Today's case is a 25 year-old man with leg pain and selling. Radiograph reveals a lucent tibial lesion with groundglass appearance, thinning of the adjacent cortical bone, and no periosteal reaction. These features favor a benign entity. Tc-99-MDP bone scan reveals intense uptake and no other lesions. Findings are most compatible with monostotic fibrous dysplasia. I need to start investigating my weird chronic ankle/leg pain.
Standing for anywhere from 10 to 30 minutes without much movement causes pain in my ankles, knees, and hips. But the pain escalates rather quickly from there and can get pretty bad.
Walking and running reduce the time/likelihood this happens. I can walk for about an hour if i don’t stop at all.
This came to a head over my vacation, where i would be limping constantly and on the ground in pain, while my two sisters were fine. We did the same amount of walking/standing, yet i had a pronounced limp and was in PAIN (on a scale of 1-10, probably a 7 or 8.) the soreness in my ankles lasted for days.
This happens all the time. It has always been a thing with me. There is no start date to when its been a problem.
Supportive Insoles do not help much. I do not wear flat shoes.
I saw a podiatrist at 22. They ran a bonescan and a live xray of my ankles. My bones were perfectly fine (????) and i was only a *little* flat footed
Y’know, i was given corrective boots/braces when i was an infant and sometimes i wonder if they actually fucked me up.
I also now have really bad chronic neck pain but thats just shit posture.

Stress fractures take place when the bones have small cracks in them due to various reasons such as strenuous physical activity or repeated stress on similar bones.
It interferes with one’s ability to move and causes excessive pain. However, there are cases where they heal naturally over time when a person rests and avoids stressful physical activities. It is best to go to a specialist for treatment as soon as possible.
The main reason for a stress fracture is the excessive stress on the bones of the lower leg over a certain period of time. The below areas are most susceptible to stress fracture:
Fibula – calf bone
Tibia – Also known as the shinbone or leg bone
Metatarsals – Foot bones
Navicular – also called tarsal bones. This is also afoot bone but shooter than Metatarsal.

Swipe! A broken bone can heal with or without the supervision of a doctor, with or without manipulation, immobilization, or surgery. But having it seen by a professional is a key step in having less pain, getting good outcomes and being productive once more. For this patient, pain got worse after surgery, routine medications weren’t helping, and motivation was poor to do physical therapy. (A) The bone scan was performed to confirm clinically evident complex regional pain syndrome, Sudeck atrophy. There is increased uptake on all three phases with diffuse activity around the knee and ankle. (B) AP and lateral radiographs show an AO 42B3c fragmentary wedge fracture to distal tibia diaphysis and 4F2Ba multifragmentary fracture of the proximal fibular diaphysis. (C) Closed reduction internal fixation with minimally invasive plate osteosynthesis was done. (D) Postop one year with good remodeling. (E) Postop five years with radiologic union. (F) Bone scan at seven years postop shows decreased uptake compared to baseline. Clinically, the patient has improved to work again. We should strive for patient comfort first and foremost. Good x-rays do not necessarily mean happy patients. Patients with FD can undergo MDP bone scan to assess for polyostotic involvement. Polyostotic FD typically involves "long lesions in long bones." Patients should be evaluated for endocrine abnormalities, particularly pediatric patients (we will cover this in more detail tomorrow).
Today's case is a 30 year-old woman with longstanding leg deformity and new leg pain. Radiograph reveals multiple lucent lesions with cortical thinning and no periosteal reaction. Findings are suggestive of FD. At bone scan, there is uptake in the left pelvis, femur and tibia, compatible with polyostotic FD.
