Kris Dreemurr (pre-canon) has clinical lycanthropy and is aware of their delusions! They believe they are secretly and will eventually grow up to be a monster, specifically the goat-like kind like the other Dreemurrs. Despite being aware that this is not the case, Kris still had these beliefs. Due to a lot of internalized bad stuffs around always being the only human in town, they didn't identify as alterhuman, therian, or similar identities, and wouldn't let themself do so. Again, I'm only certain about this applying to pre-canon.

John Ward from FAITH the Unholy Trinity struggles with delusions of people & religion! I find the idea that after the events of the games he has a hard time properly knowing it people are actually people and will once in awhile believe that someone is a demon instead.
