several years before. Intermittent complaints of swelling and rash on the upper eyelids, erythematous rash on the nasal dorsum and photosensitivity were present, for which the patient had been admitted to a rheumatology clinic for examination. A systemic connective tissue disease was suspected there, as elevated total AHA, Anti-SS-A and Anti-SSB antibodies with very high titers and low C4 complement level, without significant proteinuria, were found. The dermatological examination showed pathological skin lesions of the nose with erythematous-edematous plaque, 6 cm in diameter. Sjögren's syndrome or systemic lupus erythematosus were suspected in this patient. The pathohistological examination of the minor salivary glands from the lower lip mucosa found that this was not a case of Sjögren's syndrome. Biopsy of the plaque of the nasal lesion was recommended.

Results

The definitive diagnosis was made based on a pathological examination and the tests performed in the rheumatology clinic, which found that this was a case of necrotizing sialometaplasia of the minor salivary glands and a concomitant systemic connective tissue disease.

Discussion

Necrotizing sialometaplasia is extremely rare, accounting for less than 1% of oral lesion biopsies [3]. The mean age of onset is 46 years, and the male:female ratio is 2.7:1 [4]. The disease is prevalent among the Caucasian race, with Caucasian:African American ratio of 5:1, according to Brannon [5]. In most cases (80%), the minor palatine salivary glands are affected. Although rarely, it can occur in the retromolar space, gingiva, lips, tongue, cheeks, and nasal cavity [1,6]. This disease can also affect the large salivary glands in more than 10% of the cases [3]. The etiopathogenesis of necrotizing sialometaplasia is unknown, but it is thought that the lesion develops as a result of previous ischemia in the salivary gland. In experimental models, disruption of the arterial blood supply to the salivary glands in rodents results in a NS-like histopathological picture. This disease is found in patients with sickle cell disease, Buerger's disease and Raynaud's phenomenon, which are all vasculopathies that predispose to ischemia. Other risk factors for the development of NS include: smoking (and alcohol consumption), use of cocaine and anabolic steroids, hot food, fellatio, traumatic vascular injury, and bulimia [7-9]. The synergistic action of NSAIDs and alcohol over a long period of time results in a change in the oral mucosa functions due to suppression of prostaglandin production and reduction in the blood supply to the minor salivary glands, which causes ischemic events [10]. The iatrogenic factors for the development of NS are the use of local infiltration anesthesia with an anesthetic with a higher concentration of correctives, intubation, bronchoscopy, local radiotherapy, as well as surgical procedures in the vicinity of the affected area [3,7,11,12], which, in the case described by us, had not been conducted for a period of more than 6 months. Senapati et al. [13] reported that NS is a manifestation of local vasculitis. It may be associated with other tumours, in particular Warthin's tumour, Abrisokov's tumour, lip cancer, rapidly growing malignant mesenchymal disease and salivary gland tumours. There is also a connection with previous upper respiratory tract diseases (chronic sinusitis and allergies) in the past few weeks. It is possible that ischemia is due to immune complexes, resembling the pathogenesis of erythema multiforme or benign trigeminal sensory neuropathy. In our case, levels of immune complexes in the body were elevated.

Anneroth and Hansen described the following five clinical stages in the development of necrotizing sialometaplasia: infarction, sequestration, ulceration, repair and healing [6]. A subacute variant of this condition was also described in the literature. Histological features are ischemic lobular necrosis of seromucinous glands, with maintenance of intact lobular architecture, despite coagulative necrosis of the mucinous acini. Pale acinar outlines often persist, but the cell nuclei are hypochromatic or absent. Mucin extravasation into the adjacent tissues triggers an inflammatory reaction dominated by histiocytes and granulation tissue. Within the necrotic lobules, the inflammatory component is often minimal, but is usually found in the surrounding tissues. Although squamous metaplasia of ducts and acini is typical (which makes the diagnosis challenging due to its similarity to malignancies), the metaplastic cells have benign nuclear morphology, with minimal pleomorphism or hyperchromatism and few mitotic figures. Nests of squamous epithelium, which usually have smooth contours, occasionally may have an irregular outline.

Pseudoepitheliomatous hyperplasia, where the overlying or adjacent epithelium is markedly hyperplastic, together with extensive ductal metaplasia, may resemble malignant condition of the epithelium, which could be the reason for misdiagnosis and radical ablative surgery. It may be difficult to distinguish NS from squamous cell carcinoma, low-grade mucoepidermoid carcinoma and oncocytic tumours. Specific histopathological characteristics may have some relation to the “age” of the lesion at biopsy. Coagulative necrosis is more common in “new” lesions, whereas fibrosis and squamous metaplasia are typical of “older” lesions. In our case, biopsy was taken nearly 2 weeks after the onset of the first symptoms, and therefore the changes correspond to an “old” lesion. Management of this disease includes monitoring, use of topical antiseptics, and pain control until recovery [3]. In the presence of predisposing factors, their correction is necessary.

The candidate must complete their order of preference in order to apply for Army, Navy, Air Force and OTA. Male candidates are allowed to choose the order of their preference, female candidates can only apply for OTA should make sure they choose OTA as their primary and sole preference.

Selection Procedure

There are three more options for selection after the written test:

Personality Test/Personal Interview This test is performed through the Services Selection Board (SSB). In this test, a panel of officials evaluates all shortlisted candidates for their verbal and non communication skills. Medical Examination of selected candidates. After being shortlisted during the first three rounds, each candidate is examined for medical issues through the Board of Service Medical Officers. This is the final step of the CDS selection process. The objective of this exam is to verify that the selected candidate is able to conform to the physical standards set by UPSC, e.g. candidates selected to join to join the Indian Air Force must have excellent eyesight.

Examination Pattern

The exam pattern for students who want to be within those in the Army, Navy or Air Force or the Officer's Training Academy, the examination pattern follows: General Knowledge The first exam is based about General Knowledge that comprises of 120 questions about History, Geography, National Politics, Current Affairs and General Science etc.

Questions on current national/international events and matters of everyday life are generally asked in this section. A few general questions about History as well as Geography of India are also included in this test paper. English Second test paper in English.

The test tests the level of your English proficiency, particularly your grammar skills. Het doel van de training is om de exporteurs en potentiële exporteurs bewust te maken van de verpakking en etikettering vereisten bij het exporteren naar de Europese Unie (EU). De training is online via Zoom van start gegaan op 26 maart en was verdeeld over drie (3) dagen. Op vrijdag 5 april 2024 hebben 22 participanten een certificaat van deelname ontvangen. Hierna werden hun producten tentoongesteld en ten verkoop aangeboden op het terrein van het ministerie van EZ. Tanwir Hassankhan, waarnemend directeur van de SSB, heeft aangegeven dat er tijdens de training gefocust wordt over wat verpakking en etikettering inhoud en wat de procedure is om deze conform de standaarden te verwerken. I bought an old Ford 9N last October that was more than a labor of love - although the hood, engine and transmission compartments were laid out, the rest was in 5 gallon oil buckets! I knew this would be the perfect project for my son and I. I bought a service and parts manual from SSB and that Ford became a 6 month learning project for us. We overhauled the engine, reworked the brakes, rewired the electrical system, fixed the cooling system and found original paint and decals, with parts from SSB. It might have been faster to buy a ready-built tractor, but I wouldn't trade that time for anything!

From buckets to showroom quality! I bought an old Ford 9N last October that was more than a labor of love - although the hood, engine and transmission compartments were laid out, the rest was in 5 gallon oil buckets! I knew this would be the perfect project for my son and I. I bought a service and parts manual from SSB and that Ford became a 6 month learning project for us. We overhauled the engine, reworked the brakes, rewired the electrical system, fixed the cooling system and found original paint and decals, with parts from SSB. It might have been faster to buy a ready-built tractor, but I wouldn't trade that time for anything!

How to Join NDA - National Defence Academy

Joining any armed forces is never has been an easy task ever in the world. Because it is a career where very few smart, intelligent and strong people can enter only. And if we talk about Indian defence forces then it is the most challenging task to enter in it. Because Indian defence forces is the most hard, toughest and disciplined army in the world. Here only very hard working and mentally strong and fit youngsters can join only. NDA is one of a path you can say which allows you to enter in Indian defence forces and get a higher rank position officer as well. But it’s not easy to enter in NDA because there are lots of youngsters who apply every year very dedicadly and there is lots of competition between them for just few numbers of seats. So let’s talk about some detailed info about how to join NDA?

What is NDA

NDA (National Defence Academy) is situated in Khadakwasla, Pune, Maharashtra is a joint training defence academy of Indian army, navy and air force where all three trio cadets got the chance to train together and then they go to their respective choice field of army. To enter in National Defence Academy there is NDA exam conducts every year twice for the Indian students. From which our army gets its future high ranked officers. The training of NDA is the most difficult phase of living in the life for the selected cadets. In NDA cadets got the real tough life lessons of every difficult situation which they can phase in war. They have to go through very tough training where they face mental, physical and emotional situations. Many of them are not capable of facing these hard training and they left their NDA training in between also. So youngsters who are not mentally and physically very tough for them NDA is not a good career choice.

NDA Exam

As we read early NDA exam is conducted every year twice times. After passing 12th standard education students can apply for this exam. But many of the youngster’s starts preparing for this exam from very early. Students who want to be successful in this exam should be good enough in subjects like physics, chemistry, mathematics, geography and current affairs or GK. Students who are perfect in these subjects can clear NDA exam some how easily. But clearing NDA exam is just not enough, there are thousands of students who clears this exam very successfully every year but could not qualify after that, because the merit list of this exam is very very high. So there is no chance of attempting too much wrong answers. So students who are not good enough can join coaching classes also. Procedure of selection

After the exam students who clear their exam got the chance to be selected for the SSB (Service Selection Board) round. They do different types and levels of screening tests, physiological tests and interview of the candidate. They provide candidates individual tasks and group tasks also. The SSB round of selection is goes around 3-4 days where the job of SSB team is to know and check about the mental fitness and awareness of the candidate. Clearing SSB round is also a very difficult tasks for the candidates but it also show candidates their level of preparations and mental toughness.

Medical Examination

In medical examination the team of medical board takes different tests of candidate and check whether they are medically fit or not. They do X ray and ultrasound tests of the candidates and checks height, weight, blood pressure, heart beat tests also. Candidates who are fully medically fit got selected for other final round and others who are not physically good enough fit have the chance to be overcome with some treatments after some time of given period.

Merit List

After the so many different phases the final list of candidate is been selected on the basis of merit. In merit lists the candidates who got best high marks in the exam, SSB round and after that medical examinations the final merit list has been prepared. Every year the merit list of NDA exam comes is getting much higher and higher thereafter.
