help with GeMS paperwork pls

My therapist recommeded me to GeMS at boston children's hospital after i came out to my parents last month. Today I sent her a text asking if she'd called or set anything up yet, and she said no but she would and there was a lot of paperwork we had to fill out as a family (she hasn't been overwhelmingly supportive, more just,,, tolerant). She's working out right now so I printed out the patient self-report questionnaire because that was the only paperwork (besides a family self report questionnarire for parents) that I could find on their website because if I show her that I already did my piece, maybe it'll encourage her and my dad to do theirs.

Now I'm realizing I've never filled out any medical paperwork in my life (I'm 15) and I have some questions:

-can I fill it out in pencil?

-it asks for a signature and printed name separately from legal name and chosen name,,, which one do i put as a signature/print here?

-it asks for "current gender presentation and expression",, is that if I'm socially transitioned or just how I dress/act?

-for "current gender identity" there are 2 full lines to write on but isn't the answer just male or trans guy or however you choose to word it??

-for pronouns do i need to write out the full 5 piece set or just "he/him"

-does cultural/ethnic background mean race or ethnicity? does it matter that much?

Hey, I'm Liam! I'm a 15 year old trans guy in 10th grade. I've known I was trans since middle school but only started coming out to people a couple months ago. My parents know and are somewhat supportive but don't use my name and pronouns (as of halloween 2019 at least). My therapist is working with them and me to work on coming out to my school and getting me into GeMS at boston children's hospital.
