I was one of the person who were off putted when Mugino killed Frenda during Index III. Remembering when I saw them in Railgun S, I thought Mugino going to such an extreme to be unlikely.
But I read the Railgun manga and got to see how, even without the argument that Battle Royale was made before Railgun, it made sense.
First, the bond of ITEM and Mugino wasn’t as strong as I remembered. I knew Mugino had a scene where she showed deep care for her team. But I didn’t remember that it was mostly a ruse to get a 1v1 with Misaka. But still her overall care for her team was there nonetheless. What made sense was the next point.
Second, oh boy I didn’t remember how psychotic she could be. Since she’s so powerful, she’s not used to struggling and really doesn’t like to be faced with hardship, getting her angry really quickly. And also with that sense of pride, she dislike the ranking system of level 5, since it’s not based on power. So she hate to have lesser rank than people she might be stronger than, especially with Misaka since she’s directly ranked above her, but I guess it works to a lesser extent to the higher lv5 as well.
And so, the two ways to trigger Mugino are as follow : hardships and the top 3 levels 5. The anime made a poor job at really explaining what happened to her, but yeah, the fact that she got her ass destroyed by Kakine TWICE in the same day would easily be enough to put her anger as high as possible, and make her loses it.
In all this context, yes : the death of Frenda was perfectly understandable.
(LIGHT NOVEL SPOILERS THERE) : it’s even backed up from what I saw in the wiki. Mugino highly regrets what she did and Frenda’s death plays a huge role in her unstable mental state.

mugino from the start didnt personally care about frenda's death. it was only after she started to change that she did.
