But during this short time of free weekends, you would rather be on call, have some action and learn hands-on to be ready. Anyways, I reflected on one of my patients back in my intern year. She had a recurrent glioblastoma and I’ve met her when she already had developed a full blown psychoorganic syndrome (POS) talking rather gibberish and wandering aimlessly around the halls. . One day I was assisting one of the attendings in the OR and at some point he stopped and asked me: is the patient from our department or did she just came in from outside? I explained who she was and that she was in ours and he, very surprised, sighed. Yeah, I know her from way before [POS]. It’s a horrible story. He told me from his initial seeing her at the neurooncological consultations to her glioblastoma progressing and recurring repeatedly and her having an abortion at one point when she has developed POS. . I’ve rarely seen my consultant talking so sincere and sad when he continued with the surgery. And I realized once again what a privilege and obligation neurosurgery and of course medicine in general carries. People come with horrible diagnoses, sometimes you might be able to help them, very very often not, and often you accompany them all the way, regardless of what the outcome might be. . Charles is a 69 year old who came to me because he was having severe, electrical shocks across the left side of the face. The pain lasted for a few seconds at a time and he had multiple episodes a day. #Repost @neurodreamteam Trigeminal Neuralgia ⚡️ ———————————————— He had initial improvement with medication, but eventually the medication dosing had to be increased and eventually wasn’t effective. We took Charles to surgery for trigeminal neuralgia. I found an artery pulsating against the nerve next to the brainstem. I moved the artery away from the nerve.
