If you’re a goat, there’s one thing you really like to do: eat. And for the past few years, BART has put goats’ proclivity for munching to work to reduce fire danger on its grassy properties.
Currently, the goats can be seen grazing away on hillsides in Walnut Creek and Hayward. You might spot them from the highway or even a BART train if you keep your eyes glued to the sandy-colored hills or perk your ears to their signature “bah” sounds.
On a recent weekday, high on a hill off Camino Diablo Road in Walnut Creek, wedged between a ravioli factory and the BART tracks, a herd of goats could be seen in silhouette from the road, cresting skittishly over the hillside.
At a closer glance, the goats were in fact hard at work, eating away at the dry brush and grass below their feet as a natural means of reducing fire risk on the property. They stopped only when their herder, Zenobia Mancha, got too close or when a BART train rolled by below (the animals are protected from the tracks by an electrified fence). One kid, or baby goat, who stood just below a human’s knee height, paused to grab a drink of milk from its mother’s udders.

Though adorable, the goats are also highly utilitarian. According to Glen Eddy, BART’s Assistant Superintendent of Way and Facilities, Grounds Maintenance, the goats are used in place of fossil-fuel-reliant lawn mowers and other power tools, which increase the risk of sparking a fire on the drought-stricken grass and increase the safety of human workers, who once had to scale large inclines to clear the same – and very steep – hillsides. Goats are also naturally quiet creatures, thereby decreasing disruption to neighboring areas.
“Most people don’t realize just how much land and properties the District owns and maintains,” Eddy said. “Most of our rights-of-way consist of wide expanses of property to allow for a safety buffer and access to the tracks as need.  Almost all are covered by wild grasses and vegetation that requires maintenance year-round.”
Originally, BART would plow or mow the grass and weeds “as much as possible,” Eddy said, “but in many locations, there just isn’t room for equipment to get in there.” Likewise, performing such work by hand was “very slow and costly,” and worker injuries “is a real concern as many of these areas are on hillsides and slopes too steep to be safely cut by hand.” Poison oak proves an issue, too, but goats eat it happily as a “dessert,” Eddy said. Several areas on BART property are also “environmentally fragile” and require less-invasive treatment.
“Goats are all-natural and pretty much go anywhere, too,” Eddy explained. “And they’re just plain fun to watch.”
BART’s goats are contracted by the Coalinga-based, family-owned business Living Systems Land Management, which owns about 4,000 goats. The animals are a cross between Spanish and Boer goats, breeds ideal for this type of work because they typically eat every edible morsel in sight. The goats also work faster than any human could, clearing about one acre each day.
Mancha stays with the goats 24 hours a day, sleeping in a trailer beside the grazing property at night. It’s essential that he’s near the goats – all 450 of them – 24/7 to ward off predators, such as mountain lions, and to make sure the goats don’t “escape by night,” Mancha said, speaking in Spanish through a translator.

“This is a safe area,” Mancha said. “But there is a risk of them wandering onto roads or into homes or onto the freeway and getting run over.”
Mancha, a Peruvian national who worked as an electrical engineer back home, has resided in the United States on an H-2A temporary agricultural worker visa for nearly three years. He intends to return to Peru – with the hope of one day settling in the U.S. permanently – at the end of this year. The herding work, he said, has enabled him to put his four children through university. Two of his children are now engineers, and two are studying accounting and law respectively.
“There’s a lot of pleasure in life,” Mancha said beaming, “But nothing like the pleasure of educating your children.”
Back home in Peru, Mancha grew up around grazing animals. His eighty-year-old father still maintains a herd of sheep, he said. The work can be challenging – and lonely – but it’s worth it for Mancha, who takes great pride in his children’s educations.
“I get lonely, yes, but what can you do,” Mancha said. “I’m pretty used to it after three years. I prefer to have a job.”

BART has used goats to mow its properties for about five years, Eddy said. They mow anywhere from five to 25 acres at each location where they’re grazed. The goats’ work typically costs BART about $800 to $1,200 an acre, and they usually work for about six weeks in the fire-prone summer months. Late-season rain events may require them to return to certain areas “for another ‘haircut,’” however, Eddy said.
BART has a history of getting creative with land and station management. Earlier this year, the transit agency began contracting a falconer and his hawk at El Cerrito del Norte Station to mitigate the station’s pigeon population. The falcon has been a hit with passengers – and has markedly improved the presence of pigeons on the property.

John and Jenny Shaw have been living on this property in Macedon for 50 years, around half the time there’s been a house on it. The residence dates back to the first World War, and some of the flora has been there even longer. 
There are three oak trees dating back a century, and a stately mannagum that John estimates is 300-400 years old. These ancient trees are complemented by new plants and flowers that John and Jenny have added over the years: camellias, ash trees, productive veggie beds, 30 rhododendrons, peach, apricot, apple trees and hazelnuts that refuse to bloom. Sometimes people gift them specimens, which the pair incorporate into the existing space. 
It’s an intuitive garden – serviced by a large composting system – that has evolved with the whims of its owners. 
In the ‘70s, the pair ran the house as an event-space for private functions, putting the Federation-era wine cellar to use and serving retro menus from their own kitchen. The house was filled with people and conversation – it was a place for gathering and meeting new neighbours, who dined amongst the Shaw’s family life. 
‘Growing up in this place has had an impact on the kids,’ says John of his sons. One was married in the yard and another is Ben Shaw, permaculture expert on the Victorian surf coast. 
This culture of sharing has even impacted the evolution of the garden. There is no real fence at the rear, meaning the Shaw’s garden spills into the neighbour’s land – an arrangement that suits them both perfectly. He describes the sprawling vista from his window as 100 metres of ‘combined garden’ that stretches out to the neighbouring house. 
‘We don’t have a conventional two metre wooden fence, which gives me an extra 30-40 metres of the other people’s garden,’ says John. ‘And likewise with them, they have the benefit of our garden. It gives a sense of space.’
This notion of non-proprietal gardening underpins another key part of John’s plot: the verge garden. 
A verge garden is a shared productive garden cultivated on a lawn or nature strip, from which passers-by can collect fruits, vegetables and herbs as they please. The verge garden is protected by an unspoken contract between neighbours: trust that no one will pilfer or destroy it. This public part of the garden planted and maintained by John and hedged by hay bales is potentially his favourite piece of land, stretching up to 30m long beside the road outside his house.
‘The more that people could do that the better,’ says John of the community space. ‘If it’s treated with respect, it does something for people’s sense of equanimity or safety, that they’re living in an area where someone can do something like that and it will be respected. It’s a rather nebulous idea. It has a benefit that is not quantifiable.’

How to Rent or Airbnb Your Home the Smart Way
(TNS)—On a corner lot, surrounded by lush live oaks and Florida-style bungalows, sits Jonathan Wilson’s first real estate investment: a 2,100-square-foot, bone-colored duplex. The area was a big draw for Wilson. Seminole Heights is part-hipster and part-historic, as one of Tampa’s oldest neighborhoods and home to local breweries, art galleries and yoga studios.
Some 2,400 miles due west in the Mojave Desert is a multi-colored spectacle, called “Rancho de Colores,” one of the most popular Airbnb listings in Joshua Tree, Calif. The 1,500-square-foot, three-bedroom house, painted in blazing reds, oranges and greens, stands out among the placid chestnut-colored playas of the desert. Popular among Pinterest and Instagram enthusiasts, it was conceived by artist Patrick Hasson.
Located on opposite coasts, in contrasting environments and rented out via different channels, these two properties share one important feature: They’re both part of a long-term personal investment strategy.
Wilson and Hasson each wanted a way to create security for themselves in a time when few employers offer pensions and most people must rely on their own savings to keep them afloat in their later years.
“In 2013, I was living in Los Angeles. I realized then that I’d never be able to afford a house in L.A. unless I hit the lottery. I thought, ‘I don’t have a pension. I don’t have a retirement. What do I do?’ I had already been interested in Joshua Tree, so I started asking people how much they paid for their house,” Hasson recounts. “I bought my first house in Joshua Tree for $73,000 in 2014. I was excited I had a house, but now what?”
An Airbnb in the Desert Hasson batted around the idea of renting the house which, he figured, would bring in just enough to cover the $600 monthly mortgage payment. He wasn’t keen on managing tenants, however, so he decided to try the Airbnb option.
The house he bought was once home to meth dealers, so to communicate that the house was under new ownership—and to deter meth seekers from stopping by—Hasson painted the house in vivid colors, using a rainbow theme throughout. Ironically, this eye-catching palette was a hit with Airbnb renters who wanted a great deal in a unique space.
The experiment worked. Airbnb was generating enough income that Hasson was able to buy a second house in Joshua Tree, on three acres of property, as well as a trailer. He moved to Joshua Tree full-time in 2016 and now rents out the trailer, dubbed “The Color Trip Trailer,” and continues to Airbnb what he calls Rancho de Colores, while living in his second house, which de named “Rancho El Reposo.”
A Duplex in a Booming Neighborhood Like Hasson, Wilson wanted to invest in real estate as a way to help secure his future. He knew he wanted to buy property that could accommodate him and a tenant, but he wasn’t particular about what form that took, as long as the location was desirable and the price was right.
“I was very flexible in what I was looking for. I was looking for something that had a mother-in-law suite or an extra lot. And when this came up, it was a perfect fit,” Wilson says.
Wilson originally offered $225,000 with no contingencies for the duplex with a two-car garage, but the owners passed in favor of a cash buyer. When that deal fell through, Wilson ended up getting the house for $210,000. It had tenants in place, so he was able to generate income from one unit immediately, while moving into the other unit.
He plans to convert the two-car garage into another rentable space, which would cost him about $30,000 and add approximately $150,000 in value, he estimates.
For both men, buying real estate as a way to generate income was a new venture. Neither had deep pockets, a background in real estate investing or a huge nest egg to fall back on, and they both admit to making mistakes along the way.
And they’re not alone. More than 33 percent of renters live in single-family homes, which are largely owned by individuals like Wilson and Hasson, rather than corporations or banks. This is a $2.3 trillion industry, according to a paper by the UCLA Anderson School of Management.
Single-Family Rentals Are Historically Good Investments It turns out, however, that investing in property was a good decision. A recent study by Andrew Demers, a real estate specialist at Structured Portfolio Management, and Andrea Eisfeldt, a UCLA finance professor, shows that the yields of single-family rentals are on par with stock market returns.
The pair examined the net yields and price appreciation of single-family rental homes in the 30 largest metro areas between 1986 and 2014. Separately, net yields resulted in a 5 percent return while price appreciation accounted for a 4 percent return, on average. However, both factors must be looked at to get an accurate picture of value, argue the researchers.
Both factors combined averaged a 9 percent annualized rate of return. Compare that with the S&P 500’s annualized long-term rate of return of about 10 percent and investing in single-family rentals doesn’t seem like such a bad idea.
The ratios of the total return, the study found, depended on the price range of the house. For example, the top 20 percent most expensive homes made the bulk of their profits from price appreciation, around 5 percent, whereas net yield only made up about 3 percent.
The reverse was true for houses in the bottom 20 percent. In that case, net yield was the big earner at around 6 percent and appreciation only brought in 3 percent.
Ultimately, the lower-priced houses fetched more overall—earning a total of 9 percent, compared with 8 percent from the more expensive homes. That would mean people who want to earn more today should invest in a less expensive house, whereas those who are looking for a windfall when they cash out should consider more expensive homes.
What to Consider Before You Become a Landlord or Airbnb Host Wilson and Hasson both cite mistakes they made when they first began renting their property. The main takeaways from both of them is to limit liability, treat others fairly and spend every dollar on repairs and upgrades wisely.
Put everything in writing and leave nothing to the imagination.
If you’re renting out your home, then you’ll want to have a well-articulated lease in place. This should include when rent is due, when the lease expires, what the property owner is responsible for and what the tenant is responsible for, including lawn and large appliance maintenance. Some places require tenants to care for amenities like pools and Jacuzzis.
“Everything should be spelled out in the lease. You want to eliminate any risk of ambiguity,” says Joe Santoro, founder of Personal Property Managers in Pennsylvania.
The best way to ensure your lease is well-written and amenable to both parties is to have an attorney review it.
For an Airbnb rental, the rules are different. Hosts can ask their guests to sign a contract prior to renting their house, which is a good idea for a number of reasons. First, Airbnb offers a type of insurance called a “Host Guarantee,” which is a good start, but it’s not comprehensive.
This guarantee covers damage to your property up to $1 million, with photo evidence and receipts required to proceed with any claims. However, there are conditions. This guarantee doesn’t cover damage caused by pets and limits coverage for art and collectibles, for example. For people like Hasson, who don’t allow pets at their property, creating a contract that discloses this rule is important.
Screen your tenants.
One key to minimizing risk is knowing who is occupying your space, Santoro says. A minimum screening is essential, but a more thorough background check which includes credit history, employment history, criminal infractions and past evictions is even better.
Deeper dives into a tenant’s background, as opposed to a simple credit report, are costlier, between $100 and $150, but can be a worthy investment in the long run.
“It’s better to get the right person in than to try to evict the wrong person. Evicting someone can be a long process. Every day that your house isn’t rented is another day you’re losing money,” says Salvatore Friscia, managing broker at San Diego Premier Property Management.
With something like Airbnb, hosts can check guest reviews, look at the guests’ Airbnb profile and communicate with the guest within the Airbnb website. Airbnb offers several features such as social connections, which allows guests to add their Facebook profiles, and verified profile statuses. To get verification, users must submit identifiers such as their Facebook profile, phone number, email address or government-issued ID.
Nix the expensive stuff.
Unless you’re catering to a high-end clientele, you don’t want to sink all of your capital into marble countertops and amenities. Adjust the amount of money you spend on fixing up your rental or providing amenities to the price point. 
For example, if your Airbnb is priced at $400 per night, then you’ll probably want to include a few extra treats, such as wine and late-night snacks.
However, for hosts like Hasson, who cater to budget-minded crowds, those bottles of wine can add up.
“When I first started, I’d put different color M&Ms in different dishes and leave a bottle of wine. After a while I was like, ‘What am I doing? This is expensive,'” Hasson says.
The place to splurge in an Airbnb, says Hasson, is the bed. Save money on fancy appliances and expensive furniture and put those savings into a good mattress, 500-thread count sheets and a nice comforter. Unhappy guests are guests who didn’t sleep well, Hasson points out.
Likewise, if you’re renting your home at a low- to mid-level price point, you can get away with putting in less-expensive countertops. When Wilson bought his duplex, he had to change the fixtures and he opted for the most budget-friendly ones he could find for the rental unit.
“I want the property to look nice, but I also don’t want to spend a lot on upgrades,” Wilson says.
Treat your renters with respect.
The golden rule might be the most effective mantra when it comes to renting your space.
What people don’t realize, says Santoro, is that tenants have rights. He says that many property owners are under the false assumption that because they own the real estate they can show up and even go inside whenever they want. Not only is that illegal in most cases, but it can make for a poor relationship.
“My advice is to be polite. Treat your tenants fairly. You want to give them at least two or three days’ notice before you come over, even if you live on the property. This kind of respect goes a long way,” Santoro says.
When it comes to repairs or problems with the house, be expedient. If you don’t live near the property, then you’ll want to find someone you trust who can do repairs quickly. That said, Santoro warns against would-be landlords who rely too heavily on third parties to do repairs.
“If you want to make money off renting your house, then you have to be able to do some things yourself; otherwise, those little fixes and paint jobs will eat at your profits,” Santoro says.

If you are sleeping on a flat pillow that is simply foam or feathers, you need a new one. Why? Flat foam pillows tend not to have the proper shape that supports your neck. Some foam pillows are molded, those are different. If your pillow has a neck roll that supports your neck and allows your head to drop back into extension, those are great if you are a back sleeper. If you are a side sleeper they can cause you issues. Feather pillows tend to bunch up in all the wrong places as you sleep and lose their support in all of the areas your neck and head need support.

How should you then pick a pillow? If you are one of my patients, I have a demonstration pillow in my office. Ask me to see it and explain what to look for when purchasing a new pillow.
If you are a stomach sleeper, no wonder why your neck hurts when you wake up. No pillow in the world is going to make it better. You need to learn new sleeping habits to help your neck. Lying on your belly with your head rotated to one side all night is going to cause imbalances in your neck with one side of your neck muscles growing really contracted and the other side too stretched out.
If you sleep on your side, you need a thicker pillow. A high density foam that is the same thickness as the distance between the side of your head and the outside edge of your shoulder will be the best choice. That will fully support your head so that it isn’t tilting to the side all night squeezing and pinching off your nerves.
If you are a back sleeper, you need a pillow with a neck roll built in to support the curve in your neck adequately. These pillows allow your head to remain in neutral which means your neck is a little extended. That will keep your nerves happy while you sleep and keep the tension off of them resulting in less tight muscles when you wake up.
What do I sleep on? Well, just like running shoes, pillows are very individual. What you love one week, you may detest the next. I have three that I rotate between. One is a custom measured pillow that we sell in my office. The second is a tempurpedic memory foam pillow that I purchased at Brookstone and my new favorite addition to my collection is one from the Sleep Number Store at Ridgedale. It is a cooling memory foam with a neck roll that is a little bit thicker than most and also supports side sleepers. I find that if you have a cranky difficult neck, you are well served by changing your pillow every year.
Then again there are pains which cannot be simply settled with a comfy pillow or change of positions. These types of pain may already require for you to take some medication, and these types of medicines may even require a recommendation from your doctor.

Neuropathic pain causes significant morbidity in the United States.
The incidence of peripheral neuropathy is estimated at about 2.4% of the population.1Of the 14 million US individuals with diabetes, roughly 25% experience painful diabetic neuropathy.
Despite advances in vaccination for varicella zoster virus, around 25% of patients with a herpes zoster infection will develop persistent neuropathic pain.2 More than 85% of patients with neuropathic pain caused by peripheral neuropathy will require pharmacotherapy.
Unfortunately, there are few head-to-head trials comparing agents for neuropathic pain, so selecting the best option can be difficult.
Pregabalin and gabapentin are often considered first-line treatments for various neuropathic pain syndromes, generally irrespective of cause.1 Because the products are so variable, this article compares the pharmacokinetics (PK) and pharmacodynamics (PD) of pregabalin with various gabapentin formulations, and also covers conversion regimens.
Pharmacokinetics of pregabalin and gabapentin

Both pregabalin and gabapentin are antiepileptic medications that bare structural resemblance to gamma-aminobutyric acid (GABA), though neither agent has activity in GABA’s neuronal systems.
Although the exact mechanism of action is somewhat unclear, the drugs’ efficacy in neuropathic pain is linked to their ability to bind to voltage-gated calcium channels in the central nervous system (CNS), specifically to the alpha-2-delta protein. This binding decreases neurotransmitter release in the CNS as a result of reduced calcium influx through the gated channels.3
Gabapentin is indicated as adjunct therapy for partial seizures and postherpetic neuralgia.4 Pregabalin is indicated for the same uses as gabapentin, plus the management of fibromyalgia and neuropathic pain associated with diabetes, specifically diabetic neuropathy.5
Overall, the pharmacokinetic profiles of these 2 medications are somewhat similar, but they also have some significant differences.
Gabapentin is more slowly and variably absorbed, with peak plasma concentrations around 3 hours post-dose. Pregabalin is quickly absorbed, with the maximum rate of absorption being 3 times that of gabapentin. It reaches peak blood concentrations within an hour after ingestion.
Absorption of gabapentin is saturable, leading to a non-linear pharmacokinetic profile. As gabapentin doses increase, the area under the curve (AUC) does not follow proportionally. Unlike gabapentin, absorption of pregabalin is not saturable, and the drug has a linear pharmacokinetic profile.
The bioavailability of generic gabapentin in tablet and capsule formulations equivalent to brand-name Neurontin is about 80% at lower doses such as 100 mg every 8 hours, but only 27% bioavailable at doses of 1600 mg every 8 hours.3,4 This differs greatly from pregabalin, which boasts a greater than 90% bioavailability across a dosage range from 75 mg to 900 mg daily in divided doses.3
Gabapentin’s bioavailability for its intended patient population is also more variable than pregabalin’s bioavailability. Variability among patients is believed to be 10% to 15% with pregabalin and 20% to 30% with gabapentin.3
Finally, food increases the AUC of gabapentin by about 10%, with no change in time to maximum concentration (tmax). In contrast, the AUC of pregabalin is unaffected by food, though absorption is slower.3
Distribution of gabapentin and pregabalin is very similar. Neither agent is bound by great extent to any plasma proteins, decreasing the likelihood of drug interactions due to protein binding. Both have high aqueous solubility, and the volume of distribution of each is similar (0.8 L/kg and 0.5 L/kg for gabapentin and pregabalin, respectively).3
Gabapentin formulations Gabapentin is also available in 2 extended-release formulations: a tablet (Gralise) and a gastro-retentive prodrug, gabapentin enacarbil (Horizant).
Both have different pharmacokinetics and are not interchangeable with standard formulations, the original of which is Neurontin. Side effects of these newer formulations are similar to standard formulations.6,7
Gralise is indicated for postherpetic neuralgia and taken as an 1800 mg maintenance dose once a day. The AUC of 1800 mg of Gralise is slightly less than 1800 mg of the standard formulation. In addition, the average maximum concentration (Cmax) of Gralise is slightly higher than 1800 mg of the standard form, and minimum concentration (Cmin) is slightly lower. Finally, Tmax is increased in the extended-release form.
Although food does not palpably affect the AUC of standard forms of gabapentin, Gralise should be taken with food because the bioavailability is greatly increased (33% to 118%, depending on the meal’s fat content).6
Horizant is a prodrug of gabapentin indicated for postherpetic neuralgia and restless leg syndrome. The recommended maintenance dose is 600 mg twice a day. Doses greater than 1200 mg daily are not recommended, as side effects increase without a corresponding increase in efficacy.7
Consumption of alcohol increases the release of gabapentin enacarbil from the extended-release tablet. Therefore, alcohol should be avoided when taking Horizant due to increased risk of side effects.7
Like standard formulations of gabapentin, Gralise and Horizant are not metabolized to an appreciable extent by phase I metabolism, and they are neither a substrate nor an inhibitor of p-glycoprotein. All forms of gabapentin must be adjusted in renal dysfunction, similar to standard formulations.6,7
One review found that these extended-release formulations have similar efficacy to standard ones, and they might also have fewer adverse events.8
Both pregabalin and gabapentin are well tolerated. Dizziness and somnolence are the most common side effects in both drugs (>20% seen in gabapentin).3-5 Confusion and peripheral edema have also been reported with gabapentin.4
With both drugs, side effects are dose dependent and reversible if the medication is discontinued.3 The abrupt discontinuation of any form of gabapentin is not recommended because withdrawal symptoms such as anxiety, insomnia, nausea, pain, and sweating may present. When discontinuing gabapentin it is recommended to taper the dose over a week at least.
Pharmacodynamics of pregabalin and gabapentin Gabapentin and pregabalin vary in terms of binding affinity and potency. Pregabalin has an increased binding affinity for the alpha-2-delta protein and is a more potent analgesic in neuropathic pain compared with gabapentin.3
One study developed a population pharmacokinetic model comparing pregabalin with gabapentin.9 The authors calculated values for the concentration of the drug that will give one-half the maximum pharmacologic response (EC50) and used these values to assess potency of the 2 medications.
Based on studies of gabapentin and pregabalin in epilepsy, the EC50 values of pregabalin and gabapentin were estimated to be about 9.77 mg/mL and 23.9 mg/mL, respectively.9 From these data, pregabalin was estimated to be about 2.4 times more potent. For neuropathic pain, pregabalin’s potency ratio may be even greater.
One study analyzed data from phase 2 trials of gabapentin and pregabalin and created a pharmacodynamic model.3 The authors found that in patients with postherpetic neuralgia, mean pain scores decreased as the dose of both gabapentin and pregabalin increased.
However, the study also uncovered a plateau of gabapentin’s effect on reducing pain at around 3600 mg/day. In contrast, the pain-relieving effect of pregabalin continued to increase up to the maximum dose of 450 mg/day.3
Pregabalin also exhibited a steeper dose-response curve than gabapentin. Based on the dose-response curves predicted in this model, a dose of 450 mg/day of pregabalin equates to about 3600 mg/day of gabapentin.3
Converting from gabapentin to pregabalin For clinicians who wish to convert patients from gabapentin to pregabalin, there are a few studies that reviewed such a conversion.
One cohort study reviewed the utility of switching patients with neuropathic pain due to peripheral neuropathy from gabapentin to pregabalin.10 The study followed patients who were switched from gabapentin to pregabalin and then compared them to those who stayed on gabapentin. The authors also stratified the pregabalin group further into those who responded well or poorly to gabapentin, with gabapentin stopped after the nighttime dose and pregabalin started the following morning.
Dosages were switched using the following algorithm:
Gabapentin ≤900 mg/day → pregabalin 150mg/day
Gabapentin 901 mg/day to 1500 mg/day → pregabalin 225 mg/day
Gabapentin 1501 mg/day 2100 mg/day → pregabalin 300 mg/day
Gabapentin 2101 mg/day 2700 mg/day → pregabalin 450 mg/day
Gabapentin >2700 mg/day → pregabalin 600 mg/day
The authors found that those who responded well to gabapentin and those who did not showed additional benefit with decreased pain when they were switched to pregabalin. Patients taking pregabalin also had improved pain control compared with those who remained on gabapentin.
Switching to pregabalin resulted in improved pain relief and also fewer adverse events. This was particularly true for patients who previously responded to gabapentin.
Patients who experienced adverse events with gabapentin were more likely to also experience adverse events with pregabalin. These patients were also more likely to discontinue use of pregabalin than those who responded well to both gabapentin and pregabalin.10
Another small trial compared the degree of pain relief with gabapentin to pregabalin in patients with postherpetic neuralgia in order to more closely determine equivalent dosing between the 2 medications.11
Patients were switched from gabapentin to pregabalin using one-sixth the dose of gabapentin with unchanged dosage frequency. After switching medications, patients reported similar pain relief and side effects, with the exception of an increased incidence of peripheral edema in the pregabalin group.
The authors concluded that the analgesic effect of pregabalin was about 6 times that of gabapentin.11
Other studies have looked at methods for converting gabapentin to pregabalin. One such trial used population pharmacokinetic models to examine 2 possible scenarios for converting gabapentin to pregabalin, using a ratio of 6:1 gabapentin to pregabalin.9
The first scenario involved discontinuing gabapentin and immediately starting pregabalin at the next scheduled dosing period. The other option included a gradual transition from gabapentin to pregabalin.
In this second scenario, the gabapentin dose was decreased by 50%, and 50% of the desired pregabalin dose was given concurrently for 4 days. After this time, gabapentin was discontinued and pregabalin was increased to full desired dose.
The model looked at transitioning patients from gabapentin to pregabalin at various doses, including:
Gabapentin 900 mg/day → pregabalin 150 mg/day
Gabapentin 1800 mg/day → pregabalin 300 mg/day
Gabapentin 3600 mg/day → pregabalin 600 mg/day
Both scenarios were quick and seamless, so the authors concluded that either technique could be an effective method to switch patients between the medications.9
Final thoughts Though pregabalin and gabapentin have somewhat similar pharmacokinetic and pharmacodynamic profiles, there are clearly significant differences. Overall, pregabalin has more predictable pharmacokinetics, and it also shows a stronger binding affinity to its target receptor, increased potency, and a steeper dose-response curve in neuropathic pain that does not plateau over recommended dosing levels.
A few studies have found that pregabalin has fewer side effects and may be more efficacious for neuropathic pain than gabapentin. Several studies reviewing conversion of gabapentin to pregabalin predict that a rough ratio for conversion is about 6:1 gabapentin to pregabalin. In addition, a direct switch from gabapentin to pregabalin seems to be well tolerated, making the conversion simple.
Clinicians should note that pregabalin has various pharmacokinetic and pharmacodynamic advantages over gabapentin, and a conversion between the 2 medications is often well tolerated.
