This first image illustrates the process of bone sounding and the dimensions one takes into consideration in a crown lengthening case. Gargiulo et al. 1961 is an ultra classical piece of literature, identifying that the average measurement of biologic width was 2.04mm. This dimension plays an important role in the treatment of gummy smiles and all forms of crown lengthening. It is important to respect and maintain the biologic width. Encroachment of the biologic width with a restoration margin can lead to inflammatory destruction. Additionally, the excision of soft tissue without maintenance of a healthy biologic width, will lead to an increased risk of relapse of your gingival margin. If the distance from your proposed new gingival margin to the bone crest maintains a distance of 3mm or more, a simple gingivectomy may suffice. Should this distance be less than 3mm, the raising of a flap and subsequent bone removal to maintain biologic width is indicated.
