MACULAR DEGENERATION
What is macular degeneration? AMD also called as(ARMD) occurs when cells in the macula – the central area of the retina – degenerate. Damage to the macula affects your central vision which is needed for reading, writing, driving, recognising people’s faces and doing other fine tasks. The damage occurs at the level of the layer of cells within the retina called the retinal pigment epithelium (RPE).

Who develops AMD?
AMD only develops in older people (there are other rare types of macular degeneration which occur in younger people). AMD is the most common cause of severe sight problems (visual impairment) in the developed world. It is more common with increasing age.
It usually begins in one eye. About 5 in 100 people aged over 65 and about 12 in 100 people aged over 80 have AMD severe enough to cause serious visual impairment. Women are more likely than men to develop AMD. People of Caucasian ethnicity are more likely to develop AMD than those of African or Asian ethnicity. It is also more common in those who smoke, those who are overweight and those with cardiovascular disease.

Wet macular degeneration
Wet AMD may also be called neovascular or exudative AMD. It occurs in about 1 in 10 cases. In wet AMD, in addition to the retinal pigment cells degenerating, fragile new blood vessels grow from the tiny blood vessels in the choroid into the macular part of the retina. These vessels tend to leak blood and fluid. This can damage the rods and cones and cause scarring in the macula, causing further vision loss. Wet AMD can cause distortion of your central vision, and causes severe visual loss over quite a short time – sometimes weeks or months. Very occasionally, if there is a bleed (haemorrhage), this visual loss can occur suddenly, within hours or days.
If you develop wet AMD (see below) in one eye the risk of developing wet AMD in the second eye is about 1 in 4.
Various systems exist to classify AMD. Both wet and dry AMD can be classified as early, intermediate or advanced, according to the degree of damage to the macula. 6 of every 10 cases of intermediate/advanced AMD are due to wet AMD. You may also see wet and dry AMD classified as early or late, and wet AMD as active or inactive.
Causes of macular degeneration:
In people with AMD the cells of the RPE stop working work so well with advancing age. They gradually fail to take enough nutrients to the rods and cones and do not clear waste materials and byproducts. As a result, tiny abnormal deposits called drusen develop under the retina. In time, the retinal pigment cells and their nearby rods and cones degenerate, stop working and die. This is the dry type of AMD.
In wet AMD, new blood vessels grow into the layers of the retina from the choroid. The reason why this happens in some cases of AMD is not known, although waste products or shortage of oxygen may be involved.
Certain risk factors increase the risk of developing AMD. These include:
Smoking tobacco. High blood pressure. A family history of AMD. (AMD is not a straightforward hereditary condition. However, your risk of developing AMD is increased if it occurs in other family members.) Sunlight. Laboratory studies suggest that the retina is damaged by sunlight rays (UVA and UVB rays). Being very overweight. Poor diet. AMD is more common in people from white (Caucasian) racial backgrounds than from other racial groups.
Symptoms of macular degeneration
Early symptoms AMD is painless. Symptoms of dry AMD tend to take 5-10 years to become severe. However, severe visual loss due to wet AMD can develop more quickly.
If macular degeneration develops in one eye only, you may not be aware of it until it’s quite advanced, as the other eye will still see the things you are looking at with your central vision. When both eyes are affected you are more likely to notice symptoms.
The main early symptom is worsening of central vision despite using your usual glasses. In the early stages of the condition you may notice that you need brighter light to read by. Words in a book or newspaper may become blurred. Colours may appear less bright and you may have difficulty recognising faces and facial expressions.
Later symptoms As the condition worsens, a ‘blind spot’ then develops in the middle of your visual field. This is not always initially noticeable. However, it tends to become larger over time as more and more rods and cones degenerate in the macula.
One early symptom of wet AMD is visual distortion. Typically, straight lines appear wavy or crooked. For example, the lines on a piece of graph paper, or the lines between tiles in a bathroom.
Visual hallucinations (also called Charles Bonnet syndrome) can occur if you have severe macular degeneration. People see different images, from simple patterns to more detailed pictures – often they see complicated images of children or animals. The experience can be upsetting but is less frightening if you are aware that it can happen in AMD. Importantly, it does not mean you are developing a serious mental illness. If you do develop visual hallucinations they typically improve by 18 months.
Peripheral vision is not affected with AMD and so it does not cause total loss of vision.
Always see a doctor or optometrist promptly if you develop visual loss or visual distortion.
Older people should in any case have regular eye checks to check each eye separately for early AMD (and to check for other eye conditions such as glaucoma).
Tests for macular degeneration If you develop symptoms suggestive of macular degeneration, your doctor or optician (optometrist) will refer you to an eye specialist (ophthalmologist). This should be done urgently, in case you have wet AMD (which can worsen rapidly but which can be treated).
The ophthalmologist may ask you to look at a special piece of paper with horizontal and vertical lines to check your visual fields. If you find that any section of the lines is missing or distorted then it is a possible cause of the visual problem. The ophthalmologist will examine the back of your eye with a slit-lamp microscope. Digital photographs can be taken of the retinae. The ophthalmologist will look for the typical changes that occur with dry and wet AMD.
Another test called ocular coherence tomography is becoming more commonly used. This is a non-invasive test that uses special light rays to scan the retina. It can give very detailed information about the macula and can show if it is abnormal. This test is useful when there is doubt about whether AMD is the wet or dry form, and to monitor treatment.
Diagnosis of wet AMD is further tested for fluorescein angiography. A dye is injected into a vein in your arm and then, by looking into your eyes with a magnifier the ophthalmologist can see where any dye leaks into the macula from the abnormal leaky blood vessels. This can give an indication of the severity of the condition.
Macular degeneration treatment:
Whether or not there is a treatment that can prevent progression, or even reverse your condition, it is important to maximise the sight you do have.
Stopping smoking and protecting the eyes from the sun’s rays by wearing sunglasses are important in slowing progression of the condition.
A healthy balanced diet rich in antioxidants can be beneficial, as may the addition of dietary supplements.

PARKINSON'S DISEASE
INTRODUCTION:
Parkinsonism or parkinson’s disease is a neurological disorder that causes unintended or uncontrollable movements, such as shaking, stiffness, and difficulty with balance and coordination.It is a degenerative disease which impairs the motor skills and speech of the patient.
Symptoms usually begin gradually and worsen over time. As the disease progresses, people may have difficulty walking and talking. They may also have mental and behavioral changes, sleep problems, depression, memory difficulties, and fatigue.
 EPIDEMOLOGY:
Most people with Parkinson’s first develop the disease after age 60, about 5% to 10% experience onset before the age of 50. Early-onset forms of Parkinson’s are often, but not always, inherited, and some forms have been linked to specific alterations in genes.

People with Parkinson’s disease also lose the nerve endings that produce norepinephrine, the main chemical messenger of the sympathetic nervous system, which controls many functions of the body, such as heart rate and blood pressure. The loss of norepinephrine might help explain some of the non-movement features of Parkinson’s, such as fatigue, irregular blood pressure, decreased movement of food through the digestive tract, and sudden drop in blood pressure when a person stands up from a sitting or lying position.
SYMPTOMS OF PARKINSONISM:
Parkinson’s has four main symptoms:
Muscle stiffness, where muscle remains contracted for a long time
Slowness of movement
Impaired balance and coordination, sometimes leading to falls
Other symptoms may include:
Depression and other emotional changes
Difficulty swallowing, chewing, and speaking
Urinary problems or constipation

The symptoms of Parkinson’s and the rate of progression differ among individuals. Early symptoms of this disease are subtle and occur gradually. For example, people may feel mild tremors or have difficulty getting out of a chair. They may notice that they speak too softly, or that their handwriting is slow and looks cramped or small. Friends or family members may be the first to notice changes in someone with early Parkinson’s. They may see that the person’s face lacks expression and animation, or that the person does not move an arm or leg normally.
People with Parkinson’s disease often develop a parkinsonian gait that includes a tendency to lean forward; take small, quick steps; and reduce swinging their arms. They also may have trouble initiating or continuing movement.
Symptoms often begin on one side of the body or even in one limb on one side of the body. As the disease progresses, it eventually affects both sides. However, the symptoms may still be more severe on one side than on the other.
Many people with Parkinson’s disease note that prior to experiencing stiffness and tremor, they had sleep problems, constipation, loss of smell, and restless legs. While some of these symptoms may also occur with normal aging, talk with your doctor if these symptoms worsen or begin to interfere with daily living.
CHANGES IN COGNITION:
Some people with Parkinson’s may experience changes in their cognitive function, including problems with memory, attention, and the ability to plan and accomplish tasks. Stress, depression, and some medications may also contribute to these changes in cognition.
Over time, as the disease progresses, some people may develop dementia and be diagnosed with Parkinson’s dementia, a type of Lewy body dementia.. People with Parkinson’s dementia may have severe memory and thinking problems that affect daily living.
DIAGNOSIS OF PARKINSONISM
There are currently no blood or laboratory tests to diagnose non-genetic cases of Parkinson’s. Doctors usually diagnose the disease by taking a person’s medical history and performing a neurological examination. If symptoms improve after starting to take medication, it’s another indicator that the person has Parkinson’s.
A number of disorders can cause symptoms similar to those of Parkinson’s disease. People with Parkinson’s-like symptoms that result from other causes, such as multiple system atrophy and dementia with Lewy bodies, are sometimes said to have parkinsonism. While these disorders initially may be misdiagnosed as Parkinson’s, certain medical tests, as well as response to drug treatment, may help to better evaluate the cause. Many other diseases have similar features but require different treatments, so it is important to get an accurate diagnosis as soon as possible.

Causes of Autism:

Autism is four times more common in boys than in girls. It can happen in people of any race, ethnicity, or social background. Family income, lifestyle, or educational level doesn’t affect a child’s risk of autism. But there are some risk factors:
Autism runs in families, so certain combinations of genes may increase a child’s risk.
A child with an older parent has a higher risk of autism.
Pregnant women who are exposed to certain drugs or chemicals, like alcohol or anti-seizure medications, are more likely to have autistic children.

Affections of the thyroid gland, a butterfly-shaped gland in the front of the neck has an important role to regulate numerous metabolic processes throughout the body. Different types of disorders affect either its structure or function.
It is located below Adam’s apple wrapped around the trachea (windpipe) and a thin area of tissue in the gland’s middle, known as the isthmus, joins the two thyroid lobes on each side. It uses iodine to produce vital hormones,Thyroxine, also known as T4, is the primary hormone produced by the gland. After delivery via the bloodstream to the body’s tissues, a small portion of the T4 released from the gland is converted to triiodothyronine (T3), which is the most active hormone.
The function of the gland is regulated by a feedback mechanism involving the brain. When thyroid hormone levels are low, the hypothalamus in the brain produces a hormone known as thyrotropin-releasing hormone (TRH) that causes the pituitary gland (located at the base of the brain) to release thyroid-stimulating hormone (TSH). TSH stimulates the gland to release more T4.
Since the gland is controlled by the pituitary gland and hypothalamus, disorders of these tissues can also affect thyroid function and problems.

ATOPIC DERMATITIS/ECZEMA

This chronic disorder associated with pruritus usually starts in infancy and presents with dry skin, eczematous lesions and lichenification. It is believed that Atopic Dermatitis(AD) is associated with other IgE associated disorders like allergic rhinitis, asthma and food allergies.It has significant morbidity and it appears that the prevalence of the disorder has been increasing over the past few decades.
Etiology of Atopic Dermatitis/Eczema:
Patients with the atopic triad have a defective barrier of the skin, upper respiratory, and lower respiratory tract. If one parent is atopic, there is more than a 50% chance that their offspring will develop atopic symptoms and If both parents are affected, up to 80% of offspring will be affected. Genetic alterations include loss of function mutations of filaggrin,an epidermal protein that is broken down into natural moisturization factor. Filaggrin mutations are present in up to 30% of atopic dermatitis patients and may also predispose patients to ichthyosis vulgaris, allergic rhinitis, and keratosis pilaris. Ninety percent of such flares are caused by eggs, milk, peanuts, soy, and wheat. Atopic dermatitis has a higher incidence at higher latitudes, which may be related to decreased sun exposure and lower humidity levels.
 Atopic dermatitis/Eczema is divided into three subsets based on the age of onset:
Early-onset atopic dermatitis (birth to 2 years old):  is most common type of dermatitis, with approximately 60% of cases starting by age 1 and resolve by 12 years old. Late-onset atopic dermatitis: symptoms begin after the onset of puberty Senile onset atopic dermatitis: an unusual subset with onset in patients older than 60 years old.

In AD, there is significant water loss across the epidermis and is believed that filaggrin, which is critical for epithelial integrity, may be dysfunctional.
History and Physical:
Onset and distribution of lesions The severity of pruritus (e.g., keeping patient awake at night) Family and/or personal history of the atopic triad Presence of contact allergens The presence of triggers including allergens (e.g., dust mites, animal dander), hot showers/sweating, soaps, fragrances, food hypersensitivities, and synthetic fabrics such as polyester. Classic physical exam findings depend on age group.
 Infants:develop edematous papules and plaques that may have vesicles or crust on the scalp, face, and extensor extremities. Infants rarely have atopic dermatitis lesions affecting the diaper area but may be susceptible to other causes of diaper dermatitis such as candida or seborrheic dermatitis.
 Children: classically have less exudative patches and plaques on antecubital and popliteal fossae; adults have chronic lichenified (enhanced skin markings) lesions that have a predilection for hands. Individual lesions may also be further classified into acute (edematous, erythematous papules and plaques and/or vesicles/crusting), subacute (erythema, scale, variable crusting), or chronic (thick plaques with lichenification and scale) stages.
Evaluation:
Atopic dermatitis/Eczema is typically a clinical diagnosis given the classic distribution of lesions in each age group and the presence of e.g., keratosis pilaris may facilitate diagnosis. A biopsy will show an eczematous pattern and in childhood cases that are recalcitrant to treatment, fluorescent enzyme immunoassays or skin prick testing can be performed to detect immunoglobulin E (IgE) antibodies against specific allergens.

BRONCHIAL ASTHMA
INTRODUCTION:
 Bronchial Asthma is a chronic respiratory disease characterized by recurrent attacks of breathlessness and wheezing. During an attack, the lining of the bronchial tubes (airways) swells, causing the airways to narrow, making it more vulnerable to irritants in the environment, and thus reducing the flow of air into and out of the lungs. 
The risk factors for developing asthma include inhaling “triggers”, such as allergens pollens, tobacco smoke and chemical irritants. Several patients may also have allergic rhinitis which is characterized by sneezing and “runny” nose and tend to have their first symptoms by the age of ten years, and many children had their first attack before the age of six. Asthma cannot be cured by conventional medical system but homeopathy has an answer to it along with appropriate management one can control the disorder,shorten the episodes and enable people to enjoy a good quality of life.
Symptoms:

Homeopaths believe that when there is a tendency or predisposition for a disease – it first manifests on the less vital organs, towards the periphery (like skin). If this manifestation is suppressed than the disease shifts inwards, towards the more vital organs (like lungs, heart, brain etc).

INTRODUCTION:
Focus on Mental Health.W.H.O defines health as a complete sense of well being in our spirit, mind and body. It is mind (mental) health that we are going to discuss. Even science today agrees that for every bodily ailment there is a mental cause labelling it as psychosomatic. (Psycho coming from psyche(mind) and soma (physical).
Mental Health Symptomatology:Eustress/Distress:
 There is no health without mental health and the number one thing that is directly associated with mental health is ‘’STRESS’’. So let’s understand what stress is ?
 STRESS:
 is an emotionally disruptive state of mind which occurs as a result of adverse external influences. It simply means when we cannot handle a situation where we don’t feel adequately equipped. Often we speak about this stress which is called ‘distress’ but stress can often be interpreted as a challenge which generally denotes positive stress or ‘EUSTRESS’. Eustress keeps us excited about life (eg; winning a race or accomplishing a task. It refers a positive response to a stressor.(A stressor is a negative threat) so,Eustress is very important for a person to generate maximum output and performance as It helps improve our efficiency at work thereby improving our personal and professional life, Eustress can also help us get rid of our chronic stress. It helps us to consider the threats in our life as challenges and it simply fuels success and achievements and gives us motivation and productiveness.
Treatment:
Homoeopathy is helping humanity by converting stress into eustress as a habit. Basically it helps us change to the situation. Our situation does not change, we change first than the situation changes and this boosts our mental health.
Examples:
 To give an example let’s consider three friends on a train or bus hijacked by terrorists 1st person: would freeze to the spot, give up and quit. Waiting to die. 2nd person: would look for tools and alternatives to act in this situation. A helpline, a number to call etc . 3rd person: without giving a thought would pull out a weapon or gun and kill and get killed. Well both the 1st and 3rd person have served no purpose but it was only the 2nd one who acted well. The 2nd person acted in a healthy way. Though he too was stressed, situation was not all together favourable for him, but he considered giving it a thought.
 His fertile mind(good mental health) helped him look for solutions to the problem.(This is a perfect example of eustress) Homoeopathy plays a major role in converting your distress to your eustress.
 
Homoeopathic Approach:
Every person is different and every reaction to the same situation is different. That’s why it is the ‘Individuality’ of the person that determines the choice of homoeopathic medicine. There is no generalisation but individualisation in Homoeopathy because the concept of Homoeopathy stands on the axiom mind creates matter. That simply means our mental status gives rise to our bodily ailments and that our thoughts govern our body. 
 
Spiritual quotient for good mental health:
The basis for this axiom comes from the Holy Bible which mentions that “As a man thinketh in his soul he becomes”.
Now I would like us all to consider a very important aspect in our life and that is having a spiritual quotient. You would ask why. I would say why not. Spirituality has always been the foundation and the quest for man to attain higher consciousness and the more the person is spiritually evolved the less he would suffer. The simple reason being that in spirituality we find the root words for our existence. This is the basic human need to know his creator his benefactor. Here we find the right thoughts for our believing.

Needless to say, once again I want to urge each one of us to have our thoughts and words governed well because words and thoughts are creative they shape us , they mold us and they make us what we are meant to be!!
Therefore our mental health is directly proportional to what we think and what we speak.
