Image ID: Two pictures of game stat blocks for the demonthorn mandrake. The first is for 5th Edition D&D and the second is for 13th Age. Full text is available below the read more. End ID.
Demonthorn mandrakes are a type of plant that grows in extraplanar locations, primarily in the Abyss, the Nine Hells, and adjacent fiendish planes. They look like 7-8 foot tall, 6 foot diameter tree trunks with thick, crimson roots that it can use to crawl along the ground. Red stalks grow from the top of the trunk, along with star-shaped fruits and spiny seed pods. Two of these stalks are much longer and more dexterous than the others, and can be used to attack potential prey.
The fiendish plants don’t feed on sunlight and water as most do, instead getting their nutrients from meat. They go dormant if deprived of food, and can remain alive in that state for years until something enters their senses again and catches their attention. They usually hunt in groups whenever possible, bombarding creatures with spore pods and planting thorny growths within their bodies and then attempting to pin them down with their roots and kill them. Creatures that die with spores growing inside their body birth a new demonthorn mandrake within the next week, so long as a reasonable portion of the body is still left after the mandrakes eat.
While average demonthorn mandrakes have moderate cunning and intelligence, they are still plants in their nature and have no true society. There are rumors of the Mother Seed, a demonthorn mandrake of exceptional size and intelligence that makes far reaching plans from somewhere deep within the Abyss, but no one has confirmed such a creature’s existence yet.

5th Edition
Demonthorn Mandrake Large plant (demon), neutral evil Armor Class 13 (natural armor) Hit Points 136 (16d10 + 48) Speed 20 ft. Str 18 (+4) Dex 11 (+0) Con 16 (+3) Int 7 (-2) Wis 9 (-1) Cha 12 (+1) Damage Resistances fire, lightning Senses blindsight 90 ft. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 11 (2d6+4) bludgeoning damage. Spore Pod (Recharge 5-6). The demonthorn mandrake spits a spore pod at a point within 30 feet of it that it can see. The pod then explodes in a 10-foot radius sphere. All living creatures within that area must succeed on a DC 14 Constitution saving throw or become poisoned and take 9 (2d8) piercing damage as thorny growths burst from its face and body. While poisoned in this way, the target takes an additional 9 (2d8) piercing damage at the start of each of its turns. The poisoned creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. A creature can also use an action to pour a vial of holy water onto the poisoned creature's face, ending the effect on that creature as the growths wither and die. Tangleroot. The demonthorn mandrake extends its roots in a 30-foot radius centered on itself. Each creature touching the ground that enters that area or starts its turn in the area must succeed on a DC 14 Dexterity saving throw or become restrained. The demonthorn mandrake can target a restrained creature with its slam attacks, even if that creature is outside the slam's normal reach. As an action, a restrained creature can attempt a DC 14 Strength check, breaking free of the roots and freeing itself on a success. The demonthron mandrake can't move while its roots are extended. It can retract its roots as a bonus action, ending the effect and releasing all entangled creatures.
