Treatment for urinary incontinence depends on the type of incontinence, its severity and the underlying cause. A combination of treatments may be needed. If an underlying condition is causing your symptoms, your doctor will first treat that condition.

Your doctor is likely to suggest the least invasive treatments first and move on to other options only if these techniques fail.

Behavioral techniques

Your doctor may recommend:

Bladder training, to delay urination after you get the urge to go. You may start by trying to hold off for 10 minutes every time you feel an urge to urinate. The goal is to lengthen the time between trips to the toilet until you're urinating only every 2.5 to 3.5 hours.

Double voiding, to help you learn to empty your bladder more completely to avoid overflow incontinence. Double voiding means urinating, then waiting a few minutes and trying again.

Scheduled toilet trips, to urinate every two to four hours rather than waiting for the need to go.

Fluid and diet management, to regain control of your bladder. You may need to cut back on or avoid alcohol, caffeine or acidic foods. Reducing liquid consumption, losing weight or increasing physical activity also can ease the problem.

Medications

Medications commonly used to treat incontinence include:

Anticholinergics. These medications can calm an overactive bladder and may be helpful for urge incontinence. Examples include oxybutynin (Ditropan XL), tolterodine (Detrol), darifenacin (Enablex), fesoterodine (Toviaz), solifenacin (Vesicare) and trospium (Sanctura).

Mirabegron (Myrbetriq). Used to treat urge incontinence, this medication relaxes the bladder muscle and can increase the amount of urine your bladder can hold. It may also increase the amount you are able to urinate at one time, helping to empty your bladder more completely.

Alpha blockers. In men with urge or overflow incontinence, these medications relax bladder neck muscles and muscle fibers in the prostate and make it easier to empty the bladder. Examples include tamsulosin (Flomax), alfuzosin (Uroxatral), silodosin (Rapaflo), doxazosin (Cardura) and terazosin.

Topical estrogen. For women, applying low-dose, topical estrogen in the form of a vaginal cream, ring or patch may help tone and rejuvenate tissues in the urethra and vaginal areas. Systemic estrogen — taking the hormone as a pill — isn't recommended for urinary incontinence and may even make it worse.

Absorbent pads and catheters

If medical treatments can't completely eliminate your incontinence, you can try products that help ease the discomfort and inconvenience of leaking urine:

Pads and protective garments. Most products are no more bulky than normal underwear and can be easily worn under everyday clothing. Men who have problems with dribbles of urine can use a drip collector — a small pocket of absorbent padding that's worn over the penis and held in place by close-fitting underwear.

Catheter. If you're incontinent because your bladder doesn't empty properly, your doctor may recommend that you learn to insert a soft tube (catheter) into your urethra several times a day to drain your bladder. You'll be instructed on how to clean these catheters for safe reuse.

What is Circumcision? 

Male circumcision was one of the earliest operations performed by humans. It consists of the surgical removal of some, or all, of the foreskin (or prepuce) from the penis. It is one of the most common procedures in the world and most frequently performed during the new-born period. 

When is it usually done?

The procedure is typically done on a new-born for personal or religious reasons. Circumcision in older children and adults may also be done for the same reasons but the procedure becomes more complicated and riskier in older babies, children, and men.

Is circumcision necessary?

The use of circumcision for medical or health reasons is an issue that continues to be debated.  But circumcision showed the following advantages:  

A decreased risk of urinary tract infections.

Reduced risk of some sexually transmitted diseases in men 

Reduced risk of transmitting disease from men to women especially HIV and HPV.

Protection against penile cancer.

Prevention of balanitis (inflammation of the glans penis) and balanoposthitis (inflammation of the glans penis and foreskin).

Prevention of phimosis (the inability to retract the foreskin) and paraphimosis (the inability to return the foreskin to its original location).

makes it easier to maintain good genital hygiene

Complications of Circumcision

There are risks associated with circumcision just like any other surgical procedure. 

Risk of bleeding and infection.

Risk of meatitis (inflammation of the opening of the penis)

Risk of injury to the penis

Circumcision procedures

There are several techniques for performing circumcision. The choice of which technique is used depends on the physician’s preference and experience.

The three major methods of circumcision are the Gomco clamp, the Plastibell deviceTrusted Source, and the Mogen clamp. Each one works by cutting off circulation to the foreskin to prevent bleeding when the doctor cuts the foreskin. The procedure takes about 15 to 30 minutes.

ZSR Circumcision in Bangalore

The World Health Organization has endorsed a device from China that makes it easier and faster to perform surgical circumcisions. The device – which comes in 20 different sizes- consists of two plastic and rubber rings. ZSR circumcision is a revolutionary development in the field of phimosis treatment.

ZSR is done under anaesthesia, which can be local, regional, or general. It works by folding the foreskin back over the first rubber ring. The second, larger ring, which has a sharper edge, is then placed on top. When the two rings are closed, the blood supply is gradually cut off and the tissue dies and falls off. No sutures are required, but the rings must be worn for a period of seven days to allow them to close properly. 

This procedure is rapid, painless, and causes only little bleeding.  It is considered a day-care procedure and the patient can be discharged the same day. ZSR also showed to be superior in cosmetic outcome. 

Full recovery from ZSR Circumcision requires 3 weeks. The dressing should be changed every 48 hours which can be done at home or by the physician who performed the procedure. The patient will experience only minimal pain. 

Patient are usually advised to stay away from heavy work out or jogging. Furthermore, sexual intercourse should be avoided for 3 weeks until full recovery takes place. 

ZSR is considered to be a very safe procedure; but just like any other procedure it can very rarely face some complications such as:

Injury to urethra
