Cancer in India to double every 20 years
The threat of cancer in India is growing day by day as the medical field advances, demographic factors, mutation in DNA, incompetent health services by the private sector and delayed diagnosis and treatment. In India, the cancer burden has doubled over the last 26 years. The four main cancers that are most common and pose a threat to the huge population of the country are breast cancer, cervical cancer, oral cancer, and lung cancer which together constitute 41% of cancer burden. Oral cancer is the number one cancer among males and breast cancer among females. Here is an overview of the most common cancers and their reasons in different regions of India:
·         North-east region: It has the highest cancer rate. The most common cancers are oral and stomach cancer due to tobacco and household burning of firewood.
·         West Bengal: The leading cancers are urinary bladder cancer and lung cancer due to air and water pollution.
·         South and coastal region: Due to a diet rich in salts, the people here suffer mainly from stomach cancer.
·         Goa: Due to the extreme consumption of red meat, alcohol, and tobacco, colon cancer is the most common.
·         Gujarat and Rajasthan: Most leading cancers are head and neck cancer due to tobacco and pan masala.
·         Punjab, Malwa Belt: This area has all cancers higher than average. Due to pollution and pesticide toxin in food, people here suffer from Kidney, urinary bladder and breast cancer.
As India continues to age, a recent study shows that cancer cases will double every 20 years. “People gradually move from high birth and high death state like India was in 1900 to high birth and low death and then a low birth and low death phase when deaths from infectious diseases get controlled. Life expectancy keeps on increasing and hence degenerative diseases and cancers increase,” said Mallath, who is a senior doctor at the department of digestive diseases at TMC. This is termed Epidemiological Transition Levels (ETL) by public health experts. India’s states are at different ETL stages; while Kerala is strong, Uttar Pradesh is lowest. The lower ETL countries have a stronger index of growth and higher cancer rates.
Almost 40% to 50% of cancers are due to mutations during DNA replication and the longer a person lives, the higher the chances of accumulation of random mutations. That means if a person doesn’t die at a young age due to infection or trauma, chances are that she/he will die at an older age from non-communicable disease, including cancer. Cancer in India will bear the biggest burden in the next 10-20 years, as cancer cases will double every 20 years.
For now, the only solution to this future seems early detection and prevention of cancer. Governments should do everything possible to diagnose cancer early and treat it properly. Patients suffer in the confusion between the co-responsibilities of the Center-State. The state should not abandon the cancer care system to private hospitals. There is no question that out-of-pocket spending for private care is much higher.

Indoor air pollution and its impact
We all know the harmful effects air pollution has on our health, but when we think about air pollution, we only consider the smoke from factories and cars. However, this is known as outdoor air pollution and only accounts for half-damage of air pollution. Indoor air pollution is more dangerous and harmful than outdoor air pollution due to how concentrated the air is in indoor environments. Indoor air pollution is caused when certain particles and gases that come from burning, cooking, and heating contaminate the air in indoor areas. The impact of indoor air pollution is 3.8 million deaths worldwide each year from diseases such as stroke and lung cancer.
Millions of people around the world prepare food using traditional methods – burning of woods, charcoal, coal, dungs, crop wastes on open fires. This causes indoor air pollution and people living in such poor households may suffer from serious health problems. Burning of things is not the sole reason for the harmful accumulation of toxic particles in our houses, other causes of indoor air pollution are:
·         Asbestos is six fibrous minerals that are found in coatings, paints, building materials, and ceiling and floor tiles. Asbestos is found in old houses or ancient constructed structures. Exposure to asbestos causes several cancers and diseases and it is the leading cause of indoor air pollution. It is banned in the US.
·         Another leading cause of indoor air pollution is formaldehyde which is found in paints, sealants, and wood floors.
·         Radon is a chemical element that is found in various types of bedrock and in some building materials. It can also get into walls of houses.
·         Tobacco Smoke comes from outdoor and indoor areas can also be an indoor air pollutant.
·         Many contaminants such as mildew, mold, bacteria, dust mites that grow in damp environments can be bought in from outdoor areas. Animal dander can cause various respiratory diseases.
·         The main causes of indoor air pollution are objects such as wood stoves, space heaters, and fireplaces which put out carbon monoxide as well as nitrogen dioxide.
Effects of indoor air pollution can be life-threatening and can have harmful effects on the lungs, eyes, and brain. Kids and old age people are more prone to the after-effects of indoor air pollution. The most damaging impact of indoor air pollution.
·         Lung cancer, mesothelioma
·         Asthma symptoms, throat irritation, flu
·         Defective cardiovascular system
·         Allergic reactions
·         respiratory irritation, pneumonia, bronchitis, emphysema
·         headaches
·         upper respiratory congestion
Easy Ways to Improve Indoor Air Quality:
·         Checking ingredients of all the chemical substances present in the house.
·         Have your home checked for asbestos, especially when you move into a new house.
·         Stop using gas stoves and coal burners that create open fires
·         Have your home inspected for any mold, radon, or any other harmful chemical or bacteria that may be in your home.
·         Small areas and corners of the houses must be thoroughly cleaned and it is very important to clean the house every day.

Technology is redefining healthcare diagnosis, delivery and management
We are living in a world where technology is all-pervasive. Modern-age technology is transforming the systems of almost every sector. Similarly, technology is redefining the healthcare and pharmaceutical sector, it is reshaping the way healthcare processes, diagnosis, management, finances and delivery of services are operated. Now, when we talk about modern-era health, it includes technologies like wearable, mobiles to track health status, cloud technology for administration and management, Artificial Intelligence for diagnosis and surgical operations and many more.
It is being argued that technology has made us healthier, increasing life expectancies and improving the quality of life. Even the remote areas are well connected due to mobile penetration, adoption of enabled tools for diagnosis and treatment is making healthcare delivery faster and bringing it right to the doorstep of the patients. The following are the different technology-driven tools that are making healthcare bigger and better.
Electronic health records (EHRs)
In the past, healthcare organizations had discordant information storing systems due to which sharing, transferring and security of patient records was always at risk. Now computerized, electronic health records and big data systems allow a systematic, fast and more integrated information system which makes the healthcare data secure.
Telehealth and Telemedicine
These two healthcare branches are becoming more popular, successful and accepted by the patients because they are delivering healthcare in rural and remote areas where it is needed the most and the cost benefits of these are substantial.
Remote monitoring tools
Patients are themselves monitoring their health at home through remote monitoring tools. It saves a lot of money and reduces unnecessary visits to the doctor’s office. These tools also help to digitally exchange health data during emergencies and for the patients living far away.
Wearable Technology
These are the fastest-growing health devices in the market since they allow the wearer and doctors to assess the health continuously and easily. They also alert about serious medical issues.
Genome Sequencing
This means analyzing and decoding DNAs of different organisms, which helps to obtain genomic information. Through genomic information, scientists can understand the functioning of different organisms and their bodies and organs. It is the key to unlock various treatments and personalized medicine.
These were some branches through which technology is redefining healthcare. This partnership between healthcare and technology is an important step to create a bright future for global healthcare and pharma business.

Transparency in healthcare: A crucial issue
Like every other industry, the healthcare sector has its ups and downs. While we are rapidly moving towards advancement and technical growth, there is still scope to work on other areas in this business. The healthcare industry is majorly based on trust and transparency. But what happens when officials fail to provide their patients with the data they demand to know about? Are healthcare professionals telling us everything? how much do we really know about the services we pay for?
Healthcare costs and how it affects the patient?
Healthcare cost is a controversial topic and a survey conducted by VisiQuate found that it the least transparent area in the economy. Patients mentioned that they are hit up with unexpected bills and medical procedures without a proper explanation as to why they are necessary. What is even more heartbreaking that a 2017 Gallup survey revealed that people also delayed treatments because they were worried about the costs and later it was fount that 63 percent of them needed to be administered for chronic conditions. This shows how the lack of cost transparency negatively impacts the consumer side of healthcare.
Transparency in healthcare: How it helps?
There have been multiple examples of drug scams wherein an individual was charged way out of proportion. The public is aware of exactly nothing in terms of how much money is a particular drug, service, test or procedure worth. Doctors and hospitals that provide detailed information on where and how the patient’s money is getting distributed are at an advantage, suggests the survey by VisiQuate. This is because it helps them gain credibility and a patient is more likely to recommend and visit a medical center that has their trust. Extremely high charges result in healthcare providers getting duped in bad debt as patients cannot incur such payments for the treatment they already received. They also withhold potentially important details about their health as they do not want the hospital’s fee to rise. It also makes the physician incompetent as he is not fully aware of the patient’s problem. It was also shown that medical bills bankruptcy is primarily caused due to medical bills since 2001.
How can the medical sector become more cost transparent?
Physicians should avoid referring their patients to any facility that is not explicit about their price structure. This would help avoid unforeseen charges. Another place for improvement lies in the insurance department. People should take the time to properly understand what their health policy claim covers. Then again they should not shy away from asking hard questions to those in charge to reveal what is really going on. This change needs to start and it needs to start now!

The healthcare perspective on why should movies come with a warning
On an average, a person watches about 20-30 movies annually. Many will agree that movies are something most people enjoy, be it the youth, the aged or the ill the healthy. It takes us to another world and sometimes the movie we’re watching gets us so absorbed in it that it exerts certain effects on the viewer. For example, some may get terrified because of a scary scene in a horror movie, or people may feel the rush of adrenaline caused by intense violence in the movie. Sometimes the effect is so sharp people wish it came with a warning.
Because of the technology and work that goes into making a film, the end product has a very realistic feel to it. Every regularly it has been seen that movies come with a warning regarding smoking and alcohol. But is this enough? There have been instances of people having a seizure due do movies like “Incredibles 2” and “Twilight Saga” due to the blinding light in some of the scenes. The latest Star War movie comes with a seizure warning as it is pervaded with flashing lights that may trigger people with epilepsy.
Why should movies come with a warning?
But it is not just extreme white lights that are the problem. In the hit film “Joker” there were people walking out mid-film because of the appalling level of violence the film depicted. Even though the film was not going to cause a medically ill reaction like seizures, there was a warning that was released to authorities to have police around the theatres in case of a violent outbreak by viewers. Reaction due to violence and epilepsy are adverse but there have to be other warnings too. Films should come with “trigger” warnings for films that touch mental disorders or gory incidents.
The film “Buried” could majorly set off anyone with a problem of claustrophobia. The film started in a box and ended in a box, had no other casts featuring in the movie. It was difficult for anyone to watch it without feeling suffocated, let alone those with a severe fear of closed and tight places. Similarly, there have been more instances of films depicting nausea-inducing or disturbing content without any prior warning. It may cause a lot of people to be temporarily physically sick or unsettle their mental state for an unpredictable period of time.
On the flip side movies that cause the viewer to feel extremely elated while watching it can result in a state of sadness after the movie is over. The blockbuster “Avtaar” is a specimen of this. Critics claimed that people who felt a rush of exhilaration during the movie fell into a kind of depression after it ended. This again should have come with a warning.
Movies should come with a warning as they are important because they let the viewer get prepared for the coming scene and provides them with the knowledge of they may be getting into before choosing to watch the said movie.

Specialty drugs and the challenges surrounding them
Every sector has a profit motive and it’s no surprise that the pharma business is no different. As much as client satisfaction is a driving factor, receiving good revenue as a return on investment or ROI is also a deciding element when it comes to planning business strategies. Many pharmaceutical companies have been in a state of conflict of choice between developing specialty drugs or sticking with generic ones.
Big pharma companies taking the lead:
Big pharma companies are looking onto the US specialty business with a vision to create long term growth. Sun Pharmaceutical Industries Ltd, a company dedicated to making drugs in India, takes the lead in this regard. It focuses on launching specialty drugs in dermatology and ophthalmology and its revenue earned from the global specialty market rounded up to $118 million. Other companies like Lupin, DRL, Cipla, and Aurobindo Pharma have spent heavily on specialty R&D are exploring the specialty medicines markets. They came up with complex injectables, biosimilars and other drugs to maintain their place but Sun Pharma still dominates the industry.
Challenges surrounding specialty drugs:
Disease-specific: As the name suggests, specialty drugs are used to treat specific illnesses and are made for a comparatively smaller number of clients than generic drugs.
High production cost: Their drugs require a huge amount of investment for research, production and marketing purposes. Because of the cost being so high, the revenue and profit made from these drugs may be low marginally, especially if they don’t work well in the market.
Investor’s Expectations: As mentioned above, the cost incurred to make such products are too much for the firm to carry on their own, which is why they need investors. But shareholders that are willing to put their money may have extremely high expectations from the product that they are sponsoring. Sometimes, the product due to one reason or the other may not meet the eagerness build-up for its launch leading to its fall.
Relative Profits and High risk: Though the profit margin is impressive, it is not the same for all companies. What plans work for one company may not work for the other. The winning companies are the richest ones in this area as they possess the resources to facilitate specialty R&D. For startups and rookies entering the specialty, the market seems to be a little too far stretched and complicated as they may lack the experience and knowledge required to rule this market
Thus, before diving into the specialty drug market it is important for pharmaceutical firms to weigh their pros and cons and come up with a foolproof plan. It is possible to succeed in this area but it has to be planned and executed well. With the US-based specialty drug market on a constant state of growth, it is safe to say the future perhaps holds the cure to almost all diseases.

NBE shows that 84% of foreign-educated doctors fail the FMGE test
Doctors become doctors after passing through a set of tests. And while India has limited resources to raise competent doctors, the system that evaluates them is tough. After dedicating 5-6 years of their lives to acquire this degree and spending lakhs of rupees on education, tuition and accommodation, defaulting in the FMGE test can also lead to mental disorders like depression in doctors. The damage of an inept system is not only a burden on the individual but is also felt by the entire country. With the passing system being so rigid, the number of licensed doctors are way less than the required amount. That itself has an array of negative consequences.
The struggle for medical students who have acquired their degree online does not end at completing their MBBS, in order to get the license to practice in India, they need to pass the Foreign Medical Graduate Examination or the FMGE test that is conducted by the official National Board of Examination (NBE). Only doctors attaining their education from five countries, i.e., UK, US Australia, Canada, and New Zealand are excused from the test. Statistics derived from the NBE show that 84% of foreign-educated doctors fail the FMGE test.
Graduates from different countries and their passing rates A majority of them have to enroll in medical universities abroad because India has insufficient MBBS seats in the first place. Students from France, Kenya, and Sant Marteen have a 100% passing rate and those from Uganda, Sri Lanka and Czech republic follow close behind with passing rates of 80%, 75%, and 66% respectively. Findings revealed that though 51% of the total examinees were graduates from Russia and China, they had a low passing rate. This means that very few medical graduates from these counties pass the test. The last on the list were graduates from Bulgaria, while stats show that those from Romania and Azerbaijan are barely any better. With a defaulting rate this high, the problem was brought to the attention of the Government by the Union Health Ministry.
The government’s stance One way to help the current situation is increasing the number of available MBBS seats, this will help reduce the number of students going abroad to acquire the degree. The Medical Council of India (MCI) has established guidelines that students are required to read before registering for the MBBS course in foreign countries. Further, the Medical Council of India advises foreign facilities to ensure that the students enrolling in their institutions are proficient and have enough clinical exposure. The government has also drafted the Skill Training Curriculum for those over and again failing the FMGE. While this may not be enough, it most definitely a start towards skilling Indian students to clear the FMGE test.

Are Pharma Companies set to gain profit from climate change?
The overall climate of the world has been changing with each passing day, especially in the past few decades. Regular updates of climate change such as the glacier melting, have been doing rounds for quite some time now.
It is no secret that the temperatures are soaring to newer heights and the outcome is likely to give rise to the spread of several vector-borne diseases, resulting in an increased demand for vaccinations. That means the pharmaceutical companies will need to respond to the aggravated demand for vaccines.
Let’s unravel more on how leading pharma companies are actually going to make a profit from climate change.
What do some notable health organizations say?
The World Health Organization (WHO) reported that the changes in the climate will have an “overwhelmingly negative” outcome on human health across the globe. As such, it has been already observed that the future effects of climate change are taking over almost all individuals in the world. The increasing temperatures will increase the risk of diseases and subsequently the need for more vaccinations and medicines resulting in more opportunities for the pharma companies to make and sell medicines.
For instance, as novel epidemics like the recent Coronavirus outbreak, pharma bigwigs are getting proactive in catering to the rising needs, in order to solve subsequent health crises.
Another similar report presented by Morgan Stanley states that the pharma industry is all set to reap sweeter fruits as the planet gets warmer in the coming future. In other words, epidemics and contagious outbreaks will get more prevalent, translating into significant gains for the medicine market.
How does climate change affect the spreading of diseases?
According to the CDP (formerly Carbon Disclosure Project), warmer temperatures will help lengthening the transmission period and geographical range of vector-borne diseases. In other words, tropical health complications will have more reasons and locations to survive.
The report further identifies mosquitoes Aedes Aegypti and Aedes Albopictus as carriers of fatal diseases such as yellow fever, Zika, dengue, and chikungunya. Due to their potentially larger impact than the others, the two organisms are being heavily researched upon.
Researchers predict that by 2050, around 385 to 725 million more people than today are expected to be diagnosed with infectious diseases such as Zika and chikungunya. By the time we reach 2080, the numbers would have increased more than one million.
What does this mean for the pharma industry?
The CDP further states that almost 70% of biotech, healthcare, and pharma companies already have climate change as their core of business strategies. That itself suggests that pharma businesses are expecting to rework their business plans at par with increasing temperatures and make a profit from climate change.
Let’s understand in a deeper sense of how an infectious outbreak can cause pharma companies to grow.
The selling of medications such as relevant vaccines is a crucial aspect afforded to pharma companies as a result of climate change. So much so that the researchers estimated $50 – 125 billion of incremental vaccines will be required to cater for an additional population that has been exposed to epidemic diseases.
On the other hand, the numbers could rise up to an approximate $200 billion with branded pricing of more complicated vaccines that are opted by developed countries.
The current pharma industry, which today stands at a market value of $500 billion, is set to gain new and prosperous ventures through a significantly higher sale of vaccines and other medications.
Which pharma companies are likely to gain?
The Morgan Stanley report identifies seven pharma bigwigs: GlaxoSmithKline, Sanofi, Moderna, Takeda Pharmaceutical, Merck, Janssen Pharmaceutical, and Pfizer to be more likely to profit from climate change.
Of these, Sanofi and GlaxoSmithKline are the two companies that are expected to benefit from climate change to a greater extent. As per the CDP report, both these companies have enough stocks of specialized drugs to combat tropical diseases. The development of drugs for malaria, dengue, and Zika is in process.
What can be inferred?
As we’ve known all this while, the rising temperatures are the only reason for the spread of tropical diseases – affecting millions of people worldwide. As a result, pharma companies will need to prepare themselves for supplies in larger markets. While climate change may be a big opportunity for the world’s biggest pharmaceuticals, the industry itself will also need to become less damaging in terms of their carbon footprint, as well have more accommodative pricing for its products as higher number of people in the world who may be exposed to the risk of diseases may need affordable healthcare. Climate change is probably the biggest challenge the world will continue to face, and the industry will need to do everything possible to reduce its overwhelmingly negative outcomes through the best possible efforts.

Anti-aging is fueling growth of the cosmeceutical industry
There has been an increasing amount of interest in maintaining a youthful appearance, especially by the Millenials and baby boomers. This has developed a corresponding growth of the cosmeceutical industry and anti-aging formulations. Cosmeceuticals are basically the beauty products that have biologically active and medical ingredients and therapeutic benefits. Cosmeceuticals are mainly used to treat the signs of cutaneous aging including the lack of skin surface regularity, the formation of rhagades, and the increased presence of dyspigmentation. In recent years, a plethora of beauty lotions, creams, serums, beverages, and supplements claim to decrease the visible signs of aging, mainly by improving collagen production and decreasing fine lines and wrinkles. Sunscreen is said to be the most common cosmeceutical because, over time, ultraviolet (UV) rays cause alterations in face structures due to collagen breakdown, chronic skin inflammation and biochemical changes in the skin. Some of the common types of ingredients found in cosmeceuticals are:
·         Alpha Hydroxy Acids
·         Antioxidants
·         Botanicals
·         Depigmenting Agents
·         Exfoliants
·         Moisturizers
·         Peptides
·         Retinoids
·         Sunscreens
·         Exfoliants
These are all ingredients known to stimulate collagen production, improve skin texture and decrease the appearance of fine lines and wrinkles. Some other ingredients that can be found in anti-aging cosmeceuticals include rosmarinic acid (rosemary extract), vitamin C (ascorbic acid) and tea extracts. Thus they are highly recommended to slow down aging as aging cannot be reversed, but it can be slowed. Cosmeceutical products are the best option for the prevention of aging.
As the population demands less invasive, non-surgical alternatives to slow the effects of aging on the skin, the cosmeceutical industry is becoming a big business and growing rapidly. Market research shows that the anti-aging cosmetic market in India is over 60-crore. Although this market constitutes only 2% of the country’s Rs 3,000-crore skincare market, the anti-aging segment is the fastest-growing at 93% year-on-year.
The desire to preserve youth and numerous advancements made in reversing the natural process of aging is an alluring phenomenon that is dragging all types of consumers (not only the old people). Young men and women are seen using these beauty products to avoid healthy aging. This expansion of the target market is expected to drive the growth of the cosmeceutical industry in the future. Some of the hot-selling cosmeceuticals in the market to prevent aging are P&G: Olay Total Effect, Unilever: Ponds Age Miracle, Oriflame: Anti Aging Cream, Avon products, etc.

First ever guideline on reproductive health in rheumatic disease
The American College of Rheumatology recently issued the first-ever evidence-based, clinical practice guidelines related to the management of reproductive health issues for all the patients with rheumatic diseases. Known as the 2020 Guideline for the Management of Reproductive Health in Rheumatic and Musculoskeletal Diseases (guideline on rheumatic diseases) – comprises 131 recommendations and 12 good practice statements. The 131 recommendations are graded and separated into six categories: contraception, assisted reproductive technology or fertility therapies, fertility preservation with gonadotoxic therapy, menopausal hormone replacement therapy, pregnancy assessment and management, and medication. Whereas the 12 good practice statements are ungraded and presented as suggestions because, in those, the indirect evidence is compelling enough that a formal vote was considered unnecessary.
Many of the experts involved in the development of guidelines included rheumatologists, obstetrician/gynecologists, reproductive medicine specialists, epidemiologists and patients with rheumatic diseases.
These guidelines offer assistance to rheumatologists in caring for women. Some of the notable recommendations from each category are:
Contraception
·         The guidelines specify a strong recommendation for women with rheumatic diseases who do not have lupus or APS to use effective contraceptives.
·         Women who test positive for antiphospholipid autoantibodies (APL) or APS, the recommendation against using combined estrogen-progestin contraceptives are stated.
·         Graded recommendation for fertility therapy in women with uncomplicated rheumatic disease who are receiving pregnancy-compatible medications, whose disease is stable, and who test negative for APL.
·         Conditional recommendation against increasing prednisone dosage during fertility therapy procedures in lupus patients.
Fertility Preservation
·         For men, the guidelines suggest to cryopreserve sperm before cyclophosphamide (CYC) treatment in men, if they wish, and a conditional recommendation against testosterone co-therapy in men with rheumatic disease receiving CYC.
·         For women, the guidelines state a conditional recommendation for monthly gonadotropin-releasing hormone agonist co-therapy for premenopausal women with rheumatic disease who are receiving monthly CYC injections/infusions to prevent premature ovarian insufficiency.
Menopause and Hormone Replacement Therapy
·         A strong practice suggestion to use hormone replacement therapy in postmenopausal women with rheumatic disease who do not have lupus or have a positive APL test; and who have severe vasomotor symptoms have no contraindications, and desire treatment.
·         The guidelines make a conditional recommendation for hormone replacement therapy in women with lupus and without APL.
Pregnancy Assessment and Management
·         Conditional recommendation to treat patients lupus with low-dose aspirin daily (81-100 mg) starting in the first trimester. And for women testing positive for APL who do not meet the criteria for obstetric or thrombotic APS, it is conditionally recommended to preventatively treat with daily aspirin (81-100 mg) starting early in pregnancy and continuing through delivery.
·         Strong suggestion to counsel women who are considering pregnancy about the improved maternal and fetal outcomes associated with starting pregnancy during low disease activity.
At the 2018 ACR/ARP Annual Meeting in Chicago, a draft of the guideline was presented. The guidance team has since compressed the original three-part draft into a single, descriptive manuscript, with extensive history and online discussion now available.
