He screamed like he was being burned alive. I can still hear it.

Also in April I started a new full time job. This is both the first full time and first office job I have ever had.

And then it’s May. My dad isn’t...doing well. He’s legitimately old so that’s disconcerting. And then he sits us down one night and tells us we was told in February his cardiologist told him the heart valve he had replaced like 12 years ago needed to be replaced again. Which is a thing we knew would need to happen but they last for 15ish years so we weren’t overly worried, and his cardiologist told him he had till towards the end of the year before it would be a problem. Turns out he did not have towards the end of the year. He had two months from that appointment before the valve all but totally failed. He had to go to the ER, his legs were so swollen and his heart was struggling so much to circulate his blood that his calves were leaking lymph fluid.

In the ER his heart rate was under 40 beats per minute, his whole body shuddered when he exhaled. He was pale. He wasn’t my Dad.

They transported him to the Cleveland Clinic. They did a catheter to check out his heart. No one helped him after when he tried to stand to go to the bathroom, he was dehydrated, his blood sugar was low, he’d just had a camera shoved inside his veins. He passed out, fell, and he was caught on the very last minute. But not before he fell and caught his full weight on a previously injured knee. He was immediately unable to bare weight. The knee was never properly checked.

He had surgery. It was supposed to be 3-4 hours long. While it’s heart surgery, heart surgery in the Clinic is pretty routine, simple. We weren’t worried.

His surgery took 8 hours. They replaced one valve entirely, and repaired two others and all but the entire right side of his heart was worked on. We later found out during surgery he had a small stroke impacting his right hand’s mobility. They kept him under overnight as he tends to react violently upon waking with a ventilator in his mouth. Which. Fair.

He ended up being in the ICU for just over two months. During which time he aspirated due to a feeding tube moving from his intenstines into his stomach. He almost died.

They then kept him on a vantilator for a full week, his lungs were so irritated from the aspiration and it turns out an upper respiratory infection.

All during this I was working 40 hour weeks at home alone. By myself. My mother and my brothers were in Cleveland. I got a call from my youngest brother when my dad aspirated. I answered the phone and he was sobbing.

I finally got to see him in June. He was out of the ICU and in step down. He was delirious. Said a building was a sail boat off the coast of Mexico. He barely recognized me. He had to be watched because at one point after the aspiration he’d had a tracheostomy and ripped out the valve in his sleep. Despite having someone “watch” him at all hours of the day, he pissed himself several times and the babysitter didn’t notice. Several of these people would use the clinic computer to shop.

Eventually he came back home. But to a rehab center for his knee and hand. He couldn’t even hold a phone in his right hand and still couldn’t walk. He then moved to another rehab center that started him in intensive physical therapy.

One night while we were there my mom was rubbing my Dad’s neck. She felt a lump on the side of his neck, I chalked it up to a blocked up lymph node. His doctor didn’t know what it was but wasn’t concerned by it.

He came home home after that. He uses a walker and a wheelchair still. He has to use a chair to shower. He’s almost the man I remember. Which I know isn’t fair to him, but I can’t help but feel that way.

One night he layed down flat in bed. My mom noticed a lump on his chest. Surgical hernia. We apparently aren’t worried about that. The lump on his neck though. It kept getting bigger.

Two weeks ago we found out it’s cancer. That weekend we found out it was stage 4. Then that Wednesday we found out that the scoring they use for head and neck cancer changed in January and it’s actually stage one? I don’t know. The biopsy tested positive for two kinds of cancer. I don’t understand it.

And now it’s almost October. I have some kind of head cold. It’s fashion week in my office starting Monday?

I don’t know. I don’t understand anything.

I’m scared.

But so is my dad. My mom. My one brother can’t sleep alone and the other is having problems. So I’m the rock. I’m the one this whole family is tethered to.

My boyfriend and I have been dating for just under 2 years now. I love him more than everything. We’re trying to look for an apartment but with everything that’s happening I can’t even really think about it.

Three client service aspirations to master in the new decade.

A few years back, I received an email from a doctor. I don’t recall this physician’s name, but I do recall what he said:   

“Would you consider working together on a book similar to yours, directed at the medical community?  I see lots of parallels between what you do and what I do; it really would be helpful.” 

I (sort of) get his point, but seriously? Medicine is about facts, diagnosis, and science; what I do (and write about) is about opinions, theories, and instinct.  The two could not be more different. 

But then I thought about my first-hand, operating-room experiences with the medical profession:  my hip replacement, my recovery from I-could-have-died cerebral hemorrhage, and, most recently, a hernia repair, something others might describe as ‘minor” surgery, but is what I define as, “surgery happening to someone else.”   

Getting myself fixed is like getting my car fixed: I know absolutely nothing about either.  But I do know I want at least two things:  candor and competence.   With this most recent experience, I got both, starting with my surgeon, who told me, 

“The hernia repair is relatively straightforward, but recovery can be a bitch.” 

To which I replied: “How bad, Doc?”   

“Really bad.” 

“Rank the pain on a scale of one to ten.” 

“Seven to nine.” 

“OMG; that’s kill-me-now pain!” 

Doctors rarely disclose how much you’ll hurt; they either downplay or dismiss the issue.  This one thankfully did not.  My misery didn’t approach anywhere near the level he predicted – if you must know, maybe on the worst day it crept to maybe a four – likely a testament to the surgeon’s skill in the OR.   It also might be the result of Doctor Feelgood’s ministrations – that’s the attending anesthesiologist – who mercifully plied me with the requisite, highly efficacious painkillers I took for a week post-surgery, before declaring myself sufficiently pain-free to rid myself of them.

 Speaking of competence, leavened with a healthy dose of kindness and compassion, I was the beneficiary of this from everyone else I came in contact with that day:  the admitting staff, the prep nurses, and the recovery nurse. 

Now, Napa is not New York, a metropolis of eight million, where, given its sheer size, you might rightly expect competence.  But in a town 99% smaller, I found myself struck by just how professional, efficient, effective, and yes, kind everyone was.  To a person, everyone seemed to be punching above their weight, but if I were to put this to the people I interacted with, they would look at me funny.  “Just doing my job,” they would say. 

With this latest procedure I was my usual anxious self, babbling endlessly as my nurses Marissa, Lisa, and then later Cannon, methodically but cheerfully walked me through pre-op prep and post-op recovery.  I was running on at the mouth so much they easily and rightfully could have become irritated or annoyed.  If they felt even a hint of that, they did not show it.  I frankly don’t know how they deal with nervous patients like me. That’s kindness/compassion for you. 

Tally these things -- candor, competence, kindness/compassion -- and you get the medical profession at its best.  Now think about client service.  Aren’t these the very qualities we prize? 

Take candor.  Without it, there can be no trust; without trust, there can be no relationship; without relationship, there can be no risk-taking work that performs the way it should. 

We might fancy ourselves the next David Ogilvy, Leo Burnett, or Jay Chiat, but few of us will come close to matching the creative and organizational genius of these agency founders.  And knowing how most clients are perpetually disappointed in and frustrated with their advertising and marketing agencies – witness how many clients are increasingly migrating to in-house capabilities -- I’ll gladly take simple, consistent  competence in all things client service over more inspired but less bankable options. 

The world is an ugly place – contentious and divided, with anger bordering on irrational, unreasonable, and irresponsible – where what’s needed most these days is a bit more concern and caring.  If we simply inject a measure of kindness/compassion in what we do, you put yourself  in your client’s shoes, walking in them rather than your own.  You listen a little harder, longer, and better.  You remain patient, even when the temptation is to scream. You ask about things that traverse the professional to the personal.  You connect with clients as people, not as paychecks. 

Many of us begin a new year, let alone a new decade, with all sorts of resolutions, some grand and unrealistic, others more modest and achievable, others not.  You might view candor, competence, and kindness/compassion as obvious or easy to master, but having been in this business more years than I care to admit, I think otherwise.  They are challenge to the medical profession, and they are a challenge to those of us striving to serve clients to the best of our ability. 

These are my aspirations for the new decade.  Yours? I welcome hearing from you.

JUNE 2019 Family Update #1: Hard Doesn’t Always Mean Bad

If you look back at any past blog posts on this page, you know it’s been a while and my life has changed in more than a few ways since then. You can also tell because this is tumblr, and honestly I’m not going to put much effort into the formatting (sorry to any designers reading, including my sister & my wife).

I’ve always enjoyed writing from time to time, but it’s been on my heart more recently. I have to admit with all of the recent changes in my life--becoming a husband, a father, settling down, etc.--I’ve become such a sap. I’m talking tearing up at weddings & really good movies; journaling and reflecting on every stage Jenna goes through; stopping to, literally, smell the roses...Yeah, I think it’s about time to add some testosterone to this household. Which brings me to what prompted me to start writing again.

As you all know by now, we’re expecting our second child (a boy) at the end of August. I’d like to push it to October or November but for some reason Maddie didn’t like that idea--something about being uncomfortable?  Anyway, this baby is already making things interesting. After going to his first ultrasound at 20 weeks we were thrilled to see him moving around and excited to share the news with everyone that Friday. Throughout the weekend, Maddie and I both had a weird feeling about it, but chose not to worry. The following Monday, Maddie got a call from her doctor revealing some hard news to hear-- that our baby was diagnosed with a Congenital Diaphragmatic Hernia (CDH). Discovering this was surreal, and the countless questions started swirling in our heads. A lot has happened since the time we found out, so I’ll try to keep the details brief -- I’ll say first that he’s going to be okay.

CDH is a hole in the diaphragm, which allows organs from the abdomen (most commonly intestines, stomach, liver) into the chest cavity. At first, this was a pretty scary thing to learn about, especially considering the average survival rate is 50%. We got referred to Children’s Hospital for more testing and consultations. After a long couple of days of tests on Maddie including an MRI, multiple ultrasounds, an echocardiogram, and a 2-hour long meeting with doctors, we finally had some good news. The hole in his diaphragm is small enough to keep major organs out of his chest, only allows his intestines in, and his lungs are developing fine at this point! This could change and it’s still serious, but given the potential for worse, we are very thankful.

What this means for Maddie and baby boy: she’ll continue to go in at least weekly to monitor his condition, and she’ll be watched closely as the due date approaches. When he’s born, he’ll need a breathing tube immediately to ensure proper lung inflation until he is stabilized. Then, they will perform surgery on him to put his intestines back in place and repair the opening in the diaphragm. After surgery, he will likely be in the NICU for about a month, maybe longer depending on recovery.

So, in short, it will be a difficult few months for us coming up, but God is good all the time. He has been gracious with his timing, given us an amazing support team with family and friends, placed us right by the #2 ranked children’s hospital in the nation, and given us grace in so many other ways. I’ve always said that God is faithful in tough times but this, so far, has been one of the most tangible ways I’ve seen. Maddie and I have to laugh a little because we were so determined to leave Ohio and the Midwest after college, and God had better plans. Plans to keep us near family/friends and a hospital with a team of experts on this relatively rare defect. As with most things, we think we know best and God gently leads us back to his ways (see James 4:13-15).

We are so humbled by all of you who have expressed your concern, asked thoughtful questions, and especially have been praying over us. I can genuinely tell you we feel the prayers and support (told you I was a sap now). I’ll try to post somewhat frequently to share updates on baby boy and our family. If you’ve made it this far, congrats! You’ve earned the right to find out his name (unless you cheated and scrolled to the bottom--shame on you). Given the circumstances, we decided to reveal his name before he is born. We were also worried about Jenna spilling the beans, so here it is...

Pickles: I Will Survive!

One of the farm animal species that is hardest to place, introduce into existing populations, and care for financially is also one of the species we get the most calls about, especially over the past few years: the pig. The majority of recent instances have involved people who wanted to raise a pig in their home, so went out and bought a piglet and then discovered that this is not the best idea. But we will save that discussion for another day.

It is easy to understand the draw to a piglet, but remember — 2 pounds goes to 250 pounds in no time at all. And when full grown, a farm pig female can weigh well over 600 pounds — not so great in the house. 

Pickles’ story was not that — although she did live in a home for a few short days while arrangements were made for her long-term care. Pickles was born on a farm, and a friend of a friend was notified when it was discovered that a mother had given birth to a litter in which all of the piglets were born dead except one — a tiny little 2-pound baby.

And thankfully, this friend of a friend could not let Pickles perish, with the mother pig showing no interest in nurturing her offspring. So this amazing and very tiny little bundle of piggy cuteness, named Pickles by her rescuer, came to Farm Sanctuary’s New York Shelter.  

Thankfully, she took quickly to her bottle and had a good appetite.

Her new family gave her a few days of care while trying to find permanent arrangements, but always knew that they could not keep her (although they were very attached). And it’s fortunate that Pickles arrived at Farm Sanctuary when she did — for this was not a healthy pig.

Pickles is very lucky that her new people stepped in when they did, or she too, like her siblings, would have perished. Here she is sleeping and finally feeling safe. 

Pickles had an umbilical hernia and needed surgery, both to clean up the infection and to attempt (for the first time — many more followed) to close the tiny opening in her body cavity. The smaller hernias can actually pose more of an issue than larger ones, with the slippery little intestinal tract getting into the tiny hole and strangulating. This can quickly become fatal, since the those strangulated portions of the intestines die.

So surgery #1 was an emergency. Pickles was not able to poop, and there was concern that the intestines might be trapped in the tiny hernia pouch. Under anesthesia, it was confirmed that they were, and the hernia was repaired — thankfully, just in time. 

Cleaning the very infected umbilical area — and as you can see, Pickles at just a few days old had no problem protesting this treatment. 

Besides the hernia, she also had navel ill — caused by receiving no colostrum from mom — so she was septic. So her surgery included removing the entire umbilical stump. Coming out of anesthesia, she also had a seizure — this was very scary.

Pickles with belly band — and none too happy about it. The band was not comfortable, but it was the only thing keeping her intestines from getting caught in the tiny hole in her body cavity. 

So home she came after two weeks of recovery and care in the hospital — but in that time, she had gained only two pounds. She was not handling food well at all. And she is a piglet — they do not stand still, even in the tiny enclosure we created for her for just that purpose, complicating the recovery process. So in less than two days, our tiny bundle of energy herniated again. 

So she had surgical repair #2 and came home again — this time with a belly band — and she was also finally gaining some weight (she’d grown from 4 to 7 pounds).

And besides that belly band — which had to be changed regularly so we could check her incision site — she had to stay still and calm.  

Pickles had to take lots of meds — and sadly, you cannot keep a crazy piglet down.

So I could bore you with the multiple herniations, but suffice it to say that the trouble continued, and finally the hernia was very large — allowing the intestines to easily go into this opening. This too is not ideal — since if they were to get caught on something, that hernia could open and there would be nothing holding her insides in if that were to happen. So more small stall rest, more belly-band changes, and more angry Pickles.

“Get me out of this box!” Pickles had to have a very small, confined pen to keep from opening up her hernia even further. But even a small area could not stop this little jumping bean from staying active. 

But health-wise, the stall rest and belly bands paid off, and her body healed on its own without surgical intervention. It was like a miracle. And Pickles was thrilled. She was allowed to play — run — and all this belly-band free. Psychologically, however, she has a few issues. 

“Hello Amy — what will you be feeding me next?”

We knew that she was a bit of a nipper, which is not unusual with bottle babies, but her nips were pretty significant. We then heard that, as a tiny 30-pound pig (during hospital stay #5), she attacked some of her doctors. Pickles, Pickles, Pickles. 

Even as a baby, Pickles loved to bite, but at first it was cute — and did not hurt. 

So, perhaps due to her stay in solitary (necessary for her survival, of course), she also shows signs of having porcine stress syndrome, which is likely the cause of her hyperthermia (her temperatures was 106.8° Fahrenheit) and seizure coming out of anesthesia. Could it be that having a girdle-like belly wrap that had to be constantly changed did it? But no matter — Pickles is not really predictable, and we are hoping she grows out of this, like most pigs do.

She has a bit of an edge. She is also a herd animal, and up until this week, knew nothing about other pigs and likely felt that humans were her herdmates. Remember that piglets establish hierarchy within the first 24 hours of life — so this makes sense. Attack now and get your place in the rankings — and if we are her herd, she really is just doing what comes naturally. 

Pickles is mocking me. Seriously mocking me.  

So this week we introduced her to someone who also needs a friend — a family and a herd: Mouse!

And although they are still working things out, they now have each other — and she finally seems to realize that she is indeed a pig. And she can help Mouse with his physical therapy. (Stay tuned for a whole blog post update on Mouse!)
