Two studies “Teen Longitudinal Assessment of Bariatric Surgery (Teen-LABS)” and “Treatment Options of Type 2 Diabetes in Adolescent and Youth (TODAY)” were conducted to determine the effectiveness of bariatric surgery and medications respectively in controlling type 2 diabetes.
In Teen-LABS, a group of 63 teenagers were studied while TODAY studied 30 teenagers who were treated with medication. The results of Teen-LABS showed that teenagers who underwent the weight loss surgery had significantly lower risk of cardiovascular diseases. Although the results of TODAY also showed that medications effectively control blood glucose levels, it was found that after a two year gap, bariatric surgery showed better results in blood sugar control than medication.
Around 70% people involved in Teen-LABS study got a normal or below 5.7 A1c. On the contrary, only 10% of the participants could manage it through medication. Also, people who underwent bariatric surgery weighed around 29% less on an average as compared to the 3.7% increase in weight of people who took medications to control the condition.
According to the analysis, there were considerable improvements in kidney function, cholesterol levels, and blood pressure levels in people who went for the surgery.

Gastric Bypass Surgery
Gastric Bypass Surgery is a weight loss procedure that is being performed for more than 2 decades in some parts of the world. Over a period of time it has become a more popular with patients in India, in particular those with diabetes and other associated co morbid conditions.
Gastric Bypass Surgery is being performed over several years by Laparoscopic technique wherein three to five small holes are made for the same. By Laparoscopic instruments, a small size pouch of the stomach is made (app 30 ml to 50 ml in size), which is completely separated from rest of the stomach. This small pouch is attached to jejunum (a part of the small intestine) bypassing about 1.5 metres to about 2 meters of the initial small intestine.
The effect of this re-arrangement is a restricted intake of food and due to the bypass, some malabsorption- both of these leading to weight loss. Not only food restriction, there is some hormonal mechanism behind the weight loss as well as resolution of co morbidities. The later part of the small intestines secrete a number of hormones specially when exposed to undigested food directly in their lumen that control and modify the metabolism in our body to correct diabetes, control blood pressure and also control hunger. These hormones also increase our resting metabolic rate thereby increasing energy consumption even at rest. Roux-en- Y Gastric Bypass Surgery or the Gastric Bypass surgery the “Gold standard” among all of the Bariatric Surgical Procedures in the world.
After the surgery, most of the people can expect to lose 60 to 70% of their excess body weight over a 6 to 12 month period. There is a strict dietary follow up to promote healthy feeding habits post-surgery. There is restriction of calorie rich foods with addition of protein rich diets.

Gestational Diabetes– this type affects women during pregnancy. Some women develop gestational diabetes late in pregnancy. Some women have very high levels of glucose into their cells resulting in progressively rise of insulin. The majority with gestational diabetes patients can control their diabetes with exercise and diet, and 10% to 20% will require medications.. Undiagnosed or uncontrolled gestational diabetes can raise the risk of complications during childbirth.

What are the Causes of Diabetes?
Insufficient production of insulin (either absolutely or relative to the body’s needs), production of defective insulin, or inability of cells to use insulin properly and efficiently leads to hyperglycaemia and diabetes.
Hereditary: This is one of the factors that affect a good proportion of people affected by diabetes. If either one of the parents or both have the diabetes the chances of their offspring getting the same is quite high.
