Why Are X-rays, Ultrasounds and MRI Scans Crucial For Joint Pain?
Patients suffering from joint and muscle aches, pain and stiffness, it is important to first understand the precise cause before correct treatment can be selected.
While pain can be caused by simple sprains, these usually resolve naturally or with simple medical treatments within a week. For pain lasting for longer periods of time, some possible causes include:
muscle and ligament tears
joint arthritis
cartilage degeneration
fractures
nerve compression
inflammation
infections
bone cancers
Some of these conditions are chronic and do not respond well to standard therapy. Others are urgent and potentially life-threatening if not properly treated in a timely manner. It is often acceptable to start a trial of standard therapy based on a clinical diagnosis by an experienced doctor.
If your condition does not improve in a timely manner, imaging scans are likely necessary. Imaging helps to confirm the diagnosis and rule out more serious medical conditions. Most patients understand the importance of this but are concerned about the cost of medical care. This includes scans like X-rays, Ultrasounds and MRIs for pain, aches and stiffness. For example, a knee X-ray costs $50 while an MRI will cost more.
It is good news to many that it is now possible to use up to $300 of Medisave per year for "outpatient scans for diagnosis or treatment of a medical condition". The Pain Relief Practice helps patients better manage chronic pain by providing X-rays, Ultrasounds, CT scans and MRI scans, together with same day reports at extremely competitive prices. This reassures patients that their conditions will be promptly, conveniently, and cost-effectively investigated.

Exercise Therapy: Fantastic Solution or Dangerous Waste of Time?
Among patients who visit The Pain Relief Practice, there are three broad groups:
Those who strongly believe exercise to be the best natural solution for their condition
Those who find exercise impractical despite well meaning advice from doctors, physiotherapists and gym instructors
Those who want to recover as quickly as possible from pain to resume active exercise as part of their lifestyle
To address the issues and concerns of each group, let us first explore what exercise can and cannot do.
The Benefits of Exercise:
Support - strengthening muscles through correct training helps support our joints.
Stretching - appropriate stretching helps prevent muscle stiffness and also reduces fibrosis (also known as soft tissue scarring) in conditions such as a frozen shoulder. Stretching also relaxes trigger points (muscle "knots" and spasm).
Endorphin production - exercise causes the body to release endorphins which improves our mood and promotes feelings of wellness.
The Risks of Exercise: While exercise can have many benefits, it can also cause more injury for people who are injured and suffering from acute pain. In a way it can be considered a double edged sword, with negative results if used incorrectly.

If you are suffering from pain that started recently, it is better to rest. Using some pain rubs and patches or medications for a few days could be a good idea. If the pain is severe, it may also be necessary to see a doctor for treatments that offer faster pain relief.
Frustrating Exercise Advice: We sometimes see patients with long term pain in the foot, knee or back who are told: "there is no other option, you just need exercise to lose weight".It is especially sad when there is a sense of blame felt regarding the patient's weight or lack of motivation to exercise. In reality, the medical condition causing pain is precisely why exercise is not possible. The correct thing to do in these cases is to treat the pain condition. And there is almost always a good way to treat pain with some of our new medical technology. When the pain is cured, everyone will end up doing more physical activity. Simply being able to walk better will help a person burn more calories and lose weight.
Expectations about Exercise for Pain Relief: It is important to have realistic expectations when considering exercise therapy in terms of the amount of effort and time needed. Exercise is not an overnight miracle cure. It takes many weeks to produce results and should be part of a comprehensive treatment program under the supervision of a trained physiotherapist.
People with Sports Injuries: For some patients, excessive or incorrect exercise is actually the underlying cause of their condition.Such patients are often eager to get back to an active sporting activity. In these situations, we generally advice that it is better to use medical treatments (shockwave therapy is one of many good options to consider) to speed up recovery rather than to risk further aggravating the injury with even more exercise.
What Type of Exercise is Best? This depends on the site of injury and condition a person is suffering from.In an internet age, multiple guides and videos demonstrating which exercises to do can be found with a simple Google or YouTube search for your condition. If there is any doubt, it is of course best to consult a good physiotherapist for more advice before proceeding further.

Understanding Pain and Self Treatment Options

Pain is the body's way of telling us that something is not right. It can be experienced as a result of many different types of injuries. Most of the time, we feel pain due to:
Tissue inflammation
Nerve irritation
Wear and tear of the joints
Traumatic injuries
Pain normally provides a protective signal that helps us respond to disease and injury. However when pain is severe or persistent, it also cause very significant distress and problems.
Poorly treated pain can result in frustration, low energy levels, poor sleep, irritability, depressed mood, and even affect work and relationships. It is important when experiencing pain, to consult a licensed doctor or Physiotherapist.
For many patients, there are established simple common approaches that can provide pain relief. "RICE" Therapy This is often used as immediate, temporary treatment for muscle, tendon and ligament strains. Rest - Rest the affected part to avoid further damage Ice - Apply ice wrapped in a ziploc bag to the affected part to reduce pain Compression - Wrap a firm bandage or towel around the affected area to reduce swelling Elevation - Elevating the affected area to further reduce swelling Topical Analgesia There are a variety of available medications that can be applied directly onto the skin. These can take the form of rubs, sprays and patches applied onto the affected body part. Some of these can be bought directly from the pharmacy, while others are available only with a doctor's prescription. The advantage of topical medications is that they are very easy to use and have very much less side effects compared to orally taken medications.

Why Is It So Hard To Treat Neck, Back and Knee Pain?

Pain is a very common symptom when there is injury or disease in the body.
At some point in life, almost every person suffers from the neck, back, knee or other joint pain.
While a majority of such problems disappear after a few days due to the human body's natural ability to heal itself, a small unfortunate group of patients does not get better.
They not only continue to feel pain, stiffness, and weakness, but often get worse with time, despite seeing different doctors, traditional physicians, and trying alternative treatments.
Many are often left frustrated and confused as to why the pain does not go away.
This, in addition to the damaging physical and psychological effects of persistent pain, makes their life stressful and difficult.
A key reason why many patients do not get better is that they are given an incomplete understanding of their condition, and do not properly treat the root cause of their problem.
It has been said that if the only tool a man has is a hammer, every problem looks like a nail. Some healthcare professionals tend to over-emphasize just one aspect of a person's problem, often the aspect that they have a "solution" for.
Unfortunately, this does not actually solve the underlying problem the patient has, resulting in poor results.
A common example is when a patient is told that he has inflammation in the body and given only anti-inflammatory medications.
While this stops the pain and inflammation temporarily, it does not address the underlying cause of inflammation, leaving the pain to recur when the pills are stopped. Another example is when patients get told: "there is no other option, you just need exercise".While exercise can have many benefits, it can also cause more injury for people who are injured and suffering from acute pain. In a way, it can be considered a double-edged sword, with negative results if used incorrectly. It can also be quite impractical to exercise if someone is suffering from acute pain.
The Pain Relief Practice helps our clients achieve actual pain relief using a comprehensive biophysical model of pain diagnosis and targeted solutions for each underlying pain problem.
Using a thorough medical history and clinical examination, aided by selective X-rays, MRIs or blood investigations, we break down a patient's condition to a combination of root causes such as:
Focused Joint Injury
Persistent Inflammation
Fibrotic Tissue
Neuropathic (Nerve related) Pain
Central Sensitization (brain-related pain)
Tissue Degeneration / Age-related "wear and tear"
Muscular Imbalance
Only with a thorough diagnosis do we then prescribe a holistic treatment program that may involve a specific combination of:
Tissue Repair Supplements
New Non-Invasive Medical Technology
Physical Therapy

Treatment For Your Persistent Pain

Persistent pain, generally known as chronic pain, is pain that lasts more than three months. Persistent pain is extremely complicated and can be caused by many factors. It can occur along with conditions such as arthritis, diabetes or fibromyalgia. It may occur after an injury to the body has healed. In some cases, the cause isn't known.
Persistent pain is associated with changes in the nervous system (the nerves, spinal cord, and brain). Throughout our lives, our nervous system changes and adapts to help us learn from and cope with different experiences. This is called neuroplasticity. However, sometimes this normal process of adjusting and changing becomes abnormal and it is no longer helpful causes persistent pain.
Treating persistent pain
The Pain Relief Practice is launching a new pain treatment program. The treatments are very safe, comfortable and most importantly, very effective.
There are 2 therapy options we are considering:
A) 15 min, "no-frills" sessions, costing very little but requiring 6 therapy sessions.
B) 30 min, stronger sessions, costing slightly more but requiring only 4 therapy sessions.
Most acute painful conditions gradually disappear over a few days to a few weeks. Some proportions can continue beyond three months and sometimes for much longer.
Despite the ongoing presence of pain, you can improve what you can do and how you feel.

Using Shockwave Therapy For Pain Recovery

We are going to discuss a very interesting non-invasive treatment for joint and muscle problems: Shockwave Therapy
Shockwave is interesting for 2 reasons:
Unlike the majority of other treatments we commonly use which are slow and offer temporary relief, shockwave therapy gives long term results through stimulating the body's natural healing potential and works quickly.
Even though shockwave therapy is very established in Singapore for well over a decade, it is relatively unknown. Few patients have heard their doctors recommend it.
Shockwave, also known as Extracorporeal Shockwave Therapy, is an internationally established method of treating pain and inflammation through stimulating tissue repair.
Employing high energy acoustic waves, shockwave induces new blood vessel growth and tissue regeneration.
Although recognized as one of the best treatment options available for pain relief and recovery, its use in Singapore has up until recently been limited.
One reason is that treatment with earlier generation shockwave machines were quite painful, usually requiring local anesthesia. Shockwave machines were also very costly medical devices, resulting in expensive treatments that many patients could not afford.
Fortunately, medical research in recent years has given us a new generation of shockwave machines that provide far more comfortable treatments, no longer requiring anesthetic injections.
They have also become much cheaper, dramatically bringing down treatment costs and making shockwave therapy more affordable to patients.
Conditions Treated with Shockwave include:
Plantar fasciitis (foot pain)
Achilles tendinopathy (heel pain)
Hip pain
Knee pain
Lateral epicondylitis (tennis elbow)
Rotator cuff tendinitis (shoulder pain)
Myofascial trigger points (body aches and muscle pain)
Back pain
Neck pain
Clinical studies have demonstrated extremely high success rates, even in cases that have failed to respond to conventional approaches.
What sets this treatment apart from traditional treatments is that it treats the musculoskeletal pathology instead of just offering symptomatic relief.

Diet and Pain: Does What You Eat Really Matter?
One very common question we often get asked is what we should or should not eat when suffering from pain.
Most expect either a short answer about special foods to take the pain away, or a list of things to avoid. Others prefer recommendations of particular glucosamine brands or some other nutritional supplement.
"Is there any food I should avoid?"
In most cases, the clear answer is "No".
Many patients come with a long list of foods that helpful friends and "alternative" healers warn them against: beans, red meat, alcohol, shellfish, sugar, fat, salt...
While it is good to maintain a good diet for general health and to keep our heart healthy, there is really little be gained by making life miserable with tasteless meals if our aim is to make aches and pain feel better.
The only notable exception is when someone has gout. People diagnosed with gout should maintain a low purine diet which means reducing our intake of:
Alcoholic beverages
Some fish, seafood and shellfish, including anchovies, sardines, herring, mussels, codfish, scallops
Some meats, such as bacon, turkey, veal, venison and organ meats like liver, beef, chicken, duck, pork and ham
Shellfish, such as crab, lobster, oysters and shrimp
Certain fruits and vegetables: spinach, peas, strawberries (& strawberry jam) and durian
Beans and legumes, peanuts, bean cake, moon cake
Clearly the list is very long and life can be quite bland if we need to strictly avoid these foods. Which is why we always tell patients to ignore such "food rules" unless they have been clearly diagnosed to have gout by a medical doctor.
"Does glucosamine really help?"
The interesting point here is that when we say "not really", most patients feel relieved that this is what they already know from personal experience.Glucosamine started becoming a popular joint supplement at a time when there were hardly any options for treating joint pain other than strong pharmaceutical painkillers and invasive surgery.
Patients and doctors didn't mind trying out glucosamine supplements on the basis that they were generally quite safe. Over the years, with numerous commercial companies each claiming to have a better formulation, almost all patients we see have already tried multiple glucosamine brands with minimal or short term effects before their first visit to our clinic.
Some companies claim that glucosamine sulphate is superior to glucosamine hydrochloride, others emphasize the need to add in MSM or chondroitin. Some companies market synthetic glucosamine as more suitable for people with shellfish allergy. Others emphasize the importance of taking 1500mg a day, which in reality is just 1.5 grams!
Well over a decade of experience now suggests that while glucosamine use is likely harmless (other than harming the wallet), it is also often useless.
The UK National Institute for Health and Care Excellence recommended in Feb 2015 to doctors: "Do not offer glucosamine or chondroiton products for the management of osteoarthritis."
"Is There anything I Can Eat That Helps?"
Fortunately, the answer here is "Yes". There is now a growing list of natural foods that have been found to have beneficial effects in reducing pain and also helping repair the damage.
Collagen Hydrolysate
This is probably the most important natural food ingredient patients with joint and muscle pain should first explore.
Produced through the enzymatic hydrolysis of collagenous tissue, collagen hydrolysate contains amino acids such as glycine and proline. It is an extremely safe product and can be considered an essential part of any long term recovery program.According to published research, collagen hydrolysate when consumed has been shown to be absorbed, accumulate in joint cartilage, and stimulates the activity of chondrocytes (cartilage cells).
An increasing body of medical studies and clinical experience now supports the use of collagen hydrolysate in patients with osteoarthritis (joint wear and tear), and in athletes with joint pain.
http://www.ncbi.nlm.nih.gov/pubmed/18416885
http://www.ncbi.nlm.nih.gov/pubmed/17076983

Turmeric
Commonly used as the main spice in curry, Turmeric has traditionally been recognized for its amazing ability to decrease inflammation. New medical research suggests that taking turmeric extracts, alone or in combination with other herbal ingredients, can have very positive effects in reducing joint pain.

Boswellia
Also known in ancient times as Frankincense, this extremely valuable herb is now commonly used as a safe way of treating joint problems. Boswellia has been shown to be helpful in people with osteoarthritis, chronic low back pain, and even pain after having surgery.

Ginger
Widely used as a delicacy, spice and traditional medicine, scientific research now supports ginger as a potent anti-oxidant. Ginger has been shown to have both anti-inflammatory as well as gentle analgesic properties.
The main challenge of eating ginger for pain relief is probably the difficulty of choosing the most effective form of ginger to use. Ginger can be consumed as capsules, teas, powders, oils and food made from the root of the ginger plant. Capsules generally provide the best option by allowing highly purified ginger extracts to be combined with collagen hydrolysate and boswellia for convenience.
There are currently many preparations of the above natural solutions available. Some contain a single ingredient while others combine all of these in a single pill for convenience. It is a good idea to check with a pharmacist or your doctor for advice on choosing a formulation suitable for your condition.

Treating Difficult Pain: An Evidence-Based Comprehensive Approach

We tend to find that many patients with difficult pain conditions get disappointed with the results of their previous treatments particularly when the treatment focused on a single modality like "surgery", "exercise" or "medications".
Complex pain tends to be multi-factorial and it is difficult for a single modality treatment to produce satisfactory results.
This is especially true for conditions such as:
arthritis
degenerative changes of the knee, neck, and back
fibromyalgia
slipped discs
nerve problems
sports injuries
conditions of pain after surgery
The Pain Relief Practice uses a comprehensive biomedical approach to help patients with their conditions. We take into account each patient's specific underlying medical condition and select the most appropriate combination of available medical technologies.
3 Key Targets of Treatment include:
1) Healing Damaged Tissue
As pain is often the result of soft tissue damage, it is often necessary to boost the body's ability to heal for long term results. Possible ways to achieve this include:
Collagen Hydrolysate is produced through the enzymatic hydrolysis of collagenous tissue and contains amino acids such as glycine and proline. When taken as an oral supplement, collagen hydrolysate has been shown to be absorbed, accumulate in joint cartilage, and stimulates the activity of chondrocytes. [Evidence] Shockwave Therapy is an internationally established method of treating pain and inflammation through stimulating tissue repair.
Employing high energy acoustic waves, shockwave induces new blood vessel growth and tissue regeneration, thereby treating many pain conditions.[Evidence]
2) Reducing Inflammation
Inflammatory damage is one of the main mechanisms of pain and reducing inflammation is a good way to help patients quickly reduce pain and discomfort. Possible ways to achieve this include: Nutritional Supplementation with herbal antioxidants has been shown to improve joint health naturally and safely. [Evidence]
Radiofrequency is an externally applied, non-invasive treatment. It uses patented long wave radio frequencies to create intense tissue stimulation. [Evidence]
Nanotechnology Gels are made up of millions of phospholipids, which travel through the skin and tissue upon application, the gel then coats the joint with a layer of protective lubricant, reducing frictional damage. [Evidence]
3) Relieving Mechanical and Nerve Pressure
Ongoing physical injury and mechanical pressure on the nerves is a very significant cause of persistent pain, numbness, and weakness. Possible treatment options include:
Spinal Decompression Therapy is an FDA cleared, non-surgical treatment that has an amazingly high success rate for neck and back pain. It is designed to unload the spinal disc to provide very effective relief of pain and numbness in the back, neck, legs, and arms. [Evidence]
Joint Mobilization involves careful use of skilled graded forces to move a joint in the desired direction. This form of physical therapy is usually used to improve joint motion and normalize function by improving joint nutrient supply. It also helps to control pain and relieve muscle tension. [Evidence]
