The Euthanasia Spiel
Anonymous said:  How do you go about explaining the more unexpected aspects of euthanasias to clients who want to be in the room as their pet passes? It seems like an awkward thing to do, during a somber moment, to mention “by the way, your pet might spasm/have a bladder movement as it dies.” Is it something you bring up beforehand?
You’d be right, it is super awkward to convey to someone, in the midst of their grief, that their pet lovingly cradled in their arms may well empty its bladder upon them as it passes away, but you really do need to mention that before it happens anyway.
I basically always run through a spiel for what I’m going to do, what’s going to happen, and what it might look like. I do this with nearly every euthanasia.
It’s basically on autopilot now, which is really useful. I might be able to distance myself from my emotions most of the time to get on with what needs to happen, but every now and then I’ll be struck by this feeling of grief because this patient is special, and I don’t want them to die, even if it’s the right choice. I can just run through the spiel without exerting too much brain power.
It is worthwhile, if you are a vet student, paying attention to how experiences vets explain the events to the client and emulate a style that suits you.
This is what I usually do.
“Have you seen euthanasias/a pet put to sleep before?” Truthfully it doesn’t actually matter what they reply, I’m going to explain it all to them anyway. It might have been a decade or more since they witnessed a euthanasia and things might be different. Sometimes one member in a group (often the youngest) has never seen one, so I will address them while I explain things to everybody.
“It can be confronting, but you can stay for as much or as little as you choose to. It is a completely personal choice whether you stay.” This was added because of the guilt so many members of the general public endure after unkind internet posts trying to guilt people into staying.
“We’re going to place an intravenous catheter on a long line so your pet can sit or curl up of be held wherever they like. You can hold them if you want to.” This gives people a chance to think about where they want to be, if they’re staying.
“Once the euthanasia solution is given everything will happen fairly quickly. It’s an anaesthetic overdose, so first they will fall asleep, then because it’s an overdose they will pass away while they are asleep.” This explains basically what I am going to do in the simplest terms.
“As part of that, it is normal for them to keep their eyes open. It’s only on tv that the eyes close. They may take a few deep breaths, but it’s not them, it’s a reflex, and that can look like a gasp which is confronting if you’re not expecting it. They may also shiver, or let go of their bowels and bladder, so we will place this towel underneath them beforehand.” This is where I explain the details. Some people interrupt and say they don’t want to be present, and that’s perfectly fine. They don’t have to listen if they’re not staying.
Sometimes I will also mention disease specific things, like bloody fluid coming from the mouth in heartfailure dogs. Confronting things happen, but it’s important to pre-warn people.
Then I will ask if they had any questions, ask them about aftercare if they haven’t already decided, put the catheter in, and just give them a moment to process everything. If someone is in distress or overwhelmed, it’s best to give them simple decisions (A or Not A instead of picking from the entire alphabet). So I will ask whether they want to bury at home, or have us arrange something. Some people are holding it together enough to request what they want, others you need to lead through one choice at a time, and others just don’t process and need a couple of days to think, so I give them a list of options and ask them to tell us in a few days. Then I will leave to get everything done. This gives them a chance to process and change their mind.
When I come back I will ask if they have any questions. Then ask if they are ready. This gives them a tiny bit of control in a situation that is, by and large, beyond control.
I will tell them when I start to give the euthanasia solution, and remind them to say nice things to their pet. Say it’s been good, or that they’re going to the park, or whatever it is they want to say.
Then if anything happens, like the gasp, the emptying of the bladder, I will reiterate that ‘this is the breath I warned you about’ so that even if they’re surprised they can gather themselves quickly.
The bladder and bowel emptying often happens on a delay, so after a few moments I will ask if they are ready for me to listen to the heart, which confirms death, and let them stay as long as they like. Most don’t want to stay that long, and a careful scoop of the towel means than many don’t even realise that the pet evacuated their bladder or bowel.
What’s the alternative, anon? Not telling the pet owner that their pet may empty its bladder after death while they’re cradling it, and walking out covered in both tears and urine? Better to be up front, simple and factual before the high emotions hit so you can hand hold them through it.
