Studies about head trauma are experimental or have either a clinical or a prognosis purpose. In this study human samples from autopsies are used with the purpose of answering medical-legal questions. Four experimental groups with a total of twenty-one individuals and a control group of four individuals were studied. Samples were obtained from the injured area of each individual, fixed in 10% formalin during 24 hours and preserved in 70% ethanol. Each experimental group was then compared to the control group using statistical analysis after cell count to diagnose hyperplasia. Glial Fibrillary Acidic Protein (GFPA) shows some processes that take place after the injury and their sequence: blood brain barrier (BBB) begins to change two hours postinjury, hypertrophic astrocytes are present twelve hours postinjury, hyperplasia and the different types of BBB reactions start twenty hours postinjury, astrocytes arrange geographically and hyperplasia reaches its peak on day seventeen, and finally, on day thirty, astrocytes begin to return to their normal morphology and constitutes the scar. Hyperplasia doesn’t return to basal levels for months or even years. CD68 shows that macrophages begin to migrate almost immediately. It also shows that a restrained microgliosis begins on the second hour postinjury. These two processes reach their peak between days 17 and 20 postinjury, at the same location where astrogliosis reaches its peak.

Review: Panic’s Nova text editor
Panic, the long-established makers of Mac utility software, seems fully aware that introducing a new, commercial code editor in 2020 is a quixotic proposition. Is there enough of an advantage to a native editor over both old school cross-platform editors like Emacs and explosively popular new editors like Visual Studio Code to persuade people to switch?
I’m an unusual case as far as text editor users go: my primary job is technical writing, and the last three jobs that I’ve worked at have a “docs as code” approach, where we write documentation in Markdown and manage it under version control just like source code. The editor that works best for me in tech writing is the venerable BBEdit. When it comes to editing code, though, BBEdit lags behind. My suspicion is that BBEdit’s lack of an integrated package manager has hurt it here. Also, BBEdit’s language modules don’t support extending one another, making it effectively impossible to do full highlighting for a templating language like JSX or Jinja.
When I was a web programmer, I was one of many who moved to TextMate, and used it for everything for a while. When the Godot-like wait for TextMate 2.0 became unbearable, I wandered the text editing wilderness, eventually splitting my loyalties between BBEdit, Sublime Text, and more recently VS Code. At this point, I suspect nothing will pull me away from BBEdit for technical writing, but for programming I’m open to persuasion.
So: meet Nova.

I’ve been using Nova off and on in beta for months. I’ve reported some bugs, although I may mention a couple here that I didn’t catch until after 1.0’s release. And, I’m going to compare it to the GUI editors that I’ve been using recently: BBEdit, Sublime Text, and VS Code.
Nova is a pretty editor, as far as such things go, and with files of relatively reasonable size it’s fast. With stupid huge files its performance drops noticeably, though. This isn’t just the ridiculous 109MB, nearly 450,000-line SQL file I threw at it once, it’s also with a merely 2MB, 50,000-line SQL file, and Nova’s offer to turn off syntax highlighting in both files didn’t help it much. This may sound like a silly test, but in my day job I’m occasionally stuck editing an 80,000-line JSON file by hand (don’t ask). This is something BBEdit and VS Code can do without complaint. Panic wrote their own text editing engine for Nova, which is brave, but it needs more tuning for pathological cases like these. They may not come up often, but almost every programmer has one stupid huge file to deal with.
Nova has an integrated terminal and an integrated SSH client, and even an integrated file transfer system based on Panic’s Transmit. In fact, if you have Transmit and use Panic Sync, it knows all of those servers out of the box. Nova has a task workflow system for automating building and running. You can associated servers, tasks, and more with individual projects; Nova’s project settings are considerably more comprehensive than I’ve seen in other editors. You can even set up remote tasks. Nova has a serviceable Git client built in, too. Like VS Code, Nova uses JavaScript for its extension API, and it has built-in Language Server Protocol support—it’s a superbly solid foundation.
Beyond that, some smaller features have become table stakes for modern GUI editors, and Nova handles them with aplomb. “Open Quickly” can jump to any file in the open project, as well as search by symbols or just symbols in currently open files; it has a command palette; you can comprehensively edit keybindings. It has multiple cursor support for those of us who like that, and a “mini map” view for those of you who like that, although know that you are wrong. Nova’s selection features include “Select all in scope” and “Select all between brackets,” a command I often use in BBEdit and miss dearly in Code. (Both Nova and BBEdit select between brackets and braces, although BBEdit also selects between parentheses.) This effectively becomes “Select between tags” in HTML, a nice touch. There are a few other commands like “Select all in function” and “Select all in scope” that I didn’t have any luck in making work at all; a little more documentation would be nice.
That’s worth an aside. Panic has created a “library” of tech note-style articles about Nova sorted by publication date rather than an actual manual, and it’s not always easy to find the information you want in it. I know this is just what a technical writer would say, but I’d dearly like to see a human-organized table of contents starting with the editor basics and moving to advanced topics like version control, server publishing and extension authoring.
The Zen of Language Servers
A lot of Visual Studio Code’s smarts depend on the implementation of a “language server” behind the scenes: language servers offer almost spookily intelligent completion. For instance, take this PHP snippet:

If you have the Intelephense PHP language server plugin, Code understands that $response is an instance of Response and, after you type the > above, offers completions of method names from the Response class.
Right now, Nova’s mostly limited to the language servers Panic provides, and they’re… not always so smart. In that snippet above, Nova starts by offering completions of, apparently, everything in the open project, starting with the variables. If I type “s,” it narrows things down to methods that begin with “s,” but it’s all methods that start with “s” rather than just the methods from Response. The “Jump to Definition” command shows a similar lack of context; if I highlight a method name that’s defined in multiple places, Nova shows me a popup menu and prompts me to choose which one to jump to, rather than introspecting the code to make that decision itself.
But, this is a solvable problem: there’s (I think) no reason someone couldn’t write an Inteliphense plugin for Nova. If Nova’s ecosystem takes off, it could be pretty formidable pretty quickly.
Walk like a Mac
Even so, LSP support isn’t Panic’s biggest selling point. Unlike Sublime Text or VS Code, Nova isn’t cross-platform: it’s a Mac-only program written to core platform APIs. Is that still a huge draw in 2020? (Is it instead a drawback?)
You can definitely see a difference between Nova and BBEdit on one side and Sublime and Code on the other in terms of resource usage. With the two Ruby files shown in the screenshot above loaded, I get:

Sublime Text: 338 MB, 2 processes
Nova: 101 MB, 2 processes
BBEdit: 97 MB, 1 process
Code is an Electron-based program, although Microsoft famously puts a lot of effort into making it not feel like the black hole a lot of Electron-based apps are. Sublime uses its own proprietary cross-platform framework. In fairness, while us nerds like to harp on research usage a lot, if your computer’s got 16G or more of RAM in it, this probably isn’t a big deal.
You notice Nova’s essential Mac-ness in other ways. Its preference pane is, like BBEdit’s, an actual preference pane, instead of opening in another tab like Code or just opening a JSON file in a new tab (!) like Sublime. And while all editors better have first-class keyboard support—and Nova does—a good Mac editor should have first-class mouse support, too, and it does. You notice that in the drag-and-drop support for creating new tabs and splits. Nova’s sidebar is also highly customizable, possibly more so than any editor I’ve regularly used. (Yes, Emacs fans, I know you can write all of Nova in Lisp if you want. When one of you does that, please get back to me.)
Unlike BBEdit, though, Nova doesn’t have a Mac-like title bar, or a Mac-like outline view of the project files, or Mac-like tabs. (Well, BBEdit doesn’t have tabs at all, which turns out to be a great UI decision once you have a dozen or more files open, but never mind.) This isn’t necessarily bad; people often say BBEdit “looks old,” and it’s hard not to suspect that what people mean by that—whether or not they know it—is that it looks like the long-established Mac program it is. Nova is relying less on “we have a Mac UI and the other guys don’t” than on “we have Panic’s designers and the other guys don’t.” Make no mistake, having Panic’s designers counts for a lot.
What may be more disappointing to old school Mac nerds is AppleScript support: none whatsoever. It doesn’t even have a vestigial script dictionary. Again, this may not be something most people care much about; personally, I hate having to write AppleScript. But I love being able to write AppleScript. BBEdit’s extensive scriptability is one of its hidden strengths. Nova’s Node-based JavaScript engine is probably more powerful for its own extensions and certainly more accessible to anyone under the age of 50, but it may be hard to call it from external programs.
So is it worth it?
That probably depends on where you’re coming from.
If you loved—or still use—Panic’s older editor, Coda, this is a no-brainer upgrade. If you used Espresso, a Coda-ish editor that always seemed to be on the verge of greatness without ever reaching it, Nova may also be a no-brainer for you.
If you’re a fan of Sublime Text, BBEdit, TextMate, or another editor that doesn’t have native Language Server Protocol support, you should definitely try Nova. Sublime and TextMate have more plugins (especially Sublime), but many extensions seem to be languishing (especially TextMate). BBEdit never had a great extension ecosystem to start with. All of these editors have strengths Nova doesn’t, but the reverse is also true, and Nova may catch up.
If you’re an Emacs or Vim power user, we both know you’re just reading this out of academic interest and you’re not going to switch. C’mon.
If you use Visual Studio Code, though, it’s way tougher to make the case for Nova. Code has a vastly larger extension library. It has the best support for LSP of any editor out there (LSP was developed for Code). Despite being Electron-based, it’s pretty high-performance. Code doesn’t have an integrated SSH or FTP client, but it does have an integrated terminal and task runner and Git client. If you don’t object to using an editor that isn’t a “perfect fit” with the Mac UI, Code is very, very good… and it’s free.
I don’t object to Nova’s pricing model—$99 up front including a year of updates, $49 for future years of updates—but I can’t help but wonder if Panic should have gone with super aggressive introductory pricing. Also, I saw more than a few suggestions on Hacker News about how there should be a Code-to-Nova extension translator; I’m not sure automatic conversion would be practical, but a guide on manual conversion seems like an excellent idea.
For my day job of technical writing, I’m going to stick to BBEdit. (One day I’ll write up an article about why I think it’s the best “documentation as code” editor on the market.) For programming and web editing, when I was working on both a Ruby and a PHP project—the former a Rails learning exercise, the latter an obstinate “I am going to write a modern PHP app without using a framework” exercise—I kept trying Nova’s betas and then switching back to Code for Inteliphense and, I swear to God, MacVim for Tim Pope’s amazing rails.vim plugin. I suspect Nova could duplicate both of those, but I’m not sure I want to be the one to do it. (Also, while Panic has decent reference documentation for writing extensions, I’d like to see a few simple end-to-end walkthroughs for those of us who look at a huge list of reference topics and don’t know where to start.)
But Nova isn’t just pretty, it’s powerful, and has a lot of promise. The editors I’ve been comparing it to have been around since 2015 for VS Code, 2008 for Sublime Text, and 1992 (!) for BBEdit; it’s not reasonable to expect Nova to blow past them in every respect right out of the starting gate. Even so, they are Nova’s competition. Catching up fast is an essential requirement.
So: yes, I’ve bought Nova, and I’m rooting for Panic here. I’ll come back in a year and report if I’m willing to stay on the update train.

Do you ever get that feeling like you're supposed to be doing something else, but you’re just too stubborn to acknowledge it? I always do. But I'm slow in processing signs and was too scared to try different things. So when a hiking club was formed in my company that I've always wanted to establish or at least be a part of, I chickened out. Instead, I joined a more comfortable club: the book club.
I enjoy reading, discussing books with people, and recommending books, but the club I joined in wasn't in any way what I've expected it to be. I still tried to help make changes and move it towards a more stable direction, though, but I still can't quite feel the belongingness I should get from being a part of a group that does everything about what I'm infatuated to. All the while, I've been feeling a great and heavy connection to the email blasts of the newly established hiking club inviting other employees to join their climbs.

On November 2018, I decided to participate in one of their climbs. TUCLAS (TaskUs Climbing Association) facilitated a climb for a cause to Gulugod Baboy and a part of the fee the participants have paid for was allotted for the benefit of the Little Angels Home, Inc. Foundation. I did what I could to persuade my friends to climb with us, but no one would want to come with me except for a friend who has already hiked Gulugod Baboy. So needless to say, I went on my own. This is by far the only time I've tried being a lone joiner in a travel adventure, but I'd definitely do it again if I have to go back in time.
We met all the participants around 2:30 AM and waited for the others to come until 3:00 AM. You see, every one already knows every body. I remember how I nervously walked from the terminal to our meet-up place thinking of good ways to approach someone. The stupid little dork that I am stood a few feet away from those that I'm supposed to climb with who have already formed themselves in a semi-circle, tapping my phone- pretending I'm texting someone, thinking this wouldn't make me look awkward. But it did. Eventually, I mustered the courage to approach. I've decided to talk to someone and blurt out the lines I've been trying to memorize while I tell my inner self that I can do it and that I will make friends; I just have to start it right. Before I could even speak, the organizer noticed me walking towards them and asked if I've already filled out the attendance sheet. My enthusiastic greeting line faded and I only came up with, "not yet."
I filled out the sheet feeling like a dumb-ass, and went to being a tree trunk again; away from everybody. I wouldn't have been this awkward, I thought, had I attended the pre-climb meeting where I would have met all the participants before the actual date of the hike. It's basically a briefing and an opportunity to get to know everybody, and I failed to attend that. So I had to suffer the consequences and be left out of place feeling jealous of them having someone to laugh with. I was full of self-pity, but I decided to use this as an opportunity to learn how to make friends with strangers (which I'm terrible at), and enjoy how things will unfold (even when I'm surrounded by people I don't know and can't seem to connect with).

The road trip was basically silent. We're all tired and some of us went straight from their shifts. We arrived at the junction around 6 AM. There, we did our prayers, stretching, the negotiations with the coordinators and there, we also discussed who would form the head, mid, and the tail of the group. I was supposed to be a part of the mid, but I was too fast for it (since I don't have anyone to talk to and no one to distract me) so I eventually transitioned to head. We were going too fast so there were a lot of stops to accommodate those that were being left behind. There were a few times when we had to stop to treat those who felt nauseous and dizzy. The org. officers are really efficient in those times. They're very caring and flexible to go back and forth, to check up on the others. Sometimes I wonder if they were able to maximize the hike while carrying those responsibilities.

The hike was still fun. I allowed myself to be present in that moment. I allowed myself to enjoy the opportunity to be within and without. The goal changed from definitely making friends there so I wouldn't feel left out to just enjoying their company without feeling the need to be a part of their circle. I felt like a spectator, but not as detached. Weird, but oddly satisfying. I got to observe my body as I go through the entire experience. My mind was thinking non-stop, but it's filled with good thoughts unlike the usual. I get to practice listening without the intent to respond, but to understand. Most importantly, I've learned how to turn this trip from a sad and uncomfortable one to a fruitful one without having to try too much.

After a few hours, we finally reached camp. I've heard that there were 3 peaks at Gulugod Baboy (from SE to NW: Gulugod-Baboy, Gitna, and Pinagbanderahan). We required ourselves to visit two, I guess. Those peaks were where we took our group photos. We then stayed at the sari-sari store near the summit so that everyone can get their breakfast. We rested there for a while. The owner said she has tableas for sale which many of us bought for souvenir. I didn't. Our hike was along the days of petsa de peligro so reasonably, I didn't spend more than I'm supposed to. After that, we went back down and stayed at the Philpan Dive Resort. There were some miscommunication between the officers and the coordinators there so we had to shell out a hundred peso on top of the Php 600 we paid for- all to get us a cottage. Some of us went to the lomihan first before staying by the beach for lunch.
Before leaving the place, we did a game which wasn't planned at all. I recommend Pinoy Henyo at any team building or group trips. It’s hilarious! After the game, we did a post-climb meeting. The organizers thanked us for participating, cooperating and for being understanding. They then asked each one of us the things we could say about the hike- the pros and the cons. Only a handful of people mentioned the cons of the trip. Someone said that it would have been better if there was an activity that will let us to get to know each other better. Someone said that there was a little problem when it comes to money management- that they should have foreseen the conflicts so they could have been more prepared for it. The officers took it nicely and reassured us that this will be practiced in the coming hikes.

Many among us mentioned how nice it was to have a sound trip while hiking. We all agree as well that the officers were really responsible. Some have commended one of them for constantly checking up on those being left behind. I commended all of the organizers for practicing what they preach. See, when they announced that there will be a pre-climb meeting, they've sent out a presentation with the agenda of the meeting being included there. They said that we should prepare our own first-aid kit (I didn't), and when someone got wounded after the hike, two or three from them immediately went for their own kits, and I think that is something to be recognized.
I thought we'd leave after lunch, but we stayed there laughing the hours away until the sun set. There were still a few moments where I go, "fuck it. I just wanna go home," but I reminded myself to find something to be grateful for. Luckily, I hiked with funny people. Luckily, the sunset was so good, and that I get to experience a few moments of stillness. I think I needed this, but I wasn't aware or was too stubborn to notice. There have been many things going on around us and within us, but we don't understand what those are for until the very thing everything happened for comes. There are many things knocking at our doors, but when we’re too caught up with other things, we choose not to listen. And maybe sometimes we don’t have to, but the gut feeling lingers there and it stays at the back of your head, telling you something.
Maybe I needed to experience how to be alone again in a crowd and be okay with it. Maybe I needed to learn how to go along with a situation without the intention of controlling it to my preference. Maybe I needed to learn not to crave fitting in. Maybe this experience will prepare me for something in the future. I don’t know. I don't think we'll ever fully understand how this world works. I don’t think we’ll ever be certain on what doors to open and when to answer a knock, but we always have a choice on how to look at things. I chose to look at things as opportunities to learn. I chose to look at things with gratitude. I hope you do too.

Hip Hop Music - Hip Hop Is Alive
What has happened to hip hop music? I have seen a disgraceful turn for the worse on my tv screen, but let's review some real hip hop music. Fakahiphop.com
We will stick to only albums dropped recently. Of course, I highly recommend of all of these albums. I'm not going to criticize any of the albums. Simply because there is already a million people who have done so - with or without justification.
Let's get started with the "Conscience Rapper" Common. I wish we didn't have to use that particular terminology to categorize the rapper because all rappers should be conscience. It is basically the definition of hip-hop.
Common - Finding Forever
From the incredible "unforgivable" all the way to the radio smash hits, "The People" and "The Game" (no similarity to the west coast rapper at all) ", this album is worth your purchase.
Common puts together rhymes like a puzzle. Each piece being essential as the last. He uses intricate rhyme schemes, but still gets his point across in the most immaculate manner.
Common exchanges punchlines with Kayne West on "Southside". My favorite collaboration between the two; up until this point. Kayne also produces this track as he does most of the album. West spits at the top of his game while spitting lyrics e.g. "I know you must be thinking its armorall flow because it never gets rusty". Common on the other hand spits amazing lyrics himself, for example: "Your career was a typo, mine was written like a haiku."
Though Common never comes out and blames the south for hip-hops decline ( that he mentions a multitude of times) the following lyric from "Start the Show" could be subject to misinterpretation, "You sing along wit it, inside you knowin it's wack, Young who? I don't need a openin act". Common never backs down from a fight. He also mentions the lack of hip-hop spirit in the song "The Game".
Is this Common's best work? I don't know. I personally like it better then his last album. Be had a completely different ambience to it though. I guess, we should give credit to Kayne.

Talib Kweli - Ear Drum
Talib's recent effort has sixteen tracks that will fulfill any fans need for another album from the rap superstar.
He spits some of his most intellectual and deep thinking lyrics to date in "Ear Drum".
Electrify one of the most soulful tracks on "Ear Drum" is perfectly titled as it does electrify you. Especially your mind as most of the album does.
The guests help the momentum of the album more than they hurt it. Demonstrated in songs like "Hostile Gospel" with Bun B and Pimp C, who seem to fit in very well in the banger by Just Blaze.
If Talib had to take it back to square one. I'm talking about the hip-hop classic "Reflection Eternal". He definitely took a few paces in the right direction.
I'll finish the review by saying that the album isn't perfect, but perfection is in the beholder.
Brother Ali - The Undisputed Truth
Brother Ali probably would get his mic cut off by Bill O Reilly with his very open minded views on America. Actually more times than less, pessimistic views.
A "Letter From The Government" basically denounces the entire war. Ali blatantly states in the chorus, "I got a letter from the government the other day I opened and read and burned that man, The way that I live don't concern that man, We gon' have to settle this another way". I guess he might also be talking about any possibility of a draft and his reaction. Maybe just a letter asking him to join the military. Nevertheless, one of the most intriguing songs that has been released this year.
The underground artist doesn't stop there with his attitude towards America. Yes, his home country. On the track "Freedom ain't free". He annihilates our foreign policies. Especially when it comes to people who are starving in other countries. "So I use blood and sweat to butter my bread, Cos this cold world couldn't give a fuck if I'm fed".
If you are a liberal, you will probably take well to this album. He doesn't like the war, he doesn't like our foreign policies, and he doesn't like how Muslims are being treated. He claims to be a devout Muslim in a few tracks.
If this album doesn't make you sit back and think. You might want to get your brain examined.

Could Helicobacter Pylori Infection Be A Risk Factor for Hyperemesis Gravidarum

Summary
Background: Hyperemesis gravidarum may be severe enough to affect the patient physically and psychologically. Previous studies indicated a relationship between Helicobacter pylori and hyperemesis gravidarum. The aim of this study is to evaluate if Helicobacter pylori infection has a possible etiological role in hyperemesis gravidarum.
Methods: The study was a case control study which was conducted at the obstetrics and gynecology department, Al saidy hospitals Makka and included 120 patients pregnant between 7 and 16 weeks and presented with hyperemesis gravidarum. Patients underwent a stool analysis for detecting Helicobacter pylori antigen and according to the test results were included in either case group (antigen positive, 60 patients) or control group (antigen negative, 60 patients).
Results: The cases group showed ketonuria in 22 (36.7%) patients, protinuria in 34 (56.7%) patients and urinary tract infection in 26 (43.3%) patients. The control group showed ketonuria in 4 (6.67%) patients, protinuria in 18 (30%) patients and urinary tract infection in 8 (13.3%) patients and these differences were all significant statistically.
Conclusions: Helicobacter pylori infection possibly contributes to the etiology of hyperemesis gravidarum. Also, the severity of the disease seems strongly related to Helicobacter pylori infection.

Background
Nausea and vomiting during pregnancy are the most common conditions affecting pregnancy, occurring in about 80% of all pregnancies and commonly disappears by 16 to 18 weeks of gestation. Hyperemesis gravidarum is the most severe form of nausea and vomiting in pregnancy, it is accompanied by starvation, dehydration, acidosis, alkalosis from hydrochloric acid loss in the vomits, hypokalemia, weight loss and transient hepatic dysfunction [1]. Many hypotheses have been published to explain the etiology and pathophysiology of hyperemesis gravidarum including infections, gastrointestinal tract dysfunctions, endocrinal changes, anatomical, metabolic and immunological factors. However, no single theory is enough to give an adequate explanation for all the properties of hyperemesis gravidarum. Some studies done on patients with hyperemesis gravidarum indicated a relationship between Helicobacter pylori and hyperemesis gravidarum, in addition multiple gastrointestinal and extra digestive pathologies may be associated with Helicobacter pylori infection [2].
Helicobacter pylori is one of the most common bacteria affecting humans. It is a gram-negative helix-shaped microaerophilic bacterium transmitted by the oro oral or feco oral route. It is more prevalent in developing countries and young children. Types of diagnostic methods currently used include invasive and non- invasive methods for detection of Helicobacter pylori bacteria. The more widely accepted methods by the patients are the non-invasive methods, such as stool antigen detection, serum antibody detection and carbon labeled urea breath test. The investigation of choice for Helicobacter pylori in pregnancy is the stool antigen detection test [3].
The consequences of activated latent Helicobacter pylori could have an impact not only on the mother’s health (organ injury and nutritional deficiency), but also on her infant (malformations and inta-uterine growth restriction). Gastrointestinal manifestations during pregnancy, maternal anemia and fetal neural tube defect due to decreased iron and vitamin B12, are indications for investigation for Helicobacter pylori infection [4].
Most studies conducted on the correlation between pregnancy related disorders of gastrointestinal tract and Helicobacter pylori infection was cross sectional investigations in which the pregnant female was detected to be Helicobacter pylori positive during pregnancy or soon after delivery. However, there is no follow up study was conducted to explain the complete immune response against helicobacter pylori. One prospective study tested the presence of Helicobacter pylori infection before conception and showed that early pregnancy loss was associated with the presence of positive Helicobacter pylori before intra-cytoplasm sperm injection [5].
The aim of this study is to evaluate if Helicobacter pylori infection has a possible etiological role in hyperemesis gravidarum.
Methods
This study was a case control study which was conducted at the obstetrics and gynecology department, Al saidy hospitals Makka during the period from July 2017 to April 2019. The study was approved by the medical ethical committee and an informed verbal and written consent was obtained from all patients before inclusion.
The study included 120 pregnant women between 7- and 16-weeks gestational age with singleton pregnancy and presented with severe intractable vomiting more than 3 times per day and subjective weight loss as described by the patient and / or her relatives. The exclusion criteria were: 1) history of vomiting not related to pregnancy, 2) history of gastric troubles not related to pregnancy, 3) cases suspected to have acute condition which may be associated with vomiting e.g. appendicitis, intestinal obstruction, 4) multiple gestation, 5) associated disease e.g. hepatobiliary disorders, diabetes and 6) psychological disorders.
After consenting, patients underwent stool analysis for detecting HelicobacterpPylori antigen in stool (specific test) and according to the test results they were included in either the study group which included 60 patients with positive Helicobacter pylori antigen or the control group which included 60 patients with negative Helicobacter pylori antigen.
Then all patients underwent full history taking, thorough clinical examination and an ultrasound assessment to confirm dating, viability and placental formation. Laboratory investigations included: 1) Urinary sample to test for ketonuria, proteinuria and urinary tract infection. 2) Blood sample for routine laboratory investigations including complete blood picture to see (hemoglobin levels, total leukocytic count, platelet count), blood creatinine, aspartate transaminase (AST), alanine transaminase (ALT) and electrolytes (Na and K).
Sample size calculation
Sample size was calculated using G* power program and a sample size of 120 patients in both groups will achieve 90% power with 95% confidence interval for frequency of Helicobacter pylori of 88% in pregnant patients with HG as previously reported [6].
Statistical analysis
Data was collected throughout history, basic clinical examination, laboratory investigations and then it was coded, entered and analyzed using Microsoft Excel software. Data were then imported into Statistical Package for the Social Sciences (SPSS version 20.0) software for analysis. According to the type of data qualitative represent as number and percentage, quantitative continues group represent by mean ± SD, the following tests were used to test differences for significance; Differences between frequencies (qualitative variables) and percentages in groups were compared by Chi-square test. Differences between parametric quantitative independent groups by t test. P value was set at <0.05 for significant results &<0.001 for high significant result.
Results
This study included 120 pregnant females between 7- and 16-weeks gestational age presented with hyperemesis gravidarum who were equally divided into two groups according to presence of Helicobacter pylori antigen in stool. The age of the included patients ranged between 20-35 years and there was no statistically significant difference between both groups. The body mass index in the study group ranged from 21 to 29.1 with a mean value of 25.29±2.34 and in the control group it ranged from 21.1 to 29.8 with a mean value of 25.27±2.90 which was not significant. Gestational age in study group ranged from 7 to16 weeks with a mean value of 12.33±4.32 weeks while in the control group gestational age ranged from 7 to 15 weeks with a mean value of 12.07±4.05 weeks.
Thirty patients (50%) in the study group were rural residents compared to 24 patients (40%) in the control group and this difference was not significant (p>0.05). Table 1 shows comparison between the two studied groups regarding occupation and the difference between both groups was also not significant.
Routine laboratory investigations included hemoglobin levels, total leukocytic count, platelet count, blood creatinine, AST, ALT and serum electrolytes (Na and K) and the difference in these tests was not significant between the study and control groups.
Regarding urine analysis (Table 2), ketonuria was found in 22 (36.7%) patients in the study group compared to 8 (13.3%) patients in the control group which was significant (p = 0.037). proteinuria was found in 34 (56.7%) patients in the study group versus 18 (30%) patients in the control group which was also significant (p = 0.037). urinary tract infection was diagnosed in 26 (43.3%) patients in the study group compared to 8 (13.3%) patients among controls (p = 0.01).
