GRI system formulation

Micro/nanogels

Macroscopic hydrogels’ response is too slow to allow an efficient insulin release mechanism, so micro and nano systems have been evaluated as a possible option, since they can expand by more than 5 times in volume, thus facilitating the fast release rate of insulin. They have been evaluated in vivo by subcutaneously injecting a GOx microgel to T1D mouses, and it achieved self-regulation of BG levels under hyperglycemia conditions.

Core-shell particles

Typically micelles and liposomes, they contain an inner core, which plays the role of loading the molecules, and an outer shell, that releases said molecules. An in vivo study of a GOx base polymersome indicated that the formulation used was able to effectively regulate BG levels for up to 5 days. Another study, using PBA-based micelles, showed that the GRI release was achieved by swelling and disassembling or degradation of the micelles in response to BG concentration changes, and that the nanosize of the micelles accelerated the insulin release at hyperglycemia levels.

Microcapsules

They only contain a shell, in contrast with core-shell particles, made of alternated cationic and anionic polyelectrolites, and they release the drug through controlling the shell permeability.

In vivo tests

They are made both to make sure that the insulin release is working, injecting the GRI to the diabetic animal and conducting an intraperitoneal injection glucose tolerance test (IPGTT, basically, injecting glucose to the animal), and checking how the BG behaves (in an ideal GRI formulation there should be an instant response and an effective BG reduction), and that the insulin release is prevented at low BG levels, injecting the GRI to healthy animals and observing that no hypoglycemia is occurring.

The implantation of an insulin device (nanocomposite glucose-responsive plug covalently bound to an insulin reservoir made of surface-modified silicone) into STZ-induced diabetic rats made the BG go from >450 mg/dL to less than 100 mg/dL in less than a day, remaining under 150 mg/dL for 5 days, after which they started increasing to hyperglycemic (ca. 250 mg/dL) levels. After a glucose injection, which brought BG levels to over 450 mg/dL, the rats went back to a normal range in less than 1 hour, showing an increase in plasma insulin in just 2 minutes.

The drug previously named SmartInsulin® , developed by Smart Cells, is currently in early humans trials at Merck under the name of “MK-2640”.

Conclusions

It is clear that smart or glucose responsive insulin is still several years away. And even then, they may not be good enough to hit the market (because, honestly, if the GRI lets me get to >300 mg/dL before starting to act, I don’t think it’s worth it). Despite that, there are some promising results in vivo, in which they maintained considerably good BG levels for almost a week.

In the end, in my opinion, all depends on the efficacy of the device. The insulin release has to be fast enough (main problem, I think) to allow BG levels to be, at least, under 180 mg/dL after eating, and to stop the release even in the BG is getting down really fast (for example, the Medtronic 640G isn’t able to avoid some hypos if I have too many insulin in my body or if I’m doing exercise).

And even if the “perfect” chemical formulation is achieved, then we would have the problem of… How to use it. I honestly think that injections wouldn't be such a bad idea. Instead of 4-6 daily injections, we would only need one each 1-3 days. The implant would be a good option too, if it allows for a greater period of time and it’s relatively easy to insert. If you have to go to the hospital every week, that’s bullshit. If you can use the implant for 2 months and/or you can do it yourself at home, then it wouldn’t be so bad (we are doing it with pumps every 3 days, so…)

Finally, the last big problem I see is the daily control. You would still have to monitor your BG, to be sure that the GRI is working, that you don’t have an hypo… And you would need to be able to “see” how much insulin is still available. You can’t allow the GRI device to run out and then, when you’re over 300 and rising, insert another one.

Maybe 30, 50, 100 and 120 Units daily injections would work great here: you would only need to know your average insulin consumption, and what are you going to do that day (or the next day). And if you still have “leftovers” from the previous day, it doesn’t matter, it wouldn’t be like multiple injections, where you can have overlapping.

Or maybe weekly implants, if they are easily enough to insert. Maybe a combined GRI implant + CGM, which would last 7 days, and that you could insert yourself at home. Longer time would mean bigger implants (because even 7 days could mean, for some people, 500 - 700 Units, or more! Imagine that much insulin in a “pump reservoir” inside your body), and would make it more difficult to predict how much insulin you would need, so you may run out of insulin earlier.

Or even better, a combination of the 2? You could have a CGM+GRI implant for X months, but somehow refill the GRI with new insulin every couple of days. I think the Eversense CGM lasts 3 months, and the Senseonics can last up to 6, so maybe a visit to the hospital every 6 months to change the device wouldn’t be so bad, you have to go to the endo anyway!
