Pathological Gait Classification and Segmentation by Processing the Hip Joints Motion Data to Support Mobile Gait Rehabilitation- Crimson Publishers
Pathological Gait Classification and Segmentation by Processing the Hip Joints Motion Data to Support Mobile Gait Rehabilitation by Wasiq Khan in Research in Medical & Engineering Sciences
An accurate detection of the gait sub-phases is fundamental in clinical gait analysis to interpret kinetic and kinematic data. In general, detecting the gait events that mark the transition from one gait sub-phase to another as well as the sequence of sub-phases is essential to evaluate gait abnormalities. However, finding a reliable segmentation for pathological gait has been a challenging task. This manuscript entails a generic approach for the gait segmentation into sub-phases in the CORBYS1 system. A number of distinctive features are extracted from the Hip joints motion data which are able to partition and segment the gait cycles in an efficient way. The degree of deviation (i.e. anomaly) in each sub-phase is then calculated with respect to an optimal gait reference which is used for robot-assisted gait rehabilitation. The proposed gait segmentation method is applicable to gait with many types of pathology since training on the pathology specific templates is not required. Performance of the proposed algorithm is evaluated by statistical analysis of results which produced 100% gait segmentation accuracy for healthy subjects and over 99% for pathological subjects.

Metamaterials in Medicine: A New Era for Future Orthopedics _ Crimson Publishers
Tumblr media
Metamaterials in Medicine: A New Era for Future Orthopedics by Hamid RS Hosseinzadeh* in Crimson Publishers: Peer Reviewed Orthopedic Research Journals
Metamaterials are synthetic materials engineered to provide properties which “may not be available in nature that we know”. These materials usually earn their properties from structure rather than composition. The essential property in metamaterials is their unusual and desired properties that appear due to their unique design & structure [1].
In 1898 J.C. Bose showed the possibility of existence of artificial material by conducting microwave experiment on twisted structure. Later, in 1968, Russian physicist Victor Veselago considered the electromagnetic properties of a hypothetical materialwhose electric permittivity and magnetic permeability were both negative [2]. Such a material, as Veselago mentioned, did not exist in nature. Smith et al. had recognized gradient refractive index medium to bend electromagnetic waves. Metamaterial opened up a new exciting world for the scholars.
The word was first coined by Rodger M Walser (2001) who gave the following definition: Metamaterials are defined as macroscopic composites having a man-made, three dimensional, periodic cellular architecture designed to produce an optimized combination, not available in nature, of two or more responses to a specific excitation [3]. In metamaterials, inclusion of small inhomogeneities can produce effective macroscopic behavior. So, embedding artificially fabricated inclusions in a specific host medium will provide the designer with an ample collection of independent parameters such as properties of host materials and properties related to size, shape, and compositions of inclusions. All these design parameters can play a key role in getting the expected result.

Fundamental scientific research of domestic and foreign scientists strongly suggests that the progress of medicine is impossible without the widespread use of modern physical factors in the diagnosis, prevention, treatment and rehabilitation of almost all nosologically forms of diseases from newborns to old age of patients. Each micro-and macro-organism has individual bioenergetic characteristics corresponding to its type, which is the main condition for normal life activity of the organism. In the case of “alien” bioenergetic characteristics, specific biological processes inherent in this organism are disrupted, which leads to its death [1]. The biopotential of each person is strictly individual in both normal and pathological conditions. The degree of deviation of the biopotential corresponds to the stage of development of the disease, i.e. the formation of intermediate States of the body with a violation of its supramolecular structures

General Evaluation of Covid-19 Diagnosis Methods
After the first case has appeared in China, the disease, COVID-19, continues to spread an omnipresent threat to global health. The reliable, fast, and effective diagnosis methods are important to control the pandemic. Today, the real-time Reverse Transcriptase (RT-PCR) test is the main diagnostic practice as a gold standard method for accurate diagnosis of this disease. On the other side, serological assays are easy to be implemented for disease screening. However, the actual limitations of these methods are required skilled personnel and specialized infrastructure. Thus, in this direction, there is an urgency to be implanted an appropriate method for the diagnosis of COVID-19 disease. Nowadays, the Reverse Transcription Loop-Mediated Isothermal Amplification (RT-LAMP) technique is preferred because of the portable, easily implanted, and effective for diagnosis. In this assay, three main diagnosis methods (RTPCR, serological tests, and RT-LAMP) are evaluated s within the scope of the COVID-19 diagnosis.

The Importance of Body and Action in the Design of Technological Devices: For an Embodied Shift as an Alternative Conceptual and Methodological Framework in Gerontechnology
Abstract
To what extent is the design of technological devices for older adults symptomatic of our way of thinking about human cognitive aging? So far, technology-based solutions for the older adults are employed as reactive rather than preventive interventions and are often focused on impairments and age/dementia-related declines. In more specific terms, technologies are designed as cognitive crutches, rather than extended-mind devices. The aim of the present paper is twofold. First, pointing out researchers’ responsibility in the spreading of conceptual frameworks that drive the development (and failure) of technological tools for older adults. Second, proposing an alternative perspective about cognitive aging across the embodied shift. The embodied cognition paradigm provides both on descriptive and explicative view about how individuals engage their environment. Indeed, the methodological benefits of such a perspective stem from a better understanding about how the cognition emerges from the interaction between the bodily abilities and available resources in the current environment. It is argued here that the embodied cognition paradigm is a relevant theoretical framework for the gerontechnological domain.

Comparison of Posterior Vs Double Approach in the Treatment of Adult Degenerative Scoliosis
Background: The prevalence of adult degenerative scoliosis increases with age and can appear in a previously aligned column. Its prevalence is from 2 to 32% in people over 50 years of age.
Material and Methods: a prospective, longitudinal, comparative controlled study with two-year follow-up, in adult degenerative scoliosis, divided into two groups Group I treated with posterior approach and Group 2 treated with the double approach. 43 patients with an average age of 63.2 years, 30 women (69.76%), with curves with magnitude from 10 to 42°. 35 were subjected to a posterior approach, while 8 were treated with a double approach (ALPA Anterior-Lateral-Transpsoas-Approach + posterior approach).
Results: the Roland-Morris scale in group 1 was 13.17 (0-22) and at 2 years 9.75 (0-22) for group 2 with a preoperative mean of 14.71 (8-20) and 7.28 (0-14) at 2 years. No statistical significance between groups (p=0.356). The Oswestry in preoperative group 2 patients was 49.71 (26-74); 2 years in 21.85 (2-44). For group 1 in 54.27 (18-82), and 35.51 (2-82) in 2 years. pelvic parameters have no difference and only correction of scoliosis and sagittal balance was better in the double-approach group.
Conclusion: The meta-analysis revealed that CR MB and PS MB could achieve similar clinical outcomes, whereas the rate of complications and revision was significantly increased in CR MB. Based on the outcomes of this meta-analysis, the choice of the CR MB does not appear to be justified, and further studies of high methodological quality with long-term follow-up are required to confirm our conclusion.

Risk Communication for Poultry Farms by Detection of HPAI (H5N1) Virus in Wild Birds during the Winter Season of 2021/2022 in the Republic of Korea
The outbreak of Highly Pathogenic Avian Influenza (HPAI) is known due to the movement of migratory birds. Migratory birds change their habitats along the latitude in search of breeding and wintering sites based on temperature and length of day. The geographical route through which migratory birds move is defined as a flyway, and nine major flyways are widely known. Avian Influenza Virus (AIV) sharing may occur among migratory bird groups in breeding and wintering sites where flyways overlap, and it is spread to other parts of the world when such bird groups move back to wintering and breeding sites [1]. This mechanism can make the same AIV found in Asia, North America, and Europe. Thus, the Republic of Korea focuses on the status of Avian Influenza (AI) in wild birds and poultry in northeastern China, Mongolia, Siberia, and Alaska where summer breeding sites overlap with flyways passing through East Asia, Europe, and/or North America [2]. In 2021, the number of AIV detected in wild birds in Europe and Asia increased compared with previous years, and different types of viruses were detected. However, no detection of AIV was reported in North America at that time. The number of wild bird AI cases in Europe was 608 in 2017, 73 in 2018, one in 2019, and 771 in 2020, but 1,059 AI cases were recorded in August 2021. Different types of viruses, including H5N1, H5N3, H5N4, H5N5, H5N8, and H7N7, were detected in the first half of 2021, but after June, only H5N1 and H5N8 remained. In poultry, 1,134 HPAI outbreaks, including H5N8, H5N1, and H5N5, were confirmed by August 2021. After June, only H5N1 and H5N8 (excluding N-type unidentified H5) were observed. In Asia, 70, 31, 17, 50, and 45 AI in wild birds were recorded in 2017, 2018, 2019, 2020, and 2021, respectively. The common virus type was H5N8 with 41 cases (91.1%), and one case each of H5N1, H5N2, H5N5, and H5N6 was reported. In poultry, 260 outbreaks of HPAI were confirmed by August of 2021, of which 125 (48.1%) H5N8 and 65 (25.0%) H5N1 were the common viruses found. H5N6, H5N5, and H5N2 were also detected [3].

Green Algal Pesticide–Fertilizer for Sustainable Agriculture and Food Security
Opinion
World population expansion and climate change have posed greater challenges to food production. It is believed that food production would need to be raised by 100-110% in 2050, higher than the 70% expected in the current scenario [1]. As a result, more herbicides and fertilizers are being utilized in contemporary agriculture to improve crop yields. However, the frequent application of these agrochemicals offers several ecological losses and health risks [2]. The severe ecological losses and inefficiency in food production continue to confound efforts in achieving and maintaining food security [3]. Therefore, developing and exploring smart and eco-friendly strategies without compromising crop yields and food security is an exciting new futuristic research direction in this area. To date, various advanced techniques have been developed, including nanotechnology and slow and controlled release technology [4]. Smart nanoparticle (NPs) based technologies are great appeal since these strategies respond to a variety of stimuli to achieve controlled release of agrochemicals. These techniques can improve the usage efficiency of the active ingredients and minimize their application rates and overspills [5,6]. However, most of these traditional controlled–release materials have limitations owing to their single purpose application (either fertilizer or pesticide), expensive and non-biodegradable chemicals, and unsustainable supply of agrochemicals [7]. Therefore, the combination of low-cost pesticide-fertilizer, which can safely realize the integration of pest control and nutrient supply, is highly desired.

Metal Additive Manufacturing and its Scope in Self Reliant India
Introduction
Additive manufacturing has changed the whole layout of the manufacturing industry with its layer-by-layer approach. With the help of this technique, the fabrication of complex and intricate parts in the different industries was possible in less time and reduced portfolio costs. A lot of research is going on in the scientific community for the fabrication of metal parts using additive manufacturing. Metal Additive Manufacturing (MAM) techniques are coming up with a method to fabricate complex intricate and large-scale metal structures.
Many countries like India are focused on developing this technology. It is believed by many experts that the proper development of AM infrastructure in India is very fruitful for the nation’s economic and technological growth. In 2020, the Prime Minister of India, Shri Narendra Modi, launched “Atmanirbhar Bharat,” for the country to become self-reliant. Various measures have been proposed in this area to increase efficiency, competitiveness, and resilience. The five obligations of ‘Atmanirbhar Bharat’ are Economy, Technology-driven systems, Infrastructure and Vibrant Demography. Therefore, AM can be seen as a powerful tool in advancing India towards self-reliance.

Which Type of Breakfast Cereals is Healthier: Evaluating Labels in the Portuguese Market
The need for nutritional improvement of Breakfast Cereal Products (BCP) has been pointed out, namely regarding sugar, salt, and fat contents. Fibre has several proven health benefits but has been ignored in previous studies. This study aimed to evaluate the nutritional quality of BCP in the Portuguese market, especially regarding fibre content. A total of 247BCP available in online stores of the leading retailers were evaluated regarding the nutritional profile, using the information given in the products’ label. BCP were divided by types according to texture/appearance and production method. Nutritional differences between branded and white-label BCP were also investigated. The majority of BCP have healthy values of total fat, saturates and salt. BCP marketed for children have the highest sugar content values (p<0.05). Extruded rich in fibre and Flakes have the lowest sugar content and the highest fibre content (p<0.05). Few BCP enriched in fibre were found (3.8%), with 10 to 29g of fibre/100g of product. The healthiest products are in the groups of Extruded rich in fibre, flakes, Extruded for adults, granolas, and muesli. However, there are also unhealthy products in flakes and extruded groups. There were few differences between branded and white-label BCP. In case of difference (p<0.05), the branded cereals were healthier. Many BCP in the Portuguese market needs improvement regarding sugar content, namely all BCP marketed for children. Generally, a low percentage of the recommended daily dietary fibre intake is obtained from one BCP serving. Brands are rarely healthier than white-label products.

Multilevel noncontiguous thoracic spine burst fractures are rarely reported in the literature [1-5]. They are usually described as floating spine injuries. The reported incidence of noncontiguous spine fractures varies from 1.6 to 19%, with a usual range of 3 to 8%. However, this incidence has been historically underestimated due to overlooked injuries or delayed X-rays diagnosis.  Recent evidences based on routine full-spine CT-scan suggest the actual frequency may be as high as 20% [6]. These injuries are strongly associated with high-energy trauma, mainly fall from high height or traffic accident [7]. The average age at the time of accident is 30,9 years. They are frequently associated with life threatening organ injuries and neurological deficit, resulting in high Injury Severity Score (ISS) [3,4,6,7]. In such clinical situations, early surgical fixation has shown its efficacy compared to conservative treatment. It allows better comorbidity management and leads to better clinical outcomes in terms of mobilization. Recently, percutaneous posterior spinal fixation has become the new gold standard compared to a conventional open approach. It reduces the invasiveness of the procedure and offers better precision

Behaviors of the Tropical Pacific Barrier Layer during ENSO Events and its Mechanism
Abstract
By using gridded Argo data from 2004 to 2017, a systematic comparison study is conducted on the evolution features of the tropical Pacific Barrier Layer (BL) between El Niño and La Niña (ENSO) period. As of climatology, the BL in the tropical Pacific Ocean presents three distinctive zonal bands at about 12° N, 5° N, 10° S, separately, extending from the western Pacific warm pool eastwards to the eastern Pacific Ocean. Among them, the BL (>20m) in the warm pool occurs throughout the whole calendar year, whilst the BL at both 12° N and 10° S bands exist mostly often in Winter and Summer periods Composite analysis reveals that the ENSO-relevant BL variations are largely confined within the equatorial Pacific west of 160° W. Specifically, overwhelming thick (thin) BL occurs east of 160° E during El Niño (La Niña) year, respectively. It is deduced that the heavy rainfall, horizontal transport of low saline water and downwelling Kelvin waves are three main contributors for the unusual thick BL occurrence east of 160° E during the El Niño period. Correlation diagnosis verifies that the interannual BL changes lags the Nino 3.4 index by one month. Conversely, the BL variability leads the Nino 3.4 index by one month for the La Niña event and anomalous thickening of the Isothermal Layer (IL) triggered by water convergence and subduction is believed to be the main cause for the abnormally thick BL emergence west of 160° E.

An Independent Review of Several Successful Hospital Incident Command System (HICS) Implementations and Lessons Learned _ Crimson Publishers
An Independent Review of Several Successful Hospital Incident Command System (HICS) Implementations and Lessons Learned by Julie Bulson in COJ Nursing & Healthcare
Tumblr media
Abstract
US hospitals are required by Joint Commission to utilize a command structure that is compliant with the National Incident Management System (NIMS). Most utilized is the Hospital Incident Command System (HICS) which guides any disaster response. Further, hospitals are required to NIMS guidelines when responding to events that predicate the implementation of disaster plans or protocols - especially when hospital leaders will interact with local, state or federal authorities. These mandates have grown in depth and breadth since all first responder disciplines began to update their local policies, procedures, and education requirements in the post-9/11 preparedness environment.
This new environment includes ongoing federal grants designed to increase hospital preparedness and capacity, but with the fund are mandates requiring hospitals to educate staff members in NIMS education and its use. As a result, many hospitals have adopted HICS and NIMS as the general frameworks on which they build disaster plans, education sessions, and disaster drills. While federal and state officials continue to track compliance with federal requirements, there seems to be a gap in tracking use and effectiveness of HICS and NIMS in real-life incidents, whether planned or emergent. This article examines the real-life experiences of one 1,000+ bed Michigan hospital system that used HICS and NIMS during responses to a state funeral,a credible bomb threat, and a helicopter crash.
