#but I hate blood-borne pathogens even more

Note
Just read your plastic bug review ( absolutely delightful!) and Id love to hear your mosquito hot takes if you have the time.
Well there's a viral lie that they're ecologically valueless and of course, no, there's really no such thing, and their importance goes far beyond just another food for insect-eaters, but that's more "scientific facts" than "hot takes" so as far as "hot takes" go:
Tumblr media
They're both beautiful and cute animals, more charming than butterflies.
The fact that they're bothersome vampires is part of the charm, it's just plain cool that our planet has swarms of night-flying blood-sucking swamp creatures.
I completely reject their status as "deadliest animal" on the basis that a mosquito by itself is irritating but inherently harmless. The diseases they can carry are distinct organisms that evolved to exploit the mosquito as a vehicle. It is worth noting that only a few mosquito species can even transmit illnesses to humans at all!
Speaking of which I notice people are quick to defend bats, raccoons, rats and other more popular animals that can spread disease, but use mosquito borne illness as justification to want mosquitoes totally eradicated, and I think that's pretty transparently a matter of petty spite. Take away the diseases and the mosquito is still an "inconvenience," however harmless, and humans just have a very difficult time with the reality that nature does not exist for our comfort and fun.
Can't help noticing that research into just killing them all off gets more attention and funding than the equally viable and environmentally safer research into simply making them inhospitable to pathogens, and I'm sure that's driven partially by the above biases, but partially because there's probably money and clout in being the one to reduce Pesky Bugs from popular tourist destinations like Florida.
Even having said all of the above, an animal never needs to be harmless, pleasant, or ecologically "essential" to be worthy of admiration. Each is a unique and special sculpture of evolution and a "character" in the vast varied cast of living organisms. Some of them are allowed to be bad guys. Some of them are allowed to just be background filler. The total sum of the different forms taken by life on our planet is what's precious about life on our planet, including every part of it anyone has ever feared or hated.

Listeria
For whatever reason it reminds me of Desperate Housewives and Wisteria lane. Or just Wisteria in general.
Tumblr media
It's unfortunately it's anything but wistful or romantic.
This is inspired by the netflix series Poisoned. I hate that title but it's a great watch. I learned a lot from it. I highly recommend it particularly for medical students and residents etc.
So, listeria are gram positive rods (most bacilli are negative) making this relatively easier to remember (purple rods). Also catalase positive. IT's also a facultative anaerobe - so both cultures may be positive (aerobic and anaerobic bottles).
Tumblr media
the species that is the culprit for major human pathogens is Listeria monocytogenes.
At increased risk groups (more likely to die of Listeriosis) - the immunocompromised and extremes of life, including the elderly, pregnant women and neonates. Immunocompromised: HIV, anyone on long term highish doses of steroids, leukaemia/oncology patients etc. Worryingly, incubation time is 3-90 days in these groups, so it's difficult to really trace or keep track of. Pregnant women are at 20x higher risk of acquiring the severe form.
Source: CDC
Tumblr media
Hence why during pregnancy you're advised not to consume raw salads, fruits, cold cuts, soft cheeses, sprouts and smoked salmon or sushi, no mayo and no raw egg (definitely no raw milk) etc. Similarly, common recommendation is you serve none of these to children under the age of 1. It naturally lives in soil (hence avoidance of raw sprouts) and can live in animals. So it can frequently contaminate food.
Tumblr media
As with COVID (I can't believe I'm using COVID as a measure of things), it can cause mild food borne illness (nausea, vomiting diarrhoea, myalgias, even fevers) to invasive disease causing sepsis and even meningitis or encephalitis. Incubation time is a few days in the mild form.
Tumblr media
Death by Listeria when you have the severe form (Listeriosis) is 20%, astonishingly high given you have a 1% chance of dying from Salmonella.
It's a small risk but the issue is that complications and fatality if you do acquire is high. It's also highly preventable, but the challenge is food safety and avoiding certain foods as a consumer.
i.e. in pregnancy, most will have diarrhoeal illness that's mild. But in the subset that get invasive disease and really unwell, there's a 20% risk of miscarriage, risk of premature labour and risk of still birth at 3% in the US. In the states, pregnant women are also 10x more likely to get Listeria infection. That is, illness after exposure.
On the final note of prevention, also always properly refridgerate food to 4 degrees celsius and cook meat to 165 fahrenheit or 73 degrees celsius. As I've learned from the Poisoned documentary, you can ask restaurants to do this, ask them to use an internal thermometer to measure - as rare/medium rare etc have no meaning as it pertains to food safety. If they can't, order something else.
Tumblr media
With meningoencephalitis, we just presume that Listeria is a possibility and treat empirically while awaiting investigations.
Investigations: - CSF (lumbar puncture) in event of signs of meningism or encephalitis (classic headache/fever/stiff neck/rash and/or acute confusion or seizure AND fevers) --> expect the classic features of bacterial meningiits and gram stain positive for purple rods - PCR - stool cultures have no value - blood cultures--> look for gram positive rods in the preliminary findings, expect a call from the lab Empirical therapy: - none in asymptomatic or mild disease. just monitoring until symptoms resolve and supportive care (fluids etc.) - Listeria is a notifiable disease to the health department in most Western countries that keep track of outbreaks. - in the US, standard treatment is ampicillin. - in Australia at least, standard therapy is IV benzylpenicillin, 2.4 g 4 hrly in meningitis or encephalitis and everyone is just started on this until bug identification/gram staining and sensitivities return. - in hypersensitivity, bactrim is used. - cephalosporins have no activity against them (or "inherently resistant), hence you can see IV ceftriaxone and benzylpen in the drug regimen for empirical therapy of meningitis - cef covers gram negatives, strep pneumo (most common cause of meningitis) and neisseria meningitidis - duration of therapy in severe disease: 3 weeks, 6 weeks if immunosuppressed
Really great summary here.
Random historical trivia
listeria is named for Dr. Joseph Lister, a British surgeon who introduced sterilisation of equipment and antiseptics to surgery, improve post op care and observed that microbes are the cause of cases of poor wound healing or post op infections. he also began to look at them under the microscope as an early pioneer of bacteriology.
Tumblr media
Lister's father by the way, made compound microscopes for a living, so lister became proficient at using this and started publishing articles during medical school
this also led to a lot more research on inflammation and coagulation
weirder trivia: in his med school days, surgeons commonly did not wash hands between patients and some didn't even change gowns, glorifying how busy they where by how many stains were on it by the end of the day
Tumblr media
so unsurprisingly his early battles to pioneer antiseptics and aseptic techqniues to prevent the transmission of infection in surgical patients were pretty uphill
Resources CDC guidelines WHO guidelines FDA Statpearls --> great at covering basic physiology and pathology etc. in a short form. Australian therapeutic guidelines - unfortunately not free.. so won't bother to link. If you work at any large-ish Australian hospital you'll have 'free' access. Animal: Research on Bed bugs
The animal I was given to research for this project was Bed bugs (Cimex Lectularius). They’re a small type of flightless insect that are known for being blood sucking parasites. As they are nocturnal, they take this opportunity to feed on those who are sleeping, so when they are still small in numbers it makes them harder to find. This hugely benefits them as they need to feed on blood as much as possible in order to shed and reach maturity, to then reproduce. Being so small with a flat build, means they can travel quickly across different surfaces and can safely take refuge in all kinds of crevices. The most common place for a bed bug to reside would most likely be in mattresses and headboards, this is because it creates easy access to their victims at night. Unlike other small insects’ bedbugs do not have nests and do not rely on each other for survival but will congregate in groups when resting. However, once their numbers have grown too many they tend to disperse and find shelter in other things such as furniture, clothing and can even travel across houses (Dunkin, 2022).
When someone has been bitten by a bed bug, they usually experience symptoms such as, itchiness, irritation, and soreness. These bites can often be found in straight lines or a uniform patten. In some cases, people can suffer with a more extreme reaction, causing the bites to turn in to lesions, welts or even infection (Arezki, 2020). These infestations can also have a large impact on a person’s mental health, which can often lead to social isolation. Sleep deprivation from being awakened by scratching bites creates this “Itch-scratch cycle”, this then can cause psychological distress to manifest as nightmares (Arezki, 2020). While bedbugs get quite a bad rap for their nature and can carry pathogens on their bodies, there is little to no proof that they transmit disease to humans.
In the 1940’s a pesticide called DDT was created to decrease the spread of diseases such as Malaria, Typhus, and other insect- borne illnesses to those at war. It also became one of the main ways to get rid of bed bugs too as it was effective. However, in the 70’s it became banned as the harsh chemicals was found to be extremely damaging on wildlife populations such as birds (Davies et al., 2012). Since then, there has been a large increase of bed bug infestations as they have built up a tolerance to these pesticides. I also think that there has been an increase due to more people travelling, especially after the Covid-19 Pandemic. Most Bed bugs will spread through luggage when people go abroad, this makes it very difficult to contain and control as you can’t stop people from travelling.
There is so much stigma against animals that don’t fit our society’s standard, animals that aren’t seen as “cute” and “fluffy” but rather seen as “gross” and “ugly” are cast away and demonized. We anthropomorphise animals that we have a sense of sentimentality towards, like with dogs we obsess over them because we get their love regardless. I think we connect with animals more than insects because they have similar eyes to us, it’s almost like we can connect with their soul because of that similarity. Peter Singer argued that we should reject speciesism and that animals that have an interests like humans (Not wanting to die) should be considered equal to us (Singer, 1975). I feel as though most animals and insects have that natural urge to survive so in this case should be treated equal. I think it’s weird that we have the choice to make these decisions, animals are just as important, in fact you could say they are more important because they keep the natural cycle of life going. The human population is incredibly vast and take up a lot of space, yet we are the ones trying to make animals and insects extinct. This fear against insects like bed bugs comes from not really understanding them, they are hated because of how they naturally are. To put it into perspective it’s like having a dislike to a person that can’t see, it’s outrageous because it’s not something they can control. From doing this research it’s raised the question whether bed bugs are directly the problem or is it the social and economic repercussions that are the issue.

The Making of... Myself?
Today I think we’ll talk about a thing that many people always want to know, once they’re aware that the mortality-inept exist and have come to accept that we’re here, some of us are indeed queer, and we’re most definitely NOT going anywhere. How was I made? I have been asked this by many a being, both human and otherwise. Apparently the making of vampires is interesting. I am not an expert! None of what I know makes me so, I can only tell you what I was told, as it was told to my Maker, and on into the annals of history. First off, I was made in the 1800′s. 1827 to be quite exact, tho I don’t remember when in the year. I know it was after May, because I had turned 35 by then. But that is about as exact as I can get. Somewhere in the middle to later half of 1827. I had been regularly going out with friends, all of us dressed as men, going to clubs and such as a group and having a good time. This was the Victorian era, wearing men’s clothes was quite inappropriate, but so was being queer. We were all LGBT+. Some of us were male, some of us were women dressed as men. Honestly, we just liked being in one another’s company, and the only way to do so without someone asking if one was engaged to one of the others, was to be innocuous. And men’s clothes are simply more comfortable and have infinitely less layers. I believe it was somewhere between taking a series of hackneys from a dinner hall to a boxing match, when a gentleman slipped into the cab I was in, along with a couple of other chaps, and he apologized for intruding but it was late, and he would pay for the cab if we would allow him to ride with us, as he was going on after we would get off. Since it wasn’t a big deal for us, as we weren’t going anywhere special, and we would be out before him, we let the gentleman ride in our cab. At some point in the ride he had chatted with us enough that we were comfortable telling him where we were going, and he was quite polite. He was a gentleman, he had the natural bearing of a born lord of the empire. We arrived at our stop and we left him in the cab. It was the first meeting, and he seemed perfectly ordinary, a bit older, perhaps my uncle’s age, but not particularly suspicious or standout. The next outing we had, I was staying over with a friend, we were supposedly going to the seaside to take the air, but honestly, we were going to a house party with our clique. We would play cards, gad about the woods, ride horses, and have a generally good time. As we were out for a couple of weeks, we ran into a younger man, who said he lived near by our friend’s family estate, with his uncle, and that we were welcome to call, or to make use of the lake on their property if we liked. It turned out that his uncle was the gentleman we had met a few months ago. He seemed surprised, but he was quite welcoming and invited us to dinner a time or two with himself and the younger man. So we included the young man, as he had no idea that a great deal of the “men” who he was out riding with or climbing cliffs with, were in fact women! We had all been doing this for many years after all. When the end of our trip was coming upon us, we had a last bash, a party where we could have a good time, and we invited the younger man and his uncle out of courtesy. At some point the party felt a bit much, and I cried out on the game of cards so I could go get some air. I had anxiety even then, and sometimes I would just need a break. My friends knew and were quite okay with it. The older gentleman joined me for some air, and we left the house for the gardens. I thought I had separated from him when I nearly walked into him around the curve of a hedge, out of sight of the windows and open patio doors of the house. That was the point at which he caught me by the arm, and he apologized, and said he had lost sight of the house and was a bit lost. As I turned to point out the path that would take him back, a hand clamped on my shoulder and a sharp, burning pain struck me! I couldn’t see what was going on, but I quickly caught a breath and was about to shout, when his other hand covered my mouth. Yes, this seems quite violent, and a bit like assault. It was. I was assaulted. I had no idea what was going on, but I started to feel faint and I sagged in his grip. Then he shoved a slashed wrist into my face. If I was going to survive to see the morrow, he said, I would drink the blood welling up. I refused, but he could easily manipulate me, as I had been quite drained. I ended up drinking. I had no wish to die. He scooped me up and took me inside, and said he’d found me outside, fainted, and asked if he should take me to my rooms. He did so, and tucked me in, but when he had pulled my coat and such off to make me ready for sleep, he said something in a language I don’t know, and I can only assume that he hadn’t expected me to be a woman. Hah, shows how observant he is! The vector for becoming a vampire is a combination of some sort of blood infection, and death or near death. One must be weakened, mostly by blood loss, and then ingest the infected blood, giving it chance to take over the weakened host body. I slept for nearly 24 hours, and when I woke I was ravenous, but water made me vomit. Then there was a knock on my window and it was the younger of the two and he said that his uncle (it was not his uncle, if you haven’t surmised that yet) would like to see me. I was taken to the neighbouring estate and given a cup of something that basically smelled like the most delicious of wines, and after I drained it, it was explained that I had been turned, and he actually apologized for assaulting me, as he hadn’t known I was a lady, he’d thought me a fine strapping lad, and thought I would make an amusing and interesting companion. Turned out he was a lonely old gay man, quite old. The other young man was his previous lover but, was ready to go out into the world on his own to see what was to be seen. The younger man was already well into his 90s it turned out. He apologized several times in his explanation. But what was done was done, there was no undoing it. I had the expected reaction. I was furious, I didn’t believe him, I said several angry and unkind things about what he had done too me without my consent. And I stormed home. That was an extremely poor choice of ideas. I nearly attacked a sibling, at which time the younger man called on me, and said that our Maker had tasked him with keeping an eye on me, in case of emergency. I was still pissed, but I couldn’t put my family in danger. I “married” him, and we left for the Americas. We remained “married” for about 15 years, at which point we had an unfortunate accident and “died”. I then recovered my masculine persona, and began my own life, with occasional support from my Maker. He did feel bad, and while I no longer hate his immortal soul, I also have no desire to keep contact with him. He made a decision that was not his to make. Also, about the blood infection that causes the vampirism effect, apparently it does not react to any currently available treatments. Many have been attempted, even those for cancers of the blood. Nothing. The best that vampire science can tell is that it is some sort of blood pathogen that affects ones DNA in some subtle way, and then replicates itself. It is resilient and rejects any treatment of medical science. We even tried radiation therapy. We suspect it has some element of magic too it, but it cannot be proven. We’ve had magic tried. No dice. If there’s a vampire out there who knows where it comes from, they are keeping it to their damn selves. But that is what happened. A lonely old man thought I would be a good companion, but ended up with someone who was officially female, which didn’t suit. I have never Made anyone, as I found the process unpleasant and vile. I wouldn’t do that to another person. I’m very much of the “Live and let live, so long as no one is harmed and you respect other being’s boundaries and lives” set. The right of others to live their lives as they see fit is important. so long as they live with integrity and don’t cause harm to others, you do you, boo. I remain -Ashe, the Asexual Vampire

Captive State
Tumblr media
I will give even the most obscure, low budget trash a chance if it is an original piece of speculative fiction. There are already so few original stories being told by Hollywood now, and sci-fi, fantasy, and horror are the genres of cathartic myth-making that are most likely to have something interesting to say about any current cultural moment. Captive State is no exception. On the surface it looks like a pretty standard “aliens have invaded” sci-fi flick, but dares to ask what the world would look like 10 years down the line if humans weren’t wiped out, but instead cooperated with their new spiky alien overlords. And what if there’s a group of humans that refuse to roll over and go underground to form a resistance? Does this dystopian nightmare have anything new or relevant to say? Well...
It may not be a game changer for the entire world of sci-fi cinema, but this is a more than solid entry into the pantheon of dystopian alien future movies. Its characters are strong, its world-building is thorough and interesting, and its story is one that may seem overly simplistic in its hopefulness, but feels sorely needed right now.
Some thoughts:
The character design of these aliens is T E R R I F Y I N G.
The first 3 minutes of this film are flawless and do so much in so little time to set up this world and solidify the mythos, explain the conflict, and absolutely establish the new world order. It’s a deft and vibratingly tense sequence.
Benny from The Mummy! What have you been doing the last 20 years, man?
It’s the little details that really make the world-building sing. Love all the lo-fi tech used by the resistance (since everything digital is traceable) including Polaroids, reel-to-reel film, and carrier pigeons.
Although the real force of the film is the resistance plot, there are still some great characterizations and performances on display here. John Goodman is the bureaucratic Man in Black working for the government (and, ipso facto, the new alien overlords) to ferret out the resistance, and he’s doing a classic John Goodman workhorse performance here. But the real bright spot is Gabriel (Ashton Sanders) and his relationship with long-lost brother Rafe (Jonathan Majors).
There is a dog used as a signal in this “twilight bark”-esque notification of the resistance, and he does his job perfectly (Good Boy!) BUT towards the end of the movie, the dog is shown in the same place he was and now it’s nighttime and he’s still barking. There wasn’t a plan in place to retrieve the dog? He’s just been chilling out there this whole time, vulnerable and chained so he can’t get away? This is how you get people to hate your resistance, y’all.
Also I recognize that you’re in a hurry and everything, but that guy didn’t clean the bloody bug extractor in between people and I wanted to vom. Blood-borne pathogens are no joke, even during dystopian alien dictatorships.
Super into this cool lady mechanic with the hella gay haircut (Caitlin Ewald). Correction: TWO hella gay haircuts.
The soundtrack and score did an excellent job of setting the tone and keeping things tense - one recurring motif was a steady, thumping heartbeat that seemed to ratchet up during every step of the resistance’s Big Plan.
Did I Cry? Only the teeniest, tiniest bit at the very end. Mostly I was just tense and riveted.
If you like anything involving dystopian futures or rebellions or aliens or John Goodman, you honestly owe it to yourself to check out this movie. I was pleased as punch at how much I enjoyed it going in with next-to-no expectations. It may not reinvent the wheel, but it will definitely hold you captive for a good 109 minutes.

My piercing apprenticeship, pt 1.
The story begins with me, not the brightest student, struggling through A levels after just barely surviving GCSE’s. It was the time when everyone was either partying hard or studying to get into university and I was doing neither. I felt as though I was at a stand still... till my lovely dearest friend just told me to go for everything and anything and see where life would carry me. So I did exactly that, all the things that I was passionate about I threw myself into, I started writing more and posted my work online, I applied for tv and radio internships and I also applied for piercing apprenticeships to every studio within a 10 mile radius of my town.
And I got turned down for every single one of them.
Fast forward about 2 years, I was still as unaccomplished and in a job I hated but more secure in myself I guess. Then came the one uneventful day, I got an email, from probably the most notable piercing shop in my town, saying they’d been going through their emails and my cover letter and CV impressed them.
I was a little gobsmacked to be honest, like I said this was probably the most popular piercing shop in my town and I know for a fact they must have had thousands of people asking for apprenticeships. But never the less, I responded immediately (after calling my friend to squeal down the phone),and from then the correspondence led to an interview. When this interview came around I had come to learn that the business was being taken over by new owners who were expanding the business. That’s fine, the shop was reputable and really loved in the community but there were some faults, the wait time, only being able to pay in cash, the opening hours etc. So this new management is exciting me to be honest.
So for my interview, I’d prepared myself to the fullest, printed off a copy of my CV AND emailed one, printed my passport, bank details and printed my study materials for blood born pathogens and first aid and COSHH safety standards and I just about robbed my current job of printer paper and ink doing so!
I was prepared but I also like to mentally prepare myself for failure (because let’s face it even in this niche market, job prospects for my generation aren't the best) but it turned out be the best interview I’ve ever had. (BTW; if anyone wants a post on what the interview and questions were like, let me know and I’ll make a separate post, because I’m someone who likes to prepare thoroughly and I know I couldn't find much when searching for what kind of questions they’d ask me.)
And VOILA, a few days later, I get a cal saying I GOT THE JOB. I start 2nd of November 2017.

I have to get a shot today and I have Trypanophobia here's ways to help cope
Shockingly I'm calmer due to I realized once I did research on why I shouldn't be afraid and the procedures I managed to desensitize myself from fearing sharps containers. Bless Esthican School for being similar to medical school. • If they didn't have a scarps container then you should be terrified because the scarps container is meant so bacteria and other harmful blood born pathogens don't come in contact with people. So if anything it's a relief to see protocall is being met! • I tend to panic when they have to cleanse my skin. Cleansing the skin is also a relief because if they didn't do that you could get infection and probably have to get more shots so yay for cleansing! • When getting blood drawn I hate when they have to tie my arm to see my vein (reminds me of heroine addicts idk) so in reality a addict would never cleanse that's dangerous! Also by tieing the arm it makes it easier to get it over quick. I don't like when I can't see things my mom said and it helped its literally as if you were pinning on a safety pin on a shirt it's only on the top layer or the skin if it was underneath the dermis we'd have a problem so it's good that it's only the top! • Also a shot is literally not even going into the skin it's literally as if you were sewing and you pricked yourself on accident Hope this helped people a little bit! You're not alone!
