i don't have hardly any followers on here, so this is as good a place as any to put this: today i got reconstructive surgery to fix damage done to me during an abusive relationships years ago. and even though the area is still dressed/bandaged and swollen I looked in the mirror and straight-up wept with joy for not having to carry that reminder of him around on my body every day. also my students keep messaging me offering help and support and my cat won't leave my side. i don't think i've been this happy in years.

I finally got an answer from my podiatrist about why the outer part of my left foot hurts so much. I've brought it up before but there was a lot going on that also demanded attention but is better now.

That meant we could focus on my biggest problem area, and I showed him how my toes on the left side look more smooshed together than my right. I didn't understand when surgery didn't happen in that area. He explained that a muscle is pulling on my left side, making that area pull down and look scrunched at the toes. He proceeded to hold my right foot in a way to show what he meant and I proceeded to see it looked similar to the left.

In short, I need to walk more on my orthotics, which has already been happening. I've been taking my dog for more walks and even hiked on a trail near my sister's house in NH and I was fine. In fact the nature of the pain has been changing, in a way that makes me think it's getting better because it's starting to get pins and needles on the edges of the pain.

The other thing I need to do is just concentrate on walking better. I need my left foot to land properly on the pad of my foot. The muscle pulling the way it does makes me land on the side of my foot first, rather than evenly across the pad. That will also be helped by continuing some physical therapy moves at home.

It's been a year and half since foot surgery to help correct high arches that makes me prone to nasty falls. I originally didn't want surgery but it already hurt so much to walk, and I had a fall holding my nephew that really prompted me to do this. Stabilizing my ankle so I don't fall was the big main thing to fix and it's been a success.

My second goal was to walk with less pain overall. That success is still pending but I feel like the right things keep happening just at a very slow pace. I can be patient if I feel like I'm still moving in the right direction.

Bunlar hem görsel hem de dokunma hissi açısından farklı görünümlere yol açarlar. Örnek olarak deriden kabarık bir görüntü renk farkı olmasa dahi dikkati çeker. Veya deri ile ayni seviyede ancak farklı renkteki bir oluşum da dikkati çeker. Renk veya görüntü farkı olmasa bile duyuda farklılık var ise örneğin ağrıyor veya kaşınıyorsa gene dikkatimizi çeker. Bu durumların her biri deri lezyonu olarak değerlendirilir. Çok kez birkaç bulgu birliktedir.

Aniden çıkan yaygın cilt lezyonları (bazı hastalıkların derideki belirtisi)

Aniden çıkıp sonra kaybolan ancak bazı koşullarda yeniden gelişen lezyonlar (alerji veya bazı deri hastalıkları)

Deri lezyonlarının tedavisi

Çok sayıda ve ani olarak çıkan deri lezyonları vücuttaki sistemik (vücudun bütününü tutan) bir hastalığı düşündürür. Tedavisi esas hastalığa yönelik olmalıdır. Tek veya az sayıda deri lezyonları veya deri tümörlerinde eğer tıbbi (ilaç) tedavi mümkün değilse cerrahi tedavi uygulanır.

Cilt lezyonu ne zaman cerrahi olarak çıkartılmalı?

Bir deri lezyonu yıllardır hiç değişmeden ayni bölgede duruyor ise genellikle iyi huyludur. Ama sırf görüntüde rahatsızlık yarattığı için estetik açıdan çıkartılması istenebilir ve çıkartılır. Prensip olarak görüntüsü nasıl olursa olsun çıkartılan her deri lezyonunun mutlaka tetkike yani patolojiye gönderilmesi şarttır. Nedenine daha sonra değinilecektir. Şüpheli görünen deri lezyonları ise mutlaka cerrahi olarak çıkartılmalıdır.

Plastik Rekonstrüktif ve Estetik Cerrahide Deri Lezyonu Cerrahi Tedavi Prensipleri

İyi huylu bir lezyonun estetik veya fonksiyonel amaçlı çıkartılması

Özellikle yüz bölgesindeki et benleri sayı ve görüntü olarak rahatsızlık yaratabilir ve tedavileri cerrahidir. Ayrıca iyi huylu tümörler gözkapağı, dudaklar, burun, genital bölge gibi yerlerde fiziksel sorunlar yaratabilirler ve fonksiyonel amaçlı olarak cerrahi tedavi uygulanır.

Kötü huylu olma ihtimali olan lezyonlarda cerrahi

Çeşitli bulgular bir deri lezyonunun kötü huylu olabileceğini düşündürür. Bunlar:

Mevcut lezyonun birden büyümeye başlaması ve komşuluğunda yeni benzer lezyonlar çıkması

Renginin değişmesi

Şeklinin değişmesi

Kanaması

Aniden çıkan ve hızlı büyüyen lezyon

Şüphe uyandıran her deri lezyonunu en garantili tedavisi çıkartılıp patolojiye gönderilmesidir. Bir lezyonu ortadan kaldırmanın mutlak şartı lezyonun tamamının hem kenarlar hem de derinlik olarak çıkartılmasıdır. Bir lezyonun kenarlarının nereye kadar uzandığı gözle görülebilir. Ancak gerçekte daha derinlerde lezyon gözle görünen kısımların ötesine de ilerlemiş olabilir. Bu nedenle her lezyon güvenli cerrahi sınır denilen bir mesafeden çıkartılmalıdır.

Güvenli cerrahi sınır nedir?

İyi huylu deri tümörleri çok kez görünen kenarlarının en fazla 1 mm kadar ötesine uzanır. Bu nedenle iyi huylu olduğunu düşündüğümüz bir nevüsü (beni) görünen kenarlarından 1 mm sağlam deri bırakarak çıkartmak yeterlidir. Ancak kötü huylu olan deri tümörlerinde cerrahi sınır yani çıkartılması gereken sağlam deri ve derialtı dokusu tümörün cinsine göre değişir. Orta derecede kötü huylu olanlarda bu sınır 0.5 cm olabileceği gibi ileri derecede kötü huylu olan tümörlerde bu sınır 5 cm ye kadar çıkabilir. Plastik Cerrahlar tanısı şüpheli olan tümörlerde bir sorun ile karşılaşırlar. Bir tümörü 2 mm sağlam deri ile çıkarmak ve 5 cm sağlam deri ile çıkarmak arasında çok önemli fark vardır. Eğer ne olduğu belli olmayan tümör 1cm veya daha küçük çaplı ise tamamı en az 2 mm cerrahi sınır ile çıkartılır ve patolojiye gönderilir. Raporda lezyon iyi huylu çıkarsa ilave bir tedavi yapmaya gerek yoktur. Rapora lezyon kötü huylu gelmiş ise şuna dikkat edilir:

Cerrahi sınırlar temiz ise geride görünen bir tümör kalmamış demektir. Tümörün cinsine bakılır orta derecede kötü huylu tümörlerde tedavi yeterli sayılır ve ameliyat bölgesi herhangi tekrarlama için aralıklı olarak izlenir. Tekrarlama olursa yeniden ameliyat edilir. Tümör raporu ileri derecede kötü huylu tümör gelirse geride herhangi bir şey kalmamış gibi görünse de 2 mm cerrahi sınır yetersizdir ve ayni bölgeye beklemeksizin daha geniş ameliyat ve ilave tedaviler yapılır.

Cerrahi sınırlarda tümör mevcut ise ameliyat kesinlikle yetersiz yapılmıştır ve hemen daha uygun bir cerrahi tedavi yapılır.

Çapı geniş olan şüpheli deri lezyonlarında cerrahi tedavi

Şüpheli lezyon iyi veya kötü huylu olup olmadığı bilinmeyen lezyondur. Eğer iyi huylu olduğu bilinirse cerrahi işlem yapılması şart değildir. Buna karşılık kötü huylu ise etrafında bir miktar sağlam deri ile birlikte çıkartılacaktır. Zaten geniş olan lezyon çok daha geniş olarak çıkartılacağından ortaya çıkacak olan yaranın (defektin) çapı da büyük olacak ve kapatmak için ilave işlemler yapılması gerekecektir. Bu durumda şüpheli lezyondan biyopsi(ler) alınır. Biyopsi bir lezyondan kapatmak için ilave işlem yapılmasına gerek kalmayacak kadar küçük parça(lar) almak demektir. Çok kez bu parçalar muayenehane şartlarında lokal anestezi ile alınır ve hemen patolojiye yollanır. Gelecek patoloji raporuna göre uygun tedavi planlanır ve daha sonra yapılır.

Deri lezyonu alındıktan sonra ortaya çıkan yaranın kapatılması

Derinin bir kısmı kesilip çıkartıldığında ortaya çıkan boşluk değişik yöntemler ile kapatılır.

Primer kapatma

Derinin bir esnekliği vardır. Bu nedenle birçok bölgede 1 cm çapa kadar olan deri eksiklikleri kenarlardaki sağlam deri ortaya çekilerek uç uca dikilir ve açıklık ortadan kalkar. Derinin bulunduğu bölgeye göre açıklığı 3 cm veya daha geniş çapa ulaşan deri açıklıkları primer olarak yani uç uca dikilerek kapatılabilirler.

Deri grefti ile kapatma

Çıkartılan tümörden sonra ortaya çıkan açıklık geniş ise deri kenarları ortada birleştirilemez. Bu durumda açıklık başka bir bölgeden alınan deri yaması (grefti) ile kapatılır.

Deri flepleri ile kapatma

Deri lezyonu çıkatıldıktan sonra açığa çıkan yara yüzeyinde önemli yapılar var ise (damar, sinir, kemik, kıkırdak) bunların üzerinin sağlam ve kalın bir deri tabakası ile örtülmesi gerekir. Bu durumda deri flepleri kullanılır.

Sonuç

Deri lezyonları estetik veya sağlık nedeni ile çıkartılabilir. Her lezyon çıkartıldıktan sonra patolojiye gönderilmeli ve lezyonun ne olduğu ile cerrahi sınırların temiz olup olmadığı öğrenilmelidir. Deri tümörlerinin tedavisi Plastik Rekonstrüktif ve Estetik Cerrahi’nin işidir.
