Currently waiting to find out if I have rhabdo, and it’s like I get it, it takes time, and testing to confirm that I’ve got it, but waiting to find out if I’ve got an autoimmune disease is not fun.

Pts with EtOH use disorder often have low phosphate because they don't eat. They also have low vitamin D intake-> secondary hyperparathyroidism-> high urinary phosphate excretion. Their serum phosphate may appear normal due to extracellular shift of phosphate. When you feed them or give IV fluids with glucose, insulin causes phosphate to shift intracellularly, so their phosphate level decreases. Respiratory alkalosis also shifts phosphate into cells. Rhabdomyolysis can occur from hypophosphatemia. Check CPK level. Pts with EtOH use disorder often have myopathy, so hypophosphatemia + myopathy-> symptomatic rhabdomyolysis.

What caused you to choose family practice after med school? What different specialties did you consider?
I kind of always knew--I started out before I knew anything thinking I wanted to specialize, maybe neurology given my history with Neuroscience, but once I shadowed a couple of doctors I was like "Oh. Family Medicine. That's what I like."
It's got a little bit of everything, it's impossible to be truly good at--I'm constantly forced to learn new things about every specialty because out here we have no one. I need to be half a nephrologist, half a rheumatologist, half a dermatologist. I do a crap ton of procedures--I like them, I like the satisfaction of an IUD insertion or a cyst removal done well. I will stay busy forever. I will never have a chance to get bored. I can do migraine care, manage Parkinson's, diagnose dementia, remove some skin cancers, differentiate between polymyalgia rheumatica and statin rhabdo--if I wanted to, I could still be doing hospital care, I could still be delivering babies. I have so many patients who either don't want to or can't drive two hours to the closest city for specialty care, and if I don't learn this, fast, and well, they'll get nothing. So there's always a fire under my ass. Rare genetic blood disorder? Shit! Read up! Molar pregnancy? I've seen two. Rural medicine is fucking wild and it's my happy place.

quick disclaimer: I am not medical professional. Take all this with a grain of salt. Or like, 110% of your daily sodium intake.
So, electrical injury severity and pattern is influenced by a couple of different factors. The voltage of the current, weather it’s AC or DC, the surface area the current made contact with, and pathway the current took through the body all play a role in how your character would be injured.
One of the most dangerous things that electrical shocks cause are Arrhythmias (irregular heart rhythms). Some arrhythmias can cause cardiac arrest, but you’ll need a voltage higher than 50 volts if you want to go down that route. Cardiac arrest can have all sorts of complications, but that’s out of the scope of this post lol.
Electrical injuries also cause electrical burns. The burns will be visible at both contact points: where the current entered the body, and where it exited into the ground. There will be a trail of damage following the path the current took to the ground. Electricity doesn’t necessarily have to travel far to hurt, and cause a lot of damage. The burns can get infected, leading to sepsis, and significant electrical damage to muscles can cause rhabdomyolysis and compartment syndrome. Both rhabdo and compartment syndrome can cause an acute kidney injury (AKI), which could definitely affect someone long term.
DC current is normally a single shock (through I assume yours is a little more intentional than usually) that throws someone and can cause blunt trauma.
AC current causes tetany, or involuntary muscle contractions. These are very painful and can lead to muscle damage. AC injuries have a higher complication rate than compared to DC injuries.
On the neuro side, we’ve got peripheral neuropathy. This occurs in about 17% of electrical burn cases, so it’s fairly common. Symptoms include muscle weakness, paralysis, atrophy, and tingling, pain and numbness in the affected limb(s).
As long as your current passed through the limb you want to injure, nerve damage is absolutely in the cards for your poor fictional characters. Repeated current applied to a limb would also likely lead to muscle damage as well, particularly at the joints, on top the nervous system damage.

Oh.
Oh.
I had a whole lot of thoughts about this episode and then that scene happened. (You know, the one where Carina forces Maya to stay in the hospital for 72 hours and Maya starts yelling and everything is falling apart.) I have a lot of feelings about it, because oof, it was hard. I mean, we knew it wasn’t going to be easy, that it’s going to get worse before it gets better, but watching it happen was heartbreaking. 💔 💔
Oh, Carina. We saw her do it to her brother and now she’s had to do it to her wife, too. No wonder she’s at breaking point, no wonder she’s packing a bag. At some point, she has to protect herself and that’s exactly what she’s doing. Watching her cry as Maya was screaming after her floored me.
And Maya... what I loved about Danielle’s performance is how panicked she looked the whole time. She knows. She knows she’s pushed herself too hard, she knows she’s not well, and she knows that everyone can see it. But Maya still won’t accept it so she’s doing what Maya does and is pushing back - hard. 
It’s sad and angsty - and we just have to trust the journey. (Or otherwise sue the show for emotional trauma!)
Congrats to the fic writers who called it as rhabdo. 🤕
Jack... hmm. A dumbass. If he picked up Maya’s turnout coat from the floor, why did he a) not wonder why she wasn’t on the call, or b) not wonder why she went on a call without her turnout coat? Wouldn’t it have sparked some concern?
Also yes, let’s have Jack lean on Carina when he’s all up in his feelings. Don’t get me wrong, his feelings were valid, but it was Carina’s wife in the hospital bed and Carina’s wife who is in crisis, and not once did he (or anyone else) ask if Carina was okay. 🤦🏼‍♀️
“I’ve heard concerns about your ability to lead this time” - “I’m fine” - “Ok good” - good pep talk, Ross. But then I really liked seeing the side to her where she went up against Dixon and the police, I’d like to see more of that. The Ross/Bob scene at the end? Meh.
Nice to see Beckett making use of the gift that Maya gave him.  🤭
The ladies who turned up at the station on their scavenger hunt were a lovely bit of light relief, I liked them! I don’t care about Travis’s mayoral race at all.

Trend UA to see if rhabdomyolysis is improving. CPK should not be trended. UA will show blood without RBCs because it’s myoglobin not blood. If UA clears, the rhabdomyolysis is resolved.

Because I am taking very long on bios (I am, it turns out, terrible at describing personality) here's some blurbs on my various wols, for reference purposes. Unless they are both on the list, the partners in question are my girlfriends, @ximmeas. Also only putting characters on the list if I have spent a decent amount of time with them because this list. Aganaran Qalli- a sweet but stubborn girl from the Steppe who came to Eorzea to find her brother and instead found a new family in the Scions. Doesn’t exactly have a partner?
Chiaki Fujimori- once a sickly child from a noble Doman house; now that they are able to, they wish to help the realm that helped them. Partner is in early development;;;
Vespera Phaethon- a Garlean runaway with an exuberant and theatrical personality that hides any insecurities. While she comes across as a ditz, she’s a genius with Magitech. Partner is Dianthus Phaethon, fellow Garlean runaway and her brother adopted at the Adventurer’s Guild registration when they both needed a fake last name.
Liamaine Leveilleur- the second child of famed Archon Louisoix Leveilleur, she aims to perfect being messy into something of an artform- until she is blackmailed into the Scions and forced to help with this whole realm saving business (and reunite with her niblings). Partner is Orion Strand, who becomes something of a surrogate daughter to Liamaine.
Orion Strand- a quiet young lady who joins the Scions with a mysterious air and sinister intentions. Partner is Liamaine Leveilleur, the hero Orion had pumped herself up to advise and then betray who turned out to be. Very different then what Orion was expecting. 
Alan de Gagnon- the kind and quiet but…dim adopted son of a noble Ishgardian House. When his father was executed for heresy, Alan was exiled to Eorzea. His partner is Azriel Embers, an Ul’dah street kid who took Alan under his wing because he super needed a tank.
Primrose Lyons (might change her last name)- while she is technically a princess, Primrose was born and raised in exile in Radz-at-Han. She came to Eorzea to help fight the Empire (and avoid her ex-fiancee). Partner is idk rn.
Dion Rhabdos- an ex black mask Ascian who finally realized he was little more than cannon fodder and tried to retire- unfortunately for him, Hydaelyn blackmailed him. Partner is Avierre Boucliefort, an ex Sharlayan schoolteacher who burned down his in-law’s house.
Neptune North- a cheerful but mysterious man who arrived in Limsa Lominsa to escape…something. Though he plays the part of the hero perfectly, there’s a strange, melancholy air about him. Partner is P’elya Tia, a Lominsan pirate who is something of a wallflower.
Rhode Elya- A strange girl who was created after Neptune’s death, in a sense. She is wide eyed and curious, eager to experience the world. P’elya is her dad.

My mother is the one that watches Station 19, and I sometimes just tag along. It seems that whenever I do that, things happen. So I watched last night’s episode and I have THOUGHTS:
I’ve watched enough of this show to know that Maya and Carina have been having some issues this season. But the whole rhabdo thing just…broke me. And maybe it’s because I can’t suspend disbelief for the sake of narrative but I have some questions. Why was Carina the one to tell her she was being held? Wouldn’t it have been easier on Carina and Maya for Dr Altman to tell Maya and then Carina can just say that her hands are tied? She’s being involuntarily admitted, so where is psych in all this? And I can’t possibly be the only one wondering how she reacted when Andy showed up? What did they do after the episode ended?
