I think the word you're specifically looking for is transmigration isekai. Isekai is pretty expansive, so there are definitely works out there that touch on this, but certainly, it's rarer to address it and even rarer to go into detail.
Sometimes the original soul is still there, and sometimes it's confirmed that the soul died, which is what Ascendance of a Bookworm does.
There's always going to be an oversaturation with lots of "junk food" isekai that take the cheap and lazy route, but there are definitely also a lot of isekai that allow the MC to keep their psychological hangups.
It wouldn't be a very entertaining story if they didn't progress at all, but a good few do mess with it and I usually appreciate when they do. Famous examples would include Mushoku Tensei, Am I Actually the Strongest, and Konosuba. I also really like The NPCs in this
I feel like wanting them to struggle to survive is a separate feature of the story to look for though.
I think what always bothered me with the idea of isekai is that it is essentially stealing another persons consciousness, and it is never touched on ever again. Like where does the owner of the previous body go to? Are they just dead? And then the protagonist has no guilt over this.
The protag also seems to not care about their previous life or family at all, and in some cases I can see why that'd make sense but like- sometimes the characters have friends they really liked in their previous life and they just straight up get over them as if it is no big deal. (Like how is it not traumatizing?)
Thirdly, I feel like while the idea of starting a new life from scratch and becoming increasingly productive is really fun I feel it is unrealistic? Like if the protag had mental issues in the previous life, why doesn't it carry onto their new life?
I also feel like alot of isekais have it where characters know everything about the body they reside in and the cultures and everything already, but I would like to see something where a character has no knowledge of the world or body what so ever.
I guess my last complaint over it is that I wish I saw more mentally ill, pathetic, struggling to survive type characters. A american protagonist for a isekai would be really fun too, but I get why a typical isekai wouldn't have that. "a connecticut yankee in king arthurs court" has this idea of a 1900's American being isekai'd essentially but thats the only example I know of.

The parallel world has at least three summoned Heroes at all times.
After being summoned from another world, a Hero is able to live in the parallel world for up to 30 years. After the time has passed or in the event that they die, they are returned to their old bodies at the exact moment they left.
To prepare for the end of their tenure, an apprentice Hero is summoned around the 25 year mark so they can mentor a successor before departing.
Because summonings can target people from anywhere and anywhen on Earth, sometimes a former Hero can meet their own mentor or peer years before they've had their initial summoning.

Superman 64.
Infamous enough, but a Superman video game done well would be dope.
Remake it as an open world sort of game, probably with better cutscenes and completely revamping the levels.
Also, instead of flying through rings, make it flying around catching falling people and destroying hazards at super speed to save everyone in 2 minutes of in-game time and only 5 seconds of real world time.
Hypothetical for you all: You have been given the resources, time, and creative control to remake a game of your choosing! However, there's a caveat... it has to be either one of the worst games of all time (stuff like Sonic '06, Bubsy 3D, ET for the Atari, etc.), or one of the worst games you, personally, have played, whichever you think would make for a more interesting answer. What game are you remaking, and do you think it could be salvaged? If so, how do you think you'd try to rework it to fix the issues? If not, why?
Tl;Dr: If you could get total creative control over a remake of the worst game you've ever played/one of the worst games of all time, what would that game be, and how would you fix it?

If I were a king, I think a mundane nice thing I'd try to do for my staff is give them nice experiences disguised as regular duties for their birthdays.
Like, my poison tester is a good dude. He makes sure I don't die from what I eat, nutritional imbalances and junk foods aside. So the least I could do is make sure to order all his favorite foods for my meal on his birthday.
And maybe I'll happen to visit the local lord over the hometown of my personal bodyguards. Official business, of course, but just to be safe, I'll bring extra guards when I do so they can work in shifts that conveniently leave their schedule open when their families aren't busy.
Hey messengers, you should go to that one really nice city on the crown's dime to deliver this message to, uh...the local bird expert. I just happen to have a question about canaries, okay?
I can't think of anything for my advisors, so I guess they're not part of this.

My roommate has cancer, and she's been refused an oncologist locally despite several refferals by multiple doctors. She's been in and out of the ER for over a month, and this might actually kill her before she receives care locally.
She has an appointment on the 16th on the other side of the country with a nationally acclaimed cancer center. She'll have housing and some support while she's there, but she needs to get a one way ticket. We have been trying to raise enough for this and prices have gone up since we started trying.
We also need help covering some general survival stuff because I've not been able to work nearly as much since this started. My other roommate has labword he needs done, we are out of two prescriptions, we are about to lose our storage unit, and we need food.
Dm me for proof or details. Anything helps.

You know...maybe it's me reading too much into it, but I can't help but feel there's still some elitist (or perhaps this is the wrong word for it?) prejudice in that "the real working man's hero" story.
From what I could gather, the crux of the argument there was "Thompson is a real working man because he started from a middle-lower class and rose to where he was while Luigi comes from money".
Can we unpack the logic there real quick? At the very least, they're hoping that the masses believe that the family you're born into define who you are more than your actions ever could.
On Luigi's side, they're hoping that the lower class have an innate resentment towards him for being rich. Considering the idea some of us ascribe to that there is no such thing as an ethical billionaire, I can somewhat see why they'd try. But of course, this tries to throw out the legitimacy of Luigi's actions and says who you're born as is who you'll ever be.
On Thompson's side, the idea is also that his actions, intentionally causing the indirect death of hundreds of thousands of people, don't matter as much as who he was born from.
In more outdated but clearer terms, it's like the article is assuming "someone of noble blood will always be a nobleman while a peasant, no matter how rich, will remain a dirty peasant". While this is obviously just an attempt to manipulate the narrative to break class solidarity, the fact that they even think this will work kinda suggests to me that this is what they believe?
NYT has been discouraging reporters from sharing photos of luigi mangione - not due to concerns for his safety but to dissuade sympathy - and refuses to post his alleged manifesto in full. and today, they published an opinion by bret stephens about how brian thompson is the "real working class hero" of this story.
reddit deleted luigi's account, which was completely innocuous and consisted mostly of him giving out advice in health subreddits. they're also deleting any post that includes his alleged manifesto and banning users for sharing it.
luigi yelled that this was "an insult to the intelligence of the american people" while he was being dragged by police and news pundits are framing it as a deranged and violent outburst. news media are picking apart details of his life to paint him as a cold-blooded and mentally ill individual. even something as innocent as playing "among us" with friends is being framed as some insidious look into an assassin's disturbed psyche, ffs.
news media are also capitalizing on luigi's supposed "bizarre and impossible to understand" politics as an obvious way to paint him as a scary individual when the guy is... a centrist, at most. whose views are similar to those of your average college-educated white guy.
multiple news media also keep harping on about how luigi comes from a rich family. an obvious attempt to break the class solidarity that's been formed around this case by continuously trying to tell us "oh he's not like you guys" while ignoring the now pretty well-documented accounts of his multiple health struggles throughout the years.
and all this, and he hasn't even been found guilty of the crime he's allegedly committed. luigi is, as of right now, still innocent until proven guilty. and the news are trying to tear him apart because they obviously fear the symbol he's becoming for low and middle class america.

I'm not one to assume how good or bad other people have it, so I'll take from the general theme here something pretty bad is going on with you as a result of the prospects of trans rights being very bad.
Whatever it is that has you in such a dark place, I hope things get better moving forward. It will be an uphill battle, but all anyone can do is push and try to survive every inch taken.
I’m sorry but if there is one thing the Tumblr left needs crucially, it’s the ability to celebrate.
I remember when marriage equality was called and there were waves of rainbows and love wins posts. When we successfully defeated Donald Trump, there was lukewarm relief, a reminder that you were only allowed one or maybe two days to celebrate and then it was back to work. That is if you were even a good person for voting Biden. We never did settle if he was better than Trump. (We did.). We didn’t celebrate student loan debt relief or any of the accomplishments of the Biden administration, or any of the times Trump was blocked, or other countries succeeding in keeping fascists out of office. Who cares if we had successes? It’s not good enough. Back to work!
And this anti-celebratory attitude stretches back to the past. On the 100th anniversary of female suffrage in America, we were reminded that not all women had the vote and so we weren’t allowed to celebrate. The only post I saw about Juneteenth was reminding us that there were enslaved people who were killed instead of freed and therefore celebrating the end of chattel slavery was wrong, and besides, we have prison labor so nothing really changed or got better and there’s nothing to celebrate anyway. Trans Day of Visibility comes with Trans Day of Remembrance so that people don’t fill the tags with hate crimes and death. So on and so forth. Nothing gets better. Nothing changes. Back to work!
So of course when we have a major setback, we fall apart and have to start frantic damage control. Frantic discourse ensues over how much people are allowed to unplug before it becomes bad and selfish. Yes, maybe you can have this one day off Mr. Cratchit but you better be here and miserable early the next morning. Like abusive bosses always insisting you squeeze out more, more, more, and any achievement is just proof you were lazy the other times and impetus for more work.
If we are never allowed to acknowledge any of our victories, how are we supposed to survive our defeats?

I gotta agree with this.
The thing the shooting gave a whole lot of people was hope. Because attention was on something that hurt everyone. Because the people normally running their grift were in a panic trying to propaganda and spin this into a culture war and power doesn't panic.
