Er is een groeiend aantal mensen dat nu al nauwelijks kan rondkomen en voor wie de energierekening onbetaalbaar dreigt te worden.” Deze raming is traditiegetrouw het uitgangspunt voor het kabinet voor de finale besluitvorming over de begroting van het komende jaar. Op Prinsjesdag presenteert het CPB de definitieve Macro-Economische Verkenning, waarin ook deze laatste besluitvorming is verwerkt. Koopkracht hard onderuit, armoede neemt toe Door de hoge inflatie en de daarbij achterblijvende loonstijging daalt de koopkracht in doorsnee 6,8% in 2022. Daarbij is er inflatie-ongelijkheid: hoe lager het inkomen, hoe sterker de financiële last van de inflatie wordt gevoeld. Het standaard koopkrachtbeeld is gebaseerd op de gemiddelde inflatie en houdt dus geen rekening met dit soort verschillen tussen huishoudens. Het CPB publiceert voor het eerst een raming van de ontwikkeling van de armoede. De armoede neemt toe: door de inflatie is steeds meer geld nodig om aan de basisbehoeften te kunnen voldoen, terwijl de inkomens beperkt stijgen. Als gevolg hiervan stijgt de armoede in personen in 2023 tot 7,6%, voor kinderen is dat zelfs 9,5%. Sri Lanka prepares for twice-delayed poll amid militarized COVID-19 response

Sri Lanka is set to hold its parliamentary elections on August 5 which was delayed by more than three months due to the Coronavirus crisis. The election was originally scheduled on April 25 but it was postponed to June 20 because of COVID-19 health concerns. The June election was postponed again for the same reason. The August election was announced after Sri Lanka had started to ease its lockdown restrictions. Sri Lanka is doing better in containing the spread of COVID-19 compared to its South Asian neighbours. At the time of writing this report, Sri Lanka has recorded 1,950 Covid-19 infections and 11 deaths with the number of active cases dropping and the number of deaths remaining stagnant for a couple of weeks.

Snap elections

President Gotabaya Rajapaksa, who was elected in the November 2019 presidential elections, used his constitutional power to dissolve the parliament on March 2. This was six months ahead of schedule which he insisted was necessary because the opposition-dominated parliament has been undermining his power. One of the goals of the Rajapaksa's party, Sri Lanka Podujana Peramuna (SLPP), is to regain its majority in the 225-member parliament. Rajapaksa has called for a snap election on April 25 which was criticized by many as untimely since the country is facing a pandemic. Human rights lawyer Bhavani Fonseka tweeted about the appeal to postpone the election:

Amid calls from the opposition and civil society to postpone the polls, Election Commissioner Mahinda Deshapriya announced the indefinite postponement of the election on March 19, citing the COVID-19 threat. Harim Peiris writes on Groundviews about the declaration:

Given that the electoral process requires a mass domestic human migration or movement of people and that campaigning makes social distancing impossible, the decision of the Elections Commission was inevitable. Perhaps in hindsight, the hasty dissolution of parliament when Covid-19 was known as a global pandemic was unwise.

On March 22, Sri Lanka imposed a seven week-long strict lockdown to prevent the surge of COVID-19 cases. On April 20, the Election Commission announced that the parliamentary election will take place on June 20.

A constitutional crisis?

Analysts immediately pointed out that the new election date violates a constitutional provision which requires an election within three months after the dissolution of the parliament. Asanga Welikala writes about this issue on Groundviews:

The Constitution requires even a dissolved Parliament to be recalled in an emergency to fulfil the requirements of legislative oversight of the executive (including the exercise of any emergency powers), and to approve appropriations of public funds for government expenditure. The President, however, has steadfastly refused to do so. Coupled with this refusal, the practical inability to hold an election during the pandemic has resulted in the Election Commission having to set 20 June as the new date for the election. This is a date that is prima facie in breach of the constitutional stipulation that an election must be held, and a new Parliament must meet, within three months of the date of the dissolution of the old Parliament.

Many questioned the practicality of holding an election during a pandemic. They also accused the government of mobilizing the military to consolidate power. Tisaranee Gunasekara warned about the dangers of militarizing the COVID-19 response:

The danger of holding an election in such an atmosphere is obvious. The government will not hesitate to arrest opposition activists and voters for violating this or that anti-virus rule while giving a free pass to its own supporters. But postponing the election repeatedly carries its own dangers. It will habituate the president into acting outside the constitution and strengthen those who are advocating Gotabaya rule.

On June 2, Sri Lanka’s Supreme Court dismissed a number of petitions challenging the decision of the election commsion to hold the polls on June 20. This was regarded by many as a vindication of Rajapakshe's actions to dissolve the parliament and his refusal to call the parliament into session.

However, educator Dr. Asanga Welikala disagrees with the court decision:

On June 10, Election Commission Chairman Mahinda Deshapriya announced that the date of the election will be rescheduled for the second time to August 5.

Health guidelines during the election

Along with the announcement of the August 5 polls, the Election Commission also published its health guidelines for conducting the election amid the COVID-19 outbreak. Salient features include:

Wearing a facemask in the poll booth

Keep a distance of one meter in the queues for polls

Not allowing those who have not completed their quarantine

Limiting attendance of election propaganda meetings to 100

Meeting participants will wear a facemask and practice social distancing in the seating arrangements

No processions allowed

Distributions of handbills discouraged

The Election Commission has so far arranged two mock polls to test the preparedness of people during the pandemic.

Many election candidates are spending on social media ads for their political campaigns. According to reports, Facebook will release information on the advertisement spendings of Sri Lankan political candidates. The European Commission, meanwhile, has decided not to send a full-fledged observer mission for the August 5 polls citing COVID-19.

As Sri Lanka is set to open its borders to tourism once again on August 1 as a step towards resuming normalcy, it remains to be seen how free and fair the election will be.

Corona virüs aşısı hayvan deneyi aşamasına geldi!

Bütün dünya corona virüsüne karşı aşı geliştirme çabasında. Dünyada çok sayıda merkezde aşı geliştirmek için çalışmalar sürdürülüyor. Bu aşı araştırmalarının büyük bir kısmı, rekombinant yani sentetik aşı üretimi üzerine.

Aşılar canlı virüslerin kullanıldığı atenue-inaktif veya virüsün sentetik parçalarından oluşturulan rekombinant olmak üzere iki yöntemle hazırlanıyor. İnaktif aşılar, rekombinant aşılardan çok daha hızlı hazırlanabiliyor, viral mutasyonel değişikliklerden daha az etkileniyor ve daha uzun süre etkin kullanılabiliyor. Öte yandan dezavantajları da var; kitlesel olarak çok yüksek sayıda üretilemiyor, daha yüksek maliyetli, virüsü canlı olarak laboratuvarda çoğaltmak gerektiği için maksimum düzeyde güvenlik uygulamaları gerekiyor, bağışıklık oluşturma yüzdesi rekombinant aşılara göre daha düşük.

Acil müdahalelerin hayat kurtarabileceği bu özel süreçte, inaktif aşının olumlu taraflarına odaklanmak ve kısa dönem avantajlarını göz önüne almak gerekiyor. Yüksek sayıda üretilebilecek rekombinant aşılar geliştirilene kadar, doğası gereği az miktarda üretilebilen inaktif aşılar, özellikle risk altındaki sağlık personelini, ileri yaşlıları ve yine kanser, astım gibi ek hastalıkları bulunan risk altındaki kişileri koruyacak sayıda üretilebilir ve çok önemli bir yarar sağlanabilir. 

Biz, TÜBİTAK’ın destek verdiği COVID-19 inaktif virüs aşısı çalışmamızda üretimi tamamlayarak hayvan deneylerinin yapılacağı noktaya geldik. Hayvan deneylerinden sonra Faz-1 çalışmaları başlıyor, gönüllülerde 3 hafta süren yan etki ve antikor oluşturma açılarından değerlendirme yapılıyor. Sonrasında daha geniş bir popülasyonda aşı çalışmaları devam ediyor, yeterli güvenlik ve etkinlik elde edildikten sonra yine çalışma daha da genişletilebiliyor… İdeal koşullarda tüm bunlar, ortalama 3 ayda sonuçlanabilir. Tüm bu aşamalar, Sağlık Bakanlığı’nın onay süreciyle gerçekleşiyor. Peki, başarılı olunamazsa?… Pek çok bilimsel araştırmanın başına geldiği gibi, hayvan deneylerinde ya da sonrasında istenmeyen bir etki ile karşılaşabiliriz, bu durumda yine laboratuvara girer, yine çalışırız.

Kök Hücre Çalışması: 7 Hasta Üzerinde Araştırma Yapıldı COVID-19 Tedavisinde Mezenkimal Kök Hücrenin Etkinliği; Faz II Klinik Çalışma olarak Sağlık Bakanlığı’na sunulmuş olup, münferit hastalar için bakanlığın izniyle uygulama yapılmaktadır. Kontrol grubuyla birlikte giden standardize çalışma için proje onayına ihtiyaç duyulmaktadır.

COVID-19 hastalığının tedavisinde mezenkimal kök hücre uygulaması yapılabiliyor. Sağlık Bakanlığı tedavi rehberinde de yer almaktadır.  Bu ek tedavi şekli konusundaki klinik gözlemlerimize göre; uygun endikasyonla (ileri hastalık, entübe hasta) kök hücre uygulanan 7 hastada iyi sonuçlar elde ettik. Sağlık Bakanlığı’na yapılmış başvurumuz onaylandığı takdirde kontrol grubuyla beraber bilimsel bir veriye ulaşmak için klinik çalışmayı başlatacağız.

İmmün Plazma: Her Kan Grubuna Uyumlu Bir Plazma Ürettik ACU-IP1.0 (Bağışık Konsantre/Patojen Free Plazma); üretimin yapılabilmesi için tüm şartlar oluşturulmuştur. Sağlık Bakanlığı’nın COVID-19 İmmün Plazma kullanımıyla ilgili genelgesi sonrası bu ürün de kullanıma girmiş olup, laboratuvarlarımızda üretilmektedir. Faz II Klinik çalışma için onay beklenmektedir.

TÜBİTAK tarafından desteklenen bu araştırma projesi, hastalığı yenerek bağışıklık kazanmış kişilerden kan alınması ile elde edilecek virüs spesifik antikorların toplanması ve bunların yeni hastaların tedavisinde kullanılmasına dayanıyor. Sağlık Bakanlığı, COVID-19 tanısı alıp iyileşen kişilerden alınan immün plazmanın içerdiği antikorlar nedeniyle hastaların tedavisinde kullanılabileceğini onaylamıştır. An itibariyle hastalarda kullanılacak olan immün plazmanın hazırlanması laboratuvarlarımızda en uygun şartlarda gerçekleştiriliyor. Birden fazla kişiden alınarak havuzlanan plazma; tüm virüs ve bakterilerden arındırılıyor, konsantre ediliyor, içerdiği antikor düzeyi ve çeşitliliği, ayrıca bunların virüs nötralizasyon kapasitesi de ölçülebiliyor ve standart düzeye getiriliyor (üründen ürüne farklılık olmuyor). Hazırlanan plazmadaki antikorların titre edilerek miktarlarının bilinmesi ve virüsü nötralize etme kapasitesinin ölçülebilmesi, plazma hazırlık aşamasında çok önemlidir. Ayrıca kan grubu uyumu sorunu oluşturmayacak şekilde hazırlanmaktadır. İmmün plazmanın hastalara yönelik klinik uygulamaları da devam ediyor.

Covıd-19 virüs spesifik t-lenfositler: virüsle savaşacak t lenfositler hazırlıyoruz! COVID-19 Spesifik T-lenfositler; üretimi tamamlanmış olup Faz II Klinik çalışma için TÜSEB’e destek başvurusu yapılmıştır. 

T-Lenfositler insanlarda bağışıklıktan sorumlu savaşçı hücrelerdir. Laboratuvar ortamında da bazı özel yöntemlerle virüsün tipine özgün olarak hazırlanan bu hücreler, bugün dirençli viral enfeksiyonların (EBV, CMEV, BK, AV) tedavisinde tüm dünyada çeşitli merkezlerde uzun süredir başarı ile kullanılıyor. Özellikle kanser, bağışıklık yetmezliği ya da bağışıklık baskılayıcı tedavi gören hastalar için uygulanılıyor. Günümüzde ise corona virüse karşı savaşmak için yine laboratuvar şartlarında hazırlanan T-lenfositlerin klinik uygulamalarda başarılı olabileceği öngörülüyor. Corona virüsüne özgün üretimini tamamladığımız T lenfositlerin COVID-19 hastalarının tedavisinde etkinliğini kanıtlamak üzere TÜSEB’e destek başvurusu yapılmış olup, Sağlık Bakanlığı’na da klinik araştırma başvurusu yapılacaktır.

Dornaz alfa çalışması: klinik gözlemler denemeye devam edilmesi gerektiğini gösteriyor!

COVID-19 Tedavisinde Dornaz Alfa’nın etkinlik ve güvenlilik çalışması; Tüm laboratuvar süreçleri tamamlanmış olup, Faz II Klinik çalışma için TÜSEB’e başvurusu yapılmıştır.

In vitro şartlarda (laboratuvar ortamında) beyaz kürelerin oluşturduğu jelimsi tabakayı süratle eritebildiğini gözlemlediğimiz bu ilacı kullanan kistik fibrozisli hastalarda da COVID-19 enfeksiyonun daha hafif seyretmesi nedeniyle, standart tedaviye dirençli 3 hastada uygulama sonucu iyileşme olduğunu gördük. Bu bulgular daha önce laboratuvar ortamındaki bulgulara uyumlu olarak COVID-19hastalarının akciğerlerinde gelişen solunum zorluğunun kontrol edilebildiğini gözlemledik.

Sonuç olarak bu ilacın tedavide özellikle erken dönemde (entübasyon öncesi) uygulanabilirliği ile ilgili olarak TÜSEB’e destek başvurusu yapılmış olup, Sağlık Bakanlığı’na da klinik araştırma başvurusu yapılacaktır.
