Jegulus thoughts:

Sirius actually loving Jegulus happening (after the hilarious initial shock and surprise of finding out) because it means maybe Reggie has a chance to be better. To want to be better and get out from the dark shadow of their family. Plus if Jegulus gets married and have kids he'll really be a part of the Potter family in a way. I just want more fics where Sirius actually like the idea of the few people he loves the most getting together.

Reg lives au where he looks so different after the cave (prosthetic leg, deep scars all up his arms and face, one dead eye, a bit of a tremor sometimes) that when Sirius eventually gets out of Azkaban and moves back in, he doesn't recognise him at all. Regulus takes it as Sirius' memory being so fuzzy due to the dementors and dark/harrowing setting that he didn't bother clinging to the memory of people he didn't care about

so from there it's easy to keep his cover as 'Lycan' long-term member of the order, dumbledore's second most trusted man, famously vague and impossible to research

The ruse only fails when one day he accidentally calls Sirius 'Ris' and he KNOWS because nobody else calls him that and oh hell how did he not recognise his own eyes looking back at him and what the fuck happened to his Reggie?

Jegulus and Sirius shenanigans:

James asking Sirius permission to date Regulus and Sirius being all dramatic and silly and doing a bit from the godfather in the gryffindor common room. He even conjures a cat for this moment. Sirius using his lord black voice, "So you've come to me to ask for the hand of my only baby brother." Remus behind them face palming as he watches this while Peter is eating popcorn. James gets into the bit and kneels and acts all dramatic too. Begging to date his lovely brother. Regulus when he finds out about this pranks his brother. Or maybe tells Remus about Sirius feelings for him.
