Understanding Urethral Stricture: Causes, Symptoms, and Treatment
Urethral Stricture: Causes, Symptoms, and Treatment
Urethral stricture is a narrowing of the urethra, the tube that carries urine from the bladder out of the body. This condition can lead to various urinary problems and discomfort. Understanding the causes, symptoms, and treatment options for urethral stricture is essential for proper management.
Causes of Urethral Stricture
Urethral stricture can be caused by several factors, including:
Trauma: Trauma to the urethra, such as from a pelvic fracture or a straddle injury, can lead to the formation of scar tissue, which can narrow the urethra.
Infection: Infections, such as sexually transmitted infections (STIs) like gonorrhea or chlamydia, can cause inflammation and scarring of the urethra, leading to stricture formation.
Inflammation: Chronic inflammation of the urethra, often due to conditions like urethritis or balanitis, can result in the narrowing of the urethra over time.
Medical Procedures: Certain medical procedures, such as catheterization or surgery involving the urethra, can increase the risk of developing a urethral stricture.
Congenital Factors: Some individuals may be born with a narrow urethra, predisposing them to developing strictures later in life.
Symptoms of Urethral Stricture
The symptoms of urethral stricture can vary depending on the severity of the narrowing. Common symptoms include:
Difficulty Urinating: Narrowing of the urethra can make it difficult to start urination or maintain a steady stream.
Urinary Retention: In severe cases, urethral stricture can lead to the inability to urinate, which requires immediate medical attention.
Urinary Tract Infections (UTIs): Narrowing of the urethra can increase the risk of UTIs due to incomplete bladder emptying.
Urinary Frequency and Urgency: Strictures can cause the bladder to contract more frequently, leading to a constant urge to urinate.
Spraying or Dribbling of Urine: The narrowing of the urethra can cause the urine stream to spray or dribble instead of flowing in a steady stream.
Pain or Discomfort: Some individuals may experience pain or discomfort during urination, ejaculation, or while passing urine.
Treatment Options for Urethral Stricture
Treatment for urethral stricture depends on the severity of the condition and may include:
Dilation: This involves stretching the narrowed portion of the urethra using a series of dilators of increasing size. It is a common treatment option for less severe strictures.
Urethrotomy: This procedure involves using a special instrument to cut the stricture, widening the urethra. It is often used for short strictures.
Urethroplasty: In more severe cases, surgery may be required to remove the narrowed portion of the urethra and reconstruct it using tissue from other parts of the body.
Medication: In some cases, medications may be prescribed to help reduce inflammation and prevent further scarring of the urethra.
Self-Catheterization: Some individuals may need to perform self-catheterization regularly to help keep the urethra open and maintain urine flow.
Conclusion
Urethral stricture is a condition that can cause significant urinary problems and discomfort. Understanding the causes, symptoms, and treatment options for urethral stricture is crucial for effectively managing the condition. If you experience symptoms of urethral stricture, it is important to consult with a healthcare professional for proper evaluation and treatment.

a story of one fake penis, 39 surgeries and counting.
This story I found through this post. don't click it unless you want to see frankendick. Frankly, the phalloplasty looks okay (only compared to most I've seen and I've seen too many. too... too many doing this work) besides the enormous scars and color mismatch. It's a low bar. But I was interested to see the user's progression.
That post, of surgery results the user admits took three years to achieve (""), is from two years ago. According to the user's more recent post history, it's been downhill from there.
11 months ago, the user's erectile implant began pushing through the skin, requiring surgery. Three months later, after the revision, the complication repeated. In this post from r/Everythingphallo 4 months ago, called "Is there anyone here who has had a phalloplasty reconstruction?"— the user states:
Initially, everything was perfect – it looked aesthetically pleasing, had great functionality, and the sensation was at 100%. However, over the past 3 years, I have had 15-20 surgeries due to complications and issues. The appearance is now completely deteriorated, and my functionality is greatly limited. The scars are contracting more and more, causing a loss of 4 cm in length, which wouldn't be so bad if it still looked good and functioned properly.
in the comments, the user clarifies a major reason for the many revision surgeries was problems with the constructed urethra. Multiple surgical revisions to the urethra caused stiffening, hardening, constricting scar tissue. A bacterial infection was additionally overlooked for "several years" which worsened the damage, and no doubt put the user at severe risk of bladder and kidney infection.
As indicated by the post's title, the user does not want to stop surgery, but instead wants to entirely reconstruct the phallus using the same technique, radial free flap phalloplasty, as performed the first time. This would mean she would have massive scars and limited mobility/strength in both of her arms and hands.
In this post from two months ago, things are somehow worse. (How..?!!)
user states:
Recently, I lost all sensation (previously 100%), can no longer experience orgasms, and suffered a 50% reduction in length due to a parasite that damaged the tissue. Furthermore, I am scheduled for an emergency surgery because my ED has once again resulted in the skin breaking, marking the 5th or 6th time in a year. In general this is my 39 surgery and I’m so tired.
Parasite?!? Which apparently doctors missed for OVER TWO YEARS?? Jesus Christ, if she was playing medical bingo she'd have won years ago. But of course, she's lost everything.
All that the first phalloplasty got her was 39 surgeries, urethral strictures, loss of all sexual sensation, bacterial infections, parasites, she still wants another one.
Don't you dare say this is life-saving surgery in any capacity. It is life-ending, mentally, physically, figuratively, literally.

Vaginoplasty in India: Procedures, Complications, and Aftercare
Tumblr media
Introduction Vaginoplasty is a type of gender-affirming surgery that some transgender women undergo as a part of their transition. It involves the surgical construction or reconstruction of the vagina. The procedure can involve creating a vaginal canal, labia, and other features to align with the individual's gender identity.
procedure The availability of vaginoplasty in India has increased over the time. The preferred vaginoplasty method is a variation on the penile inversion process. In this technique, a vaginal vault is formed between the rectum and the urethra, in the same position as a non-transgender female between the pelvic floor (Kegel) muscles, and the vaginal lining is made of penile skin. After an orchiectomy, from scrotal skin the labia majora are created, and from a section of glans penis, the clitoris is formed. The prostate is left in place to prevent complications like incontinence and urethral strictures. Furthermore, the prostate has erogenous sensation and is anatomically similar to the "g-spot." A vaginoplasty's external scars are kept to a minimum by strategically placing the incisions and meticulously closing them. Non-transgender females typically have a vaginal depth of 9-12 cm (3.5-5 inches). In the case of previous circumcision, a skin graft, typically scrotal in origin, may be required. If there is insufficient skin in the penis and scrotum to achieve a depth of 12cm (5 inches), a skin transplant from the hip, lower abdomen, or inner thigh can be performed. Scrotal skin contains an abundance of hair follicles, and it is feasible to move skin with thin hair growth into the vagina until hair is cut off beforehand. A few surgeons treat all visible hair during surgery by aggressively thinning the skin and heating visible hair follicles. However, because hair grows in stages, this approach may not effectively address inactive follicles. The most effective method of preventing vaginal hair growth is to perform scrotal electrolysis, with at least three full clearings 8-12 weeks apart, depending on the electrologist's preference, hair type, and distribution. Surgeons ought to offer a diagram depicting the region that needs to be cleared.
Complications Bleeding, infection, skin or clitoral necrosis, suture line dehiscence, urinary retention, and vaginal rupture are some of the immediate risks. Fistulas in the rectum, urethra, or bladder are common in the early stages. Acute bleeding typically starts in the urethra and can be regulated with local pressure. If local pressure is insufficient to achieve hemostasis, inserting a larger catheter (20F) into the urethra by itself may stop the bleeding. In almost all cases, wrapping a suture around the bleeding site (while still holding the catheter) will stop the bleeding. This usually happens a week or more after surgery, when the hematomas liquefy. The blood appears dark and old, with no clots. Although frightening for the patient, no treatment is required. Because the genitalia and perineum have an excellent blood supply, infections should be uncommon and rarely require more than a broad-spectrum antibiotic. Skin peeling or loss is also uncommon and should be treated conservatively. Suture line separation is possible, most commonly at the posterior perineum, as a result of the pressure and stretching caused by dilation. Separations should be treated conservatively with antibiotic ointment; the majority will heal without complications. Urinary tract infections tend to occur after a vaginoplasty because the urethra is shortened. Proper hygiene and hydration are generally effective preventative measures. Patients with recurrent urinary tract infections should be evaluated for urethral strictures.
Granulation tissue in the vagina is commonly caused by delayed healing. The requirement for frequent dilation in the early post-operative period aggravates the problem by triggering repeated trauma to the region of granulation. Aftercare
Dilation Instructions: Please keep in mind that everyone's dilation schedule may be different. Before inserting the dilator into the vagina, make sure it is clean. Use antibacterial soap and warm water to clean the dilator. Rinse thoroughly, then dry with a clean paper towel or cloth. Before inserting the dilator, apply Surgilube or KY Jelly. Only use water-based lubricants. Avoid using silicone-based lubricants. Gently insert the dilator into the vagina at a 45-degree angle until it is under the pubic bone, then continue straight inward. Expect to feel a little resistance and tenderness. If you encounter too much resistance or severe pain, stop immediately. Place the dilator in the vagina to its full depth (until you feel slight pressure or resistance)  and leave it there for 10 minutes. Insert until there are only one or two white dots outside the vagina. Begin dilating three times per day for three months on the day the vaginal wrapping is removed. After three months of dilating, you may progress to the next size dilator. You should wear the following size for three months. Dilation frequency: 0 to 3 months after surgery. 3 times a day for 10 minutes each, 3-6 months after surgery. 1/day for 10 minutes each, more than 6 months after surgery. 2-3 times per week for 10 minutes each, with 1-2 times per week after 9 months.

Understanding Potential Risks and Side Effects of Urological Treatments
Urological treatments encompass a wide range of interventions aimed at addressing conditions affecting the urinary tract and male reproductive system. While these treatments can often provide significant relief from symptoms and improve quality of life, they may also carry potential risks and side effects. Understanding these risks is essential for informed decision-making and optimal management of urological conditions.
Tumblr media
Here are some common potential risks and side effects associated with urological treatments:
Medication Side Effects:
Many urological conditions are managed with medication therapy, which can sometimes cause side effects. For example, antibiotics commonly prescribed for urinary tract infections (UTIs) may lead to gastrointestinal upset, allergic reactions, or antibiotic resistance.
Medications used to treat erectile dysfunction (such as sildenafil or tadalafil) may cause headaches, facial flushing, nasal congestion, or visual disturbances in some individuals.
2. Surgical Risks:
Surgical interventions for urological conditions, such as prostate surgery or kidney stone removal, carry inherent risks associated with anesthesia, bleeding, infection, and damage to surrounding tissues.
Specific procedures may have additional risks. For example, transurethral resection of the prostate (TURP) for benign prostatic hyperplasia (BPH) may lead to retrograde ejaculation or erectile dysfunction in some cases.
3. Incontinence and Sexual Dysfunction:
Some urological treatments, particularly those involving surgery or radiation therapy, can impact urinary continence and sexual function. For instance, radical prostatectomy for prostate cancer may result in urinary incontinence or erectile dysfunction.
Pelvic floor exercises and other forms of rehabilitation may help mitigate these effects, but they may persist to some degree in certain cases.
4. Scarring and Stricture Formation:
Procedures like urethral dilation or urethroplasty, performed to treat urethral strictures, carry a risk of scarring and recurrent narrowing of the urethra.
Patients undergoing these procedures may require periodic surveillance and possible repeat interventions to manage recurrent strictures.
5. Allergic Reactions:
Some urological treatments, such as the injection of certain medications or contrast agents for imaging studies, may trigger allergic reactions in susceptible.
It important for patients to inform their healthcare providers about any known allergies before undergoing urological procedures.
6. Long-Term Complications:
Certain urological treatments may carry the risk of long-term complications that develop over time. For example, patients who undergo radiation therapy for prostate cancer may experience urinary or bowel problems years after treatment.
Regular follow-up with a urologist is crucial for monitoring and managing any potential long-term complications.
7. Psychological Impact:
Urological conditions and their treatments can have a significant psychological impact on patients, affecting self-esteem, body image, and intimate relationships.
Psychological support and counseling may be beneficial for patients navigating the emotional challenges associated with urological conditions and treatments.
In conclusion, while urological treatments offer effective solutions for managing a wide range of conditions, it's important for patients to be aware of the potential risks and side effects associated with these interventions. Healthcare providers should thoroughly discuss these risks with patients and tailor treatment plans to minimize adverse outcomes while optimizing therapeutic benefits.
