OKAY SO. This scene. For a bit of context, Wilson just quit after being fired from the hospital's board. This removal happened because he opposed Vogler's notion to revoke House's tenure. This is the first conversation they had after that, and is the first time House learns of any of these votes happening within the board. I promise I have actual analysis and it isn't just self-indulgent Hilson posting (although it is mostly self-indulgent Hilson posting).
The big thing to note here is the big reason Vogler wanted House fired: the speech he made, as mentioned by Wilson here. He had put up with quite a lot for Vogler up to that point. And when he tried to force him to endorse a new medicine developed by one of his pharmaceutical companies, he mad a public mockery of both Vogler and the company by explicitly stating at a cardiology convention that it was no different than the previous drug but for a significantly higher price.
Earlier in the arc, Cameron discussed the issue with House. She claimed that he doesn't do what he does because it helped people, he does what he does because it's right. While never explicitly stated, I think Wilson knows this well. House sacrifices a lot for what he considers to be the greater good, and he often has very little regard for the consequences.
But after seeing these consequences affect his best and only friend so significantly, he feels some sort of regret. Though, he still knows what he did was right. Wilson knows this as well, which is why after he expresses his frustration, he seems sort of defeated. There is also a brief moment where House almost seems relieved.
The last thing he wanted was this to hurt Wilson, but he also doesn't intend on changing. This would be such a fundamental disagreement if Wilson hadn't understood that he didn't truly regret what he did. It might've even fractured their relationship beyond repair. Wilson felt betrayed by House. He was angry, and thought he'd been selfish. House shows he cares here, trying to placate Wilson briefly and try to remind him that he does in fact care about him. Wilson sees through this of course, and acknowledges both that he knows that House cares, and that he respects that House felt he had to stand up to Vogler.

Okay but the lighting in this scene is immaculate, I could go off about that forever. The clever use of cold versus warm colors and lights versus shadow immediately caught my eye. The use of that yellow-orange light to keep Rebecca lit up with no shadows, it highlights every feature and imperfection (which, is purposeful of course, her character is very ill) contrasted with the dark shadows covering more than half of House’s face in some of these shots, and the blue, low-lit, dreariness of the shots with him in them.
As for performance, Hugh Laurie KILLS this scene. It gives me chills almost every time. This is the scene where were first hear a bit about what happened to House’s leg, and he becomes vulnerable- as much as he can be. He goes from unbothered to a tense sort of anger and distress as she is refusing a possible cure in favor of “dying in dignity”.
This scene is gorgeous, why the hell did they go so hard with this pilot.
