In an official(?) comic Dream was there and Killer literally had to pinch his cheeks to make sure he was actually there(that sucks)

Same goes with his memories so like if nm or somebody else tells him to do something and be doesn't then they ask him about it- "that was real?"

elaborating on this cuz that is actually one of my fav canon killer detailz

derealization n disassociation (which may i note is a big DID thing) is a big element in his character, often hinting at killer not bein able to tell from what is real and what'z not in comicz and such. he'z constantly "zoning out" or "daydreaming", almost as if half-asleep the entire time.

i like to think that killer will pull a joker and talk about different "endingz" or "storiez" of his life (since there are different endingz to his story) and you have to guess which one is right. they could all be wrong, or could all be correct. think cheshire cat. he'll speak in riddlez and puzzling sentencez just to mess around with you.

anyway back to the reality thing ! in the comic where dream approachez killer, he "checkz" to see if he'z real by pinching his cheekz. he has to be physically in contact with someone to know he isn't dreaming or just imagining this. which further explainz my headcanon that killer is a very physical person ^_^
