What Is DeQuervain's Syndrome?
Mothers and fathers get this thumb injury from picking up their babies... Gamers from their controllers... Cell phone users from too much texting – And let's not forget percussionists, golfers and anglers!
You might think it's just a SIMPLE case of Tendonitis but it's actually a slightly more complicated tendon syndrome known as a 'Tenosynovitis.'
In the case of DeQuervain's, one that involves two thumb tendons and the special wrappers called 'Synovial Sheaths' around them where they cross the wrist.
TENO refers to the tendon – (Tendons are the "ropes" which connect muscles to bones)
SYNOV refers to the Synovial Sheaths – (which are special protective tendon sleeves) and
ITIS had to do with irritation, inflammation and pain – (although inflammation is not the primary issue and is often wrongly and unfairly blamed!)

'Smartphone Thumb' Becoming a Big Issue in the US
According to researchers at the Mayo Clinic, smartphone thumb causes joints in the hand to get loose, and the abnormal thumb bone movement could be causing pain and eventual arthritis.
TennisElbowClassroom's insight:
'Smartphone Thumb' is caused by the repetitive thumb motions of typing on a smartphone keyboard and is usually, technically, De Quervain's Tenosynovitis - but it can also sometimes be a more serious Osteoarthritis thumb joint problem.

What Is Mommy Thumb or Mother's Wrist?

If you’re a new parent or you care for an infant, you could be at risk for a type of tendonitis called mommy thumb. It can be painful, but it’s treatable.
TennisElbowClassroom's insight:
New mothers are especially prone to this injury from the repetitive lifting and awkward wrist positions needed to support the newborn baby’s head while cradling and nursing.
Hormonal changes during pregnancy and after birth related to Estrogen, which tends to increase fluid retention and Relaxin, which increases connective tissue laxity may also play a role.
The common names ‘Mommy Thumb’ and 'Mother's Wrist' are often used - but, technically, the injury is known as 'De Quervain’s Tenosynovitis'

"Many tendons pass through the wrist in six anatomical tunnels called extensor tendon compartments. The NCBI notes that intersection syndrome (IS) occurs when tendons from the first compartment cross over and rub against those from the second."
TennisElbowClassroom's insight:
Intersection Syndrome (IS) is very similar to De Quervain's Tenosynovitis and can occur simultaneously.
The injury occurs where the tendons involved with De Quervain's (abductor pollicis longus and extensor pollicis brevis) in the first compartment pass obliquely over the tendons in the second compartment (which contains the extensor carpi radialis tendons).

Test Yourself for De Quervain’s Tenosynovitis
"There is a special clinical test known as Finkelstein’s test (see image), in which you make a fist with your thumb bent underneath your fingers. You then bend your wrist downwards toward your little finger. If this causes pain on the thumb side of your wrist, you likely have de Quervain Tenosynovitis."

De Quervain’s Tenosynovitis and the Extensor Retinaculum
"The tendons and their sheath lie underneath the extensor retinaculum: a fibrous band of tissue that holds the tendons of the extensor muscles in place. Thickening of the extensor retinaculum has been associated with De Quervain’s"

There are several layers of tissue involved with De Quervain’s Tenosynovitis that can become adhered and fibrotic, and the Extensor Retinaculum is the uppermost layer.
This illustration shows the retinaculum, which "holds" the tendons close to the wrist where they cross - and the tendons in their Synovial Sheaths underneath it, which is often the location of the most acute symptoms.

Jul 9, 2022 - De Quervain's tendinosis is a condition that causes pain, tenderness, and swelling along the thumb side of your wrist. The condition develops when the tendons around the base of the thumb become irritated or constricted.

Here's an excellent illustration of the two thumb tendons involved in De Quervain's Tennosynovitis (or Tendinosis).
The muscles (red) become tendons (white) and pass into their protective Synovial Sheaths (purple) and under the Extensor Retinaculum (white)
This area and just beyond it, toward the back of the thumb is where the syndrome gets painful, due to swelling, fibrosis and degenerative changes.
