Zandy speed-walking to your room wrapped in his blanket whenever he got nightmares wasn't anything new.

In the beginning, he felt guilty for bothering you in the middle of the night, but ever since you found out the child had bad dreams once in a while, you urged him to come find you no matter what time it was. Thankfully, they had significantly decreased since he met you, but there were still times he woke up scared and sweating, as if flames had nearly licked him. Admittedly, a part of him wished you could sleep in his room every day so it wouldn't happen, but he knew it wouldn't be fair to you.

At first, Zandy would knock on your door and wait for you to open before intruding, but eventually, you managed to convince him to just let himself in and onto your bed. During these instances, you'd wake up to the child cuddling to you, sometimes with Foxttore squished in between. It was healing for both of you. And tonight, Zandy thought as he tip-toed to your room, would be no different. Quietly twisting the doorknob and entering the room, he already felt a bit more at ease, especially at the sight of your silhouette. The boy walked to the bed and began to climb on it when suddenly a hand shot out to grab him, and he was faced with none other than fellow blood-red eyes.

Quickly, Zandy figured out the situation, when he noticed that you were actually on the other side of the bed, this person was none other than Prime Dottore which he didn't realize in the darkness, and that he had accidentally woken him up. It was only natural that Dottore would be woken up at the slightest noise - he had never been a deep sleeper.

But more importantly, the situation had turned awkward once Dottore had removed his grip from the boy, realizing it was just him. Immediately, Zandy tried to shuffle off the bed - he and Prime had been on way better terms lately, no longer being ignored, which was wholly thanks to you - but he was still hesitant to interact without you. But Dottore wasn't going to let him off that easy of course.

"... What are you doing here?" The boy's eyes nervously flitted from your sleeping body to Prime, hoping you'd wake up to get him out of the situation.

"W-Well..." Zandy's voice came out louder than he wanted, "I wanted to... to..." he then drifted off as he tried to think of a reasonable excuse. He could tell Dottore wasn't particularly impressed, moving to get up when suddenly a new voice entered the conversation.

"Zandik, what's wrong?" You yawned as you began to sit up in bed. "Usually, you aren't this noisy when you're sneaking off to go do work instead of sleep-" You stopped your sentence slowly and rubbed your eyes, as you now realized someone else was in the room too. "Zandy? Is that you?"

"Yes..."

"Another nightmare again? Poor thing, come here," you cooed, stretching your arms open. Dottore remained silent but quickly pieced together what happened. Well, it made an awful lot of sense - you somehow always managed to be soothing after experiencing the terrors of the mind. He too remembers the times he rested his chest on your chest after his own nightmares. Logically, when the scholar thinks about it, his child segment's nightmares were probably similar to his too.

However, Zandy didn't seem very certain of the idea anymore, more interested in heading out the door. You furrowed your eyebrows in confusion before you quickly remembered that Dottore was likely causing his hesitation.

"There's no need to be shy. You can stay, we don't mind," you said softly, moving to the edge of the bed and grabbing both his hands. "Right, Zandik?"

"... Go ahead. There's only a few more hours until I get up, anyway."

"See? Now come here." With only a bit of persuasion, the child was in between the two of you. Well, he was mostly attached to you while Dottore was just on the other side, but it was still sweet. You gently patted Zandy's head as you began yawning again, sleepiness still coursing through you. At that, Dottore pulled up the covers, making sure you (and the child) were warm.

"Good night, Zandy, Zandik..."

"Good night, [Name]... fa-" The word quickly softened and dried up on the boy's lips as he realized that he was still not brave enough for that yet. Zandy swiftly shut his eyes and willed his body to sleep as fast as possible.

A little while later, after Dottore was sure you both had dozed off, he sighed at the sight that was mostly covered in darkness, but he could still make out his child self's body wrapped by your larger one... as well as the rather wide and palpable gap in between his and his segment's.

Briefly turning over, the Harbinger glanced at the time. It would be time for him to return to his work soon, this break only a haven you begged to grant him. Dottore looked back at you, reaching out to caress your cheek, letting out another sigh. He moved his body closer, to where his own was only a little bit from touching Zandy's, and then wrapped a protective arm over you, and by extension, the child.

He would be gone soon enough, anyway - this wouldn't hurt for now.

It was moments like this that made what the future held hurt even more.

preferred roleplay type: multi-para just for how i divide my replies? so it isn't one big wall of text, but i personally have no issue if people do similar or if they prefer one para of text ! i usually do one para, divide in the middle to tag the person im replying too, and then another para underneath. im also open to all kinda of plots , aus or plots !

pet peeves & dealbreakers: i dont have too many ? but, i dont like people who spread a lot of hate/hate on characters, ships, or fandoms constantly. bigotry is a 100% no from me. of any kind. i also don't like when people ignore female muses in favor of male muses or who only seek out ships and not other kind of dynamics. when people don't specify on starter, ship calls, or memes i kinda a pet peeve of mine as well. it's frustrating from someone who is indecisive as it is and esp if i ask for people to specify.

plots or memes: i love love love both. love plotting things, love getting into the dynamic or the ship or the plot with someone. makes me giggle n kick my feet. but, i also love memes as very easy, very simple way to get us kick started with a pairing or with general interactions. and on my blog, memes never expire.

are you like your muse?: i'm similar to some of my muses, for sure. some of their qualities. i think i'm drawn to similar type of personalities in a sense, too ? like some of my muses are similar to one another, personality wise or they share traits. then, i have some of them who i am nothing alike at all n i love them just as much.

Slowly, as if to keep from spooking a dove, he reached down for his hair. And very, very aggressively, he snatched to mash his fingers into its locks and ruffle it into a deranged red mess.

        Ah yes,      the burden of being handsome, helpful  &  utterly heroic takes a toll on Bernard from time to time. Finding a comfortable little spot under an old oak, letting the afternoon hours pass by, the tiefling was half-asleep   —   arms crossed over his chest, chin already lazily resting on his chest.

   Until his slumber was interrupted.

The sudden, rough assault of his hair brought a gasp up his throat   —   which, honestly, did not make it past his lips. Bernard jolted, Shoulder shooting up in raw panic, trying to orient himself.

All prompts and ideas are just suggestions, if you are the kind of Yuletide writer who likes to follow them. If not, cool, they are certainly not the end-all of what I love about these fandoms and characters.

I like found families, oddball friendships, sympathy toward outcasts, characters who fully embrace being outcasts, melancholy, a sense of place, bittersweet accomplishments, and a stubborn flicker of hope in an overall bleak world. Dreamlike atmospheres that aren’t necessarily scary, some sort of reassurance that can be found in the weird and the profoundly unnatural.

My only ship in these three fandoms is Atrus/Catherine. DNW CW/Farley and Margot/Turner.

Myst: any (Atrus, Catherine, Yeesha, Worldbuilding)

30th anniversary! 🥳🍉 how about "30 years later" as a prompt? Counting from whenever you want. 30 years after Myst 1? After Yeesha left Tomahna? After Atrus (or either other nominated character) first made contact with the people of an Age he wrote? After the ice cream expedition first brought cones to the Cavern? Or "celebration" or anything with a celebratory twist. Atrus and Yeesha reuniting after End of Ages? Catherine having a nice day? Yeesha in Chiso Preniv?

I love the whole cast, so feel free to set the fic in any era of canon, following pretty much anyone along with the worldbuilding or nominated characters. If you want to play with, idk, Ri'neref or the Watcher or Gehn or Esher or Zandi or Nelah or some schmuck on Releeshahn or whatever, go for it! I am not particularly well-versed in pre-Fall D'ni shenanigans - I’d be interested in reading something set anywhere in those millennia, just please write it assuming that your reader may not be already up to speed on what was going on historically at the time.

What’s your favorite spot on Hunrath, dear author? What’s your favorite odd item (is it RIUM+’s Myst book or the Unwritten manual)? Tell me a story about it. Or tell me about how it’s like to live in that new world, humans, villein, arai and so very few mofang, and Farley’s plan was what got them there for better and for worse. Or a night at that cute little pub they added with the patch. Tell me about the differences between Farley’s worldview, since Hunrath is all she knows, and CW who misses his life on Earth so much. What’s Josef’s story in this regard? Or focus on any aspect of Earth culture that got lost on Hunrath, or one that got preserved, or how the 19th century habits of some obductees clash with their neighbors who come from a few centuries later, and how it all eventually becomes Hunrath’s unique culture. Or what are your favorite character’s feelings on the Sorian sky hanging on their head? Is there someone in Hunrath who insists on calling the seeds #spacepinecones, to everyone else's dismay? What's the smallest recorded obduction, a tiny seed that barely got the person from hair to toes, and what's the largest?

I don’t ship any combination of main characters but I am interested in all platonic interactions between the main trio (as well as actually seeing them forced to act like a trio. What would it take?). I am also interested in seeing any of the characters who are only mentioned in the various journals getting fleshed out a little: if your story needs an additional character or two, you could see if anyone there suits your needs! (and if nobody there does, actual OCs are welcome)

Firmament: Margot | The Mentor, Worldbuilding

Journals? Got any journals, dear author? Notes? Codes? Anything to make the Realms feel more lived-in, from the perspective of Margot or any Keeper? Or what do they find after the end, how do they settle?

I'm not necessarily looking for a fix-it for Margot, but if you've thought of something, I'm listening. Or at least, is it possible for the survivors to know what she's done for them, and to understand who she was as a person, what she went through? (which I suppose brings us back to "journals?" ;^; ) In general, has anyone ever tried to hack the memory loss situation through the written word? Were songs composed and shared from Keeper to Keeper until at last they made it back to the ears of their author?

While I didn't request Turner, and I'm not interested in fic about his character pre-amnesia, I'd be good with any worldbuilding idea that features him freshly post-canon coming to terms with being a blank slate with such a weighted past.

If you're into any of the irl Founders, I'd also love a deep dive in-universe on what their angle was in sponsoring the expedition. Like, Marx, I have Some Questions, and I'm sure there's plenty to say about the others as well! What they hoped for, which of their ideals were pivotal to the eventual success of the expedition VS which ones could've been thought through a bit better maybe…

reader refusing cuddles from dottore or the segments habjdbuhsuws hear me out. imagine the panic like did they do something wrong or what ⁉️⁉️

Rejecting cuddles and general affection from Dottore and the segments is possibly the easiest way to have them pause for once and simultaneously think something is terribly wrong. The immediate collective thought is that you're upset with them - so much so you don't even want to touch them, which is really saying something considering you're always snuggling by their side, always asking for attention. The possibility of it being a prank is low for this same reason.

The hive-mind is somehow dead quiet yet buzzing with thoughts at the same time, the minute you maneuver out of Omega's grasp, Beta's tugging, and even Prime's hold. Seeing as this has literally never happened before, the younger segments, especially, are antsy over it, individually picking out each interaction over the past few days that could possibly have you this mad. And oh, if it happens again? Work is halting in that lab. The agents are very worried. Please save them, they beg of you!

If you're actually upset, Dottore will just cut straight through the heart of the problem and ask you what's the matter. He could just let it draw out and wait for you to come to him, but he deemed that an ineffective solution for this, considering the dire situation. Rest assured that you two will come to an understanding eventually, though you should know that Dottore can be stubborn at times, even if he really does love you. He'll find it hard to do anything if he really thinks he wasn't wrong. But he won't be able to withstand the cold shoulder from you for very long after being so accustomed to your warm hugs, so he'll give in. Everything will be well! Regularly scheduled and spontaneous cuddles are once more!

If it just turns out you're teasing them, mercy will be out the window. Prepare for some of them to be petty, and some of them to act like they weren't worried for you. They will get back at you, but in reality, they're more glad they didn't upset you, because that would be difficult to navigate for people like them. (Zandy secretly giggling the whole time, knowing he was still getting hugs from you.)

My name is Behrooz Zandi. I am a musician from Tehran, Iran. raised in Iran, I familiarized myself with middle eastern modal music by playing the Tar, a traditional instrument in my country. When i was young, I moved to Malaysia to explore the world beyond the comfort of home. While studying visual arts in Cyberjaya I started making electronic music in my free time and found my true passion with the endless possibilities it gave me. Making sounds from scratch and modulating them into something melodic felt like magic to me. My process of making music is always a visual experience. I want to make a bridge between reality and imagination. Aiming to trigger a sense of visual perception for the listener - you could see my music as characters interacting with each other to tell a story. I have learned that the insecure side of being is the most loyal source for self identity. This inspires me to create something that lasts forever in the memory of people who feel connected to it. I often work with microtonal melodies and ethnic elements, blending them with organically driven sound design into the sound of Behrooz Zandi. This piece was posted on 2/19/20 but it is originally from 2018 prior to LM/DC. It’s always amusing to read their theories about big upcoming changes in Darren’s love life in light of the fact that Mia has been a steady force in Darren’s life for 10 years.

Here they are debating whether Lea will be Darren’s new beard- a theory that required them to ignore both Mia and Zandy. In this post we see justcantgetenoughcc using the “trust me I know more than you do but I can’t share it” strategy. Honestly, this is the one that baffles me the most. While I can see the slow-motion gifs for what they are- pure manipulation, I know that TLOS isn’t the Klaine bible they want it to be and I can understand how other’s can be conned with that evidence. What I can’t understand is why anyone would trust someone who continues to say “I know more than you do but I can’t share it with you...just trust me”.  Those very words are literally the exact words that should raise one’s caution flag. Mommas, the one thing you should teach your children is to never trust someone who claims to know the truth but can’t share any evidence. Especially if that story goes on for 5 or 10 years.

The primary reason was always to break Chris and Darren up.

RM/Ricky all playing together. AB imo not that invested but to a degree (at least initially). Even beard (she realized what she can have by getting rid of Chris and why she too is very much part of the game now). Each had their own reason for being invested, as Chris and Darren in love and united as a team, made it difficult to manipulate Darren. In a way, that - Darren and Chris in love - was also their Achilles heel. Each put the other ones’ interest first - and got manipulated in giving up their autonomy. Chris had nothing to hide as he was already out. Why would he even need a beard? They were so young and so naïve to Hollywood’s manipulation tactics. I don’t think they had anyone with industry experience, with their interests at heart, to advise them or look out for them. It was too late and they were in too deep, by the time they realized what they had agreed to. They still tried to back out of some of their worst mistakes. Some day… they will tell those stories or it will turn up in Chris’s books.

Initially people assumed that all that proximity and playing boyfriends onscreen - and no doubt their amazing chemistry - must be why they were together. I am sure they thought it will never last or they will get over it or get tired of it and move on to others once the itch was scratched. Like most young guys (as young gay guys - not my opinion at all - but a lot of people believe that).

But it didn’t happen that way. They didn’t break up. Instead they went and got engaged over Dec 2012/NYE 2013 (that was from when we have the pic of Chris looking at his ring standing next to his suitcase at an airport). They were serious from the get-go. They were ‘IT’ for each other.  They were even living together (even before Glee Live 2011 - in Chris’s first rented apartment in LA). I think it was Joey who once commented that he saw Darren only when he needed more clothes (Don’t quote me on it though. Ha). When Chris bought his first home in Laurel Canyon, guess who moved in there with him? It wasn’t W as most of the fandom was led to believe. There’s enough proof in their own words and since this is going to be a long post, I am going to leave some of those details out of this. And some I can’t reveal.

Why do you think LU Tour happened? It was another attempt to break them up and also to get Darren to declare his longtime gf of a hundred years. There was no way Darren would agree to it when Chris and Darren are together. And so his team came up with the clever “DIVIDE and CONQUER” tactic. Get Darren away from Chris and break him down.

Nothing ELSE came out of LU tour and so I am right in my assumption here.

A Darren who missed Chris on the road, who was mentally and physically exhausted from his days on the tour… gave in… WITH TEARS… a heartbroken Darren was FORCED to declare a gf - reading from a script, answering questions from a script, at the Toronto Radio Interview - WHILE he was already ENGAGED to CHRIS.  There are many ways I can cement the claims I am making. But they are NOT mine to share.

If you doubt me, that the intent ALWAYS was to break them up (come on guys! connect the dots!), so much has been exposed in the last two years, especially in the last six months.

Why were they forced to have beards? It wasn’t FOX (in fact there's a story not known to many that FOX was going to let Chris and Darren come out (heck they even knew of their engagement - again stories that I cannot reveal) but others disagreed/disapproved of that move and stopped it). Some of the old fandom crowd know of this. Why were their beards ALONE always allowed access to the glee set? Why were M and W paraded so much on the sets? Why was the beard woman given a job at FSO (where she did nothing really but was given free credit for work others did). Chris was already OUT. He was a grown up guy who always took care of himself even when he was much younger.

During 5.14 filming in NYC, it was clear that the film crew took care of Chris and Darren. All their needs on the set was met along with the rest of the cast present (Amber, Chord, Kevin, Lea and Darren). Even holding up their winter coats for them to slip their arms through. W didn’t do anything other than obstruct the filming (and there were a lot of complaints about W and he had to be told many times to move out of the crews way). Do you think Chris paid for W to accompany him to NYC where he wasn’t needed at all? Who had the most to gain from that decision to have W there?

The beards were ALWAYS meant to WEAR THE GUYS DOWN. To create discord between them. To grate on their nerves. To be a thorn in their side. You get the drift.  It still didn’t work. Those two guys still stayed strong.  

Yes, there was an attempt to END KLAINE early in S6 and use Darren/Blaine as Lea/Rachel’s LI and have them be the prime couple who got married to each other (instead of a Finchel wedding that RM always talked about) at the SERIES Finale. That didn’t work either. That story is known to many in the fandom. But is not mine to tell.

Look at all the attempts made to prevent Darren and Chris coming out. Since 2015.

I have repeated this so many times since 2015 and am going to repeat it ONE LAST TIME for the people who have joined the fandom since 2015.

Glee was filming the last season. Beards were barely on the set even with all the heavy make-out scenes (heavier Kliss scenes, emotional scenes, the elevator scene, the wedding, future Klaine etc). Guess everyone felt bearding was ending too as Glee only had a few more months left. We got the NOW FAMOUS BTS interview from the ranch - the day the Klaine wedding was being filmed - where Chris and Darren were interviewed by Leanne Aguilera (and not M). Where they admitted so much and looked radiant and vibrant. Best of all they admitted to being good friends in real life.

Then we got the Ellen Show where Darren and Chris interacted and wrote each others names (not to mention the incident where Chord slipped up and mentioned meeting Chris and Darren. Nothing dramatic happened by way of reaction or retaliation from PR).  Then came the Paleyfest and the Mario Lopez radio Interview - where Chris was present with Darren and Mario in the recording room. We counted Chris’s laughter interspersed  - at least five times. It wasn’t edited out. We rioted over all of those events repeatedly. It was like the CC drought was finally over. They were allowed to breathe free. And acknowledge each other. Say they were good friends with each other.

[We have proof on several claims I make here and are known to a lot of people but we can’t share them yet].

We knew they went on a European vacation that ended in Paris where Darren had to attend the CON with a few other Glee cast. Darren alone didn’t stay at the hotel where the CON was held, where the rest of the cast stayed. Ask yourself why. Darren returns to NYC and starts rehearsing for Hedwig Broadway that starts mid-April. Everyone was happy and there were several SM follows of both Darren and Chris back to back by Hedwig Crew and BTS crowd. Lot of happy tweets and fun stuff.

It felt like Darren had a few more months left to freedom. Chris plans his TLOS4 book tour around Darren’s closing show at Belasco - so that he can sit in the audience and not watch from the shadows as he did during the first 119 days of the run. Alla Plotkin, Chris’s publicist, allowed it and supported it (if there were any known contracts or clauses that prohibited Chris’s presence at the closing show, Alla wouldn’t have /couldn’t have allowed it) and planned the whole tour accordingly with Chris.  

So close to freedom and to coming out… and then out of the blue, Darren’s perfectly written Broadway BIO was edited to a mess of a BIO and  ‘xos to mia’ was included. Overnight Darren’s personal life got hijacked by a scheming woman and Darren’s manager. Eleni who was a longtime good friend of Jeff Jernigan, was appointed as Darren’s assistant (watchdog) at Belasco. Darren who hadn’t even seen or heard from the beard for many months, and Darren who had never displayed any real closeness to the woman - posts a dressing room kiss (long story about that too. (Read mleigh69’s post on how the Belasco kiss was staged).

From there on, everything went to hell in a hand basket. All their hopes and plans were dashed.

From that time to the present, how closely Darren is monitored and watched and babysat…is ridiculous. Darren has more people on his secret service detail,  sometimes more than even the President himself (it feels like). He sang 4 songs in Utah recently and he had Ricky, AB, Jeff Jernigan, Eleni, beard and Ken Sunshine (the boss of Sunshine Sachs) with him in Utah.

All their attempts to coming out was thwarted. Chris wasn’t allowed to attend the Hedwig closing show even though he had timed it to end the day before, giving him time enough to fly to NYC from LA. Chris who had never made a career misstep, chose to put his reputation as a children’s book author on the line by acting drunk - just so he could get on a plane to NYC. He had to watch the closing show from the shadows but am sure that was preferable to being beaten or giving in to two conniving people.

Even the ENCAGE was partially to stop CrissColfer from coming out. See what the encage did. All of Darren’s commitments and obligations were OVER AND DONE with by then… except for the NOOSE called the encage now. That was pretty timely, wasn’t it?

Why is Darren going on a tour with Lea Michele when Lea’s album was poorly received and didn’t do well at all? All her shows failed. She really has nothing much going on.  Hmmmm…

If am guessing, let me be wrong for one last time… WHAT IF IT IS TO KEEP CHRIS AND DARREN APART? As all their attempts and excuses to prevent Chris and Darren FROM COMING OUT ARE EXHAUSTED?

So now there are tours and MORE TOURS… where Darren's TEAM THINKS he has to remain NO HOMO for the tours to sell. They quickly got him committed to a LONDON concert while ACS was still airing.

Now ACS is over.

And sure enough, just as the ENCAGE IS COMING APART AT THE SEAMS… we have THE LM/DC TOUR!!!!

A man who became manic after being infected with coronavirus got so delirious he confessed to his wife that he used to have sex with men, doctors have revealed.  The unidentified 41-year-old, who was treated at St Thomas’ Hospital in London, also became ‘highly aroused’ and uninhibited, questioning and inappropriately touching nurses tasked with treating him. He also became obsessed with ‘grandiose ideas’ and tried to smear water on fellow patients as if he were baptising them, medics said in a bizarre case report. The man was sick with a cough and fever for 10 days before he ended up in hospital and tested positive for Covid-19, which doctors believe triggered unusual symptoms. He eventually had to be sectioned under the Mental Health Act because he became so out of control. Describing the experience in his own words after recovering as ‘fascinating’, the man said he thought he was ‘trying to help the doctors as much as I could’. He added: ‘I began to think that I was part of a TV show, in which I was sent back from the future to save the NHS, and I was curious to see how this would end.’   The doctors said it was possible his episode was the first sign of a condition such as bipolar disorder — but did not diagnose him with that and instead put it down to the coronavirus despite admitting they can’t prove it for certain. One mental health expert told MailOnline it is well documented that immune system reactions can affect the brain and trigger mania such as what the man suffered, calling his episode ‘bizarre but not extreme’. There is growing evidence that Covid-19 can affect the brain and nervous system in various ways, the most common of which is losing the sense of smell and taste.  There is growing evidence that the coronavirus, and the body’s immune response to it, can affect the brain and trigger mental health symptoms (stock image of a brain scan)  Writing in the BMJ Case Reports medical journal, Dr Jamie Mawhinney and colleagues said: ‘This is, to the best of our knowledge, the first report of an acute episode of mania or psychosis as a result of SARS-CoV-2 infection.’ The man went to A&E in the early hours of the morning at the London hospital where Prime Minister Boris Johnson was treated for Covid-19.  He had woken in the middle of the night feeling like his ‘brain was racing’ and telling his wife he thought he would die, doctors reported. He had told his wife about sex he had had with men, ‘mostly’ before they were married, which she had never heard him talk about before. The man also confessed to other ‘uncharacteristic’ sexual behaviours and became uninhibited and acted inappropriately while in the hospital.  In the report the doctors said: ‘He was loud and highly aroused with sexual disinhibition and overfamiliar behaviour, inappropriately questioning and touching members of staff.  ‘His speech was pressured, and his mood subjectively and objectively elevated.  ‘His thoughts were grandiose with persecutory elements, and he had persistent strong religious ideas, manifestations of which included attempts to anoint fellow patients with water.  ‘He also obsessively wrote down every personal interaction and bodily sensation. He said he found this experience “liberating”.’ The man’s behaviour became so uncontrollable he had to be sedated and was transferred to intensive care and supported with ventilation. Investigations confirmed that he was positive for SARS-CoV-2 – the virus which causes Covid-19 – but the virus was not found in his spinal fluid, which would have proved it was in his central nervous system and could have travelled to the brain. After 24 hours on ventilation, he was moved to a ward where his coronavirus symptoms lessened, but mental state remained abnormal. COVID-19 CAUSES DELIRIUM AND STROKE IN ‘HIGHER THAN EXPECTED’ PATIENTS  Infection with the coronavirus can cause delirium, stroke and nerve damage in ‘a higher than expected number of patients’, a study has found. Experts from University College London have reported a ‘concerning increase’ amid the pandemic of a rare brain inflammation known to be triggered by viral infections. Typically seen in children, acute disseminated encephalomyelitis — or ‘ADEM’, for short — affects the both the brain and spinal cord. The condition — which can follow on from minor infections such as colds — sees immune cells activated to attack the fatty protective coating that covers nerves. The researchers have warned that clinicians need to be aware of the risk of neurological effects to help early diagnoses and improve patient outcomes. ‘We identified a higher than expected number of people with neurological conditions such as brain inflammation,’ said paper author and consultant neurologist Michael Zandi of the University College London. The appearance of these conditions, he added, ‘did not always correlate with the severity of respiratory symptoms.’ ‘We should be vigilant and look out for these complications in people who have had COVID-19.’ ‘Whether we will see an epidemic on a large scale of brain damage linked to the pandemic — perhaps similar to the encephalitis lethargica outbreak in the 1920s and 1930s after the 1918 influenza pandemic — remains to be seen.’ The researchers also found that other neurobiological complications — including delirium, stroke and nerve damage — appear to be associated with coronavirus. In their study, Dr Zandi and colleagues studied 43 patients — aged from 16-85 — with both neurological symptoms and either confirmed or suspected COVID-19 that were treated at the National Hospital for Neurology and Neurosurgery in London. According to the researchers, many of the patients did not experience any of the respiratory symptoms often associated with the coronavirus. Among the cohort, the team identified 10 cases of temporary brain dysfunction with delirium, eight cases of strokes and eight cases with nerve damage. There were also 12 cases of brain inflammation — with nine of such patients being diagnosed with ADEM. Under normal circumstances, the London-based team said that they would only see around one adult patient with ADEM per month, on average — but that this figure has increased to at least one patient per week amid the pandemic.  Further studies are needed to identify exactly why some COVID-19 patients are developing neurological complications, the researchers concluded. ‘By day eight, his behaviour had escalated further culminating in a security call and emergency sedation for the safety of himself, the ward staff and other patients,’ the report said.   Psychiatric assessment found features consistent with acute mania – a state of extreme energy and arousal which can be euphoric but lead to violence – and he was detained under the Mental Health Act.   Mania is often conceived as a mirror image to depression, with the two moods associated with bipolar disorder.  The man was transferred to an psychiatric hospital and commenced on regular olanzapine – used to treat schizophrenia and certain types of bipolar disorder. Twelve days after being sectioned, the patient’s mania finally subsided and he was discharged. Reflecting on his experience, the man said: ‘I was taken to hospital on the 4th April with what I would describe as the worst headache of my life. At this time, I had been suffering with the symptoms of Covid-19 for over a week. ‘I was in hospital for a total of 20 days with psychosis and mania, which I experienced as fascinating.  ‘This may seem strange from an outside perspective, but I was, in my mania, trying to help the doctors as much as I could, while at the same time trying to make sense of my condition. ‘For my family and friends it was frightening. Luckily, they had a lot of support from each other, and from the great team of doctors at St. Thomas hospital.’ On follow-up, doctors revealed the man was slowly being weaned off antipsychotics while his wife said he is now back to ‘his baseline level of function’. The doctors said they couldn’t rule out the first manic episode of bipolar disorder – which the patient’s sister had been diagnosed with previously.  But it was clear the drastic changes in behaviours started at the same time as his tell-tale coronavirus symptoms. Professor Anthony S. David, director at the University College London Institute of Mental Health and author of ‘Into the Abyss: a neuropsychiatrists notes on troubled minds’, told MailOnline the man’s condition was ‘bizarre but actually not so extreme’ He said: ‘Psychiatrists talk about hypomania – a mild form of mania with elevated mood and feeling “high” – but mania proper includes delusions, typically grandiose, e.g. that one is god or has special powers, talking very fast, spending money and being disinhibited sexually. So this is what mania looks like. ‘Mania can be triggered by stress, lack of sleep, physical illness, drugs etc – but you have to have a predisposition. This man’s family history is probably relevant.’ He said that doctors are seeing growing numbers of neurological symptoms of Covid-19, but that they usually seemed to be caused by the immune system, rather than the virus itself getting into the brain – although this was possible, too. Professor David added: ‘Fever on its own disrupts thinking and could be enough to tip someone over into mania, if they were predisposed. A change in sleep-wake cycle is another cause.  ‘The immune system can affect the brain – that is why we all feel generally yuck when we have an infection, even if the infection is in another part of the body. Inflammatory chemicals and cells circulate and some can affect the brain.  ‘The more specific effect – which is rare but well recognised following other viruses – is that an immune response is triggered and the antibodies end up attacking the person’s brain in an effort to kill the virus – sort of “collateral damage”.  ‘Depending where the damage is concentrated, this can cause delusions as well as affecting consciousness, memory etcetera.’ He said some medications being used to treat coronavirus, such as steroids, are known to cause mania and psychosis. The man is not known to have been receiving treatment before his episode began, however.        Doctors pointed out Covid-19 manifests in a number of ways affecting multiple systems – including the central nervous system (CNS). The coronavirus enters human cells by latching onto ACE-2 receptors on the surface. Although the lungs are the key site for expressing ACE-2 in the body, it is also found in endothelial cells in the brain. This may provide a route into the CNS.  Previous reports have linked SARS-CoV-2 in the development of viral encephalitis – inflammation of the brain causing delirium, changes in personality and confusion. Yesterday University College London revealed study findings that the coronavirus can cause delirium, stroke and nerve damage in ‘a higher than expected number of patients’. Experts from University College London have reported a ‘concerning increase’ amid the pandemic of a rare brain inflammation known to be triggered by viral infections. Of the patients they studied with altered brain function, none had detectable levels of the coronavirus in the brain or spinal fluid – much like the 41-year-old case report.
