Understanding Congenital Heart Defects

Heart is the vital organ that is responsible for maintaining circulation in our body. Any disease in the heart can pose substantial risk to the life of a person. When a defect in heart is present at birth, it is called as congenital heart disease (CHD). Roughly 1 in every 100 children born in India has CHD.
CHD is not a single disease, but it is a spectrum involving a wide variety of different conditions that disrupt the normal anatomy and physiology of the heart. Sometimes it can be a simple hole in the heart with good prognosis but on the other end of the spectrum, there are children with complex defects that require extensive work up, complicated surgeries and long-term follow up.
Types of CHD
Classically, CHD is divided into two main types; Cyanotic heart disease and Acyanotic heart disease. Cyanotic heart disease refers to those diseases which causes bluish discoloration of the skin because of low oxygen in blood (so called “blue babies”). Acyanotic heart disease does not cause bluish discolouration because oxygen saturation of blood is normal even though structural defects exist in the heart. For the ease of understanding, let us breakdown the common types and their abnormal physiology below:
Hole in the heart: This is a common heart defect relatively heard by everyone but what does it mean? A hole in the heart refers to atrial or ventricular septal defects (ASD or VSD). We have four chambers in our heart; The top two chambers are called Atria, and the two chambers below are called ventricle. The wall that separates the two atria are called interatrial septum. Similarly, between ventricles there is the interventricular septum. Sometimes due to abnormal development of these walls during fetal period, a defect or a hole develops between the atria or ventricles and these are termed as septal defects or “Hole in the heart”. 
Obstructive lesions: In this type of CHD, there is obstruction to the normal blood flow in the heart or in the major blood vessels. It can be the form of narrowing of the valves inside the heart (valvular stenosis) or narrowing of blood vessels, an example of which is Coarctation of aorta where a part of aorta gets narrowed.
Cyanotic heart disease:  This includes diseases like Tetralogy of Fallot, Transposition of great arteries, Persistent Truncus arteriosus, to name a few. Low oxygen saturation of blood is mainly caused by a mixture of oxygen rich blood with deoxygenated blood due to structural abnormalities in the heart.
Why does CHD occur?
There are many factors that can increase the risk of CHD. In some cases, the exact cause is unknown. Some factors related to CHD include:

Genetics: CHD is associated with genetic disorders such as Down syndrome. It is caused by changes in some genes or chromosomes. The child born with CHD is more likely if there is a family history of the disease.
Maternal factors: The risk of baby getting CHD is more if the mother suffers from any infections during pregnancy such as Rubella. Other conditions such as diabetes and maternal risk for toxins such as alcohol or some drugs (eg: sodium valproate) can also increase the risk of CHD in the child. 
Symptoms of CHD
With the advent of technology in the health sector, CHD is diagnosed at birth for most children. In some cases, CHD can be picked up by symptoms such as poor feeding, bluish discolouration of skin, poor weight gain, swelling in legs, among others. These symptoms should prompt for further evaluation of the child.
Treatment of CHD
Based on the type and severity of CHD, a wide variety of treatments are available. In some children, minimally invasive catheter-based procedure can be done, one of which is device closure of ASD which is a hole in the heart treatment with minimal complications. For complex conditions, open heart surgery is required after careful planning. Long term follow up is a must for most children with CHD. 
As much as advances in modern medicine has enabled us to effectively treat such complex conditions, it is still an impossible task for many families in our country owing to the high cost of treatment.
