Strategies for Reducing Failures
Why do failures happen?
All production equipment is subjected to stress of one type or another. It may be operational (mechanical or electrical) stress applied in order to make the equipment work, or environmental stress (temperature, humidity, vibration etc.) coming from its surroundings. Equipment breaks down when excessive stress combines with insufficient strength (the equipment may have been too weak to begin with, or allowed to weaken through deterioration). It can be caused by one or more of the following five factors:
Failure to maintain basic conditions
Failure to observe correct operating conditions
Failure to reverse deterioration
Failure to correct design weaknesses
Lack of skill
All of these are human responsibilities. Equipment doesn't want to break down; people make it break down, so only people who can prevent it from breaking down, and they can only do so if they change their attitudes and behaviours. Slide summarises the causes of different types of failure.

Process Modeling
I am always intrigued to look back and see how off my perceptions and expectations were in September 2015 when first embarking on this journey.  It is often humorous how superficial my understanding of BI was.  I was especially intrigued to look at this month’s synopsis as I assumed it would be extremely entry-level.  Much to my surprise, it was very accurate.  
I am impressed with the depth of decision making theory and process modeling covered in the course.  My initial expectation rested in BI systems supporting process modeling, the outcome is much more impressive.  The breadth of applications for the various modeling techniques coupled with the knowledge of theory unleashes the potential to truly break down all aspects of business for optimization and scalability.
 Research on PCSM software will need to be completed outside of the course as this was not covered. Nonetheless, the course did forego exploration of new practical realities for application. This course has strengthened the potential connections for my B.S. Psychology and M.S. Business Intelligence through fuzzy logic applications and sparked interest in HR Analytics. I have joined the HRPS community and am considering other I/O Psychology applications under this umbrella also.  
 I was thinking yesterday that previous co-workers of mine always used to laugh that I was ‘so crazy about process,’ my bosses would roll their eyes at my insistent need for “process and procedure”. I thought it was just my analytical nature that I wanted to be consistent and I seemed to be the only one concerned with this.  I am so relieved to know that I can pour this organic idiosyncrasy of mine is a field, one that I so happened to fall upon in my current pursuits.

Forgetting, Forgotten, Forgot

How happy is the blameless vestal's lot!
The world forgetting, by the world forgot.
Eternal sunshine of the spotless mind!
    Many people wish to be able to forget their worries and sad memories. They feel that those bad memories will only bring them down. The movie Eternal Sunshine of a Spotless Mind shows a different reality where people can selectively delete memories. 
    Lacuna Inc. is one of the companies that offer services that can delete certain memories of a person of another person. In the movie, after learning that his ex-girlfriend (Clementine) has deleted him from her memories, Joel Barish went to lacuna to avail their service. There are steps to be able to remove a person from the memories of another person. First, the patient should retrieve all the things that may remind him of that other person. These includes photographs, journal entries, gifts, and clothes that the person being removed owns or gave to the patient. Second, the items collected are used in a process called mapping. The doctors create a map of the patients brain and when the objects are shown, certain points in the map of the brain lights up.This shows the parts of the brain that experiences most activity and shows where the memories of the other person is stored.Third, the map is used to guide the doctors to know where the memories should be deleted.The memories are deleted while the patient is sedated so that there will be no problems. Lastly, after the processes are done, the patients wake up as if nothing happened. All memories of the other person would have been deleted upon waking. 
   In the movie, however, Joel realizes that he doesn't want to delete Clementine from his memories after all. Unfortunately he realizes this in the middle of the process. There was no stopping the doctors from deleting Clementine because Joel cannot move because he was sedated. Like Joel, I too feel that i want to forget things. I want to forget all the bad things and i want the good memories to remain. It was easier to live having forgotten all the bad things that has happened. Joel and i have the same thought that if we forgot about the bad things, we would be happy. But now I think that it's wrong to forget. There will be always bad things and I think that it is sadder to have forgotten the good along with the bad. Joel realized that and made attempts to be able to not forget about Clementine. I guess he realized that there will be always bad things and we must take that with the good things. We should just learn to be able to be content with the good things and take the bad things in a stride. Unlike what the poem is saying, there is no happiness in forgetting, only emptiness and sadness. There is happiness in beng able to appreciate the good things while learning to deal with the bad things.
