Alternate Reality: Kawasaki-Meguro KZ79SP Raven by Yoshiki Sawamura of @crows_japan. Meguro was the Japanese brand of police, military, racing, and consumer motorcycles that Kawasaki purchased in 1960 to form Kawasaki-Meguro and enter the motorcycle market. Sawamura-san envisioned a parallel universe or alternative history where the name, spirit, and soul of Meguro never died out, so that the mighty Kawasaki Z/KZ machines still bore heavy Meguro influence. "We were able to inject the soul of the Meguro brand that once existed in Japan." Of course, Kawasaki has revived the brand themselves with the Meguro K3 now available! Many people around the world look for ways to build container images in Kubernetes without the need to mount the docker socket or perform any other action that compromises security on your cluster. With the increased need, a famous software engineer, Jessie Frazelle saw the need to introduce Img image builder. This is an open-source, daemon-less, and unprivileged Dockerfile and OCI compatible container image builder. Img is a wrapper around the open-source BuildKit, a building technology embedded within Img. There are many features associated with the img image builder. Some of them are: Img CLI, a responsive CLI that provides a set of commands similar to Docker CLI commands when dealing with container image building, distribution, and image manipulation. Rootless Builds: img can be run without requiring the –privileged Docker flag or the equivalent privileged: true security context in Kubernetes. BuildKit: defined as one of the next generation build engines for container images. Parallel Build Execution: BuildKit assembles an internal representation of the build steps as a Directed Acyclic Graph (DAG), which enables it to determine which build steps can be executed in parallel. Cross-Platform/OS Builds: it’s possible to build images for different combinations of architecture and OS on a completely different platform In this guide, we will take a deep dive into how to build container images on Kubernetes using img image builder. Setup Pre-requisites This guide will work best if you have a Kubernetes cluster set up. Below is a list of dedicated guides to help you achieve this: Install Kubernetes Cluster on Rocky Linux 8 with Kubeadm & CRI-O Install Kubernetes Cluster on Ubuntu using K3s Deploy Kubernetes Cluster on Linux With k0s Run Kubernetes on Debian with Minikube This guide will demonstrate how to build container images from Dockerfile using img image builder in Kubernetes with Github. So, you will also need: Access to Kubernetes cluster with permissions to create, list, update and delete pods, jobs, and services Github repository with a Dockerfile: we will use the repo URL as the path of the Dockerfile Dockerhub account: to be able to authenticate and push the Docker image. #1. Configure Build Contexts For this guide, we will use a private GitHub repository as our build context. We need to configure it with the required Dockerfile. The URL to my private git repository used in this article is: https://github.com/computingforgeeks/kubernetes-demo In the repository, I will create a Dockerfile with the contents below: FROM ubuntu ENTRYPOINT ["/bin/bash", "-c", "echo hello"] Now obtain a Personal Access Token to your git account. #2. Create the Img Pod Manifest We will have two containers: Git-sync: an init container to clone the private git repository img: that builds the docker image and pushes it to docker hub These two containers share a volume git-repo mounted as emptyDir at /repo Create a manifest for the pod. vim pod.yml Add the below lines to the manifest: apiVersion: v1 kind: Pod metadata: labels: run: img name: img annotations: container.apparmor.security.beta.kubernetes.io/img: unconfined spec: securityContext: runAsUser: 1000 initContainers: - name: git-sync image: k8s.gcr.io/git-sync:v3.1.5 volumeMounts: - name: git-repo mountPath: /repo env: - name: GIT_SYNC_REPO value: "https://github.com/computingforgeeks/kubernetes-demo.git" ##Private repo-path-you-want-to-clone - name: GIT_SYNC_USERNAME value: "computingforgeeks" ##The username for the Git repository - name: GIT_SYNC_PASSWORD value: "ghp_JilxkjTT5EIgJCV........" ##The Personal Access Token for the Git repository - name: GIT_SYNC_BRANCH value: "master" ##repo-branch - name: GIT_SYNC_ROOT value: /repo - name: GIT_SYNC_DEST value: "hello" ##path-where-you-want-to-clone
- name: GIT_SYNC_ONE_TIME value: "true" securityContext: runAsUser: 0 containers: - image: r.j3ss.co/img imagePullPolicy: Always name: img resources: workingDir: /repo/hello command: ["/bin/sh"] args: - -c - >- img build -t docker.io//helloworld . && img login -u -p && img push docker.io//helloworld volumeMounts: - name: cache-volume mountPath: /tmp - name: git-repo mountPath: /repo volumes: - name: cache-volume emptyDir: - name: git-repo emptyDir: restartPolicy: Never In the above file, replace the values appropriately. You can also notice that the destination folder for git-sync is the working directory for img. If you are using a public git repository, you may not be required to provide the Personal Access Token for the Git repository. #3. Run img image builder in Kubernetes Using the manifest, run the pod using the command: kubectl apply -f pod.yml Now follow the image build and push process with the command: kubectl logs img --follow Output: From the above output, we are safe to conclude that the image has been successfully pushed to DockerHub #4. Pull and Test the Image You can now pull and test the image using: 1. Docker Ensure that Docker is installed on your system. The below guide can help you achieve this: How To Install Docker CE on Linux Systems Now run a container with the image using the command: docker run -it / For example: docker run -it klinsmann1/helloworld:latest Sample output: 2. Kubernetes The image pushed can still be used on Kubernetes. Pull and test the image as below; $ vim deploy.yml apiVersion: apps/v1 kind: Deployment metadata: name: hello-world spec: selector: matchLabels: app: hello replicas: 1 template: metadata: labels: app: hello spec: containers: - name: hello-world image: klinsmann1/helloworld:latest Apply the manifest: kubectl apply -f deploy.yml Check the status of the deployment: $ kubectl get pods NAME READY STATUS RESTARTS AGE hello-world-7f68776d79-h4h4z 0/1 Completed 1 (4s ago) 6s img 0/1 Completed 0 13m Verify if the execution was successful. $ kubectl logs hello-world-7f68776d79-h4h4z --follow hello The end! We have successfully walked through how to build container images on Kubernetes using img image builder. I hope this was significant to you.

Rambles About Short Story - Alone
Well, I’m pretty bored, so... I’m going to scan through the last short story and point out all the references and parallels I made in it! How many can there be, right? It’s pretty weird to make references/parallels to parts of the story that don’t officially exist, but I did it anyways.
-----------------------------------------------------------------------------------------------------------
This whole short story focuses on Red’s loneliness when Blue’s not around! Just like in Legacy after her death, just... without the tragedy this time. So Red’s depression actually becomes rather cute, if you think about it. And unlike in Legacy, where everyone else is unable to cheer him up, this time, he has people helping him out.
Blue: “What if you ran into a clone of me, who looks and sounds the same, and has the same memories and everything, but is not the real me? Could you replace me with her?” Red: “Uh… well, if I had you, I don’t think I can replace you with anything, a clone or not.”
This is probably the most obvious one, since Blue asks something very specific. The bad ending of Legacy happens because Red could not replace her, not even for a completed clone of her. Not to say that it was a bad choice on his part. Thus the tragedy. Was it a wrong choice? Well, discussing right/wrong morality in a fictional setting that has no ultimate right/wrong is completely meaningless, so... moving on.
Crystal notices Red’s loneliness and calls Platinum to help. In Legacy, Crystal sends Platinum (and Gold) to find Red once he succumbed to his depression and ran off. And in the bad ending, Crystal is the one to notice his depression after he had doomed both Platinum and Blue/K3 to their fates.
Crystal gets groped by a pervert known as “Ass Catchem”. Well... that’s a substory from a different game. And a reference to a short story that could’ve been.
Tumblr media
Red had disguised himself as a clown, wielding a party bat on this mercenary job. Just like in Hitman.
Tumblr media
THE CROWD’S LOVING IT! - Party Bat. Yes, the bat talks. It’s a... pretty f*cked up game considering all the “for fun” parts.
The pervert tries to sue Silver for “ruining his sense of touch”. Well...
Tumblr media
Pervert mistakenly thinks that Silver is a girl, and... yeah.
Tumblr media
So Silver’s ass has... hehe. Gold will mock the sh*t out of him for that. I wonder how Blue would react.
Red doesn’t want to sleep on the bed now that Blue’s not around. In Legacy, the empty bed is one of the biggest reasons why Red can’t stand his house anymore. And so he gets really mad when Platinum puts him there after she finds him drunk. I didn’t have the space/time to write that out in the short story though. Originally, as Platinum gets ready to sleep, she’d find Red on the couch and ask why he isn’t sleeping on the bed upstairs. And after explaining his situation, Red would sleep on the couch while Platinum sleeps on the set-up blankets on the floor. But it felt kind of weird to write it out given the overall mood, so... ehn.
Platinum is going through issues of her own but still tries to help Red in his lonely times. In Legacy, Platinum definitely has her own issues, but still tries (and fails) to help Red.
Y is recovering from an injury at this time. In Legacy, Y is nearly killed by her own Mega Garchomp and falls into a coma. And Platinum blames herself for failing to save her junior. This time though, it’s nothing too major and is unrelated to Platinum, so she’s not devastated by it. It would’ve been a potential DE chapter or a short story, as X takes care of Y all day every day.
Platinum and Moon have a “fight” because Moon had been lying and hiding something important from her, and Platinum absolutely hates deception among those she trusts. Platinum says something she quickly regrets, but it’s too late and she had hurt Moon quite badly. Diamond and Pearl find Moon before she does something drastic. In Legacy, Moon runs off before anyone could catch up to her, and she ends up doing something drastic that ultimately gets her killed, thus starting the whole Corpse Party shenanigans.
Tumblr media
So comparatively, Platinum is still falling apart a bit in the short story, but not nearly as much as she would have in Legacy.
Platinum deeply hurts Moon, a younger sister figure, emotionally in a “fight” out of anger, which she quickly regrets but is too late. And then instead of making amends, because of her own fears and guilt, “flees” to Kanto to seek advice. In Legacy’s bad ending, Red deeply hurts Platinum, a younger sister figure, both physically and emotionally out of anger, which he quickly regrets, but it’s far too late since she’s bleeding out. And instead of trying to help her and make amends, he flees the scene, ultimately buries her alive, and flees again to Unova to finish the fight against the enemy. Red also does the same to Blue’s clone. He deeply hurts her emotionally, regrets it later on but it’s too late, and tries to flee from his guilt.
Platinum is ashamed by what she had done. In Legacy’s bad ending, Red is also very ashamed by what he had done, and the tremendous guilt forces him to sacrifice himself to be freed of it all in the end.
Red can’t imagine hurting Platinum... which is exactly what he ends up doing in Legacy’s bad ending.
Red: “I don’t know if it’ll be the same, but… think how you would feel if I hurt you like that. Would you actually want me to go away forever, or would you want me to return to talk to you and make things better again?”
Red does exactly that in Legacy’s good ending, after he finds enough courage to talk to Platinum again. And in Legacy’s bad ending, one of Platinum’s last thoughts would be wishing to have the old Red back like the happier times in the past.
-----------------------------------------------------------------------------------------------------------
Whew. I think that’s all of them?

HTGAWM Finale Thoughts
(obviously contains spoilers)
BONNIE AND FRANK. WHY?? for WHAT purpose? i’m so so so angry. their deaths were actually for nothing, and they were finally about to be happy. i liked them together, they both deserved a good ending. i’m pissed abt this
the whole incest story line about Sam & Hannah being Frank’s father came from nowhere and was completely unnecessary. they rlly sprung that on us in the final few episodes for what? for nothing. it added nothing, it only explained Frank’s death which was also unnecessary. this plot annoyed me.
connor & oliver’s relationship in the final episode was so poorly written. seriously? threatening each other with divorce? why? i’m glad they stayed together but i’m mad that was this a part of the finale instead of showing them soaking up their time together before connor went to jail (which i get helps him with his guilt but i still don’t think he deserved it). i wish they had just let them be happy, instead of having connor actually present oliver with divorce papers. i thought it was a poor way to conclude a relationship that spanned the seasons and the fans loved.
don’t get me started on Michaela. she always had Connors back, and I get she always has hers first, I still think her negotiating probation & leaving connor out to dry was out of character. She ended up alone, her best friends either in jail or refusing to talk to her. I hated her ending, she deserved so much better, especially after Ashers death. She got to be a judge, I guess, but I wanted more for her. i’m upset about this, she was one of my fave characters even if season 6 did her dirty.
i guess its realistic that the k5 (or k3 i guess bc two are dead) doesn’t talk anymore, but I wish they did. they went through so much together. I get Laurel not talking to the other two, as she disappeared the last season and chose being a mom which is understandable, but Connor and Michaela? They were best friends, who loved each other so deeply. I don’t like that they all went their separate ways- i think the two of them could’ve stayed friends. Even just to make viewers happy, yk?
Why did they kill Asher too? They killed him & sent Laurel away, and it resulted in all of season 6 feeling like Connor, Michaela and Oliver’s interactions were incomplete. They were missing someone- two people- but I think it was dumb to not include both Asher and Laurel in the final season. I thought both plots were unnecessary, I think the writers should’ve kept one of them for the final season.
WHY WAS NATE THE ONE W THE BEST ENDING? i’m sorry he started out innocent and then lost that- killing Miller and Xavier pissed me off. I don’t have sympathy for him, and i’m mad he got the best ending??? ugh
I am glad Annalise and Tegan ended up together tho. that was cute. i liked the parallel with Christopher as well.
DONT GET ME STARTED ON THE AGE MAKEUP HAHAHAH. WHY DID EVE LOOK YOUNGER THEN LAUREL??? I HATED IT.
overall, I didn’t like this finale. I loved this show, stuck with it when it got slow, stuck with it for 6 seasons, and the finale just made me sad for how the characters ended up, instead of sad that the show ended. I’m most mad about Bonnie, Frank and Michaela. the others were meh, but those three endings make me incredibly mad.

I’m actually super intrigued by the idea that Kreese wants Miguel at Cobra Kai - I’m not sure if Johnny would ever agree to those terms tho but I’ve also been thinking about Terry Silver being reintroduced to the series and how Kreese might deploy him. It would be really interesting to see him use Silver as a way to cause a rift at the newly combined dojo’s and target Miguel specifically, the way they targeted Daniel in K3. Especially since we didn’t get to see much of Miguel’s emotional arc after he regained the use of his legs - he was mostly used to move the plot along instead, and if they’re going to flesh that out, Silver would be the perfect villain to prey on Miguel’s insecurities especially if Johnny keeps wanting to not push Miguel too quickly. I feel like Kreese’s look during the all valley vote could also just show that Kreese wants Miguel out of commission and that he wants to break his spirit and in the process break Johnny and the former Cobras who defected. And between the narrative parallels we get with Miguel and Daniel, I’d be really interested to see if they explore what Silver being back could mean for Daniel’s guilt and clearly unresolved trauma and how Silver targeting Miguel could affect Miguel’s own journey towards recovery and the rest of the dojo.
Silver did so well at presenting himself as a friendly, trustworthy adult and I want to see how/if he uses that trick in the series. The difference this time, of course, will be that Daniel knows about Silver. But this might come down to how skilled a manipulator Silver is. Daniel might try to warn Miguel, but would Miguel listen?
And as for Kreese wanting Miguel out of commission, well, he was. I’m not sure if they would take the story there a second time. The conflict with Cobra Kai isn’t just about the dojos - it’s about different philosophies, and what better way to ensure your victory in an ideological conflict than to successfully pass your ideas on to the next generation? In that way, having Miguel back as his student would be a win for Kreese.

This carved wooden statue of Tutankhamun (ca. 1345-1327 BCE, found in the pharaoh's tomb), smoothed with gesso and then painted, is a life-size representation of the king 𓇓𓈖𓏏 “nsw”. Carter recorded that the figure was beneath one of the large ceremonial chariots 𓌝𓏏 “wrry.t” in the southern 𓇔𓏭𓈇 “rsy” end of the Antechamber. The arms 𓂝𓂝 “rmn.w” have intentionally been severed to just below the bicep, and the body 𓄡𓏏𓏤 “ḫṯ” extends to just below the hips. Tutankhamun (𓇋𓏠𓈖𓏏𓅱𓏏𓋹𓋾𓉺𓇑) “twt-ˁnḫ-imn~hekA-iwnw-šmAi)” ‘The Living Image of Amun, Ruler of Southern Iwnw (Heliopolis) wears a simple white 𓌉 “ḫḏ” garment and a flat-topped crown with protecting uraeus, similar to a crown that Nefertiti (𓇋𓏏𓈖𓇳𓄤𓄤𓄤𓄤𓄤𓏏𓏭𓇍𓍘𓁐) “nfr-nfrw-itn nfr.t-iy-ti” ‘ Beautiful are the beauties of Aten, the beautiful one has come’ often uses. His ears 𓋹𓋹𓄔𓄔 “ˁnḫ.wy” are pierced, and his skin is painted a reddish brown in the convention typical for representing males. Although Carter had suggested that the figure may have been a mannequin, similar to a clothes-dummy, which would hold the garments and jewelry of the king, it is difficult to find parallel pieces. Other scholars have pointed out that this torso and several statues from the Middle Kingdom, which are somewhat similar in appearance, may all be related to the rebirth or resurrection of the king in association with his identification with the god Osiris 𓊨𓁹 “wsir” In tombs 𓂝𓂝𓇋𓉴 “ˁˁı͗” of the Old Kingdom, there are also busts similar to this one. They are heads or torsos sculpted to look as if they were emerging from the floor or the wall. They appear to be images 𓅲𓏏 “twt” of the ka 𓂓 “k3” (the corporeal twin and essential nature of the deceased) coming up from the burial chamber and entering the offering chapel through or near the false door, the place where offerings to the deceased could be made. Because of these parallels it is likely that this enigmatic statue 𓂙𓏏𓏭𓀾 “hnty” has a funerary, rather than domestic purpose. Once upon a time, I bought and started trying to translate “Die Göttin Mr.T: Entstehung und Verehrung einer Personifikation”, the monograph about the goddess Meret by Guglielmi. I had gotten The Ping from her after reading a few comments about her in one of TDuQ’s articles (“Raising the serpent power. Some parallels between Egyptian religion and Indian Tantra.”) I’ve never lost interest in learning more about this beautiful but obscure goddess, but have lost the ability to gather the intense amount of concentration needed to do extended translations of that nature. To those ends, I am going to try approaching my research into her from a different angle, which is basically by flipping through the monograph’s indexes until something catches my eye and then working on that individual thing. The first entry for #gnwt Meret is this intriguing epithet for her: TpH.t-wdj.t-k3.w
The LGG has it as meaning, “The esophagus of the gods”, and specifies that it is an epithet of the Upper Egyptian Meret. Just below this entry is a very similar one listing Ma’at as the esophagus of the goddesses while Guglielmi gives a more literal translation: “Cave that transfers the Ka-food to the gods (esophagus.)” There is also a footnote: “TpH.t-wdj.t-k3.w belongs to the “throat" designations of Meret assimilated to Maat and also forms an epithet of the Hathor assimilated to Maat.”

Treatment of Proximal Interphalangeal (PIP) Joint Fracture by Dynamic Kirschner Wire Fixator
Tumblr media
Abstract
Aim: To describe the clinical outcome of treatment of proximal interphalangeal (PIP) joint fractures by dynamic Kirschner wire fixator.
Methods: The Kirschner wire traction fixator was used in treatment of 22 patients with pilon fracture and fracture-dislocation of proximal inter phalangeal joint. Little finger affected in eight patients, ring in three patients and middle and index fingers were affected in five and six patients respectively. The patients age at time of surgery range from 18 - 58 years (average 35.4 years). The base of proximal phalanx was involved in an average 45% (range from 35% to 70%). We used the Michigan hand score to evaluate the clinical outcome after average follow up 8 months (range from 6-9 months).
Results: Average Michigan hand questionnaire was 88% (range from 63%-100%). The average active flexion-extension ROM of proximal interphalangeal joint was 93 degrees (from 50-120 degrees). No instability or angular deformity at the end of follow up. Pin tract infection reported in four patients and two patients had moderate pain with proximal interphalangeal joint flexion more than 50 degrees.
Conclusion: The Kirschner wire traction fixator was easy to apply, can reduce and maintain reduction of unstable proximal interphalangeal joint fractures and allow immediate post operative (PIP) joint motion.

Introduction
Proximal interphalangeal (PIP) joint injuries remain challenging to hand surgeons [1]. Painless, stable and mobile PIP joint are the main goals for any treatment option of PIP joint [2,3]. Closed reduction and extension block pins [1,2] Open reduction of PIP joint fractures [3,4] and osteochondral hemi-hamate autograft arthroplasty [5,6] described for treatment of such difficult injuries. Maintenance of satisfactory alignment of the fracture and acceptable congruent joint surface, while allowing early joint motion would seem to be an ideal treatment option. Dynamic external fixation of PIP joint fractures provide indirect fracture reduction, maintain fracture alignment and allowing early movement which results in satisfactory functional outcome [7-13]. The present study describe the clinical outcome of treatment of (PIP) joint fracture by dynamic Kirschner wire fixator modified by Asal Fouad in 2009 [14].
Patients and Method
The modified Kirschner wire traction fixator applied to 22 patients with PIP joint fracture and fracture-dislocations. We exclude fractures more than 3 weeks duration and open fractures with cut tendons or neurovascular injuries. Gender, age at the time of surgery, mechanism of injury, affected side and finger, hand dominance, occupation, fracture type and degree of involvement of base of middle phalanx described in. there were four patients with open fracture, two of them had infected wounds. The average time from injury to surgery was 5 days (range from 2-21 days) (Table 1). The modified Kirschner wire traction fixator had the same principles of Suzuki et al. [15] fixator.
Surgical Steps
Without tourniquet under digital nerve bock anesthesia used 10 ml combination of (5 ml lidocaine 2%, 4 ml bupivacaine 0.25% and 1 ml sodium bicarbonate 0.25 m Eq/ml) and image intensifier the fixator applied in steps. All open fractures treated firstly by wound debridement and saline irrigation. Prophylactic antibiotics administrated for all patients (Figure 1).
Step 1 Axial traction Pin (K1):
A smooth 0.9 mm Kirschner wire was placed centrally in the proximal phalanx head perpendicularly on its long axis. The wire was bended in 90-degree angle at both sides of the finger in the direction of finger tip. Each end of the wire must be long enough to reach about 2 cm distal to the finger tip and fashioned in semicircle.
Step 2 Sliding traction pin (K2):
Sliding traction pin (K2): A smooth 0.9 K-wire was placed centrally in the head of middle phalanx. The K2 pin was bended around K1 forming a semicircles.
Step 3 The traction generated by 18 gauge cerclage wire
in a figure-of eight fashion which applied from the semicircles of the K2 pin to the semicircles of the K1 pin at each side. Traction force, traction balance on both sides and fracture reduction were assessed by image intensifier. Using the cerclage wire in traction was the difference between this fixator and Suzuki et al. fixator which used the rubber bands for traction.
Step 4 Placement of the reduction pin (K3):
The reduction pin was indicated in 12 patients because of residual displacement after traction. The K3 pin was drilled 5mm distal to the fracture line using K1 pin as a fulcrum to reduce the fracture or dislocation then it bended around K1 at both sides in a semicircle fashion. Lastly, the reduction, active finger motion and fixator stability were checked under image intensifier.
Early range of motion was encouraged for all fingers (Figure 2). The patients followed clinically and radiographically twice weekly till removal of fixator at the six week postoperative, then every three weeks for the next three months and once per month till the end of follow up period which usually range between 6 to 9 months (average 8). The subjective satisfaction was determined by the use of visual analogue scale (VAS) for pain. The Michigan hand questionnaire [16] was used to evaluate the results of using the Kirschner wire dynamic fixator.
Results
The average Michigan hand scores were 88% (range from 63% - 100%), the average active Flexion-Extension ROM of PIP joint at the final follow up evaluation was 93 degrees (range from 50 – 120 degrees) and the average active Flexion-Extension ROM of DIP was 71.5 degrees (range from 60 – 80 degrees) (Figure 3) . Twenty patients returned to previous work and recreational activities without disability and the other two patients changed their work because of pain with PIP flexion more than 50 degrees. Five patients developed pin tract infection without wire loosening resolved with oral antibiotics. One patient developed osteomyelitis treated with fixator removal, surgical debridement and antibiotics. Aseptic loosening of the wires and osteolysis in the head of proximal phalanx occurred in one patient that treated with fixator removal. According to VAS there were four patients complained of mild pain with extreme flexion which did not need treatment or affect daily activities and two patients had moderate pain treated with NSAID (Table 2).
Discussion
The management of intra-articular fractures and fracture dislocations of PIP joint is a therapeutic challenge for most hand surgeons [17,18]. In trial to stabilize and maintain fracture reduction of PIP joint a trans-articular K-wire has been used for these proposes but joint movements prevented for at least three weeks after surgery till removal of the K-wire [19,20]. Salter et al. [21] in 1981 established the importance of continuous active and passive movements in healing of hyaline articular cartilage. They concluded that, immobilization of any joint interfere with the normal distribution of synovial fluid and nutrition of articular cartilage. In a comparative study between the use of splinting, dynamic traction fixator and ORIF of pilon fracture of PIP joint Stern et al. [11] in 1991 concluded that ORIF can achieve anatomical reduction in some cases but should be approached cautiously. Extensive soft tissue dissection may deprive the smaller fragments from its blood supply and contribute to stiffness. The ligamentotaxis provided by dynamic fixator achieved and maintained the reduction of pilon fracture of PIP joint and give results which are radiologically and clinically comparable or even better than those obtained by ORIF. The results of our study support Stern et al. [11] conclusion in that the use of dynamic external fixation reduce the articular surface indirectly by means of ligamentotaxis which preserve wide joint space and allowing early active ROM of the PIP joint. The Kirschner wire dynamic traction fixator is simple in the design and application, also it allow both active and passive joint ROM. Inanami et al. [22] in 1993 treated seven PIP joint fracture-dislocations with dynamic finger fixator, the average PIP joint ROM was 88 degrees after average follow up 21 months. Allison [23] in 1996 reported average PIP joint ROM 77 degrees after treatment of 14 patients with comminuted fracture base of middle phalanx by dynamic external fixator. In this series we reported average PIP joint ROM 93 degrees in 22 patients treated with dynamic external fixator. We believed that our results was more better than Inanami et al. [22] and Allison [23] because of the use of spiral springs and the size of rings in their fixators can complicate this methods [24]. The modified Kirschner wire traction fixator is simple and had the same idea of dynamic traction fixation as PRTS of Suzuki et al. [15] but we believed that it had technically different points in the design and application 1- the bending of sliding traction pin (K2) around the axial traction pin (K1) give the advantage of stability and parallelism of (K1) to the long axis of middle phalanx. This makes the traction in line with the long axis of middle phalanx bone which results in more easily, maintained and acceptable fracture reduction. 2- Using of tension band wire instead of rubber bands was more amenable during healing time, maintain fracture reduction and give us the chance to modify the traction force by increasing or decreasing the tightness of tension band wire in outpatient clinic according to follow up radiographs. 3- In the modified Kirschner wire traction fixator we can use the (K1) pin as a fulcrum by reduction pin (K3) to get reduction because it was fixed by bending of (K2) pin around it but this option not possible with Suzuki et al fixator. Another point of view the bending of (K3) pin over (K1) pin after fracture reduction gave more stability to the reduction even if the traction lost during follow up. De Soras et al. [24]. In 1997 noted inflammation around the pin sites in up to 45% of cases and reported one case of sever osteolysis which involved almost the whole of the head of the proximal phalanx. In our series, although four patients developed a superficial pin-site infection which was successfully treated by oral antibiotics, one case developed osteomyelitis treated by surgical debridement, early removal of fixator and intravenous antibiotics. Another one case developed marked osteolysis in the head of proximal phalanx which needed earlier removal of the traction system. The final results of these two patients were completely unsatisfied; they had limited painful PIP joint ROM required regular NSAID administration. We believed that the unsatisfactory results of these two patients due to potentially infected open injuries and late presentation. Our overall results, and the mean range of active flexion, compare favorably with those of other types of dynamic traction splint [25,26] (Table 3).
Conclusion
We believed that the capsule-ligamentotaxis of the Kirschner wire dynamic traction fixator achieved reasonable articular congruity. The modified Kirschner wire dynamic traction fixator has given very acceptable results with a low rate of complication. It is light, cheap, effective and easy to apply. Immediate and continuous post operative (PIP) joint motion improved the range of motion regardless the accuracy of fracture reduction which reflected positively on the final outcome of the patients.
