What happens during a cataract surgery?
Cataracts develop as a result of many factors in old age. Most commonly it is because of long standing diabetes. There is deposition of crystalline substances on the lens within the orbit, which gradually causes your lens to harden and lose its flexibility, which eventually causes vision to deteriorate. Therefore cataract surgery involves the removal of this hardened crystalline lens, which is then replaced with an Artificial Intraocular Lens (IOL). Although you will discuss the procedure in detail with your doctor and they will explain the entire process to you, it is always good to have some information with you beforehand, so that you know that right questions to ask them.
There are different types of IOL’s which are being used in modern day cataract surgery, where some are considered to be standard IOL’s and some to be premium IOL’s. And you should have an idea about all of them in order to help you decide which one is best suited for you, along with the advice of your surgeon.
The different types of IOL’s:
Multifocal:
As the name suggests this type of lens helps you to look at a range of distances, by allowing your eye to focus on objects which are up close or at a distance. The technology which is used in the lens, helps your brain to work out at what distance the object is and then focus on it. Most people who undergo cataract surgery with a multifocal lens will not need to wear glasses or use contact lenses, following it, because the lens can now adjust the focus as necessary.
Accommodative:
Just like the multifocal lens, the accommodative lens also helps you to focus on range of distances, but the mechanism of function is different. The accommodative lens does this by changing its shape within the eye, when you want to focus on objects at different lengths. When your eye moves, the lens automatically changes its shape.
Toric:
This type of lens is sued to correct a visual problem known as astigmatism, which is problem in the spherical curvature of your eye. Therefore it works almost like an accommodative lens, changing its shape to correct the refractive error that is caused by defective eye shape.
Monofocal:
Which is the most basic IOL that is there on offer. And it helps you to focus clearly on objects at one distance only, wither up close or at distance. What happens on most occasions is that people opt to have lenses which focuses on two differentdistances on either eye, so that they can clearly objects at a distance with one eye, and objects up close with the other eye. This type of vision is known as monovision, and means that you have to wear reading glasses at all times.
All IOL’s other than the basic monofocal ones are considered premium IOL’s, and this means that your insurance will not cover for it. And the decision on choosing what type of IOL you need depends on what your expectations are from the cataract surgery.

100% of the world population with a high life expectancy will develop presbyopia or eyestrain   throughout their lives. This fact is explained because tired eyesight is one of the signs of normal eye aging of the eye, along with the development of the cataract. It is something natural, such as the appearance of wrinkles on the skin or gray hair.
According to eye doctor in Rohini, Presbyopia occurs because the lens, which is the natural lens that the eye has to focus on nearby objects, undergoes a deterioration process over the years that implies on the one hand the loss of elasticity of its anterior capsule (elasticity necessary to change the anterior curvature of the lens and focus), and on the other hand there is also the loss of contraction of the ciliary muscles, responsible for that change of lens shape to be able to perform the approach in close proximity. The sum of these two facts is that which leads to the emergence of presbyopia, which results in, for example, a progressive loss of reading ability, explains eye specialist in Rohini.

What is SMILE

The Procedure
SMILE is the latest advancement in laser vision correction. It is the THIRD GENERATION of refractive procedures. The first two are PRK, followed by LASIK. BAJAJ EYE CARE CENTRE is proud to be the first laser vision correction centre in North and East Delhi to offer the ZEISS VISUMAX femto second laser for SMILE vision correction. SMILE is an all laser, latest cutting edge technology for the correction of myopia and myopic astigmatism. It provides LASIK or even better outcomes in a lease invasive manner. SMILE uses only one type of laser that is femtosecond laser in single step procedure and hence is minimally invasive. During the SMILE procedure, the computer guided laser light energy cuts out a small corneal tissue called ‘Lenticule‘. This Lenticule is equivalent to the dioptric power of the refractive error of the patient. This Lenticule is removed through a small 2mm incision made by the femtosecond laser. This results in reshaping of the cornea and the vision gets corrected.
Precision of SMILE Procedure
The human cornea acts as a refractive surface, possessing al out two- thirds of the total refractive power of the eye. The light rays pass through the cornea, get refracted and then passing through the lens and internal structures of the eye, reach the retina to form an image. The refractive power of the cornea (and hence of the eye ) can be changed by reshaping the corneal curvature / shape of the cornea. This is done by reshaping the stroma ( middle layer ) of the cornea. The SMILE laser removes a thin layer of the stromal tissue ( in microns), created by it, called Lenticule, which is then removed through a small 2-3 mm incision by the experienced surgeon. The tissue removed creates an effect to reshape the corneal curvature equivalent to the spectacle number ( dioptric power ) of the glasses worn by the patient, making this procedure very precise. This all is done by the laser in a matter of seconds, in a painless manner.

The actual laser procedure to make the Lenticule hardly takes about 30-35 seconds. Patients do not experience any discomfort during the procedure and are able to see quickly without glasses within 24 hours of the surgery.
Benefits of SMILE
Minimally Invasive
SMILE is perhaps the least invasive technique possible in laser vision correction for the removal of glasses in comparison to PRK or LASIK Laser . In PRK , an 8-9 mm diameter epithelial layer of the cornea is removed mechanically and then the EXCIMER laser is used to reshape the cornea. In LASIK Laser, the procedure consists of two steps -
1. Making a flap - the flap is approximately 20 mm or more in circumference 2. Excite laser is used to reshape the cornea under the flap and the flap is settled back on the corneal surface. Whereas in SMILE, only a 2-3 mm keyhole is made in the cornea by the VISUMAX femtosecond laser, thereby reducing the damage/cutting of corneal nerves. This results in less dry eye and more stable corneas post operatively.
Flapless Technique
LASIK technique whether using blades ( microkeratome ) or bladeless ( femtolasik ), involves making a flap of the corneal tissue. Rarely complications can occur due to this flap called flap related complications either intra- operatively or post - operatively, though the chances are quite remote in good surgical hands. Since there is no flap created in SMILE procedure, there is no question of flap related complications, either during the procedure or after it.
Lesser Dry Eyes
Post - operative dry eyes is a relatively common side effect of LASIK / Contoura vision procedures . Because SMILE involves making of only. 2-3 mm laser incision versus 20mm incision in LASIK/ Contoura vision lasik procedures, there is potentially a minimal chance of dry eye post SMILE surgery.

What Is Blade Free Lasik ?
BLADE FREE LASIK as it suggests does not involve any kind of blade in lasik laser procedures making the procedure more precise, accurate, safe and more comfortable to the patient. Any lasik procedure consists of two steps :
In the first step a corneal flap is created and in blade free lasik this is done by utilizing a femtosecond laser instead of a blade or microkeratome(blade like computerized equipment) to create a flap as in conventional lasik.
In the second step another laser called an EXCIMER laser is directed to the layers beneath the flap to alter the shape of the cornea according to the patients requirement .

A squint (strabismus) is a condition where the eyes point in different directions.
Signs and symptoms of a squint
One of your child's eyes may turn inwards, outwards, upwards or downwards, while the other eye looks forward. Squints may not be constant, and a minor squint isn't always obvious. A squint can cause blurred or double vision, but children may not realise there's a problem. Left untreated, lazy eye (amblyopia) can develop. This is when the brain starts to ignore signals coming from the eye with the squint.
How are squints treated?
To be most successful and avoid long-term problems, most squints should be treated as soon as possible. Treatment is most effective in very young children.
Several types of treatment are available for squints, including:
1. Glasses – worn constantly to correct vision problems (refractive errors). Children are usually given plastic lenses instead of glass.
2. Eye exercises – In some cases, special eye exercises may help the eyes to work together. 3. Botulinum toxin injections into one of the eye muscles – the injection weakens the muscle, allowing the eyes to realign for around three months. The eyes may stay in position or may need further treatment. Children will usually be given a sedative before the injection.
Squint surgery
If other treatments aren't successful, surgery will probably be required. This involves moving the muscles that control the movement of the eye to improve their alignment and help the eyes work together. Risks from surgery are rare, although sometimes more than one operation will be needed. Headaches are a common complaint among both adults and children. The severity and frequency of headaches range from occasional to nearly daily. Headaches can be debilitating for some people, resulting in a lower quality of life.
