Happy New Year! I know 2020 was tough on many of us, and I truly hope 2021 is kinder to us than its older sibling. This image was taken three years ago today. On this day, the temperature was a frigid -14ºF with a -24º windchill. The waters of Casco Bay and Portland Harbor were steaming- what we call sea smoke- because the water temperature was warmer than the air temperature by a lot. Pancake ice had formed in the harbor near Spring Point Ledge lighthouse. My wife, who I had only started dating 2 months earlier, agreed to bundle up with me to shoot sunrise. Her hair literally turned to ice from her breath! I spent time climbing around the rocks at Portland Head Light, and then asked if she minded if we made a stop at Spring Point. She agreed, but stayed in the car while I stepped out onto the ice to capture this shot of Spring Point Lighthouse shrouded in fog. Rating Methodology

Health Inspections Every year, the government assigns inspectors to conduct a formal review of nursing homes for regulatory purposes to meet the mandates outlined for Medicare and Medicaid, this aims to measure and improve the safety of residents across providers. Facilities may also be inspected when complaints are submitted or based on a reported incident. When noncompliance is identified, the facility is served a citation that indicates which regulation that was identified, along with the severity of the incident. Nursing homes are subsequently required to execute a program of resolution in order to meet compliance. Some scenarios require enforcement actions to be applied, such as a civil monetary penalty or withholding of payment(s), to incentivize resolution in a timely manner. Penalties Facilities are applied 2 types of penalties due to non-compliance / accumulation of incidents. Civil penalties are monetary fines that may be applied to a facility based on citations / infractions identified during a review. The severity of a penalty is defined primarily by the size and frequency of the infraction. Quality of Residence Care There are 3 types of resident care ratings, but for this exercise, we primarily focused on the overall quality measure rating. The quality measures (QMs) include 17 data points that are derived from clinical information reported by the respective nursing home and also from Medicare claims data submitted for payment. Ratings are calculated for the QM domain using the 4 most recent quarters for which data are available. A nursing home receives points contingent on performance on each measure (weighting distribution is not equal). Staffing Staffing research is submitted regularly by the facility and is adjusted for the requirement of the facilities residents. For each of registered nurse staff and total staffing, a 1 - 5 rating is applied according to definitions established for each category. These ratings are subsequently combined to assign an overall staffing rating. As an example, to get an overall staffing rating of 5 stars, nursing homes must earn a rating of 5 stars for both registered nurses and total staffing. Nursing homes could also be assigned a 1 star rating should they not have a registered nurse on-site daily, and do not submit staffing data, or which the data cannot be verified.

A Retirement Community That Comes to You

Traditional C.C.R.C’s operate under an assortment of contractual arrangements. Some have high buy-in fees, refundable to varying degrees after a resident’s death; others function more like rentals. Depending on luxury and geography, they tend to serve seniors who are financially comfortable.

Often, residents sell their houses to pay entrance fees that average $107,000 to $427,000, according to a report from LeadingAge, the trade association representing nonprofit senior care providers. (LeadingAge has rebranded these entities “life plan communities.”) Monthly fees range from $2,100 to $4,200.

“It’s a great solution for people who either have means or good retirement plans, some wealth built up,” said Ruth Katz, senior vice president of public policy and advocacy at LeadingAge.

So far, the at-home programs carry lower price tags, though members still pay for housing and other living costs. At Senior Choice at Home in Florida, Mr. Ahmadi said, a 75-year-old would probably pay $55,000 to $60,000 in entrance fees and about $525 a month.

At Springpoint Choice, which has about 270 members in New Jersey and Delaware, initial fees run $30,000 to $65,000, with monthly charges of $300 to $500. All the fees are tax deductible.

“If in a year they have a life-changing event, they could be paying $400 a month for skilled nursing, which on the East Coast typically costs $13,000 a month,” Ms. Laidman said.

Ms. Basso joined Springpoint Choice at a bargain rate. Because she has good long-term care insurance, her entrance fee was a discounted $25,790; she paid it with the sale of her New Jersey house and her parents’ condo. Her $128 monthly fee has since increased to $146.
