It depends. As I do love getting requests with themes, I don’t exactly do specific codes specifically made for specific users- my codes are free for everyone to use. So finally: you can request codes, but not just for you, for everybody.

Mockup | Google Forms

Features: a customizable header, multiple choice, checkbox, short, and long answer boxes that are extremely similar to the original version, submit button that turns dark purple on click like original, popup that appears when you click the submit button, link in popup that brings you to give another response

Note: Unfortunately there was a lot of CSS coding for the popup, button, and overall body, so I had to make it a little less easier to read. If you really don’t like it that way, copy the CSS into a CodePen, then click “Format CSS”. Additionally, this will not actually send your answers anywhere as it is just a mockup.
