Fucked up D&D spell idea:

Duress

Third level Enchantment spell that allows you to give the target an instruction, so long as they are reasonably capable of complying with it. If the spell succeeds, the target will take 1d4 psychic damage if they ever defy the instruction, as their body is wracked with crippling agony. The spell lasts for 8 hours, and requires concentration, but you can’t break out early with a wisdom save.

Note that this is not a charm effect, and there is no compulsion placed upon the target. It is fully up to them what they do, as long as they can withstand the excruciating pain.

There would also be a greater version and a mass version

Story idea:

A soldier, having fought for days, eventually finds himself on a silent battlefield, devoid of allies or enemies. For days he wanders, searching for a battle which ended a while ago. Eventually he may be found by others, but they will not recognize him, for he has become something truly horrific.

His mind has long been eroded away by the unending battle, and all that remains is his training as a soldier. He may still move, speak, and survive like anyone else, but there is nothing behind his eyes, just a husk awaiting his next battle.
