Pre-implantation genetic diagnosis (PGD) is a technique for analyzing embryos to differentiate those who are carriers of a genetic defect from those who are genetically normal.
The objective of this technique is to avoid the birth of children with chromosomal abnormalities since embryos diagnosed as abnormal are discarded for transfer. This increases the probability of embryo implantation and the probability of successful in vitro fertilization (IVF) treatment.

There are many new tests that are also carried out if needed — DNA fragmentation index to identify if the DNA material is damaged or intact, Karyotyping — to identify and gene abnormalities etc.
Scrotal ultrasound: This is a test that uses ultrasound waves to create images of the scrotum that contains testicles and these images are displayed on the computer screen and results are interpreted. This test helps detect any abnormality of the testicles such as:
Injury to the testicles.
Dilation or swelling of veins in the scrotum (varicocele).
Size of the testicles.
Testicular torsion (twisting of the testicle).
Infection.
Tumours or cysts in the scrotum.
Absence of testicle.
Undescended testicle.
Testing for anti-sperm antibodies: Sometimes men can produce abnormal antibodies that can attack the sperm when trying to reach the egg, and this can prevent a woman from getting pregnant.
Post ejaculatory urine analysis: A test for the presence of sperm in urine to identify any ejaculatory issues such as retrograde ejaculation.
Test for erectile dysfunction (ED): The test includes a careful examination of the external male genitalia to check for sensation. There could also be other underlying health conditions like cardiac disease or diabetes that could be causing ED. A blood sample could also be taken to identify any such condition.

Does IVF work in PCOS?
Polycystic ovary syndrome (PCOS) is a hormonal disorder common among women during their reproductive age. Women with PCOS may have irregular or prolonged menstrual periods or excess male hormone (androgen) levels. The ovaries may develop numerous small follicles, which leads to failure in the regular release of eggs. This condition causes difficulty in conceiving. The hormonal imbalance is the main reason for PCOS and could lead to infertility. As the ovaries are not receiving sufficient quantity of hormones, they are unable to develop the eggs to maturity, and so ovulation does not take place. The way eggs develop and release is affected by the excess amounts of androgens (male hormones) released by the body.
Women with PCOS related infertility are treated with fertility medication, which stimulates ovulation, although this can increase the risk of multiple births if not managed properly.
The other option is to undergo in-vitro fertilization (IVF), where the maturation of eggs takes place in a controlled process. Once the eggs are mature, fertilization with sperms takes place in the laboratory and embryos are formed. Women with PCOS generally respond well to IVF treatment. Occasionally, the problem with PCO patients is that they respond too well to stimulation, which puts them at risk of Ovarian Hyperstimulation Syndrome (OHSS). It is a condition caused by overstimulation of the ovaries. It causes ovaries to swell and leads to a variety of symptoms like abdominal distension and fluid accumulation in the body.

Ovulation Disorders and infertility
Ovulatory disorder is one of the leading causes of infertility in women and can occur when ovulation is disrupted or absent. Such women experience irregular or absent periods. The foremost common causes of ovulatory problems is related to hormone imbalance within the body.
There are several causes of ovulation disorders. Some are caused by hormonal problems when the pituitary gland in the brain doesn’t release the proper hormones. It could also be due to an imbalance in the hormone levels due to stress, diet, exercise, radiation, or a condition called polycystic ovarian syndrome (PCOS) which cause eggs to be released at irregular intervals or totally stop releasing eggs. Other ovulation disorders may be caused by issues with the ovaries. For example, some women are born with no ovaries or have ovaries that fail prematurely (premature ovarian failure). Others have ovaries that are immune to the consequences of hormones and thus the eggs don’t develop (ovary syndrome) or ovaries that are damaged by medication, radiation, or surgery.
How do Ovulatory disorders affect fertility and the ability to conceive naturally?
Infertility problems happen when you stop ovulating or if ovulation process is disrupted. This makes it difficult, if not impossible, for you to conceive. The body prepares for pregnancy by secreting hormones during a normal cycle. Some of them are

GnRH and FSH are the hormones that are liable for causing an egg to mature within a woman’s ovary. LH spurs the eventual release of the mature egg (ovum) into the Fallopian tube where it’ll be fertilized by a man’s sperm.
Though cycle lengths vary, women who ovulate regularly tend to possess a cycle that’s 28 days long and ovulate once during this 28-day period. Women with hormone imbalance or hormone deficiencies experience infrequent or absent ovulation (anovulation) and infertility as a result.

Polycystic Ovarian Syndrome (PCOS): The precise explanation for PCOS is probably the hormone imbalances affecting a woman’s androgen (testosterone) levels and insulin receptivity. Low levels of insulin receptivity can cause an increase in blood glucose levels and lead to an increase in testosterone. Testosterone is produced naturally in women. In women with PCOS, there will be elevated testosterone levels and they can experience irregular or absent periods, anovulation, and have ovarian cysts — the production of multiple cysts that blocks ovarian follicles from producing mature eggs. Testosterone excess can disrupt ovulation and cause infertility. Hypothalamic amenorrhea: FSH and LH are the key hormones essential to obtaining pregnancy. During the cycle, the pituitary releases FSH to signal to the ovaries that a follicle must mature into an ovum. Women with hypothalamic amenorrhea have irregular or absent ovulation because their bodies don’t have the nutrients or fat content which helps send hormone impulses to the ovaries. Excess stress, high or low weight, and excessive weight gain or weight loss can all be contributing factors. Hypothalamic amenorrhea is common in professional athletes, dancers, and ladies with anorexia nervosa.
Hypothyroidism/Hyperthyroidism: High or low levels of the hormone can cause ovulatory disorders and a simple biopsy can help diagnose.
Hyperprolactinemia: Prolactin is a hormone produced by the pituitary gland, and an excess of prolactin may cause hyperprolactinemia. Elevated levels of prolactin hormone can cause ovulatory disorders. Excess of prolactin reduces the estrogen levels, causing infertility. Prolactin hormone is responsible for the production of breast milk. The body is deceived with high levels of this hormone in women who aren’t pregnant and stops ovulating, assuming occurrence of child birth.
Adrenal dysfunctions: Androgens produced by the adrenal glands and abnormal levels of androgens can cause irregular ovulation. High levels of androgens can in-turn cause infertility.
Premature ovarian failure & menopause: Premature ovarian failure (POF) also called primary ovarian insufficiency (POI), is the onset of menopause prior to the age of 40. In premature ovarian failure and menopause, the ovaries stop producing Estrogen. Premature ovarian failure typically occurs because the body has “run out” of functioning ovarian follicles (the sacs that grow to be eggs) early or the ovarian follicles are working improperly. A woman’s body produces less estrogen as she grows old, but the cause of premature ovarian failure is still not known. Women with autoimmune disorders, who received chemotherapy or radiation, or having certain genetic disorders are more likely to experience premature ovarian failure.
Some women may experience ovulatory problems even if they do not have medical issues. Excess stress, high or low body weight, and excessive weight gain or weight loss can all be contributing factors. Certain factors such as being overweight, a woman’s level of activity, and medication use can affect hormone levels and cause infertility. Women who are overweight, or obese experience hormone imbalances that impact their ability to conceive.
Side Effects of Medication
Non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can affect ovulation when taken for extended periods of time. Steroids, even prescribed by a doctor for medical use, interfere with the hormones needed for ovulation as do some epilepsy medications. Many contraception methods use hormones which interfere with the ovaries’ ability to form and release eggs.
Lifestyle Factors
Eating disorders, severe stress, excessive weight loss, and weight gain, and excessive exercise can affect ovulation. Eating healthy and regular exercise is important, avoiding too much or too little exercise can affect ovulation.
Diagnosing and treating ovulation disorders
Ovulation disorders are diagnosed with a detailed case history and menstrual history and physical examination. But other disorders require laboratory blood testing to diagnose. Based on the symptoms, doctors will test for different hormone levels. If a lady has PCOS symptoms, for instance, the doctor will likely test her testosterone and insulin levels.
Once diagnosed, it is important to make lifestyle changes if ovulation has been affected by one of the factors related to lifestyle, most ovulation disorders are often treated with lifestyle changes or medication. Eating healthy and regular exercise is important, too much or too little exercise can affect ovulation. Some prescription medications can alter ovulation. It is important to talk about eliminating medications with the prescribing physician before eliminating them. Prolactin is a hormone produced by the brain’s pituitary gland. prolactin has different functions in the body but the most important of them is to regulate periods and stimulate the breasts to grow and produce milk after giving birth. Once the delivery is over the prolactin level increases which prepares the body for breastfeeding. Too much prolactin in a person’s blood is called hyperprolactinemia an increased level of prolactin reduces estrogen and interferes with ovulation, causing irregular or absent periods affecting fertility. One of the prerequisites for achieving fertilization and successful pregnancy is sperm motility. It has a contribution to your success rates in ART procedures like IUI and IVF. Healthy sperm motility is sperms with forwarding progressions of at least 25 micrometres per second. Sperm motility also refers to the ability of the sperm to swim efficiently to reach the egg during the process called fertilization.
There are different types of sperm motility issues:
Slow or sluggish progressive motility
Non-progressive motility, which is as anything less than 5 micrometres per second
No mobility
Diagnosis:
Sperm motility testing is through a routine semen analysis. These are usually obtained by masturbation at a doctor’s office or testing laboratory. A condition of slow sperm motility is known as Asthenozoospermia when less than 40% of total sperms are motile.
Causes of Asthenozoospermia:

MYTH 1. One needs to take complete bed rest during IVF: It is one of the most prevalent myth that plagues every IVF patient. Many people think that they need “strict” bed rest during the IVF and also after the embryo transfer . Many patients are scared that the embryos will fall out if they walk around. Patients need to know that nature designed the body with enough ‘ability’ that walking, running, coughing, and sneezing does not cause an embryo to fall out. The fetus remains safe in the womb, and just because they have been in an incubator in the IVF does not make any difference. IVF allows us to copy nature, and once the embryo reaches the uterus, it is just like a natural embryo.
MYTH 2. IVF is a painful procedure: Many couples believe that IVF is a painful procedure, which is not true. The needed injections are given with the help of a specialized pen, which is a painless process. In the egg retrieval process, the woman is administered anesthesia, so there is absolutely no pain in that process. After this, there are certain injections that we inject in luteal support, they are also minimally painful. In some cases, we can avoid the injections and replace the same with gel or tablets. So it is a painless procedure.
MYTH 3. IVF means we will have twins: This is also a common myth that if we opt for IVF, we will have twins only. It depends on how many embryos are transferred as part of the IVF cycle and also on how many embryos continue to grow in the womb. The chances of a twin pregnancy is higher if more than 1 embryo is transferred. But it is not guaranteed that IVF will ALWAYS result in twins.
MYTH 4. IVF children have birth defects: Many couples believe that if a child is born via IVF, he/she will have deformities. This myth is based on the wrong understanding that as the child’s conception and early embryo development have happened outside the mother’s body, there are higher chances of birth defects. This is not true. There have been many studies over the last few decades that show that the percentage of deformities in an IVF baby is the same as the percentage of deformities in a naturally-conceived child, not more, not less. It is also important to know that in an IVF pregnancy, the baby is screened with a lot more diligence when compared to a normal pregnancy and so the chances of a birth defect is much lesser.
